Circadian clock regulates intestinal epithelial cell differentiation via NOTCH/Hes1 oscillations.

The circadian clock regulates diverse cellular processes, including intestinal epithelial cell (IEC) proliferation. However, mechanisms regulating clock-dependent IEC differentiation remain unknown. We performed a time course RNA-Seq using the mouse small intestine and identified NOTCH signaling, a key mechanism regulating IEC differentiation, as one of the pathways under the control of circadian rhythms. Using mouse enteroids, we discovered that a NOTCH reporter, Hes1-luciferase, exhibits both ultradian or circadian oscillations depending on the stemness of mouse enteroids. Furthermore, singl

Non-coding RNAs in pancreatitis: from mechanisms to therapeutics.

Pancreatitis is a complex inflammatory condition of the pancreas and can be classified into acute pancreatitis (AP) and chronic pancreatitis (CP). The pathogenesis of the disease remains unclear, and effective preventive or therapeutic strategies are currently lacking. Non-coding RNA (ncRNA) regulatory networks consist primarily of miRNAs, lncRNAs, and circRNAs, which precisely regulate gene expression through complex interactions and play a key role in fibrosis and inflammation. In terms of AP, ncRNAs could modulate the activity of pancreatic acinar cells, autophagic flux, inflammatory cell i

Groin abscess outside the hernia sac secondary to perforated appendicitis in an older adult: a case report.

Groin abscesses secondary to acute appendicitis are rare, particularly when inflammation spreads retroperitoneally and the abscess forms outside the hernia sac rather than within such as occurs in Amyand's hernia. An 84-year-old woman with advanced breast cancer and a known inguinal hernia developed a progressively enlarging right groin mass 4 days after mastectomy. Computed tomography revealed a fluid collection, and needle aspiration confirmed an abscess. Conservative management led to temporary improvement; however, a recurrent abscess near the cecum appeared 31 days later. Surgical interve

Gut-Liver Axis Failure in Critical Alcohol-Associated Liver Disease: From ICU Secondary Hits to Microbiome-Targeted Therapy.

Alcohol-associated liver disease (ALD) can progress to critical illness phenotypes requiring intensive care, including severe alcohol-associated hepatitis, acute decompensation, and alcohol-associated acute-on-chronic liver failure (ACLF). In these patients, short-term outcomes are driven less by the burden of fibrosis alone than by systemic inflammation, immune dysfunction, infection, and multiorgan failure. At the core of this process is gut-liver axis failure, which links alcohol-induced dysbiosis and intestinal barrier disruption to microbial translocation, hepatic innate immune activation

Combined Omeprazole and Glycyrrhiza glabra L. Extract Attenuate Ethanol-Induced Gastric Ulceration Through Modulation of TLR4/NF-κB/NLRP3 Signaling and Upregulation of PI3K/AKT/mTOR Gene Expression.

Ethanol (EtOH)-induced gastric ulcer (GU) is a common model used to investigate mechanisms of mucosal injury and repair. Omeprazole (OMP) is a conventional anti-ulcer drug that effectively suppresses gastric acid secretion, but its efficacy may be enhanced by combining it with bioactive phytochemicals derived from a Glycyrrhiza glabra L. (licorice; LIQ) extract that possess potent antioxidant and anti-inflammatory properties. The aim of the present study was to evaluate the gastroprotective effects of OMP, LIQ extract, and their combined treatment against EtOH-induced GU in rats, focusing on m

High interobserver variability exists in grading appendiceal goblet cell adenocarcinoma using World Health Organization 5th edition criteria.

AIMS: Appendiceal goblet cell adenocarcinoma (GCA) is an uncommon malignancy that has been described under various names and grading schemes. The 5th Edition of the World Health Organization (WHO) Classification of Digestive System Tumours provides a three-tiered system for grading these neoplasms, but the reproducibility of this classification scheme has not been studied. METHODS AND RESULTS: We scanned 58 H&E-stained slides from 20 GCA and circulated the whole-slide images among seven pathologists with interest in appendiceal pathology. They evaluated each slide for the presence of 15 histo

Analysis of predictive factors and development of a nomogram for complicated acute appendicitis in adults.

BACKGROUND: The number of patients receiving conservative treatment for acute appendicitis is currently increasing. However, for complicated acute appendicitis (CAA), surgical treatment yields more definitive outcomes, and delayed surgery increases the risk of complications. Therefore, this study aimed to analyze the predictive factors for CAA and establish a nomogram. METHODS: Patients who underwent appendectomy between January 2023 and October 2025 were included. Univariate and multivariate analyses were performed to identify predictive factors and independent predictive factors, respective

A rare presentation of CASPR2-associated Morvan syndrome overlapping with GM1-positive AMSAN: a case report.

