Mechanisms of dengue virus-induced bone marrow suppression.

Infection with many flaviviruses is associated with transient suppression of haematopoiesis. Of the flaviviruses of man, none are more accessible to clinical and laboratory study than dengue. Consequently, the clinical syndrome of dengue-associated bone marrow suppression has been well documented. A review of experimental dengue infections of volunteers and histopathological studies of bone marrow from patients with severe dengue virus infection suggests that marrow suppression evolves rapidly through several phases: (1) onset of marrow suppression within 3-4 days of infection; (2) onset of ho

Gallbladder wall thickening in dengue hemorrhagic fever: an ultrasonographic study.

This study attempts to investigate whether gallbladder wall thickening (GBWT) measured by ultrasonography can be used in children as a reliable criterion to predict the onset of severe dengue hemorrhage fever (DHF). In this prospective study, we performed ultrasound examinations focusing on the gallbladder wall and the presence of intraperitoneal free fluid in 48 mild DHF cases (grades I-II) and 48 severe cases (grades III-IV). GBWT varied between 1 mm and 8 mm with a mean of 3.77 mm +/- 2.04 mm. The mean value of DHF grades I and II (2.39 mm +/- 1.48 mm) is significantly lower than that of gr

[A serological survey regarding Flaviviridae infections on the island of Réunion (1971-1989)].

Serological prevalence of Flaviviridae was studied on Reunion Island by testing 2,507 human sera from a randomised sample. Each serum was tested against 5 viruses (yellow fever, dengue type 1 and 2, West Nile and Wesselsbron) using haemagglutination inhibition test: 42.68% of human sera were found positive. The multivalent reactions represent practically three fourths of the positive ones. A severe dengue type 2 outbreak on the island in 1977-1978 and the possible circulation of a Flavivirus may explain these data. Among the tested subjects, positivity frequencies vary according to their age a

Dengue severity throughout seasonal changes in incidence in Puerto Rico, 1989-1992. The Puerto Rico Association of Epidemiologists.

To determine whether the proportion of severe dengue cases increased with the yearly seasonal increase in dengue incidence, we examined reports of disease symptoms in case surveillance data and laboratory testing results in Puerto Rico from January 1989 to July 1992. A computer algorithm was designed to identify severe cases, i.e., those that fulfilled three or all four of the World Health Organization criteria for dengue hemorrhagic fever (DHF). A monthly severity index (SI) was defined as the ratio of severe cases to laboratory-positive and indeterminate (all non-negative) cases for each mon

Changes in levels of anti-dengue virus IgG subclasses in patients with disease of varying severity.

Extensive complement activation precedes onset of shock in dengue patients and complement "split products" C3a and C5a could be responsible, directly or indirectly, for the increased vascular permeability and disseminated intravascular coagulation which characterises dengue haemorrhagic fever (DHF) dengue shock syndrome (DSS). As IgG subclasses vary in their capacity to activate the classical complement pathway after combining with antigen, we have used an indirect enzyme linked immunosorbent assay (ELISA) to assess levels of IgG1-4 against each dengue serotype in acute and convalescent sera f

Failure of high-dose methylprednisolone in established dengue shock syndrome: a placebo-controlled, double-blind study.

OBJECTIVE: Steroids are widely used in Thailand and other dengue-endemic countries to treat severe dengue shock syndrome. This study was designed to determine whether a single high dose of methylprednisolone will reduce mortality in children with dengue shock syndrome who did not respond to simple fluid and plasma replacement therapy. METHODS: A prospective, randomized, double-blind, controlled trial was conducted in two hospitals in Khon Kaen Thailand during June to September in 1987 and 1988. Sixty-three children with severe dengue shock syndrome were randomized into two groups; the first g

Immunity and immunopathology in dengue virus infections.

Dengue virus infections are a serious public health problem in tropical and subtropical areas of the world. Based on epidemiological data, it has been postulated that immune responses to dengue virus contribute to the pathogenesis of severe dengue illness, dengue hemorrhagic fever (DHF) and dengue shock syndrome (DSS). Host immune responses are also important for controlling dengue virus infection. Therefore, dengue virus infections are an interesting model to explore the interactions between viruses and the immune system which result in immunopathology or recovery from infection. In this pape

Growth of dengue type 2 virus isolates in human peripheral blood leukocytes correlates with severe and mild dengue disease.

We tested three dengue type 2 (DEN-2) isolates from children with clinically apparent but mild secondary dengue infections, and 10 isolates from children with moderately severe dengue hemorrhagic fever, and noted significant growth differences in peripheral blood leukocytes, but not in C6/36 cells. We also observed cytopathic effects in C6/36 cells that correlated with disease severity. These preliminary observations suggest the possibility that viral factors, whether surface antigens, attachment sites for entry into leukocytes, or intrinsic replication properties in human mononuclear phagocyt

Antibody-enhanced infection of monocytes as the pathogenetic mechanism for severe dengue illness.

