Histologic signatures as predictors of intestinal barrier impairment in inflammatory bowel disease: integration with endoscopic and molecular markers.

BACKGROUND AND AIMS: Intestinal barrier impairment (BI) is a hallmark of inflammatory bowel disease (IBD), driving immune activation, neutrophil-mediated injury, and chronic mucosal damage. While endoscopy and molecular profiling provide insight into epithelial and vascular assessment, the combined prognostic role of histology and BI remains insufficiently defined. This study aimed to determine the association between histology-derived signatures and BI, and identify outcome predictors by integrating histologic, advanced imaging, and molecular data. METHODS: IBD patients underwent in vivo epi

Formulation and characterization of 15d-PGJ2 loaded PLGA nanoparticles and in vitro evaluation of small intestinal submucosa integrated with nanoparticles for diabetic wound.

Diabetic foot ulcers represent chronic and often non-healing wounds. Their treatment is particularly challenging due to persistent inflammation and infection, necessitating the exploration of innovative strategies for more effective therapeutic approaches. This study aimed to develop a biodegradable extracellular matrix-mimicking wound dressing with anti-inflammatory properties designed to control inflammation and support cell proliferation and migration in the wound area. The anti-inflammatory drug prostaglandin J2 (15d-PGJ2) was encapsulated in PLGA nanoparticles (NPs). The average hydrodyna

The Pathophysiological Interrelationship Between Metabolic Dysfunction-Associated Steatotic Liver Disease and Cardiovascular Disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a highly prevalent multisystem disorder and is strongly associated with increased cardiovascular risk. Cardiovascular diseases represent the leading cause of mortality in this population. As the hepatic manifestation of systemic metabolic dysfunction, MASLD is initiated by excess lipid accumulation driven by increased dietary fatty acid intake and accelerated de novo lipogenesis. This triglyceride overload induces lipotoxicity, triggering hepatocellular injury, immune activation, and mitochondrial dysfunction. Excessive mitoch

Gut Microbiota and Diabetic Complications: Potential Mechanisms, Microbial Signatures, and Clinical Implications.

Type 2 diabetes mellitus is a systemic metabolic disorder with an extensive spectrum of complications, which still persist despite improvements in glycemic control. Emerging evidence suggests that gut dysbiosis may be an underpinning factor in the pathogenesis of both microvascular and macrovascular complications associated with diabetes. This narrative review explores the relationship between gut microbiota and the development of diabetes complications, including nephropathy, retinopathy, neuropathy, cardiovascular, cerebrovascular, peripheral vascular, and reproductive system disorders. Ischemic Stricture of the Terminal Ileum in the Setting of Superior Mesenteric Artery Stenting: A Case Report.

Chronic mesenteric ischemia is an infrequent but potentially debilitating condition that can result in irreversible intestinal injury when diagnosis or treatment is delayed. Although revascularization can restore mesenteric blood flow and improve symptoms, structural bowel damage acquired during prolonged hypoperfusion may persist or manifest later. Chronic ischemic strictures of the small intestine are rare and diagnostically challenging, particularly when involving the terminal ileum, where they may mimic inflammatory, infectious, or neoplastic disorders. We report the case of a 55-year-old

Integrative single-cell transcriptomics and mendelian randomization identifies BTN3A2 as a shared protective factor in Behçet's disease and inflammatory bowel disease.

BACKGROUND: Behçet's disease (BD) and inflammatory bowel disease (IBD) are immune-mediated chronic inflammatory disorders. Although they exhibit significant overlap in clinical manifestations and immunological features, whether they share molecular targets and pathogenic links remains unclear. This study aimed to explore the possible linking and shared treatment target between these two diseases. METHODS: We analyzed scRNA-seq data from the peripheral blood of BD and IBD patients to delineate the immune cell landscape and identify key subsets via differential proportion analysis. Microbiota-mediated mechanisms of natural products in atherosclerosis: focus on metabolic and inflammatory pathways.

