Association of pre-drainage bilirubin levels and pathological features with survival in patients undergoing preoperative biliary drainage: a single-center retrospective study.

BACKGROUND: In clinical practice, bilirubin levels are recognized as a crucial biochemical indicator closely associated with hepatobiliary function and the severity of biliary obstruction, and there is growing interest in exploring the potential correlation between varying preoperative bilirubin levels and clinical outcomes in patients with obstructive jaundice. Obstructive jaundice, characterized by impaired bile excretion and subsequent bilirubin accumulation, poses significant challenges to clinical treatment due to its heterogeneous etiologies and variable patient responses to intervention

MFG-E8-Derived Oligopeptide MOP3 Facilitates Anti-Inflammatory M2-like Macrophage Polarization in Gut Ischemia/Reperfusion.

Gut ischemia/reperfusion (I/R) injury releases damage-associated molecular patterns (DAMPs), such as extracellular cold-inducible RNA-binding protein (eCIRP). Milk fat globule-epidermal growth factor VIII-derived oligopeptide 3 (MOP3) is a novel peptide enabling macrophage uptake of eCIRP via αvβ3-integrin. MOP3 reduces inflammation in gut I/R, but its mechanisms are not completely understood. We hypothesized MOP3 promotes macrophage polarization toward an anti-inflammatory, M2-like phenotype in gut I/R. We induced gut I/R in mice through 60 min of superior mesenteric artery occlusion followed

Gut microbiota-derived butyrate contributes to baicalin-induced attenuation of hypertensive vascular remodeling via adventitial immunity.

BACKGROUND: Baicalin shows potent vasculoprotective effects against hypertension despite poor oral bioavailability. We investigated whether gut microbiota modulation contributes to the systemic vasculoprotective effects of orally administered baicalin. METHODS: We utilized an Angiotensin II-induced hypertensive mouse model, employing broad-spectrum antibiotics, 16S rRNA sequencing, metabolomics, and in vitro co-culture assays to map the gut-immune-vascular axis. RESULTS: Oral Baicalin significantly attenuated Ang II-induced blood pressure elevation and improved the intestinal barrier integri

Systemic sclerosis and small bowel involvement: A systematic review of diagnostic approaches and current evidence.

BACKGROUND: Small bowel involvement represents a common and clinically significant manifestation of systemic sclerosis (SSc). Pathogenesis is multifactorial, and diagnosis remains challenging due to non-specific symptoms, limited diagnostic tools, and a lack of standardized management strategies. METHODS: A systematic search of PubMed and Embase was conducted to identify studies evaluating small bowel involvement in patients with SSc using objective tests and patient tissue. A qualitative synthesis was performed due to the anticipated heterogeneity in study designs, interventions, and outcome

Metabolic reprogramming in chronic kidney disease-cardiovascular disease comorbidity: from molecular mechanisms to therapeutic strategies.

Chronic kidney disease (CKD) and cardiovascular disease are bidirectionally linked, with metabolic reprogramming as a central pathophysiological nexus. This review examines metabolic disturbances underlying CKD-CVD comorbidity, including energy metabolism disorders, mitochondrial dysfunction, lipid and glucose dysregulation, and mineral metabolism abnormalities. The gut microbiota-metabolic axis contributes through depletion of short-chain fatty acids, accumulation of trimethylamine N-oxide, and production of uremic toxins. These changes compromise intestinal barrier integrity and sustain syst

PANoptosis links gut dysbiosis to obstructive sleep apnea-associated atherosclerosis: a gut-vascular inflammatory axis.

Obstructive sleep apnea (OSA) is increasingly recognized as an independent contributor to atherosclerotic cardiovascular disease, yet the mechanisms linking nocturnal intermittent hypoxia (IH) to plaque progression remain incompletely understood. Beyond oxidative stress and sympathetic activation, emerging evidence suggests that the gut may function as a critical relay organ between OSA and vascular injury. IH reshapes the intestinal ecosystem by altering microbial composition, weakening epithelial and mucus barrier integrity, and remodeling microbial metabolites, thereby increasing systemic e

Transient receptor potential vanilloid channels in inflammatory bowel diseases: linking inflammation, angiogenesis, and oxidative stress.