Antibody-dependent enhancement of certain virus infections can occur in cells expressing Fc receptors. This mechanism plays an important pathogenetic role in the development of complications associated with dengue virus infection, including dengue hemorrhagic infection and dengue shock syndrome. The virulence of the virus, characterized by the ability to infect Fc receptor-bearing monocytes also influences the development of these severe illnesses.

Pericardial effusion in dengue haemorrhagic fever.

Acute shock in severe dengue haemorrhagic fever (DHF) may occur concurrently with accumulation of fluid in serous body spaces such as pleural, peritoneal and pericardial cavities. Echocardiography is a non-invasive diagnostic procedure which is sensitive to detect even a small quantity of pericardial effusion. The chest radiogram in two reported cases of DHF associated with severe shock revealed that almost half of the pleural cavity was filled with fluid. Although no signs of pericardial effusion could be determined on physical examination, ECG and radiological procedures, a small amount of f

Recovery of virus from the liver of children with fatal dengue: reflections on the pathogenesis of the disease and its possible analogy with that of yellow fever.

Dengue viruses types 2 or 3 were recovered from the liver of 5 out of 17 children who were suspected to have died of dengue. Among the 5 children with liver isolates, virus was also recovered from the heart blood, spleen and midbrain of one and from the spleen of another. Attention is drawn to the many similarities in the clinical manifestations of yellow fever and severe dengue, their indistinguishable liver lesions and the close relationship of the viruses themselves. In view of the liver isolates reported here and those described by others, it is suggested that the pathogenesis of the two d

Study of the distribution of antibody-dependent enhancement determinants on dengue 2 isolates using dengue 2-derived monoclonal antibodies.

Dengue 2 (DEN-2) strains isolated from children during the 1980 metropolitan Bangkok epidemic were shown to possess antigenic homogeneity when studied for determinants mediating antibody-dependent infection enhancement using DEN-2 monoclonal antibodies. All isolates possessed multiple enhancing determinants, but those associated with mild and severe dengue syndromes could not be distinguished. Either the basis of disease severity in dengue is more complex than the mere presence or absence of virus epitopes involved in enhanced infection or enhancing epitopes have differences not detected in th

Disease severity-related antigenic differences in dengue 2 strains detected by dengue 4 monoclonal antibodies.

Low-passage epidemic Thai isolates of dengue 2 (DEN-2) were distinguished on the basis of associated disease severity by detection of virus determinants involved in enhanced macrophage infection when incubated with a panel of monoclonal antibodies derived from a DEN-4 strain associated with grade II dengue hemorrhagic fever (DHF). DEN-2 strains associated with DHF typically had multiple determinants that were involved in antibody-dependent enhancement of infection, whereas those associated with uncomplicated disease had few. These data provide further evidence that in epidemic areas where DHF

Hemodynamic response to high-dose methyl prednisolone and mannitol in severe dengue-shock patients unresponsive to fluid replacement.

Nine children; 4 males and 5 females, aged ranging from 2 1/2 to 13 years presented with signs and symptoms of poor tissue perfusion associated with dengue shock syndrome. All these 9 patients were subjected to the therapeutic trial of high dose methyl prednisolone (MP; 9/9) and mannitol (M; 6/9) after their failure to the saline and plasma replacement. Following the high dose MP and M, a significant increment in the effective circulatory blood volume as reflected by the sustained increment in CVP, widening of PP and declining in PR as well as improvement in clinical tissue perfusion were esta

Immunoepidemiology helps to unravel the mysteries of dengue haemorrhagic fever.

It is now nearly 20 years since a strong correlation was noted in Thailand between secondary immune responses and severe dengue syndromes. It is currently thought that 'enhancing' antibodies, in an individual undergoing a second infection with a different serotype of dengue virus, promotes viral replication in monocytes which then become the targets of an immune elimination response (possibly T-cell mediated). The monocytes then release various chemical mediators which produce the symptoms of shock and haemorrhage seen in dengue haemorrhagic fever (DHF). Much new knowledge has been gained in r

Suppression of dengue virus replication in vitro by rimantadine hydrochloride.

The effects of rimantadine on dengue virus replication were examined in a variety of tissue culture systems. The growth of dengue virus type 2 in human peripheral blood leukocytes (PBL) was completely suppressed when rimantadine was included in the culture medium at a concentration of 25 microgram/ml. Similarly, rimantadine caused a significant inhibition of dengue virus replicaton in cultures of rhesus monkey PBL. Addition of drug into virus-infected LLC-MK2 cell cultures caused a decrease in the production of all four types of dengue virus. Maximal inhibition of dengue virus replication by r
