Case Report: Immune checkpoint inhibitor-induced hypophysitis presenting as acute paralytic ileus mimicking mechanical bowel obstruction in a patient affected by advanced non-small cell lung cancer.

Immune checkpoint inhibitors (ICIs) have significantly improved outcomes in advanced non-small cell lung cancer (NSCLC) but may induce immune-related adverse events (irAEs), including hypophysitis. Although endocrine irAEs usually present with nonspecific constitutional symptoms, atypical manifestations may delay diagnosis and expose patients to unnecessary invasive procedures. We report the case of a 76-year-old man with metastatic NSCLC receiving first-line nivolumab, ipilimumab, carboplatin, and pemetrexed who developed profound asthenia, altered mental status, bilious vomiting, and severe

Incidence, Predictors, Management, and Outcome of Paralytic Ileus in Acute Pancreatitis.

BACKGROUND: Paralytic ileus is a frequent complication of acute pancreatitis (AP), contributing to increased morbidity, nutritional compromise, and risk of infection. Despite its clinical relevance, data on its incidence, predictors, management, and outcomes in AP remain limited. METHODS: We conducted a retrospective study of 426 adult patients admitted with AP over a 3-year period at a tertiary-care center. The primary outcome was paralytic ileus; secondary outcomes included infected necrosis, intervention, and mortality. Multivariate logistic regression identified independent predictors exp

Robotic Transabdominal Preperitoneal Repair Versus Laparoscopic Totally Extraperitoneal Repair for Inguinal Hernia.

BACKGROUND AND OBJECTIVES: To date, no Japanese studies have compared robotic transabdominal preperitoneal repair (R-TAPP) and laparoscopic totally extraperitoneal repair (L-TEP). Herein, we present our initial experience from a single Japanese hospital, comparing both procedures and evaluating their short-term outcomes in the context of potential inclusion under Japan's national health insurance system. METHODS: Patients with inguinal hernias who had not undergone previous anterior hernioplasty were retrospectively analyzed. Demographic factors (age, sex, body mass index) and perioperative d

Paralytic Ileus in a 45-Year-Old Female with Schizophrenia on Clozapine Therapy.

A 45-year-old woman who comes with complains of recurrent paradoxical loose stools and abdominal distension for 3 days. She had type II diabetes, schizophrenia, and a history of left below-knee amputation. While there was no indication of a mechanical blockage, imaging tests revealed paralytic ileus. A potentially fatal gastrointestinal side effect that is frequently associated with anticholinergic drugs, such as clozapine, is paralytic ileus. Full recovery was achieved with conservative treatment and a reduction in the dosage of clozapine. This instance highlights how important it is to ident

SIADH as an Underrecognized Manifestation of Porphyria-like Crises in Hereditary Tyrosinemia Type 1: Clinical and Pathophysiological Insights.

Hereditary tyrosinemia type 1 (HT1) is a rare metabolic disorder caused by fumarylacetoacetate hydrolase deficiency, leading to the accumulation of toxic metabolites such as fumarylacetoacetate (FAA) and succinylacetone (SA). We report an 11-year-old boy with poorly controlled HT1 who presented with a severe neurovisceral crisis after suboptimal adherence to nitisinone (NTBC) therapy, characterized by abdominal pain, hypertension, paralytic ileus, seizures, and profound hyponatremia. Biochemical evaluation revealed markedly elevated urinary δ-aminolevulinic acid (ALA), consistent with a porphy

Gastrointestinal Diagnostic Coding After Spinal Cord Injury: Health Behavior Correlates and Implications for Neurogenic Bowel Management in a Nationwide Claim-Based Cohort.

Background: Neurogenic bowel dysfunction (NBD) is a major chronic sequela of spinal cord injury (SCI) with substantial implications for rehabilitation and long-term management. However, population-level evidence describing how gastrointestinal (GI) diagnostic codes are used following SCI, particularly within administrative healthcare systems, remains limited. Methods: We conducted a nationwide retrospective cohort study using administrative claims data from the Korean National Health Insurance Service (NHIS). A total of 584,266 adults with trauma-related SCI encounters between 2009 and 2019 we

Uremic Paralytic Ileus in a 70 Years Old Female: A Diagnostic Dilemma.

Uremia has been associated with slowed intestinal motility, resulting in loss of peristalsis and the development of paralytic ileus. We present a case of uremia-induced paralytic ileus in a 70-year-old woman. The patient was admitted with a 7-day history of abdominal pain, vomiting, and constipation. Initial evaluation revealed a soft non-tender abdomen with decreased bowel sounds. Urgent ultrasonography showed dilated gut loops and sluggish gut motility, while atrophic kidneys with hydronephrosis were also appreciated. Routine biochemistry labs revealed elevated creatinine and potassium level

The Efficacy of Lacosamide in the Treatment of Delirium Complicated With Ileus: Four Cases.

