Adult intestinal malrotation: A rare incidental finding in trauma imaging.

Intestinal malrotation is a congenital anomaly resulting from abnormal rotation and fixation of the midgut during embryonic development. It is typically diagnosed in infancy, and its detection in adults-particularly in asymptomatic individuals-is rare. In adults, clinical presentation ranges from chronic, nonspecific symptoms to acute intestinal obstruction and midgut volvulus. A 36-year-old woman presented to the emergency department following a motor vehicle collision. She reported no gastrointestinal complaints and denied any history of recurrent abdominal pain, bilious vomiting, abdominal

Anatomical Correction of Adult Intestinal Malrotation Using Kareem's Procedure: A Case Report.

BACKGROUND: Intestinal malrotation (IM) is a congenital defect in midgut rotation or fixation that results in abnormal positioning of the bowel. Although most cases present during infancy, adult presentation is uncommon and may be misdiagnosed. Kareem's procedure is an anatomical corrective approach described for gut malrotation; however, evidence regarding its use remains limited. CASE PRESENTATION: A 26-year-old female presented with sudden, severe, colicky central abdominal pain associated with non-bilious vomiting and complete constipation for 10 hours. She had experienced a similar self-

Mapping Colorectal Practice Patterns in a Community Hospital: A Correspondence Analysis Approach.

BACKGROUND: This single-surgeon, single-institution study aims to show the association between perioperative variables and surgical technique in colorectal surgery, assess discrepancies in patient management relative to established guidelines, and identify underlying causes for improvement. Because the findings reflect one surgeon's practice at one hospital, they are intended as a reproducible audit methodology for single providers rather than as generalizable clinical guidance. METHODS: A total of 171 patients underwent colorectal surgery at The Brooklyn Hospital between 2017 and 2024, inclu

Gallbladder Volvulus Mimicking Immune Checkpoint Inhibitor-Related Acalculous Cholecystitis: A Case Report.

Gallbladder volvulus (GV) is a rare surgical emergency whose clinical and radiological presentation closely mimics acute cholecystitis. This diagnostic difficulty is amplified in patients receiving immune checkpoint inhibitors (ICIs), as gallbladder inflammation may initially be misattributed to an immune-related adverse event (irAE). We report the case of an 82-year-old woman with microsatellite instability-high (MSI-H) duodenal adenocarcinoma treated with pembrolizumab. Following a temporary discontinuation of immunotherapy and initiation of corticosteroids for a suspected gastrointestinal i

Case Report: Lithium-induced bradycardia triggered by cecal volvulus: a case of obstruction-associated drug accumulation.

BACKGROUND: Lithium toxicity can rarely manifest as cardiac arrhythmias, often initially presenting as bradycardia. Bowel obstruction is a rare cause of acute lithium toxicity. We present a case of lithium-induced symptomatic bradycardia induced by cecal volvulus. CASE REPORT: An 84-year-old female with a history of bipolar 1 disorder on chronic lithium therapy was admitted for symptomatic bradycardia and abdominal pain. She also presented with myoclonus, polydipsia, and encephalopathy. Laboratory evaluation revealed lithium toxicity in the setting of renal impairment, and abdominal imaging f

[Small bowel volvulus due to duodenal malrotation].

Malrotation of the intestine is a congenital disorder that often presents in early childhood with small bowel volvulus. In this case, we present an 86-year-old patient with three previous episodes of small bowel volvulus and frequent episodes of severe abdominal pain. On reexamination of previous CT scans, isolated duodenal malrotation with the duodeno-jejunal junction being located to the right of the midline was detected. A modified Ladds procedure was performed, whereafter the patient remained symptom-free.

A rare case of gallbladder torsion with incidental stage IIIB gallbladder adenocarcinoma.

Gallbladder torsion is rare and can mimic acute cholecystitis. We report an 83-year-old female who presented with abdominal pain, leukocytosis, and hyperbilirubinemia. Work-up showed a distended, thick-walled gallbladder with stones and suspected choledocholithiasis. Because of worsening pain and concern for impending perforation, she underwent urgent cholecystectomy, which revealed a freely mobile gallbladder consistent with torsion. Final pathology showed a 0.4 cm poorly differentiated adenocarcinoma with metastatic involvement of a cystic duct lymph node (stage IIIB). Postoperatively, she u

Outcomes for operative and nonoperative management for gastrointestinal emergencies.

BACKGROUND: Gastrointestinal (GI) conditions requiring emergency laparotomy are common, but it has been increasingly recognised that many patients never undergo surgery. This study used electronic healthcare records (EHRs) to provide an insight into the number of patients admitted with GI emergencies at a population level. In this paper we present condition specific outcomes for nine common diagnoses that may require emergency laparotomy. METHODS: A single-centre retrospective cohort study was conducted at a tertiary public hospital (1 December 2013-31 January 2020). Patients were identified

Small Bowel Volvulus Diagnosed and Treated Using Intraoperative Angiography in a Hybrid Operating Room: A Case Report.

