Cesarean Delivery after Abdominal Wall Reconstruction Using an Anterolateral Thigh Flap with Vascularized Fascia Lata.

Techniques for abdominal wall reconstruction include the use of prosthetic materials (synthetic or biological meshes), component separation, autologous tissue reconstruction, or a combination thereof. Reconstruction using prosthetic materials avoids additional trauma to healthy tissues but carries risks such as infection, adhesion to surrounding structures, and bowel obstruction caused by intestinal adhesions. Therefore, abdominal wall reconstruction using autologous tissue may be considered as an option suitable for women of childbearing potential. Only a few cases of successful childbirth fo

Optimizing pediatric radiology workflow amidst workforce shortages: A single-center experience reducing off-hours fluoroscopy.

Pediatric radiology is facing an escalating workforce crisis, fueled primarily by a constrained fellowship pipeline and rising imaging volumes. This imbalance has created difficulty providing and maintaining off-hours fluoroscopic services, which require on-site radiologist presence. The resulting burden on the existing labor force exacerbates burnout, challenges radiologist retention and recruitment, and risks undermining the long-term sustainability of pediatric radiology practice. There is a shared imperative to seek alternative, creative strategies to alleviate these issues while achieving

Langerhans cell histiocytosis presenting as a cecal mass in an adolescent: a rare surgical case report.

INTRODUCTION AND IMPORTANCE: Langerhans cell histiocytosis (LCH) is a rare, idiopathic, multifocal disease that commonly occurs in the skin and skeletal system, although involvement of the gastrointestinal tract is exceptional. Only a limited number of cases have been reported in the literature, particularly at the intestinal level, often with incidental diagnosis after colonoscopic polypectomy, and presentation as a cecal mass is extremely uncommon. CASE PRESENTATION: A 15-year-old boy presented with abdominal pain and a non-tender right iliac fossa mass. Hematological and biochemical tests

Bouveret Syndrome Mimicking Gallbladder Malignancy: A Diagnostic Challenge in an 85-Year-Old Woman.

Bouveret syndrome is a rare complication of cholelithiasis in which a gallstone enters the duodenum via a cholecystoenteric fistula, causing gastric outlet obstruction. Because of chronic inflammation and nonspecific symptoms, the condition can easily be misdiagnosed as a gastrointestinal malignancy. An 85-year-old woman presented with severe weight loss (10 kg), night sweats, and non-bloody vomiting. CT imaging revealed massive aerobilia, a thickened gallbladder wall, and an ill-defined mass at the gastroduodenal junction. These findings raised strong suspicion for a malignant process in the

Meckel's diverticulum leading to axial torsion and volvulus of the small bowel.

A Meckel's diverticulum is a blind-ending pouch that arises on the antimesenteric border of the terminal ileum. It is the most common gastrointestinal anomaly and the result of an incomplete obliteration of the vitelline duct during the first 8 weeks of gestation. The 'rule of two' is a mnemonic which refers to key characteristics of Meckel diverticula: it is found in 2% of the population, it is 2 inches long, 2 feet from the ileocaecal valve and twice as common in males. Only 2%-4% of those with a Meckel's develop complications in their lifetime. The majority present in early childhood with c

A rare case of lesser omental hernia in a male patient following total colectomy.

INTRODUCTION AND IMPORTANCE: A lesser omental hernia is a rare internal hernia in which intestinal loops become entrapped through a defect in the lesser omentum, potentially causing bowel obstruction. Its diagnosis is challenging because of nonspecific symptoms that resemble those of other gastrointestinal diseases. This report presents a case of lesser omental hernia following total colectomy. CASE PRESENTATION: A 65-year-old male with a history of laparoscopic total colectomy for ulcerative colitis 4 years prior presented with acute abdominal pain and distension. Computed tomography (CT) re

Giant ectopic splenic volvulus: a rare surgical emergency.

BACKGROUND: Splenic volvulus is a rare condition most often associated with a wandering spleen (WS) due to congenital or acquired ligamentous laxity. A giant ectopic splenic volvulus in adults without identifiable risk factors is exceptionally rare. CASE PRESENTATION: We report the case of a 46-year-old woman with no significant past medical history who presented with acute abdominal pain. Computed tomography (CT) revealed a giant ectopic spleen measuring approximately 20 cm, with torsion of the splenic pedicle, consistent with splenic volvulus. Emergency surgery was performed. Intraoperative

Jejunal mesenteric cyst presenting as acute midgut volvulus in a child: a rare cause of surgical abdomen.

INTRODUCTION: Mesenteric cysts are defined as benign intra-abdominal tumors located in the mesentery. Mesenteric cysts are rare intra-abdominal lesions in children and are often asymptomatic. Acute midgut volvulus secondary to a mesenteric cyst is an exceptionally uncommon presentation that requires prompt surgical intervention to prevent bowel ischemia. CASE PRESENTATION: An 8-year-old boy was referred from the pediatric department with acute abdominal pain and recurrent bilious vomiting. Clinical examination revealed around 20 × 20 cm size cystic lump involving whole abdomen. Contrast-enhan

Giant Jejunal Diverticulosis Causing Acute Mesenteric Volvulus: a Case Report.

