Spleen-Preserving Repair of Acute Gastric Volvulus With Wandering Spleen: Anterior Gastropexy and Splenopexy in a 20-Year-Old Man.

Gastric volvulus is an uncommon condition in which the stomach twists on itself, and its association with wandering spleen is attributed to a shared defect of deficient peritoneal fixation. Both conditions are described predominantly in children, and reports of the combination in adults remain rare. When it does occur, the goal of management is often definitive fixation of the affected organs, yet there is limited guidance on the optimal approach in adults, particularly regarding splenic preservation. We present a rare case of gastric volvulus with wandering spleen in a 20-year-old man, manage

Acute mesenteric ischemia caused by adhesive-band-induced mesenteric artery compression with secondary ileal volvulus and internal hernia: a rare case report.

INTRODUCTION: Acute mesenteric ischemia (AMI) is a surgical emergency caused by inadequate blood flow through the mesenteric vessels, resulting in intestinal ischemia and eventual gangrene of the bowel wall. The condition may result from arterial thrombosis, embolism, mesenteric venous thrombosis, or non-occlusive systemic hypoperfusion. CASE PRESENTATION: We report a rare case of AMI in a 73-year-old female with a significant past surgical history of transabdominal hysterectomy with bilateral salpingo-oopherectomy. She presented with sudden, severe abdominal pain that was disproportionate to

Meckel's diverticulum leading to axial torsion and volvulus of the small bowel.

A Meckel's diverticulum is a blind-ending pouch that arises on the antimesenteric border of the terminal ileum. It is the most common gastrointestinal anomaly and the result of an incomplete obliteration of the vitelline duct during the first 8 weeks of gestation. The 'rule of two' is a mnemonic which refers to key characteristics of Meckel diverticula: it is found in 2% of the population, it is 2 inches long, 2 feet from the ileocaecal valve and twice as common in males. Only 2%-4% of those with a Meckel's develop complications in their lifetime. The majority present in early childhood with c

Giant ectopic splenic volvulus: a rare surgical emergency.

BACKGROUND: Splenic volvulus is a rare condition most often associated with a wandering spleen (WS) due to congenital or acquired ligamentous laxity. A giant ectopic splenic volvulus in adults without identifiable risk factors is exceptionally rare. CASE PRESENTATION: We report the case of a 46-year-old woman with no significant past medical history who presented with acute abdominal pain. Computed tomography (CT) revealed a giant ectopic spleen measuring approximately 20 cm, with torsion of the splenic pedicle, consistent with splenic volvulus. Emergency surgery was performed. Intraoperative

Jejunal mesenteric cyst presenting as acute midgut volvulus in a child: a rare cause of surgical abdomen.

INTRODUCTION: Mesenteric cysts are defined as benign intra-abdominal tumors located in the mesentery. Mesenteric cysts are rare intra-abdominal lesions in children and are often asymptomatic. Acute midgut volvulus secondary to a mesenteric cyst is an exceptionally uncommon presentation that requires prompt surgical intervention to prevent bowel ischemia. CASE PRESENTATION: An 8-year-old boy was referred from the pediatric department with acute abdominal pain and recurrent bilious vomiting. Clinical examination revealed around 20 × 20 cm size cystic lump involving whole abdomen. Contrast-enhan

Giant Jejunal Diverticulosis Causing Acute Mesenteric Volvulus: a Case Report.

BACKGROUND: Large jejunal diverticula are usually asymptomatic but can cause rare, life-threatening complications including obstruction, perforation, and bleeding. Secondary mesenteric volvulus caused by jejunal diverticulosis is exceptionally rare. It should be strongly considered when computed tomography (CT) reveals a mesenteric "whirl sign" in an adult presenting with acute obstructive symptoms. CASE PRESENTATION: A 76-year-old female presented to the emergency department with acute, diffuse abdominal pain and clinical evidence of mechanical intestinal obstruction. A CT scan demonstrated

Laparoscopic Ladd procedure for adult-onset intestinal malrotation with midgut volvulus: a case report.

BACKGROUND: Intestinal malrotation is a rare congenital anomaly resulting from the incomplete rotation of the midgut. While commonly diagnosed in newborns, adult presentation is extremely rare (0.2-0.5%), making its true incidence unknown. This case report describes a rare adult-onset midgut volvulus successfully managed via a minimally invasive approach. CASE DESCRIPTION: A 29-year-old female presented to the emergency department (ED) with acute abdominal pain, nausea, and a history in the last two years of intermittent, self-limiting subocclusive episodes. Abdominal computed tomography (CT)

Epidemiology of helminth infections in adults in rural rainforest communities of the Nkam-Wouri River Basin, Littoral Region of Cameroon: Implications for NTDs elimination using preventive chemotherapies.