BACKGROUND: Atherosclerosis (AS) is a chronic inflammatory vascular disease characterized by lipid accumulation, endothelial dysfunction, immune dysregulation, and plaque formation. Beyond conventional lipid-related mechanisms, gut microbiota dysbiosis and microbiota-derived metabolites have emerged as important regulators of atherogenesis. Natural products, including polyphenols, flavonoids, alkaloids, fatty acids, polysaccharides, saponins, and terpenoids, may modulate AS by reshaping gut microbial ecology and metabolic outputs. METHODS: This narrative review qualitatively synthesized Engli

Large vessel vasculitis combined with inflammatory bowel disease: epidemiology, shared pathogenesis, and therapeutic advances.

Concomitant existence of large vessel vasculitis (LVV) and inflammatory bowel disease (IBD) is rare but has gained increasing clinical and research attention. LVV, comprising Takayasu arteritis (TAK) and giant cell arteritis (GCA), is characterized by granulomatous inflammation of the aorta and its branches. Accumulating evidence demonstrates a substantial overlap between LVV and IBD in epidemiological patterns, genetic susceptibility loci, immune cell subsets, and proinflammatory cytokine networks, supporting shared immunopathogenic mechanisms. IBD, including Crohn's disease (CD) and ulcerati

Mesenteric denervation ameliorates post‑infarction heart failure alongside alterations in the gut-nerve-microbiota axis.

Sympathetic overactivity and gut microbiota dysbiosis drive chronic heart failure (CHF) progression. This study explored the associations of mesenteric denervation (MDN) with cardioprotection and potential involvement of the nerve-microbiota-heart axis in MI-induced CHF mice. C57BL/6 mice were randomized into control, CHF, and CHF_MDN groups. CHF was induced by LAD ligation, and MDN was performed via phenol ablation of the superior mesenteric artery. MDN was associated with suppressed mesenteric sympathetic activity, improved left ventricular ejection fraction, and reduced myocardial fibrosis

Gut microbiota-derived butyrate contributes to baicalin-induced attenuation of hypertensive vascular remodeling via adventitial immunity.

BACKGROUND: Baicalin shows potent vasculoprotective effects against hypertension despite poor oral bioavailability. We investigated whether gut microbiota modulation contributes to the systemic vasculoprotective effects of orally administered baicalin. METHODS: We utilized an Angiotensin II-induced hypertensive mouse model, employing broad-spectrum antibiotics, 16S rRNA sequencing, metabolomics, and in vitro co-culture assays to map the gut-immune-vascular axis. RESULTS: Oral Baicalin significantly attenuated Ang II-induced blood pressure elevation and improved the intestinal barrier integri

Metabolic reprogramming in chronic kidney disease-cardiovascular disease comorbidity: from molecular mechanisms to therapeutic strategies.

Chronic kidney disease (CKD) and cardiovascular disease are bidirectionally linked, with metabolic reprogramming as a central pathophysiological nexus. This review examines metabolic disturbances underlying CKD-CVD comorbidity, including energy metabolism disorders, mitochondrial dysfunction, lipid and glucose dysregulation, and mineral metabolism abnormalities. The gut microbiota-metabolic axis contributes through depletion of short-chain fatty acids, accumulation of trimethylamine N-oxide, and production of uremic toxins. These changes compromise intestinal barrier integrity and sustain syst

PANoptosis links gut dysbiosis to obstructive sleep apnea-associated atherosclerosis: a gut-vascular inflammatory axis.

Obstructive sleep apnea (OSA) is increasingly recognized as an independent contributor to atherosclerotic cardiovascular disease, yet the mechanisms linking nocturnal intermittent hypoxia (IH) to plaque progression remain incompletely understood. Beyond oxidative stress and sympathetic activation, emerging evidence suggests that the gut may function as a critical relay organ between OSA and vascular injury. IH reshapes the intestinal ecosystem by altering microbial composition, weakening epithelial and mucus barrier integrity, and remodeling microbial metabolites, thereby increasing systemic e

Transient receptor potential vanilloid channels in inflammatory bowel diseases: linking inflammation, angiogenesis, and oxidative stress.

