Oral inulin prebiotic encapsulating Cortex Fraxini extracellular vesicles alleviates colitis and associated renal stress via reinforcing intestinal barriers and remodeling microbiota.

Background: Gut-kidney axis dysfunction contributes to extraintestinal complications in inflammatory bowel disease (IBD), yet effective therapeutic strategies targeting this axis remain limited. This study developed an oral prebiotic delivery system by encapsulating Cortex Fraxini-derived extracellular vesicles (CF-EVs) within an inulin-based hydrogel (CF-EVs@Inulin) and evaluated its therapeutic potential in ulcerative colitis and associated renal injury. Methods: CF-EVs were isolated and characterized for morphology and antioxidant activity. An inulin-based hydrogel was employed to encapsula

Review: optimizing rescue therapy in acute severe ulcerative colitis using infliximab pharmacokinetics-a practical guide for clinicians.

BACKGROUND: Corticosteroid-refractory acute severe ulcerative colitis (ASUC) remains a key management challenge despite an increasing understanding of rescue strategies. Due to the relative paucity of high-quality evidence, the use of infliximab in ASUC should be informed by pragmatic application of key clinical and pharmacological principles. This review synthesizes emerging evidence of infliximab pharmacology, describes key concepts of mechanisms of infliximab failure, and suggests an optimized approach to using infliximab in ASUC. METHODS: Relevant studies investigating infliximab in ASUC

Survival impact of involvement of lymph node stations 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic and periampullary cancers (PANODE): prospective multicentre study.

BACKGROUND: Current guidelines do not recommend routine sampling of lymph node stations (LNS) 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic ductal adenocarcinoma (PDAC) and other periampullary cancers because prospective multicentre studies are lacking. This study evaluated the association between the involvement of these LNS and overall survival (OS). METHODS: This prospective multicentre study included patients undergoing pancreatoduodenectomy for PDAC, distal cholangiocarcinoma, ampullary cancer, or duodenal cancer between October 2015 and April 2020. Associations between inv

Bacteroides fragilis Abscess Associated With Fat Necrosis Following Breast Augmentation With Autologous Fat Grafting: A Case Report With Review of the Literature.

Autologous fat grafting (AFG) is a popular breast augmentation procedure characterized by high patient satisfaction and low complication rates. Infectious complications are uncommon with this technique, and gram-positive organisms are the predominant pathogens responsible for most cases. We report a rare case of abscess formation due to Bacteroides fragilis infection occurring in the setting of presumed fat necrosis following AFG, as indicated by ultrasound imaging and plastic surgery evaluation. A 52-year-old woman developed a postoperative infection after AFG and abdominal liposuction despit

The role of interleukins and immunological parameters in patients with colon cancer: an optical insight.

The progression of colon cancer from benign adenoma to malignant tumors that invade and spread underscores the critical need for accurate and less invasive biomarkers. The primary objective of this work was to assess the diagnostic potential of combining immunological biomarkers and optical polarimetric characteristics in colorectal cancer. We specifically aimed to evaluate serum cytokine profiles as potential diagnostic tools and to explore tissue optical signatures in a preliminary analysis. Secondary objectives included examining postoperative immune changes and exploring the involvement of

Pathogenic Keratinocyte States and Fibroblast Niches Define the Tissue Microenvironment in Severe Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease characterized by recurrent painful abscesses and tunnels in flexural sites. The mechanisms driving HS pathogenesis, particularly the interactions between epithelial, stromal and immune cell populations, remain incompletely understood. To characterise the cellular and molecular landscape of HS, we analysed lesional skin from severe HS patients using single-cell RNA-sequencing and spatial transcriptomics, with a focus on fibroblast-keratinocyte interactions. We identified a migratory S100+ pathogenic keratinocyte state enriched

Machine Learning-Assisted, Ago-Mediated Droplet Fluorescence Analysis with a Knowledge-Enabled Agent for Colorectal Cancer Diagnostics.

Effective screening for colorectal cancer (CRC) remains limited by the invasiveness of colonoscopy and the insufficient sensitivity of conventional liquid biopsy approaches. Here, we present Machine Learning-Assisted, Ago-Mediated Droplet Fluorescence Analysis with a Knowledge-Enabled Agent (MADAK-Agent), an integrated analytical platform that combines an Argonaute-mediated cascade amplification module with a monodisperse droplet-assisted, object-resolved post-amplification fluorescence measurement workflow and a knowledge-enabled interpretation interface. We developed a dual-channel cascade s

Targeted Gastrointestinal Delivery of mRNA Lipid Nanoparticles Enables Systemic Vaccination and Glucagon-like Peptide-1 Metabolic Therapy.

