Treatment of intussusception with small bowel obstruction: application of decision analysis.

Although hydrostatic reduction of intussusception has been accepted as the most desirable method of treatment, there is still no consensus regarding its application on patients presenting with signs of small bowel obstruction. Decision analysis was used to consider alternative strategies of management, that is, the selective approach of not attempting hydrostatic reduction if there is radiographic evidence of small bowel obstruction, and an attempt at hydrostatic reduction even in the presence of intestinal obstruction, thus operating only when the noninvasive treatment fails. Estimates were b

Acute jejunogastric intussusception.

Acute jejunogastric intussusception is a rare complication following gastric surgery. Three patients were treated for this condition during the past 15 years. Common presenting manifestations are vomiting, hematemesis, upper abdominal pain, and palpable abdominal mass within the left hypogastrium. The diagnosis is established by gastroscopy or upper gastrointestinal radiographs. Four categories of classification are described. Optimal operative management consists of prompt laparotomy. Manual reduction of the intussusception is followed by resection of compromised bowel. Procedures to prevent

Nodular lymphoid hyperplasia of the intestinal tract in infancy and childhood.

Over an 18-year period we have diagnosed nodular lymphoid polyposis of the intestinal tract in 6 patients. The site of the polyposis, which was due to prominent lymphoid hyperplasia, was ileal (3), colonic (2), and rectal (1). The diagnosis was made following complications arising from the polyps, which included recurrent intussusception (2), rectal prolapse (1), intestinal or pseudointestinal obstruction (2), and rectal bleeding (1). Immunoglobulin staining was performed on all the bowel specimens and in every case secretory IgA was present on the mucosal surfaces and IgG and IgA were seen in

Defecography.

Defecography is a technique of examining the rectum and anal canal in which the patient is studied while sitting down rather than recumbent and recordings are obtained both at rest and during straining. The authors describe their findings in 83 patients with dyschezia. Defecation was normal in 28 patients. Prolapse of the anal mucosa was seen in 13 patients and internal procidentia in 23, 12 of whom also had intussusception manifested as rectal prolapse. A deep rectogenital fossa associated with an enterocele was seen in 16 patients; 13 had a proctocele, while fecal retention was seen in 5. De

Rectal prolapse.

Rectal prolapse occurs mostly in the geriatric female patient and can be a very disabling condition. The etiology is intussusception of the rectosigmoid secondary to excessive and prolonged straining. Medical therapy for this disease process is not helpful and patients will require a surgical procedure. The two best surgical procedures for the correction of rectal prolapse are low anterior resection of the rectosigmoid and proctopexy. A few patients who are unfit for laparotomy may require the Thiersch Wire procedure. Two unresolved problems after surgical therapy are continuing constipation a

Evacuation proctography in obstructed defaecation and rectal intussusception.

The symptoms of obstructed defaecation may present as a number of different syndromes including descending perineum, solitary rectal ulcer, irritable bowel and mucosal or complete rectal prolapse. In order to clarify the pathophysiology of obstructed defaecation we carried out dynamic and static radiological investigations together with manometric and electrophysiological measurements in ten patients with severe, intractable obstructed defaecation. Results were compared with a total of 35 control subjects. There were no significant differences in sphincter pressures or the recto-anal inhibitor

Anatomic specificity in the diagnosis and treatment of internal rectal prolapse.

Distal bowel evacuation was studied by cinedefecography in 85 women with obstinate constipation, tenesmus, and incomplete evacuation in whom a diagnosis of internal rectal intussusception was clinically suspect. Sixty-five patients showed radiographic evidence of intussusception--mostly of the distal rectum, without rectosacral separation. Patients with distal intussusception who did not respond to nonoperative measures were treated by Delorme's transrectal excision with excellent results. Internal rectal intussusception is a real and demonstrable entity which may be symptomatically disabling

Disturbances in the defecation mechanism with special reference to intussusception of the rectum (internal procidentia).

