Specialist investigation of obscure gastrointestinal bleeding.

The investigation and treatment of 131 patients with 'obscure' gastrointestinal bleeding has been reviewed. One hundred and six patients were assessed electively for recurrent haemorrhage, 25 presented as emergencies. The major presenting feature was melaena (55 patients), anaemia (35), rectal bleeding (34), haematemesis (six) and ileostomy bleeding (one). The lesions responsible for haemorrhage were colonic angiodysplasia (52 patients), small bowel vascular anomalies (16), Meckel's diverticula (nine), small bowel smooth muscle tumours (seven), gastric vascular anomalies (four), chronic pancre

[Diffuse intestinal polyangiomatosis as a cause of recurrent intestinal hemorrhage. Report of an unusual case].

We observed a 38-year-old Italian male with recurrent intestinal bleeding and consecutive iron-deficient anemia. For search of the source of bleeding intensive clinical, endoscopic and radiological investigations had been performed. Multiple malformations of superior mesenteric artery with abnormal vascularisation of jejunum and ileum were established angiographically. In addition the lumen of small bowel loops was dilated and enlarged mucosa folds were found. Because of a prominent vascularized area in the cecum a resection of the right hemicolon had been performed with consecutive death of t

Angiodysplasia of the small bowel; a 'brilliant' technique for localizing the quiescent lesion.

Intraoperative transluminal endoscopic illumination is of benefit to locate angiodysplasias of the small bowel at laparotomy in the quiescent non-bleeding phase.

Pre- and intraoperative localization of small bowel arteriovenous malformation.

Patients with chronic gastrointestinal bleeding with no source found after standard radiographic and endoscopic procedures are diagnostic challenges. Since angiodysplasia is a frequent cause of such bleeding, selective angiography has become an essential diagnostic tool in identifying arteriovenous malformations (AVM) of the large and small bowel. In addition to preoperative identification, some method of intraoperative localization is essential to assure removal of the involved segment. In a patient with a 7-year history of gastrointestinal bleeding from an AVM of the small bowel, a technique

[Anemia in the elderly subject secondary to jejunal vascular malformation. Apropos of 2 cases].

Two cases of normocytic regenerative anemia in two patients aged 84 and 86 respectively were related to gastrointestinal bleeding. Abdominal angiography was negative in both cases. Only laparotomy provided a diagnosis of jejunal vascular malformation with the aid of peroperative endoscopy. Angiodysplasia was diagnosed in one case and capillary hemangioma in the other. In these very old people with very somber prognosis, anemia was corrected by surgery without recurrence after 8 and 10 months respectively. Gastrointestinal malformations are found in about 20 p. 100 of unexplained digestive hemo

Angiodysplasia of the upper gastrointestinal tract. Clinical spectrum in 41 cases.

Over a 3-year period, arteriovenous malformations (angiodysplasia) were identified at upper gastrointestinal endoscopy in 41 patients. The lesions were multiple in one-third of patients and were predominantly situated in the proximal stomach. We considered angiodysplasia to be the cause of upper gastrointestinal bleeding in 11 (27%) patients and of anemia in nine (22%) patients. In these two groups other mucosal lesions were not seen at upper gastrointestinal endoscopy and there had been prior undiagnosed episodes of overt or occult bleeding in almost half. In 21 (51%) patients the lesions wer

Importance of small bowel involvement in bleeding angiodysplasia.

Lower intestinal bleeding related to enterocolic angiodysplasia is now accepted as a common clinical situation in the elderly. A planned approach is mandatory to allow early localization and appropriate therapy. Colonoscopy, scintigraphy and angiography used judiciously have almost entirely replaced exploratory laparotomy as a diagnostic tool. Nonoperative treatment comprising arteriographic selective vasopressin infusion and endoscopic coagulation has been followed in some cases by hemorrhage control. Such techniques, if easily obtainable, have their place; however, surgery remains the ultima

[Vascular malformations of the digestive tract: a cause of severe digestive hemorrhage. Apropos of 2 cases].

We report on two cases of angiodysplasia of the digestive tract complicated by serious acute bleeding. The preoperative diagnoses were made by angiography. One case concerned a voluminous jejunal lesion which was macroscopically visible; the other concerned a punctiform lesion detected only by angiography and not found on the pathology specimen. The authors insist on the potential gravity of these angiodysplastic lesions, the necessity to perform an emergency celio-mesenteric angiogram when confronted with a serious digestive tract hemorrhage non-explained by conventional methods of exploratio

Selective visceral angiography for unexplained acute gastrointestinal bleeding in a district general hospital.

Eleven patients are reported with unexplained gastrointestinal haemorrhage (GIH) presenting over a 2 year period in whom selective visceral angiography (SV A) was carried out after a negative endoscopy. SV A revealed the anatomical source of bleeding in all eleven patients and the aetiology in six of them. The latter comprised two tumours of the small bowel, an A-V malformation of the jejunum, enlarged pancreaticoduodenal vessels, angiodysplasia of the caecum and carcinoma of the rectum. Of the remaining five patients, four were seen to have active bleeding into the large bowel and one into th

Angiodysplasia as a cause of upper gastrointestinal bleeding.

Angiodysplasia of the stomach, the proximal part of the small intestine, or both was diagnosed in 30 patients by upper gastrointestinal (Gl) endoscopy over a 40-month period. This diagnosis represented 4% of 676 patients referred over the same time period for endoscopic examination of suspected upper Gl bleeding. Twenty-three patients (77%) had experienced at least one episode of overt bleeding (hematemesis or melena) prior to diagnosis. Multiple gastroduodenal angiodysplastic lesions were found in 19 (63%) of the patients, and additional colonic angiodysplasia was detected in six of 12 patien

Management of bleeding angiodysplasia of the upper small intestine.

Five patients thought to have bleeding angiodysplasia in the distal duodenum and proximal jejunum were treated surgically after conservative measures failed to control hemorrhage. Aspiration through a long oral tube preoperatively and intraoperative endoscopy were most helpful in locating the site of bleeding.

Upper gastrointestinal bleeding in patients with chronic renal failure.

Endoscopy to evaluate upper gastrointestinal bleeding was done for 482 patients over a 42-month period. Fifty-nine patients (12%) had chronic renal failure and upper gastrointestinal bleeding; the remaining 423 did not have renal failure. Angiodysplasia of the stomach or duodenum was the most frequent source of bleeding in patients with renal failure. Angiodysplasia (p less than 0.001) and erosive esophagitis (p less than 0.01) were significantly commoner causes of bleeding in the renal failure population than in the group without renal failure. Recurrent bleeding was also more frequent in pat

Angiodysplasia of the stomach and duodenum.

Twenty-five angiodysplasias in the stomach and duodenum were identified with endoscopy in 12 patients. The average age of the patients was 70 years. Two patients had aortic stenosis. Six of the patients were treated with electrocoagulation, three of whom required surgery. Angiodysplasia of the upper gastrointestinal tract is considered to be more common than previously thought and an important cause of both obscure and overt bleeding.

The pattern of angiodysplasia of the gastrointestinal tract in a tropical country.

Angiodysplasia of the intestine was diagnosed by selective visceral angiography in ten of 30 patients from India with unexplained recurrent hemorrhage of the gastrointestinal tract. In contrast with the reports from the western literature, most of the instances involved the small intestine and occurred in the third decade of life.
