Angiodysplasia of the colon. A review of 17 cases.

From October 1977 through December 1982, 24 patients with lower gastrointestinal tract hemorrhage were diagnosed by visceral angiography as having angiodysplasia of the colon. Seventeen of them underwent surgery for definitive treatment of their hemorrhage. Five patients had lesions on both sides of the colon, and five had coagulation disorders. Three of the 17 patients with lesions isolated to the cecum underwent right hemicolectomy. The rest underwent subtotal colectomy with ileoproctostomy. No patient had recurrent or persistent bleeding. There were three deaths in the subtotal colectomy gr

Small intestinal angiodysplasia in the elderly.

The predominant site of bleeding intestinal angiodysplasia in elderly patients will be the cecum or ascending colon, but recent experience in the Yale-Affiliated Gastroenterology Program in 1 year indicates that elderly patients may have bleeding acquired angiodysplasia (AD) confined to the small intestine only. A review of the literature confirms that symptomatic small intestinal AD is infrequent and occurs at an average age of 32 years in some series. Five patients with symptomatic small intestinal AD diagnosed during 1981 at Yale were older, with an average of 62 years. Three of the five ca

[Angiodysplasia of the ileum and cardiomyopathy].

A patient aged 73 had repeated episodes of melaena and a systolic murmur suggesting mitral insufficiency. An obstructive cardiomyopathy was diagnosed on echocardiography, while the gastro-intestinal tract investigations proved negative (gastro-duodenal endoscopy, barium meal and follow-through), until ileal vascular tumours were diagnosed by selective mesenteric arteriography. Diagnosis could only be confirmed on exploratory laparotomy, and definitive treatment was undertaken at the same time. The authors discuss the practical and theoretical consequences, including the need for thorough gastr

[Angiodysplasias of the colon and small intestine: a not uncommon cause of gastrointestinal hemorrhage. 3 cases].

Three cases of bowel angiodysplasia with digestive bleeding were reported. In all cases, diagnosis was confirmed by arteriographic and/or histological data. Angiodysplasia of the colon was the most often an acquired and degenerative disease, and most commonly affecting patients after the seventh decade of life. This condition was a frequent and misinterpreted cause of bleeding per rectum in patients after fifty years. Selective superior mesenteric angiography, after colonoscopy (except during major bleeding) was the best investigation, showing pathognomonic images and specifing the site, often

Detection of bleeding from angiodysplasia of the jejunum by blood pool scintigraphy.

Bleeding from angiodysplasia of the gastrointestinal tract has become widely recognized. In most patients there is a single focal lesion in the colon. Small bowel involvement is rare and usually associated with colonic lesions. There have been no reports of bleeding angiodysplasia affecting the small bowel only. We were able to detect bleeding from an isolated angiodysplasia of the jejunum and correctly localize it. Our diagnosis was greatly aided by the use of dynamic data collection and by the cinematic display of the data.

New diagnostic and treatment modalities involving endoscopic retrograde cholangiopancreatography and esophagogastroduodenoscopy.

Within the past several years, there has been dramatic advances in diagnostic and therapeutic modalities involving EGD and ERCP. Endoscopic sclerotherapy of esophageal varices is effective in controlling acute variceal bleeding in approximately 90% of the patients. Once bleeding is controlled, sclerotherapy may be a useful, long term treatment to prevent recurrent hemorrhage and to improve survival. In the treatment of patients with nonvariceal upper Gl bleeding, endoscopic electrocoagulation and laser photocoagulation does not significantly alter the operative rate or mortality rate. However,

Angiography in gastrointestinal bleeding.

160 selective visceral angiograms were performed in 145 patients with gastrointestinal bleeding. In 52 studies done as emergency procedures (group A) 87% were positive, and in 108 elective procedures (group B) 74% were positive. The commonest lesion demonstrated in group B was angiodysplasia (43 patients) and most of these cases had previously undergone various investigations which had proved to be negative. Angiodysplasia is clearly an important cause of obscure gastrointestinal bleeding that may go unrecognised for many years. In addition to its diagnostic value angiography also proved extre

Angiodysplasia and Meckel's diverticulum: a congenital association?

Five patients under the age of 21 years are described in whom a Meckel's diverticulum was found in association with angiodysplasia of the caecum or ascending colon. Angiodysplasia has only been observed previously in patients over the age of 35 years and its demonstration in 5 young patients with Meckel's diverticulum points not only to an association between the two disorders, but suggests that angiodysplasia may have a congenital aetiology.

Intraoperative angiography in intestinal angiodysplasia.