BACKGROUND: Morvan syndrome is a rare autoimmune disorder characterized by peripheral nerve hyperexcitability with autonomic and central nervous system involvement, most commonly associated with antibodies against contactin-associated protein-like 2 (CASPR2). Acute motor and sensory axonal neuropathy (AMSAN) is an axonal variant of Guillain-Barré syndrome linked to anti-ganglioside antibodies and often manifests as severe limb weakness. Their concurrent presentation is unusual and raises the possibility of shared immune targets within peripheral nerve microdomains. CASE PRESENTATION: A 70-yea

Incidence Trends of Early-Onset Colorectal Cancer in Germany: A Registry-Based Study From 2003 to 2023.

Motivated by studies on early-onset colorectal cancer (EO CRC, below 50 years) from the United States (US) and elsewhere, this study aimed to provide national incidence trends of EO CRC for Germany. We retrieved colorectal cancer cases (ICD-10 codes C18-C20) using pooled data from German cancer registries with high-quality data. Appendix (C18.1) was excluded from the main analyses and investigated separately. We calculated age-standardised incidence rates (ASR) by calendar year, age group, sex, tumour site, histological subtype, tumour size, grading and estimated corresponding average annual p

Right Lateral Femoral Cutaneous Nerve Neuropathy After Surgery for Perforated Appendicitis: A Case Report.

Perforated appendicitis can be associated with a variety of postoperative complications, most notably abscess and wound infection. Perforation of the appendix can cause regional inflammation that affects neighboring tissues and organs. Occasionally, unexpected additional pathology emerges in the postoperative setting. Due to the retroperitoneal nature of the lateral femoral cutaneous nerve of the thigh (LFCN), resulting inflammation can contribute to neuropathy, known as meralgia paresthetica (MP). The findings of MP are suggested by physical examination. This report describes the case of a pa

Streptococcus intermedius Septic Arthritis of the Acromioclavicular Joint with Periarticular Abscesses in an Elderly Man with Diabetes and Recent Canine Exposure: A Case Report and Literature Review.

BACKGROUND/OBJECTIVES: Streptococcus intermedius, a member of the Streptococcus anginosus group, is characterized by a marked propensity for abscess formation but only rarely causes native-joint septic arthritis. Involvement of the acromioclavicular (AC) joint is particularly uncommon. We describe a case of native AC joint septic arthritis due to S. intermedius in a patient with multiple predisposing factors and highlight diagnostic and management considerations. METHODS: We report the clinical course of a 72-year-old man with poorly controlled type 2 diabetes mellitus who presented with prog

The Dual Roles of Gut Microbiota in Biliary Atresia: Mechanisms, Biomarker Potential, and Therapeutic Implications.

Biliary atresia (BA) is a progressive fibroinflammatory cholangiopathy of infancy that rapidly advances to cholestasis, fibrosis, cirrhosis, and liver failure if bile drainage is not restored early. Although Kasai hepatoportoenterostomy (KPE) remains the standard first-line operation, many children still develop recurrent cholangitis, persistent cholestasis, and progressive native liver injury. Increasing evidence indicates that the gut microbiota participates in this clinical course through the gut-liver axis. In BA, dysbiosis may weaken the intestinal barrier, increase translocation of micro

CD44-Targeting Hydroxyapatite Nanoparticles (HAP) Induce Mitochondrial Dysfunction-Driven PANoptosis and Immunogenic Cell Death (ICD) via Ca Overload in Colorectal Cancer.

Colorectal cancer (CRC) remains therapeutically challenging due to high metastasis, recurrence, and immunotherapy resistance driven by tumor microenvironment-mediated immune evasion. Immunogenic cell death (ICD) offers a promising strategy to reshape the immune microenvironment, yet existing ICD inducers suffer from poor targeting efficiency and insufficient death signal release. Here, we constructed a calcium overload-based smart nanosystem, HA-HAP@CUR, to achieve highly efficient ICD induction via a triple-effect mechanism: hyaluronic acid (HA)-mediated CD44 targeting enables tumor-selective

Rare Rectal Involvement of Metastatic Appendiceal Adenocarcinoma Mimicking Primary Rectal Cancer.

Primary appendiceal adenocarcinoma is a rare gastrointestinal malignancy that typically metastasizes to the peritoneum. Metastatic rectal involvement is exceptionally rare. We describe the case of a previously healthy 44-year-old man diagnosed with metastatic appendiceal adenocarcinoma. Sixteen months after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy, he developed new-onset constipation and tenesmus. Subsequent colonoscopy revealed a large ulcerated rectal mass, consistent with possible metastatic poorly differentiated adenocarcinoma based on histology, immunoprofiling

Aspergillus fumigatus brain abscess in an immunocompromised patient receiving a second-generation Bruton Tyrosine Kinase inhibitor (BTKi).

Chronic lymphocytic leukemia (CLL) is a frequent type of adult leukemia. Bruton Tyrosine Kinase inhibitors (BTKi) are first-line treatments for CLL. Despite being efficient, BTKi have also been associated with significant side effects, including arrhythmias, hemorrhagic complications, and opportunistic infections. Zanubrutinib is a second-generation BTKi that recently received FDA approval for CLL and is associated with improved outcomes and a better safety profile compared to first-generation BTKi. In the present study, we report a case of a patient with CLL, with an in-range absolute neutrop
