Neurofibromatosis Type 1 Patients With Epilepsy: A Comprehensive Analysis of Demographics, Comorbidities and Healthcare Outcomes.

Neurofibromatosis Type 1 (NF1) is the most common neurocutaneous disorder in the United States. Due to a nonfunctional mutation in the NF1 gene, the disorder may initially present with café-au-lait spots and later numerous neurofibromas across the body. Epilepsy is a rare comorbidity of NF1, with an incidence of just 4%-7%. While abnormal electroencephalograms have been seen in up to 25% of NF1 patients, very few studies have investigated the association between epilepsy and the NF1 patient profile. This study was aimed at evaluating the associations between epilepsy and demographics, comorbid

Balancing a Surgical vs. Non-surgical Approach in Perforated Appendicitis.

Appendicitis is the inflammation of the appendix, one of the common causes of acute abdomen that may indicate emergency abdominal surgical intervention. However, when complicated by perforation, phlegmon, and/or abscess formation, the standard and traditional treatment with the approach for a surgical intervention has been questioned regarding its risks and benefits. In this case presentation, our patient, who presented with perforated appendicitis complicated by phlegmon formation and ileus, was managed with a conservation approach through antibiotics and percutaneous drain placement. ALTERNATIVE THERAPEUTIC APPROACHES FOR PARALYTIC ILEUS IN PERSIAN MEDICINE.

Paralytic ileus is a prevalent medical condition following surgery, exerting a significant financial impact on the healthcare system. Despite its considerable implications, there has been limited progress in advancing both diagnostic and curative approaches to address ileus. This study seeks to introduce alternative diagnostic methodologies rooted in Persian medicine (PM) for paralytic ileus. Our investigation involved a thorough review of literature, including The Canon of Medicine, and an exploration of various PM texts for relevant references. The findings were systematically compared with

Impact of the uroperitoneum on the development of paralytic ileus: a preclinical study in the pig model.

PURPOSE: The intraperitoneal leak of urine is considered as a major cause of peritoneal inflammatory reaction that could lead to paralytic ileus. Our aim was to document the effect of urine on the peritoneal surface. METHODS: Seven white minipigs (one control-six tests) underwent standardized general anaesthesia. In three test pigs urine was aspirated from the bladder and instilled in the peritoneal cavity. In the remaining three pigs a continuous urine leak was created through section of the ureter. At 4 and 10 h laparoscopic harvesting of slices of the parietal peritoneum at the level of th

Primary Small Intestinal Leiomyosarcoma Presenting With Small Bowel Perforation: A Case Report.

A small intestinal leiomyosarcoma (LMS) is a rare malignancy that accounts for a small proportion of small bowel tumors and is often difficult to diagnose early due to its variable symptoms. This case report describes a 75-year-old female patient, previously suspected to have a gastrointestinal stromal tumor (GIST), presenting to the ED with abdominal pain and elevated inflammatory markers. A CT scan was performed which revealed a perforated mass in the distal ileum with surrounding collection. Consequently, she underwent laparoscopic washout, bowel resection, and primary anastomosis. Acute pancreatitis due to fishbone.

A 68-year-old woman with obesity and ulcerative colitis history, referred to the emergency department and transferred to the ICU, with severe epigastric belt-like abdominal pain (requiring opiates) with sudden onset 10 hours after the last meal. The clinical exploration presented painful abdomen with epigastrium peritoneal irritation. Laboratory tests showed elevated amylase (2600 U/l) and lipase (1127 U/l), with leukocytosis and neutrophilia. CT scan reported acute pancreatitis versus prepyloric gastric perforation, although no extraluminal gas was described, and a sharp foreign body was obje

Efficacy and safety of preoperative intravenous iron versus standard care in colorectal cancer patients with iron deficiency anemia: a systematic review and meta-analysis.

BACKGROUND: Anemia, particularly iron deficiency (ID) anemia, is common in colorectal cancer (CRC) patients, affecting up to 58% of individuals. This study aimed to compare the effectiveness and safety of preoperative intravenous iron (IVI) with standard care (no iron or oral iron) in CRC patients with ID anemia. METHODS: A systematic search across multiple databases identified studies comparing IVI versus no iron or oral iron in CRC patients with ID anemia. Pooled data were analyzed for changes in hemoglobin (Hb) levels, need for red blood cell transfusions (RBCT), overall mean number of tra

Pylephlebitis Secondary to Diverticulitis Diagnosed by Abdominal Ultrasound and Computed Tomography.

