Time-Restricted Feeding Delays Cardiovascular Aging by Increasing Acetic Acid Through Remodeling Gut Microbiota.

Time-restricted feeding (TRF) restricts food intake to daily windows, yet its efficacy against vascular aging and underlying mechanisms remain unclear. Here, we demonstrate that TRF attenuates angiotension II -induced accelerated vascular aging by reshaping gut microbiota, stabilizing microbial diversity, and elevating the Firmicutes/Bacteroidetes ratio. TRF preserved intestinal barrier integrity, upregulated tight junction proteins (Occludin, ZO-1, MUC2), reduced serum lipopolysaccharide, and suppressed systemic inflammation. These benefits translated into alleviated hypertension, aortic stif

Spatial multi-omics-guided drug development in inflammatory bowel disease: from mucosal immune niches to precision therapy.

Crohn's disease and ulcerative colitis are now treated with an expanding set of biologics and small molecules, yet treatment selection remains largely empirical. Primary non-response, secondary loss of response, incomplete mucosal healing, fibrostenosis, and fistulizing disease show that conventional clinical, endoscopic, and bulk molecular markers do not capture how disease is organized within tissue. This review argues that the inflamed intestine is better viewed as a set of spatially structured mucosal immune niches rather than as uniform inflammation, with epithelial, myeloid, lymphoid, st

Integrative analysis of the gut-liver-heart axis in coronary artery disease: From chronic metabolic dysbiosis to acute cardiogenic shock.

The complex interplay between chronic metabolic disorders and acute cardiovascular decompensation represents a critical frontier in cardiology. This integrative research program conducted at Chongqing Medical University systematically examines the gut-liver-heart axis across the cardiovascular disease spectrum through three interconnected studies. First, a case-control investigation of coronary artery disease (CAD) patients with and without nonalcoholic fatty liver disease (NAFLD) revealed distinct intestinal dysbiosis patterns characterized by increased Coprococcus and Veillonella alongside d

Optimized Graph Structures for Calibrating Graph Neural Networks with Out-of-Distribution Nodes.

BACKGROUND: Bone autograft options may be limited in revision spinal fusion cases. Reconstruction of the iliac bone graft (IBG) donor site with cancellous bone allograft allows for reharvest for patients who subsequently have another fusion. This study examined viability of the reconstructed IBG (RIBG) donor sites. Secondarily, we assessed the feasibility of whether the RIBG sites could be reharvested for obtaining a successful arthrodesis for a secondary fusion. METHODS: Prospectively collected data of 154 consecutive lumbar revision fusions were reviewed, of which 17 cases had their IBG don

Association between frailty and outcomes in emergency department patients requiring emergency general surgery: a retrospective cohort study.

BACKGROUND: Frailty, a state of increased vulnerability, is a well-established risk factor of adverse outcomes following surgery. While its prognostic value has been demonstrated in various contexts, data are lacking on its impact in emergency general surgery within the Swiss healthcare system. This study aimed to evaluate the association between frailty and outcomes among older patients undergoing emergency general surgery in a Swiss tertiary hospital. METHODS: We conducted a retrospective cohort study including patients aged 65 years or over who underwent emergency general surgery between J

Scurvy in a Patient with Crohn's Disease: A Case Report.

INTRODUCTION/BACKGROUND: Crohn's disease is a chronic autoimmune bowel disease that typically causes inflammation of the lining or wall of the small and large intestines as well as the entire gastrointestinal tract. Patients diagnosed with Crohn's disease can develop oral ulcers, which are also a symptom of vitamin C deficiency, also known as scurvy. Patients with Crohn's disease are prone to experiencing nutritional deficiencies, including vitamin C deficiency due to malabsorption. CASE PRESENTATION: In this case report, we describe a very interesting presentation of scurvy in the presence o

Intestinal blood vessel-associated macrophages and gut-vascular barrier dysfunction in cirrhosis.

