Mycotic Aneurysms in Intestinal Transplantation: A Case Series.

INTRODUCTION: Mycotic aneurysms (MA) are a potentially fatal complication following intestinal transplantation (ITx). Their occurrence arises from several factors, such as chronic abdominal sepsis, potential for contamination of the vascular grafts during enteric anastomoses and the heightened burden of immunosuppression. METHODS: A retrospective review of a prospectively maintained database of all adult ITx from December 2007 to June 2025 was conducted. RESULTS: Among 168 ITx, six patients (3.6%) experienced eight MA episodes. MA occurred bimodally, early post-transplant (2-8 weeks) or in a

Sleep Deprivation Exacerbates Ischemic Stroke Outcomes via Akkermansia Depletion and Metabolic Dysregulation.

AIMS: Sleep disturbances are a potential risk factor for stroke. Our clinical cohort shows that insomnia is associated with a higher 90-day stroke recurrence rate, prompting an investigation into the underlying mechanisms, particularly the role of the gut microbiota. METHODS: We utilized a rat MCAO model of sleep deprivation (SD) combined with antibiotic (ABX)-induced gut dysbiosis. Neurological deficits were assessed through behavioral tests, and cerebral infarct volume was measured using TTC staining. Structural changes in brain and intestinal tissues were observed by H&E staining, while th

[Angioedema 2025 - Advancements in Diagnosis and Treatment].

Angioedema is a pathophysiologic process characterized by increased vascular endothelial hyperpermeability, leading to fluid accumulation and tissue swelling. This process is reversible in most cases; therefore, it is clinically exhibited by exacerbations and remissions. Patients experience paroxysmal, unpredictable, and slowly progressing episodes of swelling of the face, extremities, genitals, tongue, and larynx, and severe abdominal pains. Attacks affecting the oropharyngeal and upper airways impose a considerable life-threatening risk. Intestinal wall edema can cause bowel obstruction, mim

The Gut-Oral-Skin-Heart Microbiota Axis and Oxidative Stress: Where Have We Been and Where Are We Going.

Scientific interest in the relationship between cardiovascular disease and the human microbiome has grown substantially in recent years, with particular emphasis placed on the gut microbiota. Emerging evidence suggests, however, that distinct microbial niches-including the skin, oral cavity, and gut-may interact to influence cardiovascular homeostasis through shared pathogenic pathways, most notably oxidative stress and systemic inflammation. Intestinal dysbiosis has been strongly linked to the increased production of microbial metabolites such as trimethylamine N-oxide, which promotes endothe

The gut-lung axis in ARDS: beyond microbial translocation.

The gut-lung axis has emerged as a pivotal pathway in the pathogenesis of acute lung injury (ALI) and acute respiratory distress syndrome (ARDS). Disruption of the intestinal barrier, a common event in critical illness, facilitates the systemic dissemination of live microbiota, their pathogen-associated molecular patterns (PAMPs), and bioactive metabolites. This process critically depends on the integrity of the gut vascular barrier (GVB). The GVB is the endothelial layer underlying the gut epithelium. It serves as the final gatekeeper, restricting microbial products from entering the systemic

Gut microbiota dysbiosis-induced chronic inflammation as a driver of atherosclerosis: cellular crosstalk and host-microbe interactions.

Gut microbiota dysbiosis is increasingly recognized as an upstream contributor to chronic low-grade inflammation and atherosclerosis (AS). Disruption of microbial homeostasis may impair intestinal barrier integrity, increase exposure to pro-inflammatory microbial products and metabolites, and reduce protective metabolites such as short-chain fatty acids (SCFAs), thereby activating innate immune signaling and sustaining vascular inflammation. Current evidence indicates that gut dysbiosis promotes atherosclerosis mainly through three interconnected processes: metabolite imbalance, barrier dysfun

Hepatotoxic mechanisms of functionalized nanopolystyrene: decoding the role of ionic surface groups.

BACKGROUND: With annual global plastic production exceeding 400 million tons, nanoscale polystyrene particles (nPS) have become a major health concern due to their bioaccumulation capacity and ability to cross biological barriers. Surface-charged nPS variants (cationic, anionic, and neutral) show distinct biodistribution patterns, yet the mechanisms underlying their systemic damage remain incompletely understood. This study aimed to investigate the systemic injury mechanisms of nPS with different surface charges. METHODS: Mice were exposed to fluorescently labeled cationic (amino-modified), a

Hypertension-associated gut dysbiosis drives target organ damage through impaired polyunsaturated fatty acids metabolism and immune activation.

Hypertension (HTN) is a major risk factor for cardiovascular diseases, with chronic low-grade inflammation emerging as a critical contributor to its development and target organ damage. Emerging evidence implies gut microbiota in blood pressure regulation. However, the long-term impact of patient-derived gut dysbiosis on the chronic progression of HTN remains insufficiently characterized. This study aimed to determine how HTN-associated gut microbiota contributes to sustained blood pressure elevation and target organ damage during long-term colonization, and to elucidate underlying immune-meta

GPR15-guided CD8+ T regulatory cells control intestinal inflammation.