Pylephlebitis is a rare infection, characterized by non-specific symptoms such as abdominal pain, that often leads to delayed diagnosis, yet it is a severe infection with a high mortality rate. Imaging studies are essential for diagnosis, and contrast-enhanced abdominal CT and abdominal ultrasound are commonly performed. A 51-year-old male was admitted to the hospital with fever and shock. Blood tests revealed liver and kidney dysfunction. Contrast-enhanced computed tomography (CT) is the best modality to demonstrate the portal vein abnormalities and diverticulitis. But plain abdominal CT was

A Case of Upadacitinib-Associated Ileus Secondary to Psoriasis Induced by Treatment of Atopic Dermatitis with Dupilumab.

A 69-year-old man with severe atopic dermatitis (AD) received a single 600 mg subcutaneous injection of dupilumab, which resulted in a psoriatic rash on day 10. He was then given 30 mg of oral upadacitinib daily, and after 10 weeks of treatment, both the AD and the psoriasis had significantly improved. However, at week 16, the patient had no bowel movement for a week, and paralytic ileus was suspected based on the patient's symptoms and laboratory findings. Without surgery or other treatment, one week after stopping upadacitinib, the patient resumed bowel movements and the ileus improved, sugg

Intestinal Pseudo-Obstruction After Vincristine Use.

A 73-year-old man presented with nausea, abdominal discomfort, and distention persisting for the past five days. He had previously been diagnosed with stage III peripheral CD4+ T cell lymphoma and had initiated chemotherapy comprising vincristine two weeks prior to presentation. An evaluation revealed diffuse colon distention and pneumatosis intestinalis without mechanical obstruction, consistent with acute colonic pseudo-obstruction. The patient underwent surgical intervention and was subsequently admitted to the Intensive Care Unit due to distributive shock. Vincristine-induced ileus followe

Deep-Learning Model for Mortality Prediction of ICU Patients with Paralytic Ileus.

Paralytic Ileus (PI) patients in the Intensive Care Unit (ICU) face a significant risk of death. Current predictive models for PI are often complex and rely on many variables, resulting in unreliable outcomes for such a serious health condition. Predicting mortality in ICU patients with PI is particularly challenging due to the vast amount of data and numerous features involved. To address this issue, a deep-learning predictive framework was developed using the Medical Information Mart for Intensive Care IV (MIMIC-IV) dataset, which includes data from 1017 ICU patients with PI. By employing SH

Robinia pseudoacacia poisoning in two horses: A case report.

This case report describes the poisoning of two mares from the same paddock with Robinia pseudoacacia (Black locust) bark. The poisoning manifested itself by the sudden onset of weakness and fever with transient improvement after the administration of non-steroidal anti-inflammatory drugs and fluids. After the initial stabilisation, the mares were left unattended overnight. One of them was found dead in the morning. The surviving mare developed colic and severe encephalopathy and had to be referred to the clinic. Blood tests revealed severe hyperammonaemia. After four days of symptomatic treat

Gall Stone Ileus and Recurrence: Management Dilemma for the Operating Surgeon.

Gallstone ileus (GSI) is a rare complication of gallstone disease. It occurs as a result of the passage of a stone from the biliary tract into the gastrointestinal tract via an abnormal pathway (bilio-enteric fistula). Chronic inflammatory processes result in gall bladder adhering and subsequently eroding into the intestines, leading to a fistula. It is a surgical emergency seen in the elderly who often present as being unwell, with abdominal pain, distension, and vomiting. Imaging such as a CT scan is diagnostic in confirming small bowel obstruction. Management usually involves relief of obst

Paralytic Ileus as the Initial Presentation of Guillain-Barre Syndrome: A Rare Case Report.

Emerging symmetrical limb weakness, altered sensory perception, and absent or reduced deep tendon reflexes are the hallmarks of Guillain-Barre syndrome (GBS). There are numerous studies that indicate up to two-thirds of patients with GBS may also have autonomic dysfunction. One uncommon presenting characteristic of GBS before motor weakness is evident is paralytic ileus, a sign of gastrointestinal dysautonomia. Here we describe the case of a 55-year-old man who was brought to the emergency room of our hospital with paralytic ileus, a less common symptom of early-stage GBS. The patient was init