Anorectal disorders that disturb normal defecation are described, especially intussusception of the rectum (internal procidentia). A review of 190 patients, half of whom were treated operatively and the other half conservatively, is presented. Diagnostic procedures, symptoms, and indications for operations are evaluated. We believe that intussusception of the rectum is a relatively common cause of difficult emptying of the rectum and, when the correct diagnosis is established, operation presents a fair chance for improvement.

Ileocaecal atresia.

Meconium ileus is one of the commonest causes of distal small bowel obstruction in neonates. Within the last six years at our hospital two newborns presented with identical signs and symptoms of meconium ileus. Abdominal x-rays showed distal small bowel obstruction with ground glass opacity in the right lower quadrant. Gastrografin enema in both cases demonstrated a microcolon, but there was no reflux of this contrast material into the terminal ileum. Laparotomy in both babies revealed atresia of the ileocaecal valve with dilatation of the terminal ileum loops which contained normal meconium.

[Colorectal complications of radiological examinations].

These complications are uncommon, but can be very serious. They are dominated by perforations, in the case of barium enemas. The use of rigid canulas and/or balloons are often implicated in rectal perforations which occur most readily in rectal tissue damaged by a biopsy. Colonic perforations usually occur in damaged tissues (various forms of colitis, cancer, colostomy). The hydrostatic pressure seems to be incriminated as a precipitating factor in the development of spasms in the presence of an underlying obstruction. The other complications are much less common. In children, perforation usua

Comparison of suture materials and suture patterns for inverting intestinal anastomosis of the jejunum in the horse.

In 7 horses, 4 anastomoses were done in the small intestine in each, using the combinations of synthetic absorbable monofilament and multifilament suture materials with continuous- and interrupted-suture patterns in the serosubmucosal layer of a 2-layer inverting-suture technique. Horses were evaluated 30 days after the operation for adhesion formation, lumen diameter, evidence of chronic obstruction, and suture tract inflammation at the anastomosis. Postoperative obstruction occurred in 5 of the 7 horses, and 6 horses survived. One horse was euthanatized on postoperative day 6 after 48 hours

Kidney graft survival in transfused and nontransfused sibling beagle dogs.

In 6 pairs of sibling Beagle dogs, 1 kidney was exchanged between pairs, and contralateral nephrectomy was done. Previously, one dog of each pair was given blood transfusions from the donor of its allograft. All dogs were given azathioprine and prednisone postoperatively for immunosuppression. Four of 6 dogs given pretransplantation transfusions were healthy 1 year after surgical manipulation was done, and 2 died for reasons other than graft rejection. Of the 6 dogs that were not given pretransplantation transfusions, 3 were healthy after 1 years, but 2 were euthanatized because of graft rejec

Intestinal obstruction of infancy and childhood in Benin City, Nigeria.

The pattern of intestinal obstruction observed on 168 Nigerian infants and children is presented. The commonest causes of intestinal obstruction were intussusception (21%), anorectal anomalies (20%) and Hirschsprung's disease (14%). Strangulated hernia and adhesions occurring in 3.5% and 1.7% of cases respectively were notably uncommon in this age group. The role of radiology in the management of some cases is highlighted.

Primary lymphoma of Meckel diverticulum: a case report.

A case of a primary lymphoma of Meckel's diverticulum in a 6-year-old Iraqi boy presenting clinically as acute appendicitis is reported. Exploration revealed a ruptured gangrenous Meckel's diverticulum with an ileo-ileal intussusception at the diverticulum origin. A fairly comprehensive search through the English language literature has failed to reveal any other report of a primary lymphoma of Meckel's diverticulum leading to acute intestinal obstruction.

Ileocaecal intussusception of a previously recognized Meckel's diverticulum.

A case is reported of an ileocaecal intussusception caused by a previously recognized Meckel's diverticulum. The case offers the opportunity to discuss the management of an asymptomatic Meckel's diverticulum found incidentally. The conclusion is reached that, in general, diverticulectomy is appropriate.