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is characterized by chronic relapsing intestinal inflammation driven by complex interactions among immune dysregulation, epithelial barrier disruption, vascular dysfunction, oxidative stress, and gut microbiota imbalance. Despite advances in biologic and small-molecule therapies, a substantial proportion of patients remain refractory to treatment or experience significant adverse effects, underscoring the need for novel mechanistic insights and therapeutic targets. Emerging evidence suggests that the

Aging-driven metabolic abnormalities remodel intercellular communication through the gut-liver-heart axis and may promote coronary artery disease: the key role of bile acid metabolism.

Coronary artery disease (CAD) remains the leading cause of cardiovascular mortality worldwide and shows a strong age-dependence that classical risk-factor models do not fully explain. A growing body of work indicates that aging is closely associated with CAD and, in preclinical models, can promote it through immunometabolic remodeling of the gut-liver-heart axis, in which bile acid metabolism is proposed to act as a central molecular link. Here we integrate cellular, molecular, and clinical evidence to outline how aging perturbs this axis and sustains chronic vascular inflammation. At the cell

Non-selective β-blockers reduce bacterial translocation by restoring gut barrier function during experimental cirrhosis.

BACKGROUND & AIMS: Intestinal barriers limiting gut-to-liver translocation of bacteria/l products are disrupted in cirrhosis. Cirrhosis is characterised by sustained β-adrenergic overactivity, which may contribute to barrier dysfunction but, its impact on the gut-liver-axis remains unclear. We investigated whether β-adrenergic signalling drives barrier disruption in experimental cirrhosis being amenable for treatment by non-selective beta blockers such as propranolol. METHODS: Cirrhosis was induced in rats (CCl4, BDL) and mice (BDL), followed by propranolol or vehicle treatment. Chronic β-adr

Cannabinoid receptor-mediated modulation of the gut-brain axis under chronic stress: Neuroinflammatory mechanisms, microbial metabolites, and sexual dimorphism.

Chronic stress disrupts the gut-brain axis (GBA) through coordinated dysfunction of the intestinal barrier and of the vascular interfaces that protect the brain, fostering a neuroinflammatory state implicated in mood disorders, neuropsychiatric conditions, and chronic pain. This narrative review synthesizes evidence across five domains. First, the inflammatory cascade by which stress-induced disruption of the epithelial and gut vascular barriers activates the NLRP3 inflammasome and sustains central neuroinflammation. Second, microbial metabolites, namely short-chain fatty acids (SCFAs), trypto

[When the intestine swallows the pounds].

The case of an 88-year-old male patient with progressive, unintentional weight loss is reported. The condition was subjectively asymptomatic and subsequently completely reversible. Following a comprehensive internal medicine examination, chronic mesenteric ischemia due to arteriosclerosis was identified as the cause and treated interventionally. Over time, the patient experienced significant weight gain, a comparatively rare phenomenon in very old geriatric patients. Broad and targeted investigation of the cause and interventional therapies are safe and feasible even in old age.

Allogeneic haematopoietic stem cell transplantation for refractory perforating intestinal Behçet disease in a patient with aplastic anaemia: A case report.

RATIONALE: Aplastic anemia (AA) rarely coexists with intestinal Behçet disease (iBD), and the post-partum period may further complicate its expression. Managing refractory iBD with recurrent perforation on a background of long-standing AA is challenging, and evidence on optimal treatment is limited. PATIENT CONCERNS: A 42-year-old woman with non-severe aplastic anemia (NSAA) diagnosed in 2005 (AA duration ≈ 20 years) developed recurrent fever, oral ulcers, and penetrating ulcers of the ileocaecum and terminal ileum shortly after cesarean delivery in June 2019, with one episode of vulvar ulcer

Exploring Endotoxemia in Age-Related Macular Degeneration, Glaucoma, and Diabetic Retinopathy.

Lipopolysaccharide (LPS), a component of Gram-negative bacteria, is a potent activator of the innate immune system and has been implicated in the pathogenesis of eye diseases, including age-related macular degeneration (AMD), primary open-angle glaucoma (POAG), and diabetic retinopathy (DR). LPS enters systemic circulation through bacterial lysis or active secretion, leading to endotoxemia-either acute or chronic. Chronic endotoxemia, often resulting from gut dysbiosis, may link microbial imbalances to eye diseases. Indeed, chronic LPS exposure may contribute to both systemic and local inflamm
