[Lipoma of the small intestine. A clinical case].

The paper reports a case of a 45-year-old female with long-standing anemia, recurrent abdominal pain and subocclusive crises. Following a negative endoscopy of the upper tract of the large intestine, barium enema and angiography, the patient underwent total colonoscopy. Massive bleeding from the ileal valve suggested an ileal pathology: a small intestine enema confirmed a polypoid proliferation 60 cm above the Bahuino valve with related ileal invagination 25 cm long. The patient underwent surgery and pathological findings revealed a 7 cm-wide ileal lipoma near a small angiodysplasia. The latte

[Intestinal angiodysplasia as a cause of severe intestinal hemorrhage--report of 4 cases].

Angiodysplasia of the small intestine alone cannot be differentiated histologically from hereditary hemorrhagic telangiectasia (Morbus Rendu-Osler-Weber). Isolated localisation in the small intestine is rare and requires surgery in the event of massive bleeding. Four cases of bleeding from angiodysplasias of the small intestine in patients having undergone surgery at our institution over a ten-year period are presented. In three patients the particular intestine was resected. Two of these patients had an uneventful follow-up of even and ten years, one patient relapsed and was rehospitalized fo

Jejunal angiodysplasia presenting as acute lower gastrointestinal bleeding.

Vascular malformations of the bowel are uncommon sources of gastrointestinal (GI) tract bleeding with the majority of cases occurring in the right colon in elderly patients. Angiodysplasia can be difficult to diagnose, thereby complicating management as well. In the past, the diagnosis of angiodysplasia was rarely made early and rested upon a high index of suspicion in patients with lower GI bleeding and multiple negative diagnostic evaluations. Consequently, surgery was often performed late with increased morbidity and high recurrence rates. With the advent of pre- and intraoperative selectiv

[Angiodysplasia of the colon and Crohn's disease. A study with vascular resin casts].

All the previous studies which have associated arteriovenous malformations of the colon with chronic inflammatory disease have always been performed on a diseased intestine. This study, which uses the intravascular injection of resin, shows morphological evidence that angiodysplasia of the colon and intestinal inflammatory disease can coexist independently. This fact suggests that in cases of severe hemorrhage in a patient with inflammatory disease of the small intestine, it cannot simply be assumed that it originates in the macroscopically abnormal intestine, and that angiographic examination

[Isolated jejunal angiomatosis. A rare cause of intestinal hemorrhage].

The Authors report a case of solitary small bowel angiodysplasia and emphasise the rarity of this site. The importance of angiography is stressed in terms of its fundamental role in the diagnostic process and in its ability to evaluate not only the site but the extension of this type of lesion, thus enabling a surgical approach to be defined.

Angiodysplasia of the colon, Meckel's diverticulum and cardiac malformation. A case report.

A clinical case of angiodysplasia of the colon, Meckel's diverticulum and mitral valve prolapse is reported in a 20 year old man. Association of vascular ectasia of the colon and Meckel's diverticulum has been reported in a few cases but rarely with other kinds of malformation. This brief clinical paper reports a congenital aetiology of an uncommon disease of the colon, such as angiodysplasia in a young male.

Intraoperative video panendoscopy for diagnosing sites of chronic intestinal bleeding.

Intraoperative video panendoscopy was performed in 14 patients with chronic, recurrent gastrointestinal bleeding. All of the study patients had undergone extensive and expensive diagnostic testing including multiple radiographic contrast studies of the gastrointestinal tract, upper and lower endoscopy, nuclear bleeding scans, and selective mesenteric angiography without definition of the bleeding source. Intraoperative video panendoscopy, employing a segmental advance and look technique, allowed visualization and transillumination of the entire gut and identified mucosal disease in 13 patients

Small bowel angiodysplasia in association with Crohn's ileitis. A case report.

A case of a female patient affected by Crohn's ileitis associated with small bowel angiodysplasia is reported. Despite a good clinical and laboratory response to steroid therapy the patient showed an unexplained hypochromic microcytic anemia. At laparotomy Crohn's ileitis as well as an angiodysplastic lesion were found. Both lesions were resected in continuity. During a 2-year follow-up the patient did not show anemia despite pregnancy. It is suggested that the angiodysplastic lesion was the possible cause of hypochromic anemia and that the patient should have been operated on before, based on

[Intestinal angiodysplasia--a diagnostic and therapeutic challenge].

During an eight-year period we have treated 10 patients for intestinal angiodysplasia complicated by major hemorrhage. This condition represents a degenerative, acquired lesion mostly in elder people consisting of thin-walled vessels of venous origin. Angiodysplasias are usually multiple and predominantly localized in the right colon. Besides localizing the bleeding source the exclusion of concomitant lesions capable of bleeding but not responsible for hemorrhage represents the main diagnostic problem. Segmental colectomy for angiodysplasia is rarely followed by rebleeding when preoperatively

[Angiodysplasia of the terminal ileum].

Angiodysplasia, a recently discovered disease, is a pathology of scant incidence in our environment and difficult to find because of the inconclusiveness of diagnostic proofs. The disease, generally related to adults and the elderly, and preferentially located in the large intestine, does not always manifest this way. The case we present is one of the youngest in the world literature and of ectopic localization in the terminal ileum. This confirms the enigmatic aspect of this pathology, which still has characteristics to be delimited. Its etiology, incidence and prevalence are unknown and ther

Intraoperative enteroscopy for bleeding angiodysplasias of small intestine.

Six patients with angiodysplasias of the small intestine were diagnosed preoperatively with selective angiography during the past eight years. Enteroscopy was used to identify these vascular lesions intraoperatively. Lesions were ulcerated in four patients, had a bleeding vessel tip in one patient and appeared as a nonulcerated reddish patch in the other. Exact localization of these lesions allowed limited segmental resection of the small intestine to be performed. With complete enteroscopy, we were confident that no bleeding lesion was left undetected. The feeding artery and vein to the resec

[Intestinal angiodysplasia: a presentation of 7 cases and review of the literature].

Seven cases of intestinal angiodysplasia that required surgery for hemostatic control are presented, with a review of their clinical, endoscopic, arteriographic and histologic features. The average age of presentation was 52.43 years, there was a predominance in women, and the principal clinical manifestation was recurrent rectal hemorrhage. The most common location was the cecum and ascendant colon. Arteriographic diagnosis was achieved in only two patients.

Segmental dilatation of the duodenum.

In this article two cases of idiopathic duodenal dilation are described. These cases meet the present criteria for segmental dilations of the intestine. The onset of the clinical features was acute in one case and chronic in the other. However, the radiological images were similar in both cases and provided the diagnosis. Surgical treatment consisted of duodenal resection and tapering and was curative in one case. The other patient died shortly after surgery. The duodenal specimens showed normal three-layer duodenal architecture in the pathology study. The intramural nervous plexi were unimpai

[Angiographic diagnosis in intestinal angiodysplasia. Series of 13 cases].

The authors report 13 cases of intestinal bleeding; while serials endoscopic and radiological examinations had been negative, arteriography allowed intestinal angiodysplasia to be diagnosed, and detected 18 lesions (12 of the right colon, 6 of the small bowel), confirming the fundamental role of arteriography in the diagnosis the localization and extension of these lesions. The importance of accuracy is emphasized both in the performance and in the evaluation of angiographic findings, in order to avoid missing multiple localizations (colon and small bowel). The authors illustrate and evaluate