BACKGROUND: Chronic helminth infections cause serious health problems; however, limited data on prevalence and distribution hinder effective control interventions. This study assessed the prevalence, distribution, and risk factors of helminth infections in adults across four health districts (HD) in the Littoral Region of Cameroon, while hypothesizing that helminth infection prevalences are significantly higher in densely forested and wetland forest plantation districts compared to degraded and mosaic plantation forest districts despite over more than two decades of preventive chemotherapy. Gastric volvulus following Nissen fundoplication: a case report and review of reported cases, risk factors, and management strategies.

Gastric volvulus is a rare but potentially life-threatening complication following Nissen fundoplication, with delayed diagnosis increasing the risk of gastric ischemia, necrosis, and perforation. We report the case of a 72-year-old man who presented on postoperative Day 10 with progressive dysphagia, severe epigastric pain, and intolerance to oral intake following robot-assisted laparoscopic Nissen fundoplication. Computed tomography and upper gastrointestinal contrast study demonstrated marked gastric distension with obstruction at the gastroesophageal junction. Urgent diagnostic laparoscopy

Assessing diagnostic accuracy of onchocerciasis rapid diagnostic tests using Bayesian latent class models.

BACKGROUND: Onchocerciasis elimination programs increasingly rely on rapid diagnostic tests (RDTs) that detect antibodies to the Ov16 antigen, yet the performance of currently available rapid tests remains uncertain. The commercially available SD BIOLINE Ov16 RDT has shown inconsistent sensitivity when used on whole blood and does not consistently meet international performance thresholds for mapping or decisions to stop mass drug administration (MDA). Two novel onchocerciasis RDTs have been developed, DDTD RDT and GADx RDT, but their diagnostic accuracy has not been fully evaluated using meth

Frequency of Mutations Within P-Glycoprotein and Beta-Tubulin Associated With Ivermectin Treatment in Onchocerca volvulus Adult Female Worm Populations in Some Onchocerciasis Endemic Communities in Ghana.

BACKGROUND: Ivermectin is the drug of choice for the control of onchocerciasis because of its potent embryostatic and microfilaricidal effect. However, with long periods of mass drug administration, the effect of ivermectin on the Onchocerca volvulus worms is waning. These suboptimal responses have been linked to genes such as p-glycoprotein (P-gp) and beta-tubulin (B-tub). The purpose of this study was to define the proportion of adult female O. volvulus from some endemic communities in Ghana that have mutations in these genes. METHODS: O. volvulus adult female worms were obtained from 33 pa

Sharon Operation under Spinal Anesthesia for Recurrent Sigmoid Volvulus: A Letter to the Editor.

The Sharon operation is useful for recurrent sigmoid volvulus in high-risk patients because of its minimal invasiveness and short operative time. We report the case of a patient with recurrent sigmoid volvulus who was at high risk during general anesthesia and successfully underwent the Sharon operation under spinal anesthesia. This case suggests that spinal anesthesia may be a feasible option for selected patients undergoing this procedure.

Percutaneous endoscopic sigmoidopexy for recurrent sigmoid volvulus in high-risk adults: a systematic review.

BACKGROUND: Recurrent sigmoid volvulus in frail, comorbid adults recurs frequently after detorsion and carries significant risks with resectional surgery. Percutaneous endoscopic sigmoidopexy (PES), defined as fixation of the sigmoid colon to the abdominal wall without an indwelling tube, has been proposed as a tube-free, minimally invasive alternative. OBJECTIVE: To synthesise outcomes of strict PES (fixation-only; no tube) for recurrent sigmoid volvulus in adults at high surgical risk. METHODS: PRISMA-guided review (2000-31 July 2025) registered prospectively with PROSPERO CRD420251111664.

Laparoscopic vs Open Ladd's Procedure: A Multicenter Study of Postoperative Volvulus and Small Bowel Obstruction Rate.

BACKGROUND: Whether operative approach influences reoperation for postoperative volvulus or small bowel obstruction (SBO) after Ladd's procedure remains uncertain, with limited contemporary multicenter data outside administrative databases. We evaluated whether open vs laparoscopic Ladd's procedure was associated with differences in reoperation for postoperative volvulus and SBO. STUDY DESIGN: This multicenter retrospective cohort included patients ≤15 years of age undergoing Ladd's procedure from January 2014 through September 2021 across 10 children's hospitals. Clinical applications of gastric point-of-care ultrasound in adult emergency and critical care: a scoping review.

BACKGROUND AND OBJECTIVES: Point-of-care ultrasound (PoCUS) is a cornerstone of bedside assessment in emergency and critical care, providing rapid, radiation-free visualization of anatomy and pathology. Gastric PoCUS offers a non-invasive method to assess gastric content, volume, and motility, informing management of aspiration risk, airway interventions, and enteral feeding. Despite supporting evidence, its use remains limited and poorly standardized. The objective of this scoping review is to map the clinical applications of gastric PoCUS in adult emergency department (ED) and intensive care