Intraoperative angiography is quite useful in locating the site of bleeding in patients with angiodysplasia of the small intestine. We recommend the placement preoperatively of an angiographic catheter in these patients. We have described a technique that complements this procedure with metallic clip labeling to locate precisely the area of bleeding. This method of treatment is illustrated in the present study by reports of patients bleeding from vascular malformations of the ileum, duodenum and the head of the pancreas.

Lower gastrointestinal bleeding. Diagnostic approach and management conclusions.

The management of patients with lower gastrointestinal bleeding requires a systematic approach based on defined diagnostic and therapeutic methods. Although in 80 percent of patients bleeding will stop spontaneously, 25 percent will have rebleeding and 50 percent of those with rebleeding will bleed again. Angiography documents specific bleeding sites but raises questions related to the incidence, site and frequency of bleeding, as well as the necessity of demonstrating extravasation. We reviewed 49 arteriograms performed for lower gastrointestinal bleeding. We conclude from our findings that a

Gastrointestinal bleeding in aortic stenosis.

Gastrointestinal bleeding in aortic stenosis is an uncommon condition but when present it is often undiagnosed. The usual radiological proceedures fail to demonstrate the source of bleeding. Mesenteric angiography, however, will identify the lesion. The lesion is usually a vascular malformation located in the right colon. The angiodysplasia may also occur in other parts of the gastrointestinal tract. We have encountered five patients with aortic stenosis who had multiple massive hemorrhages of the lower gastrointestinal tract who defied diagnosis by the conventional methods. Mesenteric angiogr

Gastrointestinal angiodysplasia associated with aortic valve disease: part of a spectrum of angiodysplasia of the gut.

Twelve patients with angiodysplasia of the gastrointestinal tract were seen at The Mary Imogene Bassett Hospital are presented. Six share the features of gastric or duodenal angiodysplasia, advanced age, and aortic valve disease. Of these 6 patients, 4 who bled repeatedly were treated with endoscopic coagulation of areas of gastric and duodenal angiodysplasia. Six patients with other types of gastrointestinal angiodysplasia are presented for comparison. Two had gastric angiodysplasia and no aortic valve disease, 2 had hereditary hemorrhagic telangiectasia, 1 had received irradiation, and 1 cou

Arteriovenous malformations of the intestine.

Arteriovenous malforamtions of the intestine are recognized as an uncommon cause of chronic gastrointestinal bleeding. In previous reports, the role of selective mesenteric angiography in the localization of the lesion has been emphasized; with confirmation by in vitro injection of radioopaque dye into the resected specimen. The histologic appearances of arteriovenous malformations are well recognized, although terminology is still confused. In this report, two distinct entities of small vessel malformation are described and the terms submucosal endothelial angiodysplasia and submucosal fibrom

[Vascular lesions of Fabry's disease. Optical, histochemical and ultrastructural studies].

On the basis of a new case of Fabry's disease and the various cases published in the literature, the authors analyse the vascular lesions of this angiodysplasia related to an error in lipid metabolism. They emphasise the involvement of lymph vessels (in particular those of the small bowel), and the ultrastructural characteristics and the mode of formation of deposits within the endothelial cells of the vascular walls.

Angiodysplasia of the small bowel: a method of intraoperative identification.

Two cases of recurrent bleeding from the gastro-intestinal tract are described. The initial diagnosis and the identification of the lesions at laparotomy were difficult problems. In both cases the bleeding was due to a vascular lesion of the small bowel, referred to as angiodysplasia. The lesions were detected before operation by selective angiography and identified at laparotomy by a preoperative and an intraoperative injection of a vital stain through the catheter into the feeding vessels.

Persistent gastrointestinal bleeding due to angiodysplasia of the gut in von Willebrand's disease.

Gastronintestinal vascular dysplasia as a cause of persistently recurring melaena is reported in two patients with von Willebrand's disease. Colonic wall vasculature was affected in one patient, the abnormality being demonstrated both radiologically and at operative hemicolectomy undertaken to control bleeding. Abnormalities in the vasculature of the duodenal and jejunal walls were found in the second patient at operation and necropsy, though not demonstrated by angiography.

Solitary angiodysplastic lesions in the ileocaecal region diagnosed by angiography.

Single angiodysplasias were demonstrated by preoperative angiography in four patients. In two cases the lesions were in the terminal ileum, and were situated just above the caecum in the other two patients. In three patients, contrast medium was extravasated into the bowel lumen. Histological verification was obtained in three instances. The literature on intestinal angiodysplastic lesions was reviewed, with especial regard to their distribution and radiological manifestations. The relative frequency of angiodysplasias in the right side of the colon is noted. Visceral angiography is an importa
