Comparative Outcomes of Resection: Primary Anastomosis and Hartmann's Procedure in the Management of Sigmoid Volvulus.

One major cause of large bowel blockage is sigmoid volvulus, which in more complex instances necessitates surgical intervention. Despite the widespread use of two basic surgical techniques-resection-primary anastomosis and Hartmann's procedure- comparative results in settings with limited resources, such as Bangladesh, are still not well understood. This study compared the outcomes of Hartmann's procedure and resection-primary anastomosis in sigmoid volvulus. From September 2018 to August 2019, the Department of Surgery at Mymensingh Medical College Hospital in Bangladesh was the site of this

An Interesting Endoscopic Description of Borderline Ischemia in Sigmoid Volvulus: The "Fallen Leaves in Autumn" Sign.

Sigmoid volvulus (SV) is an uncommon cause of intestinal obstruction resulting from axial twisting of the sigmoid colon around its mesenteric base. In the absence of complications, endoscopic detorsion is considered the first-line treatment, whereas urgent surgery is required in cases of perforation, peritonitis, or failed detorsion (1). The "coffee bean sign" (massively dilated sigmoid loop directed to the right upper quadrant) and the "whirl sign" (twisting of mesentery and vessels) are well-established radiological findings of SV (2). By contrast, endoscopic features are less well character

Nursing Management of Repeated Extubation Failure Following Colonic Volvulus Surgery in a Patient with Alstrom Syndrome: A Case Report.

BACKGROUND: Alstrom syndrome is a rare genetic disorder characterized by multi-system involvement, including metabolic, sensory, and cardiorespiratory impairments. Patients undergoing major surgery are at high risk of respiratory complications, though reports of repeated extubation failure are scarce. CASE PRESENTATION: An 18-year-old male with a previous diagnosis of Alström syndrome was admitted for acute abdominal pain and diagnosed with colonic volvulus. An emergency exploratory laparotomy was performed, which included the reduction and decompression of the colonic volvulus, followed by a

Cecal bascule in a patient with complete situs inversus-a rare cause of left upper quadrant abdominal pain.

Cecal volvulus is a rare cause of large bowel obstruction with cecal bascule being a rare cause of cecal volvulus. We present a case of cecal bascule in a 27-year-old female with Kartagener's syndrome and complete situs inversus with no history of prior abdominal surgeries. Computed tomography imaging revealed complete heterotaxia of the intra-abdominal organs with displacement of the cecum to the left upper quadrant with anteromedial rotation concerning for cecal bascule. The patient was taken urgently to the operating room where an upper midline laparotomy was performed. The cecum was found

Organoaxial gastric volvulus secondary to hiatal hernia successfully treated with laparoscopic surgery: a case report.

Gastric volvulus is a rare but potentially life-threatening condition characterized by abnormal rotation of the stomach. Early diagnosis and surgical management are essential to prevent ischemia and necrosis. We report the case of a 56-year-old woman presenting with abdominal pain and vomiting. Contrast-enhanced computed tomography demonstrated a markedly distended stomach with abnormal orientation consistent with organoaxial gastric volvulus. After initial decompression with a nasogastric tube, the patient underwent laparoscopic exploration. Intraoperatively, the proximal stomach was found he

Gallbladder volvulus in an elderly patient presenting as acute calculous cholecystitis: a case report.

INTRODUCTION AND IMPORTANCE: Gallbladder volvulus is a rare, life-threatening emergency resulting from torsion of the gallbladder on its vascular pedicle. Its rarity and clinical similarity to acute cholecystitis make diagnosis difficult. This is the first documented case in the Philippines, highlighting its significance for local surgical practice. PRESENTATION OF CASE: A 74-year-old woman presented with acute right upper quadrant pain, nausea, vomiting, and bloating. Laboratory workup revealed leukocytosis with neutrophilia, and imaging suggested acute calculous cholecystitis. At surgery, c

Clinical Outcomes After Failed Endoscopic Detorsion for Sigmoid Volvulus: A Single-Center Retrospective Cohort Study.

OBJECTIVES: Endoscopic detorsion is the first-line treatment for sigmoid volvulus (SV); however, detorsion is not always successful. We evaluated clinical outcomes after failed endoscopic detorsion and assessed recurrence in patients who achieved clinical success after decompression. METHODS: This single-center retrospective cohort study included 47 patients with first-episode SV, of whom 43 underwent endoscopic management. Baseline characteristics and short-term outcomes were compared between successful and failed detorsion groups. Among patients with clinical success, the in-hospital course

Caught in the Whirl: Computed Tomography Diagnosis of Transverse Colon Volvulus With Proximal Jejunal Involvement and Cecocolic Pneumatosis.