Delirium is a common disease in elderly patients and requires medications, although nondrug interventions are recommended. When delirious patients are complicated with ileus, the selection of drugs would be difficult. In this paper, three cases of antipsychotic-induced paralytic ileus successfully managed with lacosamide are presented without any severe adverse effects, including psychiatric symptoms. Lacosamide is a newer antiepileptic drug that acts on the sodium channels. Drug-induced eruptions are rare compared to other antiepileptics, such as carbamazepine and lamotrigine. Lacosamide can

Paralytic Ileus as the Initial Manifestation of New-Onset Type 1 Diabetes Without Diabetic Ketoacidosis: A Case Report.

Type 1 diabetes (T1D) is an autoimmune disease characterized by pancreatic β-cell destruction and absolute insulin deficiency. In pediatric patients, T1D typically presents with classic symptoms such as polydipsia, polyuria, weight loss, or diabetic ketoacidosis (DKA). Although gastrointestinal symptoms may occur at disease onset, paralytic ileus as an initial manifestation of T1D, particularly in the absence of DKA, is extremely uncommon. A 15-year-old boy presented with severe abdominal pain and vomiting and was diagnosed with paralytic ileus based on abdominal radiographic findings. Mild hy

Influence of Language Barriers on Postoperative Recovery After Hepatobiliary and Pancreatic Surgery: A Retrospective Analysis.

BACKGROUND: Understanding health related information is crucial for informed consent and active participation in surgical care. Language barriers between patients and health care professionals pose the risk of misunderstandings and incomplete information exchange. This can significantly impair the quality of health care. Little is known about the impact of language barriers in hepatobiliary and pancreatic surgery and its consequences for postoperative recovery. METHOD: We performed a retrospective study in a University Medical Center in Germany assessing patients from 2020 to 2023 who underwe

Reirradiation for Recurrent Cervical Cancer Within the Previous Radiation Field Using a Bioabsorbable Spacer: A Case Report.

Reirradiation for cervical cancer recurrence in previously irradiated fields is challenging due to dose limitations. To our knowledge, this is the first case report describing successful reirradiation using a bioabsorbable spacer for vaginal cuff recurrence after initial concurrent chemoradiotherapy (CCRT) and hysterectomy. A 70-year-old woman with cervical cancer Stage IIIC2r initially received CCRT and chemotherapy. Three years later, uterine recurrence led to hysterectomy. Eighteen months post-surgery, vaginal cuff recurrence was diagnosed by imaging and tumor biopsy. A bioabsorbable spacer

Divergent manifestations and management of GISTs of the small intestine: a case series.

Gastrointestinal stromal tumours (GISTs) of the small intestine are rare but clinically significant mesenchymal neoplasms, often presenting with gastrointestinal bleeding, obstruction, or perforation. This retrospective case series analyses six cases managed over 18 months, highlighting their diverse presentations, diagnostic challenges, and surgical outcomes. Gastrointestinal bleeding was the most common symptom (n = 4), followed by obstruction (n = 2) and perforation (n = 1), with one case detected incidentally. Contrast-enhanced computed tomography was crucial in preoperative diagnosis, rev

Peritonitis and paralytic ileus due to COVID-19 in a 29 weeks pregnant woman: a case report.

BACKGROUND: COVID-19 infection has caused a global health crisis with over 6,956,900 deaths. Extra-pulmonary forms of the infection are common, including digestive symptoms. Pregnant women are at greater risk of developing severe forms of the infection. They are also affected by extra-pulmonary symptoms. CASE PRESENTATION: We report the case of a 28week + 4 pregnant woman with paralytic ileus caused by inflammatory peritonitis related to COVID-19 leading to premature birth. Pulmonary deterioration was secondary to abdominal symptoms. CONCLUSIONS: Therefore, the presence of gastrointestinal (GI

Circulating IGF-1 levels and 27 digestive diseases: 461,401 UK biobank prospective analysis.

Insulin-like growth factor-1 (IGF-1) regulates gastrointestinal mucosal repair, hepatocyte function, and metabolic homeostasis, but its association with a broad spectrum of digestive diseases remains unsystematic. We analyzed 461,401 UK Biobank participants (aged 40-69 years) with baseline IGF-1 measurements (stratified into quartiles: Q1 ≤ 17.525, Q2 17.526-21.253, Q3 21.254-24.825, Q4 ≥ 24.826 nmol/L) and followed up to August 2024 (median: 66-184 months). Emergency involving four intra-abdominal pathologies.

Patients in the emergency department usually present with a defined condition that can be treated. More often than not, a cause leads to a pathology that is the leading symptom, such as sigmoid carcinoma with caecal perforation. However, we very rarely see three different conditions that lead to another and require emergency treatment. We report the case of a 70-year-old male who presented with ileus, peritonitis and an incarcerated inguinal hernia. We found a gastric perforation and a desmoid tumour of the small intestine in the incarcerated inguinal hernia. An ileocecal resection, gastric su
