Development and validation of a novel intestinal ultrasound score for predicting endoscopic activity of ileocecal ulcers in intestinal Behçet's disease.

BACKGROUND: No diagnostic criteria have been established to specifically evaluate the activity of intestinal lesions in Behçet's disease (BD). We aimed to identify intestinal ultrasound (IUS) parameters that were correlated with endoscopic ulcer activity, as well as develop and prospectively validate an IUS activity score for patients with intestinal BD. METHODS: Patients who underwent colonoscopy and IUS within 2 weeks during 2007-2019 were retrospectively included in the development phase. Correlations between corresponding endoscopic activity and seven IUS parameters [bowel wall thickness

Tumour-Associated MUC1 Exerts Multiple Effects on Cholesterol and Lipid Metabolism-A Potential Pathogenic Effector of Atherosclerosis in Cancer.

(1) Cancer has been shown to contribute to the progression of atherosclerosis, while inflammatory aspects of atherosclerosis can exert profound effects on cancer development and outcomes. TA-MUC1 (Tumour-associated Mucin 1) is a transmembrane glycoprotein that is overexpressed in many human epithelial cancers lining the intestine. Interestingly, the lack of intestinal MUC1 has been shown to impair cholesterol uptake in MUC1-/- mice. (2) To investigate whether TA-MUC1 could have specific effects on cholesterol metabolism and, thereby, have the potential of impacting the pathogenesis of atherosc

Bacteroides in Bile Detected by Metagenomic Next-Generation Sequencing: Potential Novel Indicator for Early Allograft Dysfunction After Liver Transplantation.

OBJECTIVE: Metagenomic next-generation sequencing (mNGS) is an effective method for the detection of microorganisms. Early allograft dysfunction (EAD) is a common complication after liver transplantation. The association between early postoperative microorganisms in bile and EAD is unclear, so we evaluated the association of microorganisms and other potential risk factors with EAD. METHODS: A total of 100 patients who underwent orthotopic liver transplantation with biliary T tube placement in Huashan Hospital Fudan University from March 2021 to July 2022 were studied. Clinical data, the occur

GPR15-guided CD8+ T regulatory cells control intestinal inflammation.

Inflammatory bowel disease (IBD) causes chronic suffering from gastrointestinal inflammation and dysfunction that can progress to colon cancer1,2. The prevalence of the disease is increasing, and there is an urgent need to better understand its pathogenic mechanisms to improve treatment. We show that GPR15-a G-protein-coupled receptor expressed in immune cells and described previously as an entry co-factor for human and simian immunodeficiency viruses3-is a marker and homing receptor for a subset of intramucosal GPR15-guided regulatory CD8+ T lymphocytes (CD8+ TIGR cells). Deleterious GPR15 ge

Consensus Statement on the Timing of Median Arcuate Ligament Release in Patients With Pancreaticoduodenal or Gastroduodenal Artery Aneurysms Associated With Median Arcuate Ligament Syndrome.

INTRODUCTION: Pancreaticoduodenal and gastroduodenal artery aneurysms (PDAA and GDAA) are rare but life-threatening vascular lesions. Many are associated with median arcuate ligament syndrome (MALS), a condition associated with increased retrograde collateral flow due to celiac artery compression. Although endovascular treatment is the first-line approach for PDAA and GDAA, the role and timing of median arcuate ligament (MAL) release remain unclear. METHODS: This consensus statement was developed through the Anatomy on the Border Expert Consensus Meeting, organized by the Japanese Society for

Environmental pollutants as vascular poisons: rethinking mesenteric ischemia through the lens of heavy metal toxicity.

Mesenteric ischemia is a potentially deadly condition that occurs due to the lack of blood supply to the intestine. There is also emerging evidence that the mesenteric circulation may result in vascular dysfunction and ischemic damage caused by environmental contaminants especially heavy metals such as lead and mercury. Persistent exposure leads to endothelial dysfunction through the generation of oxidative stress, nitric oxide depletion, mitochondrial dysfunction and inflammatory signaling pathways. This hypothesis-generating narrative review integrates toxicological and vascular evidence to

Behçet-like intestinal disease in a patient with trisomy 8-associated myelodysplastic syndrome (TRIAD).

A woman in her late 60s from South India presented with a year-long history of recurrent oral and genital ulcers, abdominal pain and low-grade fever. Evaluation showed ileocaecal ulcers, cytopenias and Trisomy 8-positive myelodysplastic syndrome (MDS). The constellation of mucocutaneous and gastrointestinal Behçet-like features with clonal haematopoiesis led to the diagnosis of Trisomy 8-Associated Autoinflammatory Disease (TRIAD), also known as Behçet-like intestinal disease with MDS. She initially responded to corticosteroids and azacitidine, but relapsed whenever steroids were tapered below

An oral, liver-restricted LXR inverse agonist for dyslipidemia: preclinical development and phase 1 trial.

Despite advances in lipid-lowering treatment, atherosclerotic cardiovascular disease remains the leading cause of mortality, underscoring the need to address residual risk. Targeting both the synthesis and clearance of triglyceride (TG)-rich lipoproteins is a promising approach. Liver X receptor (LXR) repression can reduce plasma TG and cholesterol and improve insulin sensitivity by suppressing de novo lipogenesis and intestinal lipid absorption and enhancing clearance of TG-rich lipoproteins, but its clinical utility remains unexplored. Here we demonstrate the role of LXR inverse agonists in

Extra-Virgin Olive Oil Phenolics in IBD-Associated Vascular Risk.

