BACKGROUND: Dengue shock (DS) and septic shock (SS) are the most common infectious causes of shock in Vietnam. Little is known about the cost of an episode of DS or SS from the patient perspective. We aimed to describe the direct medical, non-medical and productivity costs associated with DS and SS.
METHODS: We recruited adults with DS and SS to a prospective observational study at the Hospital for Tropical Diseases, Ho Chi Minh City, from 2019 to 2021. We collected hospital bills, insurance status and out of pocket payments, and conducted an economic questionnaire at discharge, 1, 3 and 6 mo

The four dengue virus serotypes (DENV1-4) and the related Zika flavivirus (ZIKV) are major public health concerns worldwide. Primary immunity against ZIKV increases the risk of a subsequent severe DENV2 infection, presenting a significant challenge for developing safe and effective ZIKV vaccines. However, the mechanisms driving this phenomenon remain unclear. Leveraging a long-standing Pediatric Dengue Cohort Study in Nicaragua, we show that anti-NS1 immunoglobulin A (IgA) antibodies in plasma, elicited after a primary ZIKV infection, drive neutrophil activation and correlate with increased ri

Dengue virus (DENV) infection can potentially lead to severe dengue disease due to the risk of antibody-dependent enhancement. This study reports a comparative analysis of the host cytokine/chemokine response triggered by primary and secondary DENV infections using an artificial ex vivo whole-blood coculture model to simulate viremia during the acute febrile phase of infection. Using ex vivo primary and secondary DENV infection modes, a dengue-specific customized multiplex cytokine/chemokine assay was employed. Secondary infection did not exacerbate DENV-induced hematological and cytopathologi

BACKGROUND AND PURPOSE: Severe dengue is characterised by systemic inflammation, cytokine storm, vascular leakage and haemorrhagic manifestations, largely driven by the host immune response to dengue virus (DENV) infection. Despite its burden, no licensed antivirals or host-directed therapies are currently available. Our group has previously identified Annexin A1 (AnxA1) as an endogenous regulator of inflammation in dengue. Treatment with the AnxA1 peptidomimetic, Ac2-26, improved clinical outcomes in murine models of severe dengue by promoting resolution of inflammation without affecting vira

INTRODUCTION: Infection-associated hemophagocytic syndrome (IAHS) is a rare complication in dengue infection that leads to multiple organ dysfunction (MOD) and increased mortality rates. The early use of hemoperfusion with hemoadsorption and standard treatment may reduce inflammation and prevent mortality. This study investigates the therapeutic effectiveness of a combination of hemoperfusion with hemoadsorption and multimodal therapy, including intravenous immunoglobulin (IVIG), corticosteroids, therapeutic plasma exchange (TPE), and/or extracorporeal organ support in pediatric patients with

Dengue poses a significant arboviral threat in Brazil, with 2024 recording the largest outbreak to date. This prospective observational study was conducted during the 2024 outbreak with unvaccinated patients at Eduardo de Menezes Hospital, Belo Horizonte. A total of 556 patients were included, of whom 169 had complete clinical and laboratory data. Patients with suspected dengue underwent clinical and hematological evaluations, as well as diagnosis by RT-qPCR, and ELISA. These parameters were employed to assess the relationship between diagnostic methods, hematological changes, and disease seve

Vascular leakage is the defining pathological hallmark of severe dengue, including dengue hemorrhagic fever and dengue shock syndrome. Despite its clinical importance, the mechanisms linking dengue virus infection to endothelial dysfunction remain incompletely understood, and no medicine is currently available. Emerging evidence implicates dysregulation of the host fibrinolytic system in dengue-induced vascular leakage, but its mechanistic role has not been evaluated. Here, we show that dengue virus infection activates endothelial fibrinolytic pathways to drive plasmin-mediated vascular leakag

The majority of the antibody response in dengue infection targets the structural envelope (E) and membrane (M) proteins of the virus. However, vaccine development is challenging because antibodies against the E protein often show cross-reactivity, leading to antibody-dependent enhancement (ADE) and worsening of disease severity. Non-structural protein 1 (NS1), secreted during the acute phase of infection, has been implicated as a pathogenic factor that contributes to vascular permeability and haemorrhage in severe dengue. In this study, we aimed to investigate the role of NS1 antibodies in dis