INTRODUCTION: Small bowel volvulus is a surgical emergency that may cause mesenteric ischemia; however, diagnosis may be difficult when bowel perfusion appears preserved. CASE PRESENTATION: An 84-year-old man presented with recurrent postprandial abdominal pain and melena 6 months after a total pharyngolaryngectomy with a jejunal free flap reconstruction. Extensive endoscopic evaluations, including capsule endoscopy, revealed no bleeding source. Contrast-enhanced CT revealed the whirl sign at the root of the small-bowel mesentery with the narrowing of the superior mesenteric artery (SMA), whi

Spleen-Preserving Repair of Acute Gastric Volvulus With Wandering Spleen: Anterior Gastropexy and Splenopexy in a 20-Year-Old Man.

Gastric volvulus is an uncommon condition in which the stomach twists on itself, and its association with wandering spleen is attributed to a shared defect of deficient peritoneal fixation. Both conditions are described predominantly in children, and reports of the combination in adults remain rare. When it does occur, the goal of management is often definitive fixation of the affected organs, yet there is limited guidance on the optimal approach in adults, particularly regarding splenic preservation. We present a rare case of gastric volvulus with wandering spleen in a 20-year-old man, manage

Acute mesenteric ischemia caused by adhesive-band-induced mesenteric artery compression with secondary ileal volvulus and internal hernia: a rare case report.

INTRODUCTION: Acute mesenteric ischemia (AMI) is a surgical emergency caused by inadequate blood flow through the mesenteric vessels, resulting in intestinal ischemia and eventual gangrene of the bowel wall. The condition may result from arterial thrombosis, embolism, mesenteric venous thrombosis, or non-occlusive systemic hypoperfusion. CASE PRESENTATION: We report a rare case of AMI in a 73-year-old female with a significant past surgical history of transabdominal hysterectomy with bilateral salpingo-oopherectomy. She presented with sudden, severe abdominal pain that was disproportionate to

Meckel's diverticulum leading to axial torsion and volvulus of the small bowel.

A Meckel's diverticulum is a blind-ending pouch that arises on the antimesenteric border of the terminal ileum. It is the most common gastrointestinal anomaly and the result of an incomplete obliteration of the vitelline duct during the first 8 weeks of gestation. The 'rule of two' is a mnemonic which refers to key characteristics of Meckel diverticula: it is found in 2% of the population, it is 2 inches long, 2 feet from the ileocaecal valve and twice as common in males. Only 2%-4% of those with a Meckel's develop complications in their lifetime. The majority present in early childhood with c

Giant ectopic splenic volvulus: a rare surgical emergency.

BACKGROUND: Splenic volvulus is a rare condition most often associated with a wandering spleen (WS) due to congenital or acquired ligamentous laxity. A giant ectopic splenic volvulus in adults without identifiable risk factors is exceptionally rare. CASE PRESENTATION: We report the case of a 46-year-old woman with no significant past medical history who presented with acute abdominal pain. Computed tomography (CT) revealed a giant ectopic spleen measuring approximately 20 cm, with torsion of the splenic pedicle, consistent with splenic volvulus. Emergency surgery was performed. Intraoperative

Jejunal mesenteric cyst presenting as acute midgut volvulus in a child: a rare cause of surgical abdomen.

INTRODUCTION: Mesenteric cysts are defined as benign intra-abdominal tumors located in the mesentery. Mesenteric cysts are rare intra-abdominal lesions in children and are often asymptomatic. Acute midgut volvulus secondary to a mesenteric cyst is an exceptionally uncommon presentation that requires prompt surgical intervention to prevent bowel ischemia. CASE PRESENTATION: An 8-year-old boy was referred from the pediatric department with acute abdominal pain and recurrent bilious vomiting. Clinical examination revealed around 20 × 20 cm size cystic lump involving whole abdomen. Contrast-enhan

Giant Jejunal Diverticulosis Causing Acute Mesenteric Volvulus: a Case Report.

BACKGROUND: Large jejunal diverticula are usually asymptomatic but can cause rare, life-threatening complications including obstruction, perforation, and bleeding. Secondary mesenteric volvulus caused by jejunal diverticulosis is exceptionally rare. It should be strongly considered when computed tomography (CT) reveals a mesenteric "whirl sign" in an adult presenting with acute obstructive symptoms. CASE PRESENTATION: A 76-year-old female presented to the emergency department with acute, diffuse abdominal pain and clinical evidence of mechanical intestinal obstruction. A CT scan demonstrated