Transverse colon volvulus is a rare cause of mechanical bowel obstruction, and preoperative diagnosis may be challenging because clinical and radiographic findings are often nonspecific. Computed tomography (CT) plays a central role in identifying the transition point, mesenteric whirl sign, associated bowel involvement, and signs of bowel compromise. We report the case of a 65-year-old man with chronic obstructive pulmonary disease who presented with a five-day history of bowel obstruction, including absence of stool and flatus and vomiting. Upright abdominal radiography showed colonic air-fl

A rare case of midgut reverse rotation presenting with cecal volvulus in a young adult.

INTRODUCTION AND IMPORTANCE: Congenital malrotation is a disease that almost exclusively affects newborns. When it is discovered in an adult, which happens seldom, it is usually linked to a protracted history of gastrointestinal problems. Reversed rotation of the midgut is the rarest form of midgut malrotations. Small intestinal obstruction caused by volvulus or peritoneal bands is a common consequence of these conditions that typically manifest in the early stages of infancy. CASE PRESENTATION: Here, we present a case of 20-year-old young adult male presented with clinical signs and symptoms

Intussusception in Neonates: Uncommon Causes and Diagnostic Dilemmas in Two Case Reports.

INTRODUCTION: Neonatal intussusception (NI), although a rare cause of bowel obstruction, presents significant diagnostic challenges due to its clinical heterogeneity. This report describes two cases of NI, both presenting with vomiting and hematochezia. In both instances, the initial radiological evaluations led to a misdiagnosis, delaying timely treatment. Surgical intervention ultimately confirmed the diagnosis in both cases, highlighting the limitations of imaging modalities in neonates. CASE PRESENTATIONS: In the first case, a full-term female neonate presented with bilious vomiting and r

Clinical outcomes and treatment costs of endoscopic-first strategy compared with emergency surgery for sigmoid volvulus in a multicenter retrospective study.

This study aimed to compare clinical outcomes and treatment costs between emergency surgery and an endoscopic-first strategy, including endoscopic decompression followed by elective surgery when feasible, in patients with sigmoid volvulus (SV). This retrospective cohort study enrolled 118 patients with SV treated at two centers between January 2019 and December 2024. Patients were categorized according to their initial treatment strategy into an emergency surgery group (n = 58) and an endoscopic-first group (n = 60). Outcomes included ICU admission, overall complications, stoma creation, any s

Computed tomography findings and surgical outcomes in acute mesenteric ischemia: a retrospective single-center cohort study.

BACKGROUND: Acute mesenteric ischemia (AMI) is a time-dependent condition associated with high mortality, frequently related to diagnostic delay and progression to irreversible bowel necrosis. Computed tomography (CT) plays a pivotal role not only in diagnosis but also in guiding therapeutic decision-making. However, the prognostic role of individual CT findings and their relationship with irreversible ischemic injury remain incompletely defined. This study evaluated predefined CT findings, AMI subtype, treatment pathways, intraoperative infarction or necrosis, and mortality in a real-world co

Intestinal failure-associated liver disease in a child with short bowel syndrome: management issues.

Intestinal failure (IF) refers to a state where the absorptive surface of the small intestine is inadequate to maintain water and electrolyte balance and to meet nutritional demands for optimal growth in children. We describe a 2-month-old infant with short bowel syndrome (SBS) and intestinal failure-associated liver disease (IFALD) following surgical resection for intestinal malrotation and midgut volvulus. He required prolonged total parenteral nutrition (TPN), which resulted in cholestasis and progressive liver dysfunction. Management included ursodeoxycholic acid, amino acid-based continuo

Preoperative Neutrophil-to-Lymphocyte Ratio is Associated with Early Postoperative Pain and Opioid Consumption after Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume represents a measurable variable with a relevant impact on effectiveness of health care. A recent Italian law, the "spending review", calls for the definition of "qualitative, structural, technological and quantitative standards of hospital care". There is a need for an accurate evaluation of the available scientific evidence in order to identify these standards, including the volume of care above or below which the public and private hospitals may be accredi

Delayed diagnosis and surgical intervention of intestinal malrotation can lead to unresolved gastrointestinal symptoms: Findings from a patient-reported registry.

INTRODUCTION: Intestinal malrotation is a rare congenital anomaly with highly variable clinical presentations and potentially devastating consequences when diagnosis is missed. Surgical intervention is considered definitive management, yet many patients continue to experience gastrointestinal symptoms after surgical intervention. This study examined whether time to diagnosis or factors related to surgical treatment were associated with unresolved gastrointestinal symptoms. METHODS: We used patient-generated data from the national IMPOWER registry. We categorized registrants by duration of sym

Phage Therapy for a Refractory and Multidrug-resistant Equine Surgical Site Infection.

A 14-year-old American Quarter Horse mare developed a refractory deep surgical site infection following emergency celiotomy for acute right dorsal large colon displacement with 180° volvulus. Initial cultures revealed a high-burden polymicrobial infection with Escherichia coli and Enterococcus faecium, associated with delayed healing, and later followed by multidrug-resistant Pseudomonas aeruginosa, further limiting therapeutic options. Phage therapy was initiated using a customized lytic phage cocktail administered topically on a daily basis over a 35-day period, with cocktail composition gui
