Validation of Claims-Based Algorithms for Intussusception Identification in Japanese Hospitals: A Multicenter Analysis From the CLEAR Study.

PURPOSE: Intussusception is a serious condition and a rare adverse event of rotavirus vaccination. While health insurance claims data are increasingly used for epidemiological research, claims-based algorithms for identifying intussusception have not been validated in Japan. This study aimed to validate multiple claims-based algorithms for intussusception identification against ultrasound findings as the reference standard. METHODS: This validation study used claims data and ultrasound findings from three Japanese hospitals between 2011 and 2024. The study population comprised all patients wh

Gastrointestinal Manifestations of Systemic Sclerosis: Screening and Management to Improve Outcomes.

Gastrointestinal (GI) involvement is among the most common and clinically important manifestations of systemic sclerosis (SSc), affecting up to 90% of patients and substantially contributing to morbidity and reduced quality of life. SSc-related GI disease results from microvascular injury, enteric nervous system dysfunction, and progressive smooth muscle atrophy with fibrosis, producing widespread dysmotility throughout the gastrointestinal tract. Clinical involvement can extend from the oral cavity to the anorectum, with esophageal dysfunction, gastroesophageal reflux disease (GERD), gastropa

Adult intestinal malrotation: imaging findings.

Intestinal malrotation is the most common congenital anomaly of the small intestine. However, it is associated with delayed diagnosis due to the lack of specificity of its symptoms, which can lead to devastating consequences such as intestinal volvulus or massive intestinal necrosis. We present a clinical case in which we highlight the importance of abdominal computed tomography and the detection of its characteristic signs for the early identification of this pathology.

Surgical Management of Intestinal Obstruction: A Prospective Observational Study.

BACKGROUND: Intestinal obstruction remains one of the most common surgical emergencies encountered in tertiary care settings globally, carrying significant morbidity and mortality if not promptly diagnosed and managed. The etiological spectrum, clinical presentation, and optimal surgical strategies vary considerably between small bowel and large bowel obstruction (LBO) and are further influenced by the patient's comorbidity burden, nutritional status, and oncological history. This study was undertaken to systematically evaluate the surgical management of intestinal obstruction at a tertiary ca

Pain Neuroscience Education for Postamputation Pain and Functioning: Testing Embedded Education in a Mobile App for Occupational Therapy Pain Treatment.

IMPORTANCE: Postamputation pain remains a barrier to return to function. Occupational therapy practitioners often include pain neuroscience education (PNE) to support occupational engagement and health literacy. OBJECTIVE: To assess perspectives about PNE for postamputation pain. DESIGN: We conducted cognitive interviews in which participants viewed PNE videos that are part of a novel graded motor imagery (GMI) mobile application for pain treatment. The videos were designed to prepare for an occupation-based rehabilitation approach. All interviews were recorded, transcribed, and analyzed usi

Immediate Lymphatic Reconstruction: A Technical Modification Eliminating the U-Stitch With a Mini-Forceps.

Immediate lymphatic reconstruction (ILR), originally described as the lymphatic microsurgical preventative healing approach (LYMPHA), reduces the risk of developing lymphedema secondary to breast cancer treatment. ILR involves the intussusception of arm lymphatic channels into a vein draining centrally. However, performing this technique in a deep surgical field is technically challenging. We introduce a technical modification to ILR by repurposing a pulmonary wire to facilitate the intussusception technique. Intraoperatively, fluorescein isothiocyanate (FITC) was injected into the first and f

Current Status of PSMA-Targeted Agents Based on Small Molecules for Prostate Cancer in Preclinical Studies.

BACKGROUND: National conversation has justifiably been concerned with firearm-related deaths and much less attention has been paid to the consequences of surviving a firearm injury. We assessed the risk of hospital readmission, length of stay (LOS) during hospitalization, and costs within 90-days after surviving an index firearm injury and compared these data with pedestrians and occupants involved in motor vehicle crash (MVC). METHODS: Nationwide Readmission Database, a nationally representative readmission database from 2013 and 2014 was used to create a retrospective cohort study. The prim

Unmasking the false cecal polyp: a case of inverted appendix-a case report and review of the literature.

An inverted appendix is a rare incidental finding on colonoscopy, often misinterpreted as a neoplastic lesion. A 74-year-old woman underwent routine screening colonoscopy that revealed a cecal polypoid lesion near the appendiceal orifice. Initial biopsies showed nonspecific inflammation, and a subsequent contrast-enhanced computed tomography (CT) scan reported only sigmoid diverticulosis. During a repeat in-hospital colonoscopy, the lesion was re-evaluated and identified as an inverted appendix, preventing unnecessary resection. Retrospective review of the CT images confirmed the diagnosis. Ap

Ileocecal intussusception due to giant submucosal lipoma in a young woman: case report.

Colon lipoma is a benign non-epithelial neoplasm. It manifests clinically at an average age of 65 years when it is >2 cm. Treatment depends on its dimensions and complications and consists of endoscopic or surgical resection. One of its complications is intestinal intussusception, which is rare; in adults it represents 5% of all intestinal intussusceptions and only 0.08% of all abdominal surgeries. Here we report the case of a 32-year-old woman with ileocecal intussusception due to a giant submucosal lipoma of the colon, who underwent surgery through a right hemicolectomy with ileostomy, which

Exploring Neural Mechanisms Underlying Error-related Impairments in Active Working Memory Suggests an Adaptive Shielding of Contents during Cognitive Control.