High-phenolic extra-virgin olive oil (EVOO) is a chemically dynamic bioactive matrix in which cultivar, ripening stage, processing, storage, and digestion shape the final profile of phenolic alcohols and secoiridoids. In inflammatory bowel disease (IBD), chronic intestinal inflammation is associated with barrier dysfunction, dysbiosis, systemic immune activation, endothelial injury, platelet hyperreactivity, and increased cardiovascular risk. This narrative review evaluates whether EVOO phenolics may intersect the gut-endothelium-platelet axis linking IBD to vascular and thromboinflammatory co

Morchella polysaccharide ameliorates myocardial infarction inflammation in mice through increasing gut microbiota-derived 12-HEPE.

Myocardial infarction (MI) initiates robust inflammatory responses. While moderate inflammation facilitates the clearance of necrotic debris, sustained inflammation promotes fibrosis and exacerbates MI progression. Fungal polysaccharides have shown significant anti-inflammatory activity, but role of Morchella polysaccharide (MCP) in modulating MI-associated inflammation remains unclear. In this study we investigated whether MCP ameliorated myocardial infarction injury and elucidated the underlying mechanisms focusing on the gut microbiota and associated metabolites. MI mice were established in

Successful treatment of non-occlusive mesenteric ischemia in a patient undergoing combined therapy with peritoneal dialysis and hemodialysis: a case report.

A Japanese man in his 60 s with end-stage kidney disease due to diabetic nephropathy, who initially underwent peritoneal dialysis (PD) and was transferred to combined PD and hemodialysis (HD) therapy 4 years prior, was admitted to our hospital for surgical treatment of lumbar spinal canal stenosis. His rehabilitation continued well after successful surgery; however, he suddenly presented with abdominal pain and cloudy peritoneal dialysate, along with an increased white blood cell count in the peritoneal dialysate effluent, suggesting PD-associated peritonitis. Intraperitoneal administration of

The Aging Microenvironment Shapes Angiogenic Remodeling in IBD-Associated Colorectal Carcinogenesis.

Chronic intestinal inflammation establishes a pro-senescent microenvironment that fuels the stepwise evolution from inflammatory bowel disease (IBD) to colorectal cancer. Although cellular senescence initially functions as a tumor-suppressive barrier, the persistent accumulation of senescent cells can promote disease progression through the senescence-associated secretory phenotype (SASP). Key SASP mediators, including VEGF, IL-8/CXCL8, and matrix metalloproteinases (MMPs), reprogram endothelial and stromal compartments, driving aberrant neovascularization, vascular leakiness, extracellular-ma

Mechanisms of fibrotic tissue remodelling: insights from systemic sclerosis.

Systemic sclerosis (SSc) is a prototypical systemic immune-mediated fibrosing disease that affects the skin, the lungs, the heart, the kidneys and the intestinal tract. Similar to many other fibrotic diseases, SSc is associated with high morbidity and mortality and therapeutic options are limited. Fibrosis arises from a complex interplay of vascular damage, inflammation and prolonged, misdirected repair responses. The progressive accumulation of extracellular matrix perturbs the physiological tissue architecture and commonly leads to failure of the affected organs. Understanding the mechanisms

Aortic graft invasion into duodenum as an atypical pathergy phenomenon in vascular Behçet's disease.

Behçet's disease (BD) is a multisystemic inflammatory disorder that can affect vessels of all sizes, often leading to significant vascular morbidity. We present the case of a 37-year-old man with BD who developed a rare and severe postoperative complication involving vascular graft invasion into the duodenum. The patient initially underwent emergency aortobiiliac bypass surgery for a ruptured abdominal aortic aneurysm. Subsequent evaluation revealed clinical features consistent with BD, and immunosuppressive treatment with corticosteroids and azathioprine was initiated; however, adherence was

Molecular determinants of thromboinflammatory activation in inflammatory bowel disease.

Inflammatory bowel disease (IBD) is consistently associated with an increased risk of venous thromboembolism, particularly during active disease and hospitalization, yet thrombosis in this context extends beyond a transient inflammatory complication. Emerging evidence supports a unifying thromboinflammatory framework in which chronic intestinal inflammation promotes systemic vascular activation. Endothelial dysfunction represents a central interface in this process and is characterized by reduced nitric oxide bioavailability, increased expression of adhesion molecules, angiogenic remodeling, a

Histologic signatures as predictors of intestinal barrier impairment in inflammatory bowel disease: integration with endoscopic and molecular markers.

BACKGROUND AND AIMS: Intestinal barrier impairment (BI) is a hallmark of inflammatory bowel disease (IBD), driving immune activation, neutrophil-mediated injury, and chronic mucosal damage. While endoscopy and molecular profiling provide insight into epithelial and vascular assessment, the combined prognostic role of histology and BI remains insufficiently defined. This study aimed to determine the association between histology-derived signatures and BI, and identify outcome predictors by integrating histologic, advanced imaging, and molecular data. METHODS: IBD patients underwent in vivo epi

Rupture Predictors and Clinical Outcomes in Jejunal Artery Aneurysms: A Literature Case Series Review.

INTRODUCTION: Jejunal artery aneurysms (JAAs) account for 1% of all visceral artery aneurysms (VAAs). Fewer than 100 cases have been reported in the English literature, rupture rates approach 60%, compared with 10%-20% for other VAAs. Their rupture risk and management remain poorly defined. METHODS: We reviewed the English literature from 1944 to June 2025 and identified 44 cases of JAAs with analyzable data. Primary objective was to explore predictors of rupture; secondary objective was management. Given the rarity and heterogeneity of reports, statistical analyses were exploratory. To the b
