Gastric outlet obstruction secondary to Peutz-Jeghers syndrome successfully managed by polypectomy.

Peutz-Jeghers syndrome (PJS) is a genetic disorder characterized by the development of hamartomatous polyps throughout the gastrointestinal tract but primarily in the small bowel. Gastric outlet obstruction (GOO) due to PJS polyps is rare in childhood. The optimal treatment of large, gastric polyps in PJS remains unclear. We report herein the case of a 1-year-old, male patient who was treated endoscopically for GOO caused by a massive PJS polyp. The patient had a history of frequent non-bilious vomiting and melena. Gastrointestinal bleeding was suspected, prompting an abdominal ultrasound and

Entering the era of disease modification in Crohn's disease.

INTRODUCTION: Crohn's disease (CD) can follow a progressive course leading to structural bowel damage and disability. The expansion of advanced therapies and recognition of a window of opportunity have shifted management from symptom control toward disease modification. AREAS COVERED: After searching MEDLINE and Embase from inception until 1 July 2027, we reviewed studies examining early intervention and disease modification in Crohn's disease. This review examines evidence for early medical and surgical intervention as individual treatment strategies to modify the course of disease, distingu

Effects of graded dietary raw bee venom inclusion on growth, immunity, and disease resistance in Nile tilapia (Oreochromis niloticus).

Nowadays, studies on the application of bee venom (BV) in aquaculture have been growing due to its bioactive constituents. However, previous studies have focused on nano-formulated bee venom or parenteral administration routes on Nile tilapia (Oreochromis niloticus). However, the effects of graded dietary supplementation of raw venom, especially at different inclusion levels, on Nile tilapia are still limited. Therefore, this study focused on investigating the effects of bee venom supplementation at different dosage levels on growth performance, hematological and biochemical parameters, antiox

Structural mechanisms for ATP-mediated inhibition of human NLRP6 inflammasome.

Inflammasomes ignite innate immune defense in response to infectious pathogens and noninfectious dangers, primarily through sensors composed of nucleotide-binding domain (NBD), leucine-rich repeat (LRR)-containing (NLR) family proteins. NLRP6 is an inflammasome sensor that plays critical roles in regulating intestinal inflammation, and its overactivation is linked to autoinflammatory diseases such as inflammatory bowel disease. However, how NLRP6 is maintained in an inhibited structure is unknown. Here we report two cryogenic-electron microscopy structures of human NLRP6 monomer in the adenosi

The Role of Lachnospiraceae in Liver Diseases: Recent Advances and Clinical Application Prospects.

Chronic liver diseases impose a considerable global public health burden, yet available treatment options remain largely inadequate. The gut microbiota exerts a key modulatory effect on liver disease pathogenesis via the gut-liver axis. Among them, Lachnospiraceae, a dominant bacterial family in the healthy adult gut, has drawn growing interest in recent years. Lachnospiraceae exert protective functions by producing short-chain fatty acids, participating in secondary bile acid conversion, and synthesizing active metabolites such as N-acetyl-glutamic acid, thereby maintaining intestinal barrier

Endoscopic vacuum therapy for leaky cavities: is it possible?

A 71-year-old male patient with a history of bladder neoplasia underwent Bricker-type surgery, during which an iatrogenic rectal injury occurred. During surgery, an unsuccessful suture attempt was made, leading to the appearance of fecaluria after 48 hours. A computed tomography scan revealed a small continuity defect in the rectal wall, accompanied by a 25-mm adjacent collection. Percutaneous drainage was placed in an attempt to achieve spontaneous closure, but this was unsuccessful. A rectoscopy was performed, identifying a wall defect in the mid-rectum. A review with a paediatric gastroscop

Investigating the Role of Sodium Butyrate in Mouse Models of IBD: A Multimodal Structural and Functional MRI Study.

Patients with inflammatory bowel disease (IBD) experience abnormalities in the central nervous system via the gut-brain axis (GBA), leading to changes in brain structure and function. Butyrate has been shown to influence the central nervous system via the GBA, with potential benefits for neurological issues in IBD. However, directly assessing butyrate's impact on the central nervous system in IBD patients is difficult due to confounding factors in human studies. This study employed a mouse model to investigate the effects of sodium butyrate (NaB) on brain structure and function in IBD through

Microbial indole-3-acetamide alleviates polycystic ovary syndrome through HIF-2α-ceramide signaling.

Polycystic ovary syndrome (PCOS) is a common reproductive endocrine disease and metabolic disorder in women that is marked by abnormalities in adipose tissue and gut microbial dysbiosis. However, it remains unclear whether and how the gut microbiota affects adipose tissue, thereby leading to the development of PCOS. Here, we find that intestinal Collinsella aerofaciens and its metabolite indole-3-acetamide (IAM) are significantly reduced in PCOS patients. Mechanistically, we demonstrate that C. aerofaciens produces IAM through an IAM synthase, which activates the adipose hypoxia-inducible fact

Harnessing NIR-II-responsive Rh single-atom nanozymes for photothermal-catalytic immunomodulation and eradication of drug-resistant biofilms in deep tissues.