BACKGROUND: Bacterial translocation in cirrhosis can trigger infection and hepatic decompensation, leading to systemic inflammation, organ failure and increased mortality. These infections often originate from the gastrointestinal tract after bacteria breach the intestinal barrier and disseminate to systemic sites. OBJECTIVE: In this study, we explore the mechanisms underlying intestinal barrier dysfunction in cirrhosis using an experimental cirrhosis model and patient-derived intestinal biopsies. DESIGN: We developed a murine model of cirrhosis through chronic administration of carbon tetra

Gut microbiota and atherosclerosis.

Atherosclerosis reflects a chronic inflammatory process of arteries. The origin of chronic vascular inflammation has been associated over a long time primarily with lipid disorders, but evidence from the past years has suggested that lipid-independent pathways are also involved. Recent research has demonstrated that the gastrointestinal microbiota has an impact on the development of atherosclerosis. Many clinical studies have revealed that there exist altered gut microbiota and increased intestinal abundance of bacteria from the oral cavity in atherosclerosis-related disorders such as cardiova

Dynamic changes to the intestinal environment occur throughout recovery from experimental ischemic stroke.

Stroke survivors can experience a plethora of complications throughout their recovery that impair quality of life and impact neurological outcomes. Intestinal dysfunction is reported to occur rapidly following stroke in both humans and animal models and alterations such as reduced barrier integrity, lymphocyte loss and an altered microbiota have been suggested to contribute to increased brain injury. Despite the persistence of gastrointestinal symptoms in many stroke survivors, how the intestinal environment changes over the course of stroke recovery remains poorly understood. Here, we use an

Phocaeicola coprophilus-Derived 6-Methyluracil Attenuates Radiation-Induced Intestinal Fibrosis by Suppressing the IDO1-Kynurenine-AHR Axis.

Therapeutic options for radiation-induced intestinal fibrosis (RIF) remain limited. This study reveals that intestinal kynurenine (Kyn) is persistently elevated after radiation and correlates with fibrosis severity in both murine models and human rectal cancer samples. Exogenous Kyn exacerbated RIF, whereas inhibition of indoleamine 2,3-dioxygenase 1 (IDO1) attenuated fibrotic progression. Mechanistically, Kyn activates the aryl hydrocarbon receptor (AHR) to promote fibroblast activation and fibrosis. Antibiotic depletion of gut microbiota abrogates radiation-induced IDO1-Kyn upregulation and

Fecal microbiota transplantation promotes Wnt3a-mediated hippocampal neurogenesis in a rat model of chronic cerebral hypoperfusion.

BACKGROUND: Limited data support the beneficial effects of fecal microbiota transplantation (FMT) against intracranial ischemic injury under chronic cerebral hypoperfusion (CCH). However, a comprehensive understanding is lacking, hindering its clinical translation. In the present study, we evaluated microbial, metabolic, cellular, and behavioral alterations to explore the roles and mechanisms of FMT in hippocampal neurogenesis under CCH. METHODS: Rats underwent bilateral common carotid artery occlusion to induce CCH. Intestinal microbiota (IM) and fecal/hippocampal metabolites were assessed b

Effects of Phytochemicals on Atherosclerosis: Based on the Gut-Liver Axis.

Background: Atherosclerosis (AS) is the primary pathological basis for cardiovascular and cerebrovascular events, with its development closely linked to dyslipidemia and chronic inflammation. The gut-liver axis, serving as a core bridge connecting gut microbiota, hepatic metabolism, and systemic inflammation, has gained increasing prominence in AS pathogenesis. Phytochemicals exhibit multifaceted biological activities, yet their mechanisms for preventing and treating AS via the gut-liver axis remain to be systematically summarized. This review aims to summarize the potential mechanisms of phyt

Obesity-induced cognitive impairment: Underlying mechanisms and therapeutic prospects.