BACKGROUND: Large jejunal diverticula are usually asymptomatic but can cause rare, life-threatening complications including obstruction, perforation, and bleeding. Secondary mesenteric volvulus caused by jejunal diverticulosis is exceptionally rare. It should be strongly considered when computed tomography (CT) reveals a mesenteric "whirl sign" in an adult presenting with acute obstructive symptoms. CASE PRESENTATION: A 76-year-old female presented to the emergency department with acute, diffuse abdominal pain and clinical evidence of mechanical intestinal obstruction. A CT scan demonstrated

Laparoscopic Ladd procedure for adult-onset intestinal malrotation with midgut volvulus: a case report.

BACKGROUND: Intestinal malrotation is a rare congenital anomaly resulting from the incomplete rotation of the midgut. While commonly diagnosed in newborns, adult presentation is extremely rare (0.2-0.5%), making its true incidence unknown. This case report describes a rare adult-onset midgut volvulus successfully managed via a minimally invasive approach. CASE DESCRIPTION: A 29-year-old female presented to the emergency department (ED) with acute abdominal pain, nausea, and a history in the last two years of intermittent, self-limiting subocclusive episodes. Abdominal computed tomography (CT)

Epidemiology of helminth infections in adults in rural rainforest communities of the Nkam-Wouri River Basin, Littoral Region of Cameroon: Implications for NTDs elimination using preventive chemotherapies.

BACKGROUND: Chronic helminth infections cause serious health problems; however, limited data on prevalence and distribution hinder effective control interventions. This study assessed the prevalence, distribution, and risk factors of helminth infections in adults across four health districts (HD) in the Littoral Region of Cameroon, while hypothesizing that helminth infection prevalences are significantly higher in densely forested and wetland forest plantation districts compared to degraded and mosaic plantation forest districts despite over more than two decades of preventive chemotherapy. Therapeutic Balloon-Assisted Enteroscopy: Recent Advances in the Management of Small Bowel Diseases.

Balloon-assisted enteroscopy (BAE) has substantially advanced small bowel disease management. This review summarizes recent therapeutic BAE advances, focusing on three major indications: small bowel vascular lesions, hereditary polyposis syndromes, and Crohn's disease (CD). For suspected small bowel bleeding from vascular lesions, morphological classification guides lesion-specific hemostasis-primarily using argon plasma coagulation and clipping-while gel immersion endoscopy helps secure the visual field. In hereditary polyposis syndromes (e.g., Peutz-Jeghers syndrome and familial adenomatous

Small intestine intussusception causing intestinal obstruction secondary to left pinna melanoma metastases: a case report.

We present the case of a 68-year-old patient with a previous history of stage IV malignant ulcerated nodular melanoma of the left pinna with pulmonary metastasis, who was admitted to the emergency department with an initial clinical suspicion of intestinal obstruction, which was confirmed by computed tomography. He underwent an exploratory laparotomy that revealed multiple small lesions in the small intestine suggestive of malignancy, causing a distal jejunal intussusception responsible for the obstruction. The patient underwent resection of the affected bowel segment and subsequent intestinal

Duodenum inversum presenting with intestinal obstruction in a 40-day-old infant: a case report.

BACKGROUND: Duodenum inversum is a rare congenital anomaly characterized by an abnormal course of the duodenum with preservation of a normally positioned duodenojejunal junction. Clinical presentation is highly variable, ranging from incidental findings to symptoms of upper gastrointestinal obstruction. Due to its rarity, diagnosis and management remain challenging, particularly in pediatric patients. CASE PRESENTATION: We report the case of a 40-day-old female infant presenting with recurrent postprandial vomiting and poor weight gain since the first days of life. Initial clinical, metabolic

Assessing diagnostic accuracy of onchocerciasis rapid diagnostic tests using Bayesian latent class models.

BACKGROUND: Onchocerciasis elimination programs increasingly rely on rapid diagnostic tests (RDTs) that detect antibodies to the Ov16 antigen, yet the performance of currently available rapid tests remains uncertain. The commercially available SD BIOLINE Ov16 RDT has shown inconsistent sensitivity when used on whole blood and does not consistently meet international performance thresholds for mapping or decisions to stop mass drug administration (MDA). Two novel onchocerciasis RDTs have been developed, DDTD RDT and GADx RDT, but their diagnostic accuracy has not been fully evaluated using meth

Frequency of Mutations Within P-Glycoprotein and Beta-Tubulin Associated With Ivermectin Treatment in Onchocerca volvulus Adult Female Worm Populations in Some Onchocerciasis Endemic Communities in Ghana.

BACKGROUND: Ivermectin is the drug of choice for the control of onchocerciasis because of its potent embryostatic and microfilaricidal effect. However, with long periods of mass drug administration, the effect of ivermectin on the Onchocerca volvulus worms is waning. These suboptimal responses have been linked to genes such as p-glycoprotein (P-gp) and beta-tubulin (B-tub). The purpose of this study was to define the proportion of adult female O. volvulus from some endemic communities in Ghana that have mutations in these genes. METHODS: O. volvulus adult female worms were obtained from 33 pa