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is characterized by chronic relapsing intestinal inflammation driven by complex interactions among immune dysregulation, epithelial barrier disruption, vascular dysfunction, oxidative stress, and gut microbiota imbalance. Despite advances in biologic and small-molecule therapies, a substantial proportion of patients remain refractory to treatment or experience significant adverse effects, underscoring the need for novel mechanistic insights and therapeutic targets. Emerging evidence suggests that the

Aging-driven metabolic abnormalities remodel intercellular communication through the gut-liver-heart axis and may promote coronary artery disease: the key role of bile acid metabolism.

Coronary artery disease (CAD) remains the leading cause of cardiovascular mortality worldwide and shows a strong age-dependence that classical risk-factor models do not fully explain. A growing body of work indicates that aging is closely associated with CAD and, in preclinical models, can promote it through immunometabolic remodeling of the gut-liver-heart axis, in which bile acid metabolism is proposed to act as a central molecular link. Here we integrate cellular, molecular, and clinical evidence to outline how aging perturbs this axis and sustains chronic vascular inflammation. At the cell

Venous Thromboembolism and Gut Dysbiosis: Mechanistic Links Between Endotoxemia, Microbial Metabolites, and Thromboinflammation.

Background: Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, is increasingly recognized as a thromboinflammatory disorder involving coagulation, innate immunity, endothelial dysfunction, and vascular homeostasis. Emerging evidence suggests that gut microbiome-related inflammatory and metabolic signals may influence pathways potentially relevant to VTE through intestinal barrier dysfunction, microbial translocation, and microbiome-derived metabolites. This review critically examines the direct and indirect evidence relating gut dysbiosis to mechanisms potenti

Interpreting MALDI imaging data for rare types of ampullary cancer using machine learning.

Rare tumor diseases are difficult to diagnose and there is a lack of routine diagnostic procedures. Approaches must be found that allow comprehensive identification and evaluation of prognostic relevant target proteins or transcripts. Analyzing rare ampullary cancer, respectively, to their prognosis and predictive factors by machine learning (ML) based matrix-assisted laser desorption/ionization (MALDI) time-of-flight (TOF) imaging is a first step towards providing new solutions for diagnostics of those cancer samples. In this study, we investigated a cohort of ampullary adenocarcinomas, inclu

Non-selective β-blockers reduce bacterial translocation by restoring gut barrier function during experimental cirrhosis.

BACKGROUND & AIMS: Intestinal barriers limiting gut-to-liver translocation of bacteria/l products are disrupted in cirrhosis. Cirrhosis is characterised by sustained β-adrenergic overactivity, which may contribute to barrier dysfunction but, its impact on the gut-liver-axis remains unclear. We investigated whether β-adrenergic signalling drives barrier disruption in experimental cirrhosis being amenable for treatment by non-selective beta blockers such as propranolol. METHODS: Cirrhosis was induced in rats (CCl4, BDL) and mice (BDL), followed by propranolol or vehicle treatment. Chronic β-adr

Glaucocalyxin A enhances colonic anastomotic healing after ischemia-reperfusion: experimental findings.

PURPOSE: Glaucocalyxin A (GLA), a diterpenoid with antioxidative and anti-inflammatory properties, has shown vascular protective effects in preclinical models. This study aimed to evaluate whether GLA improves colonic anastomotic integrity following ischemia-reperfusion (I/R) injury. METHODS: Twenty-four female Sprague Dawley rats were randomized into three groups (n = 8): control + GLA, I/R, and I/R + GLA. I/R was induced by 60-minute superior mesenteric artery occlusion and 60-minute reperfusion before end-to-end colonic anastomosis. GLA (10 mg/kg) was administered intraperitoneally. On pos

Human inflammatory bowel disease-on-a-chip for modelling disease progression, cancer initiation and sex-specific effects.