Goal-directed behavior relies on cognitive flexibility-the ability to rapidly adapt ongoing thoughts and behaviors while preserving task-relevant information. The performance monitoring system optimizes such behavior by detecting and evaluating errors, whereas the working memory (WM) system maintains relevant information and protects it from interference. We previously showed that motor errors impaired active WM maintenance (Error-Related Impairment of Active working Memory [ERIAM]; [Wessel, J. R., Jiang, J., & Stolley, J. J. Action errors impair active working memory maintenance. Journal of E

Accuracy of ultrasound for intussusception in pediatric emergency presentations: a systematic review and diagnostic meta-analysis.

INTRODUCTION: Intussusception is a common cause of acute abdominal emergencies in children. This systematic review and diagnostic meta-analysis aimed to determine the diagnostic accuracy of ultrasound for intussusception in pediatric emergency presentations, providing pooled estimates for sensitivity, specificity, predictive values, and diagnostic odds ratios to inform clinical practice. METHODS: Adhering to PRISMA-DTA guidelines, a comprehensive search was conducted in PubMed, Scopus, Cochrane Library, and Web of Science up to July 2025. Bayesian bivariate random-effects meta-analyses were pe

Laparoscopic Management of Adult Ileocolic Intussusception Secondary to an Inverted Meckel's Diverticulum.

Intussusception is the telescoping of one part of the bowel into an adjacent part, and it is most common in the pediatric population. In the adult population, intussusception is rare; however, it is likely secondary to a pathological lead point, such as a neoplasm. Intussusception has been described secondary to Meckel's diverticulum, but ileocolic intussusceptions from an inverted Meckel's diverticulum are rare. We describe here a rare case of a 33-year-old male with no past medical or surgical history presenting with an acute abdomen and imaging findings of ileocolic intussusception with bow

Intimo-Intimal Intussusception during Endovascular Repair of Type B Aortic Dissection.

OBJECTIVE: Stent graft induced aortic intimo-intimal intussusception (SAoII) during thoracic endovascular aortic repair (TEVAR) of type B aortic dissection (TBAD) is a rare complication. This study aimed to report reliable imaging features of SAoII to avoid such a potentially catastrophic event. This retrospective, multicentre observational analysis was registered as a Chinese Clinical Trial (ChiCTR2300073780). METHODS: Patients undergoing TEVAR for TBAD in three centres were reviewed from January 2014 to December 2022. SAoII was defined as intra-procedural partial or circumferential disrupti

Meconium containing intestinal structures: a histological study using human fetuses.

Meconium is changed from defoliated intestinal villi with apoptosis. A meconium corpuscle is a limited architecture in the meconium at the light microscopic level. In histological and immunohistochemical sections of the abdomen from 40 fetuses at 20-31 weeks of gestation, we found 12 fetuses with meconium and examined the architecture. A solid or loose meconium was seen depending on sites. The solid meconium in the colon and rectum contained numerous spherical or rugby ball-like corpuscles comprising of a core with tightly-packed atrophied cells and a laminar fibrous sheath. The solid meconium

Identifying Gastrointestinal Pathologies Using Point-of-Care Ultrasound.

Patients presenting with abdominal pain require expedited diagnosis and treatment. Computed tomography (CT) scans, which are frequently ordered in the inpatient and emergency departments, have high diagnostic sensitivity and specificity. However, CTs are costly, have radiation exposure, can create hospital workflow inefficiencies, and create a potential safety risk with patient transport. Point-of-care ultrasound (POCUS) use is growing as an efficient, safe, and bedside assessment tool for diagnosing and treating gastrointestinal (GI) pathologies. This manuscript synthesizes key sonographic fi

Primary intestinal lymphangiectasia presenting with intussusception in a child.

Intestinal lymphangiectasia is a protein losing intestinal disease caused by congenital malformation or obstruction of intestinal lymphatics. Based on etiology, it is classified as primary or secondary. Primary Intestinal Lymphangiectasia (PIL) is a rare disorder of intestinal lymphatic dysfunction and may present with a wide spectrum of clinical manifestations but it very infrequently presents as intussusception in children. To the best of our knowledge this is the second only reported case in children, both cases being described from the Indian subcontinent. A 12-year-old male presented comp

Determinants of ultrasound-guided reduction failure and pathological lead points in pediatric intussusception.

PURPOSE: Intussusception is a leading cause of acute intestinal obstruction in children. Ultrasound-guided hydrostatic saline enema (USGSE) is widely accepted as first-line non-surgical management, but predictors of reduction failure and pathological lead points (PLPs) are not well defined. METHODS: We retrospectively reviewed pediatric patients (< 18 years) with ileocolic intussusception treated between 2012 and 2022. Clinical variables included symptom duration, vomiting, bloody stools, and age. Univariable and multivariable logistic regression analyses were used to identify predictors of f

Variations in Paediatric Intussusception Radiologic Enema Reduction Practices Across Aotearoa New Zealand: Results From a Nationwide Cross-Sectional Study.

INTRODUCTION: Most children with intussusception are successfully treated with radiologic enema reduction. Our recent nationwide study revealed significant variation in enema reduction success and the requirement of secondary operative intervention. This may be due to variation in the management of intussusception. As such, radiologic enema management patterns across Aotearoa New Zealand (AoNZ) were investigated. METHODS: A snapshot survey of all AoNZ radiologists who had managed paediatric intussusception in the preceding 5 years was performed. Survey answers were collected anonymously via R