BACKGROUND: Dengue fever poses a major public health threat in Brazil, particularly among individuals with comorbid conditions. Chronic kidney disease (CKD) is known to impair immune defenses and may predispose patients to worse infectious outcomes, but its impact on dengue has not been comprehensively evaluated at the national level. This study aimed to assess the association between CKD and adverse clinical outcomes of dengue fever.
METHODS: We conducted a population-based analysis using 2024 data from the Brazilian Information System for Notifiable Diseases (SINAN). Patients with a confirm

BACKGROUND: Prolonged fever occurs in some adults with dengue and is linked to greater severity. However, its frequency, characteristics and clinical outcomes in pediatric dengue remain unclear.
METHODS: We performed a prospective study in a dengue hyperendemic region of Colombia, including hospitalized children with confirmed dengue presenting warning signs or severe dengue (SD). Prolonged fever was defined as persistent temperature >38.3°C beyond day 7 of illness. We evaluated prolonged fever frequency, clinical features, immunologic status and outcomes.
RESULTS: Among 253 children (204 de

Reliable prognostic biomarkers remain an unmet need in dengue. We evaluated urinary neutrophil gelatinase-associated lipocalin and soluble urokinase plasminogen activator receptor for severe dengue prediction in the febrile phase. Urinary neutrophil gelatinase-associated lipocalin and soluble urokinase plasminogen activator receptor demonstrated areas under the receiver operating curve of 0.88 and 0.79, respectively, in discriminating against severe dengue. The noninvasive sampling offers practical advantages as risk stratification tools.

Dengue, transmitted by Aedes mosquitoes, can progress to severe symptoms like hemorrhagic fever and shock syndrome. While the virus and host immune response contribute to severity, other factors, such as small extracellular vesicles (sEVs), may play a role. sEVs mediate intercellular communication by transferring cellular components; however, their role in vivo infection remains unclear. We isolated and characterized sEVs from DENV-infected C6/36 mosquito cells, finding that they interact with mammalian cells and internalize the content. Using sEVs populations (with a size between 100 and 200

BACKGROUND: Plasma leakage is a key manifestation of severe dengue, but current recommended diagnostic methods by the World Health Organization (WHO) have several limitations. Endothelial cells (ECs) activation and dysfunction are known to play a central role in plasma leakage. Endocan, a proteoglycan produced by activated ECs, has been shown to correlate with disease severity in dengue infection. This study aimed to evaluate endocan as a potential marker of plasma leakage in dengue infection. METHODS: This retrospective cohort study utilized data from the International Study on Biomarkers and

Infants represent a vulnerable population for severe outcomes from infectious diseases. For dengue virus, maternal antibodies can cause severe disease in infants aged 5-12 months through antibody-dependent enhancement, but the role of maternal immunity in neonates aged 0-30 days has not been quantified. Our study leverages the re-emerging setting of Brazil, where the proportion of mothers with anti-dengue antibodies varies across space and time. Infant dengue burden has risen 11-fold from 2000-2024, in line with expansion in the general population. We fit mechanistic models to infant dengue an

Invasive fungal infection (IFI) simultaneously due to three fungi is rare. IFI caused by two fungal agents is relatively commoner in transplant patients. Most common IFI worldwide is candidiasis, whereas in India, aspergillosis and mucormycosis infections are commoner. Severe dengue viral infection has been linked with invasive fungal infections. Following case report exemplifies more than two fungi can together cause pneumonia as confirmed by similar findings in 3 respiratory samples from the dengue shock syndrome patient. INTRODUCTION: Dengue virus (DENV) infection can present a range of clinical manifestations, from mild fever to severe complications. Emerging data suggest that specific viral serotypes may influence disease severity. This study aimed to evaluate the circulating DENV serotypes and their correlation with clinical outcomes in hospitalized patients.
METHODS: A retrospective, cross-sectional study was conducted at a tertiary care center in northern India from August 2022 to January 2023. Laboratory-confirmed dengue cases (N=356) were included, and serotyping was performed via nested reverse transc