The prevalence of obesity continues to rise worldwide. Obesity is not only a core risk factor for chronic metabolic diseases, but also significantly associated with the risk of cognitive impairment. Obesity in middle age can cause aprosexia, cognitive disorder, dementia, hypomnesis, and increase the risk of execution decline, depression and anxiety. The underlying mechanisms involve multiple pathological pathways such as neuroinflammation, imbalance of gut microbiota-gut-brain axis, metabolic abnormalities, and imbalance of adipokines. Obesity induced chronic inflammation impairs neuroprotecti

An autopsy case of extensive aortic thrombosis with acute kidney injury as the sole clinical manifestation.

Aortic thrombosis is a rare but life-threatening condition. It typically presents with abdominal or lower-limb ischemia. Although acute kidney injury (AKI) can accompany aortic thrombosis, AKI as the sole clinical manifestation has not been previously reported. A 69-year-old man with extensive atherosclerotic disease and chronic kidney disease presented with severe oliguria. Initial evaluation, including non-contrast computed tomography (CT), did not reveal any aortic abnormalities. Renal Doppler ultrasonography findings and markedly elevated D-dimer levels prompted contrast-enhanced CT, which

Identifying distinct clinical phenotypes and outcomes in adult-onset IgA vasculitis using unsupervised clustering analysis.

OBJECTIVE: IgA vasculitis (IgAV) is a small-vessel vasculitis. The diversity of clinical presentations and outcomes requires the identification of groups with a distinct phenotype for management and follow-up. METHODS: The French retrospective IGAVAS database (n = 260) includes adult-onset IgAV. Agglomerative hierarchical clustering was performed, using k-means and Ward's method, based on 7 parameters (sex, age, constitutional symptoms, skin necrosis, joint and digestive involvement, abnormal kidney function). These results were validated on an independent cohort of Slovenian patients (n = 20

Loss of Myofilaments in Gastrointestinal Smooth Muscle: A Novel Pathological Finding in MELAS-Associated Chronic Intestinal Pseudo-Obstruction.

BACKGROUND AND AIMS: Chronic intestinal pseudo-obstruction (CIPO) occurs in up to 40% of patients with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes (MELAS). While enteric nervous system abnormalities have been documented, 15%-38% of CIPO cases show normal histology, suggesting alternative pathogenic mechanisms. We aimed to investigate smooth muscle pathology in MELAS-associated CIPO. METHODS: Comprehensive pathological examination including light and electron microscopy was performed on autopsy material from a 52-year-old male with MELAS (m.3243A > G mutat

Triple-emulsion microfluidic Core-Shell hydrogel microcapsules for oral pentoxifylline Delivery: Ameliorating colitis and rebalancing gut microbiome.

Inflammatory bowel disease (IBD) encompasses chronic or relapsing inflammation within different regions of the gastrointestinal tract. Pentoxifylline (PTX), a methylxanthine derivative primarily used to improve blood flow in peripheral vascular diseases, has demonstrated anti-inflammatory and immunomodulatory properties, suggesting its potential in attenuating IBD-associated inflammation. However, its clinical application in IBD remains limited, partly due to its short half-life and poor targeting to inflamed intestinal tissues, necessitating strategies to enhance its bioavailability and tissu

Neuropeptide-GPCR Regulation of the Neuroimmune Axis in Neurodegeneration: Mechanisms and Translation.

Chronic neuroinflammation and dysfunction of the neuro-glial-vascular unit (NGVU) are central mechanisms driving the progression of neurodegenerative diseases such as Alzheimer's and Parkinson's disease. Neuropeptides, as key regulatory signaling molecules in the central nervous system (CNS), bind to specific G protein-coupled receptors (GPCRs) on the surfaces of microglia, astrocytes, oligodendrocytes, and cerebrovascular elements. Through cell type-specific biased signaling, they precisely regulate the threshold for inflammatory activation, coordinate phagocytosis and autophagy, maintain met