The rising incidence of multidrug-resistant (MDR) bacterial infections in deep tissues, such as subcutaneous abscesses and bacterial pneumonia, has become an urgent public health concern. Both conventional antibiotic treatments and nanomaterial-based therapies are inadequate due to their limited catalytic efficiency, poor tissue penetration, and the enhanced resistance they provoke. We report the rational design and synthesis of nitrogen-doped rhodium single-atom catalysts (Rh SACs) that integrate second near-infrared (NIR-II) photothermal conversion with multienzyme mimetic activities. This n

Microplastics and inflammatory bowel disease: dissecting the evidence from observational links to potential causal mechanisms.

Inflammatory bowel disease (IBD) has surged globally, and environmental factors such as microplastics (MPs) have emerged as potential contributors alongside a complex interplay between altered gut microbiota and dysregulated immune response in genetically susceptible individuals. While current evidence linking MPs to intestinal inflammation remains largely associative, a causal relationship between MP exposure and IBD in humans has yet to be confirmed. Therefore, this review aims to summarize current evidence regarding the occurrence, detection, and potential pathogenic roles of MPs in IBD. Em

The current studies on the mechanisms of lactic acid bacteria as probiotics in the alleviate of bovine diseases: a review.

Lactic acid bacteria (LAB) are an encouraging probiotic option to antibiotics in treating typical bovine diseases, including but not limited to, mastitis, inflammatory bowel disease, diarrhea, and endometritis. The review discusses the therapeutic mechanisms of LAB in a systematic manner including competitive exclusion of pathogens, synthesis of antimicrobial products such as organic acids, bacteriocins, and short-chain fatty acids, recovery of mucosal barrier integrity by regulating tight junctions, and suppression of pro-inflammatory pathways, such as NF-κB and MAPK, to modulate the host imm

Gut microbiota-regulated cell death: a review on pyroptosis, ferroptosis, and related mechanisms.

The intestinal microbiota has emerged as a crucial regulator of host physiological functions, significantly influencing various forms of regulated cell death (RCD). Among these, pyroptosis and ferroptosis play pivotal roles in immune regulation, metabolic homeostasis, and disease progression. The gut microbiota modulates these cell-death pathways through microbial metabolites, inflammatory signaling, oxidative stress, and immune-cell polarization, thereby affecting intestinal disorders, metabolic diseases, cardiovascular diseases, and neurodegenerative diseases. However, current evidence remai

Females with Normal-Weight Obesity Display Evidence of Postprandial Dyslipidemia and Basal Intestinal Permeability.

Females with a normal body mass index (BMI) and high body fat percent (i.e., normal-weight obesity or NWO) are at increased risk for cardiometabolic diseases compared to normal-weight lean (NWL) counterparts. However, underlying mechanisms remain unclear. Here, we examined postprandial lipids, gut permeability biomarkers, and arterial stiffness outcomes as potential factors implicated in cardiometabolic disease development. Females (N=33) with a normal BMI were grouped as NWO (body fat percent ≥33.0%; n=18) or NWL (body fat percent <33.0; n=15). Following IV insertion and baseline blood collec

The shared genetic architecture between acne vulgaris and inflammatory bowel disease: A cross-trait analysis.

Acne vulgaris (AV) is clinically associated with inflammatory bowel disease, yet the biological mechanisms driving this association remain unclear. We investigated the shared genetic architecture of AV and inflammatory bowel disease (including ulcerative colitis and Crohn's disease) using data from GWASs. Although AV was genetically weakly correlated with inflammatory bowel disease, MiXeR analyses estimated that 28.2% of the causal variants for inflammatory bowel disease are also causal for AV, with distinct overlap for ulcerative colitis and Crohn's disease. Using the pleiotropic analysis und

Plumieride ameliorates type 2 diabetes and colitis by regulating intestinal functions.

ETHNOPHARMACOLOGICAL RELEVANCE: Plumeria rubra L. has been traditionally used to alleviate symptoms of intestinal disorders. Plumieride (PR), a major iridoid glycoside isolated from Plumeria rubra L., possesses a rare spirocyclic structure featuring a 5/5-spiro junction annulated to a six-membered ring. This unique structure sets it apart within the iridoid group and is linked to a variety of pharmacological activities. However, there is limited research on the mechanisms underlying PR's activity. AIM OF THE STUDY: Our work aims at elucidating PR's therapeutic effects and the mechanisms media