Lipid nanoparticles (LNPs) administered parenterally often show poor localization to multiple tissue types, including the pancreas and gastrointestinal (GI) tract. Furthermore, patients typically prefer orally administered drugs to those that are given parenterally. We therefore investigated whether GI LNP administration to buccal, gastric, small intestinal, colonic, and rectal submucosal tissues could simultaneously circumvent parenteral administration while enhancing LNP delivery to previously inaccessible locations. We found that, in addition to local targeting, gastric LNP administration a

Dual therapy with tofacitinib and steroids for acute severe ulcerative colitis: a randomized double-blind placebo-controlled trial.

BACKGROUND: One-third of patients with acute severe ulcerative colitis (ASUC) fail intravenous corticosteroids. We evaluated whether adding tofacitinib to corticosteroids reduces treatment failure in ASUC. METHODS: In this single-center, double-blind, placebo-controlled trial, adults with ASUC were randomized 1:1 to tofacitinib (10 mg three times daily for 3 days, then twice daily) or placebo for 5 days alongside corticosteroids. Primary outcome was treatment failure at day 6 (<3-point Lichtiger reduction or total score ≥10, or colectomy). Secondary outcomes were relapse (≥3-point rise with t

Risk factors for postoperative intra-abdominal abscess following minimally invasive ileocecal resection in children with Crohn's disease.

BACKGROUND: Pediatric-onset Crohn's disease (CD) is associated with a more severe disease course than adult-onset CD, with earlier progression to complications, and frequent need for surgery. Ileocecal resection (ICR) is the most commonly performed surgery. Postoperative intra-abdominal (IAA) abscess is a frequent and potentially severe complication following ICR in children. The aim of this study was to identify independent risk factors associated with postoperative IAA following minimally invasive ICR in children with CD. METHODS: We conducted a retrospective single-center study including p

Safety of in-hospital delay of appendectomy in complicated appendicitis - a propensity score-matched analysis of 1027 patients.

INTRODUCTION: Appendectomy is one of the most frequently performed emergency surgical procedures worldwide. Current guidelines recommend surgical intervention within 24 h for uncomplicated appendicitis; however, the optimal timing for patients with complicated appendicitis remains unclear. Recent studies suggest that in-hospital delay may be safe in uncomplicated cases, yet patients with clinical or radiological signs of perforation are usually excluded. This study aims to identify clinical and laboratory risk factors for perforation and to evaluate whether it is safe to delay surgery by 6 to

Self-Subcutaneous Injection of Elemental Mercury: A Report of a Rare Case From Pakistan.

Deliberate subcutaneous injection of mercury by a mentally healthy individual is rare. It has been frequently reported in individuals with severe depressive disorders during suicide attempts, as well as among athletes seeking performance enhancement and in certain contexts related to sexual gratification. Here, we present the case of a 28-year-old male patient who deliberately self-injected mercury subcutaneously into his abdomen that led to the formation of a painful induration/abscess in the midline of the abdomen. X-ray of the abdomen showed multiple subcutaneous radio-opaque deposits. A su

Anatomical Correction of Adult Intestinal Malrotation Using Kareem's Procedure: A Case Report.

BACKGROUND: Intestinal malrotation (IM) is a congenital defect in midgut rotation or fixation that results in abnormal positioning of the bowel. Although most cases present during infancy, adult presentation is uncommon and may be misdiagnosed. Kareem's procedure is an anatomical corrective approach described for gut malrotation; however, evidence regarding its use remains limited. CASE PRESENTATION: A 26-year-old female presented with sudden, severe, colicky central abdominal pain associated with non-bilious vomiting and complete constipation for 10 hours. She had experienced a similar self-

Early surgery for strangulated and non-strangulated small bowel obstruction has no advantages over delayed procedure, especially in elderly patients: results of a multicenter retrospective cohort study.

BACKGROUND: Adhesive small bowel obstruction (SBO) can be managed surgically or non-operatively. However, there are still no well-established criteria for defining the optimal treatment strategy. Recent evidence suggests that early intervention within 24 hours of admission to hospital may benefit patients. METHODS: This was a multicenter retrospective study of adhesive SBO investigating the outcomes of early surgery (<12 hours), delayed surgery, and nonoperative treatment in 143 patients. In addition, the impact of time to surgery was also assessed in cases of strangulated and non-strangulate