Inflammatory bowel disease (IBD) causes chronic suffering from gastrointestinal inflammation and dysfunction that can progress to colon cancer1,2. The prevalence of the disease is increasing, and there is an urgent need to better understand its pathogenic mechanisms to improve treatment. We show that GPR15-a G-protein-coupled receptor expressed in immune cells and described previously as an entry co-factor for human and simian immunodeficiency viruses3-is a marker and homing receptor for a subset of intramucosal GPR15-guided regulatory CD8+ T lymphocytes (CD8+ TIGR cells). Environmental pollutants as vascular poisons: rethinking mesenteric ischemia through the lens of heavy metal toxicity.

Mesenteric ischemia is a potentially deadly condition that occurs due to the lack of blood supply to the intestine. There is also emerging evidence that the mesenteric circulation may result in vascular dysfunction and ischemic damage caused by environmental contaminants especially heavy metals such as lead and mercury. Persistent exposure leads to endothelial dysfunction through the generation of oxidative stress, nitric oxide depletion, mitochondrial dysfunction and inflammatory signaling pathways. This hypothesis-generating narrative review integrates toxicological and vascular evidence to

Extra-Virgin Olive Oil Phenolics in IBD-Associated Vascular Risk.

High-phenolic extra-virgin olive oil (EVOO) is a chemically dynamic bioactive matrix in which cultivar, ripening stage, processing, storage, and digestion shape the final profile of phenolic alcohols and secoiridoids. In inflammatory bowel disease (IBD), chronic intestinal inflammation is associated with barrier dysfunction, dysbiosis, systemic immune activation, endothelial injury, platelet hyperreactivity, and increased cardiovascular risk. This narrative review evaluates whether EVOO phenolics may intersect the gut-endothelium-platelet axis linking IBD to vascular and thromboinflammatory co

Successful treatment of non-occlusive mesenteric ischemia in a patient undergoing combined therapy with peritoneal dialysis and hemodialysis: a case report.

A Japanese man in his 60 s with end-stage kidney disease due to diabetic nephropathy, who initially underwent peritoneal dialysis (PD) and was transferred to combined PD and hemodialysis (HD) therapy 4 years prior, was admitted to our hospital for surgical treatment of lumbar spinal canal stenosis. His rehabilitation continued well after successful surgery; however, he suddenly presented with abdominal pain and cloudy peritoneal dialysate, along with an increased white blood cell count in the peritoneal dialysate effluent, suggesting PD-associated peritonitis. The Aging Microenvironment Shapes Angiogenic Remodeling in IBD-Associated Colorectal Carcinogenesis.

Chronic intestinal inflammation establishes a pro-senescent microenvironment that fuels the stepwise evolution from inflammatory bowel disease (IBD) to colorectal cancer. Although cellular senescence initially functions as a tumor-suppressive barrier, the persistent accumulation of senescent cells can promote disease progression through the senescence-associated secretory phenotype (SASP). Key SASP mediators, including VEGF, IL-8/CXCL8, and matrix metalloproteinases (MMPs), reprogram endothelial and stromal compartments, driving aberrant neovascularization, vascular leakiness, extracellular-ma

Molecular determinants of thromboinflammatory activation in inflammatory bowel disease.

Inflammatory bowel disease (IBD) is consistently associated with an increased risk of venous thromboembolism, particularly during active disease and hospitalization, yet thrombosis in this context extends beyond a transient inflammatory complication. Emerging evidence supports a unifying thromboinflammatory framework in which chronic intestinal inflammation promotes systemic vascular activation. Endothelial dysfunction represents a central interface in this process and is characterized by reduced nitric oxide bioavailability, increased expression of adhesion molecules, angiogenic remodeling, a

Histologic signatures as predictors of intestinal barrier impairment in inflammatory bowel disease: integration with endoscopic and molecular markers.

BACKGROUND AND AIMS: Intestinal barrier impairment (BI) is a hallmark of inflammatory bowel disease (IBD), driving immune activation, neutrophil-mediated injury, and chronic mucosal damage. While endoscopy and molecular profiling provide insight into epithelial and vascular assessment, the combined prognostic role of histology and BI remains insufficiently defined. This study aimed to determine the association between histology-derived signatures and BI, and identify outcome predictors by integrating histologic, advanced imaging, and molecular data. METHODS: IBD patients underwent in vivo epi

Formulation and characterization of 15d-PGJ2 loaded PLGA nanoparticles and in vitro evaluation of small intestinal submucosa integrated with nanoparticles for diabetic wound.

Diabetic foot ulcers represent chronic and often non-healing wounds. Their treatment is particularly challenging due to persistent inflammation and infection, necessitating the exploration of innovative strategies for more effective therapeutic approaches. This study aimed to develop a biodegradable extracellular matrix-mimicking wound dressing with anti-inflammatory properties designed to control inflammation and support cell proliferation and migration in the wound area. The anti-inflammatory drug prostaglandin J2 (15d-PGJ2) was encapsulated in PLGA nanoparticles (NPs). The average hydrodyna

The Pathophysiological Interrelationship Between Metabolic Dysfunction-Associated Steatotic Liver Disease and Cardiovascular Disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a highly prevalent multisystem disorder and is strongly associated with increased cardiovascular risk. Cardiovascular diseases represent the leading cause of mortality in this population. As the hepatic manifestation of systemic metabolic dysfunction, MASLD is initiated by excess lipid accumulation driven by increased dietary fatty acid intake and accelerated de novo lipogenesis. This triglyceride overload induces lipotoxicity, triggering hepatocellular injury, immune activation, and mitochondrial dysfunction. Excessive mitoch
