ACR Appropriateness Criteria® Abdominal Pain-Child.

The range of pathology that can produce abdominal pain in children not only is broad, but can vary with age. Due to the wide differential and the inability for a child to clearly identify and describe the nature of their abdominal pain, diagnostic imaging is often needed. Various clinical scenarios are discussed for a child presenting with acute abdominal pain. Initial imaging studies that are usually appropriate or may be appropriate (based on the best available evidence or expert consensus) for suspected constipation, intussusception, small bowel obstruction without history of surgery, surgi

Multicenter analysis of time interval to intussusception reduction: Success and complication rates.

INTRODUCTION: Recent evidence shows that a 6-8 h interval between diagnosis and reduction of ileocolic intussusception does not negatively impact clinical outcomes. The goal of this study is to investigate the safe interval between diagnosis and non-operative reduction attempt for ileocolic intussusception. METHODS: This multicenter, retrospective review assessed enema reduction success and patient complications in ileocolic intussusception without lead point in patients aged 0-4 years over an 8 year period (2014-2021) in nine tertiary care institutions with subspecialty pediatric services.

Pain and irritability in Pediatric ileocolic and small bowel intussusception: A comparative study.

BACKGROUND: Ileocolic intussusception (ILIC) represents a medical emergency in pediatric emergency medicine. Some patients suspected of ILIC actually have small bowel intussusception (SMBI), which may appear clinically similar and can only be differentiated from the ileocolic type by ultrasound. The study aimed to compare pain and irritability among children presenting with ILIC versus SMBI. METHODS: This retrospective cohort study included children aged 4 months to 4 years diagnosed with ILIC or SMBI at two tertiary pediatric EDs between January 2021 and May 2025. Pain was assessed at triage

The Taurus sign: A novel clinical sign for rapid diagnosis of patent vitello-intestinal duct with double intusussception.

A patent vitello-intestinal duct uncommonly presents as neonatal bowel prolapse. We describe a 20-day-old infant with double intussusception showing the Taurus sign, confirmed surgically and treated by resection and anastomosis with good recovery. This distinctive sign allows rapid bedside diagnosis, signals vascular compromise, guides surgery, and promotes urgent intervention, helping prevent ischaemic morbidity in this rare neonatal emergency, especially in resource-limited settings, thus improving early outcomes through timely surgical referral and management.

A Rare Lymphoproliferative Disorder in a Teenager with Spontaneous Resolution on Positron Emission Tomography Scan.

UNLABELLED: Rosai-Dorfman disease (RDD), also called sinus histiocytes, is a rare disease of enlarged lymph nodes with no known etiology. It mainly involves the cervical lymph nodes. The presentation of RDD with isolated mesenteric lymph node involvement has not been reported previously. Here, we report a case of isolated mesenteric lymph node involvement caused by RDD in a 16-year-old boy. He presented with intermittent right lower quadrant pain, fever, altered bowel habits, and unintended weight loss for the past 2 months. Initial clinical and radiological findings were consistent with lymph

Is There a Role for Intestinal Ultrasound in Polyp Detection?

We present a series of pediatric and adult cases in which intestinal ultrasound (IUS) visualized benign and malignant intestinal polyps. These cases demonstrate the potential of IUS as a rapid, noninvasive bedside modality for evaluating abdominal pain. IUS has a role in recognizing structural abnormalities and associated complications such as intraluminal masses and intussusception. Although not a diagnostic standard for polyp detection, IUS can complement endoscopy in selected cases. These findings highlight an emerging, broader application of IUS beyond inflammatory bowel disease assessment

Adult Ileocolic Intussusception Secondary to Appendiceal Mucocele: A Report of a Rare Case.

Adult intussusception is a rare and diagnostically challenging condition, often caused by an underlying pathological lead point such as neoplasms, with presentations that are typically nonspecific and intermittent, including abdominal pain, nausea, vomiting, and altered bowel habits. Among unusual causes, appendiceal mucoceles represent a rare but clinically significant entity that can precipitate intussusception, carrying a risk of serious complications like pseudomyxoma peritonei (PMP) if ruptured. We report the case of a 58-year-old male presenting with right lower quadrant pain, nausea, an

Complication imaging after laparoscopic Roux-en-Y gastric bypass: clues to the diagnosis and pitfalls.

Obesity is a complex chronic disease with a rising global prevalence and significant health implications. The laparoscopic Roux-en-Y gastric bypass (LRYGB) is one of the most widely performed bariatric procedures worldwide, ensuring significant weight loss and reducing obesity-related comorbidities. However, the risk of postoperative complications remains considerable. Multidetector computed tomography (MDCT) is regarded as the primary imaging modality for the assessment of suspected complications, due to its high diagnostic accuracy. This review offers a comprehensive overview of early (≤ 30

Update on Management of Incidental Findings Seen on Imaging Studies of the Abdomen and Pelvis.

Incidental findings (IFs) are findings on imaging studies obtained for indications unrelated to the IF. IFs may represent clinically important lesions such as malignancy. However, the vast majority are benign with no clinical relevance. IFs can lead to unnecessary follow-up imaging, procedures, and iatrogenic complications, resulting in significant financial and psychologic burden to patients and the health care system. To address this, the American College of Radiology released a series of white papers that provide management guidelines for a variety of IFs in different organs. Since the orig

Risk factors for early recurrence after hydrostatic reduction of intussusception in children and development of a nomogram prediction model.

OBJECTIVE: To identify predictors of early recurrence after hydrostatic reduction of pediatric intussusception and develop a nomogram. METHODS: A total of 412 children with intussusception treated between January 2022 and January 2025 were enrolled. Based on recurrence within 48 h after successful hydrostatic reduction, they were assigned to a recurrent group (n = 37) or a non-recurrent group (n = 375). Clinical and ultrasonographic variables were compared between the groups, and a nomogram was developed and validated from the identified predictors. RESULTS: Following hydrostatic reduction, so

Hidden Malignancy: Ileocecal Intussusception as an Atypical First Presentation of Diffuse Large B-cell Lymphoma in Adulthood.

Intussusception is predominantly a pediatric condition, with only about 5 % of all intussusception cases occurring in adults with primary intestinal lymphomas comprising 1 %-4 % of gastrointestinal malignancies, 90 % of which are B-cell non-Hodgkin lymphomas (NHL). Diffuse large B-cell lymphoma (DLBCL), the most common subtype of NHL, often leads to intussusception without classical B symptoms. We present the case of a 33-year-old male with ileocolonic intussusception caused by a 4.8 × 5.3 × 5.5 cm enhancing mass. Histopathology and FISH confirmed DLBCL without MYC, BCL2, or BCL6 rearrangement

Laparoscopic treatment for an inflammatory fibroid polyp complicated with ileocecal intussusception: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Adult intussusception is a rare clinical entity, accounting for less than 5% of all cases and often associated with an underlying lesion. Inflammatory fibroid polyps (IFPs) are rare benign mesenchymal tumors that may act as a lead point for intussusception, most frequently in the small intestine. Herein, we report the case of a 52-year-old woman presenting with ileocecal intussusception secondary to an IFP. CASE PRESENTATION: A 52-year-old woman presented with intermittent abdominal pain and symptoms of subacute obstruction. Imaging revealed an ileocolic intussusc