Patients affected by inflammatory bowel disease (IBD) exhibit compromised intestinal barrier function and decreased mucus accumulation, as well as increased inflammation, fibrosis and cancer risk, with symptoms often being exacerbated in women during pregnancy. Here we replicate these IBD hallmarks in human-derived organ-on-a-chip devices lined by colon epithelial cells from individuals with IBD when interfaced with matched fibroblasts, cultured under flow, exposed to peristalsis-like motions and perfused with circulating immune cells. Use of heterotypic tissue recombinants revealed that IBD f

Cannabinoid receptor-mediated modulation of the gut-brain axis under chronic stress: Neuroinflammatory mechanisms, microbial metabolites, and sexual dimorphism.

Chronic stress disrupts the gut-brain axis (GBA) through coordinated dysfunction of the intestinal barrier and of the vascular interfaces that protect the brain, fostering a neuroinflammatory state implicated in mood disorders, neuropsychiatric conditions, and chronic pain. This narrative review synthesizes evidence across five domains. First, the inflammatory cascade by which stress-induced disruption of the epithelial and gut vascular barriers activates the NLRP3 inflammasome and sustains central neuroinflammation. Second, microbial metabolites, namely short-chain fatty acids (SCFAs), trypto

Gastrointestinal involvement in Ehlers-Danlos syndrome classical-like type 2 associated with a novel AEBP1 splice-site variant.

Ehlers-Danlos syndrome classical-like type 2 (clEDS2) is a rare autosomal recessive connective tissue disorder caused by biallelic loss-of-function variants in the gene encoding adipocyte enhancer-binding protein 1 (AEBP1). While cutaneous and skeletal manifestations are commonly observed, gastrointestinal complications, including bowel rupture, have been reported only rarely, and their histopathological basis remains poorly characterized. Here, we report findings of molecular investigations, gastrointestinal histopathological evaluation, and long-term clinical follow-up in the 16th reported p

[When the intestine swallows the pounds].

The case of an 88-year-old male patient with progressive, unintentional weight loss is reported. The condition was subjectively asymptomatic and subsequently completely reversible. Following a comprehensive internal medicine examination, chronic mesenteric ischemia due to arteriosclerosis was identified as the cause and treated interventionally. Over time, the patient experienced significant weight gain, a comparatively rare phenomenon in very old geriatric patients. Broad and targeted investigation of the cause and interventional therapies are safe and feasible even in old age.

The Gastrointestinal Barrier-Mechanisms of Barrier Dysfunction in Liver Cirrhosis and Spontaneous Bacterial Peritonitis.

The gastrointestinal (GI) barrier is a highly coordinated, multilayered defence system that maintains intestinal homeostasis by separating the luminal microbiota from the internal milieu. In liver cirrhosis, this barrier undergoes profound structural and functional disruption, emerging as a central driver of bacterial translocation and infection-related complications. Among these, spontaneous bacterial peritonitis (SBP) represents a major determinant of morbidity, mortality, and disease progression. Barrier failure in cirrhosis is not attributable to a single defect but results from the conver

Allogeneic haematopoietic stem cell transplantation for refractory perforating intestinal Behçet disease in a patient with aplastic anaemia: A case report.

RATIONALE: Aplastic anemia (AA) rarely coexists with intestinal Behçet disease (iBD), and the post-partum period may further complicate its expression. Managing refractory iBD with recurrent perforation on a background of long-standing AA is challenging, and evidence on optimal treatment is limited. PATIENT CONCERNS: A 42-year-old woman with non-severe aplastic anemia (NSAA) diagnosed in 2005 (AA duration ≈ 20 years) developed recurrent fever, oral ulcers, and penetrating ulcers of the ileocaecum and terminal ileum shortly after cesarean delivery in June 2019, with one episode of vulvar ulcer
