Is percutaneous sigmoidopexy a useful alternative to high surgical risk patients to treat sigmoid volvulus?

This article presents two cases of elderly patients with recurrent sigmoid volvulus and significant comorbidities, making them unsuitable for surgery. Instead, percutaneous sigmoidopexy assisted by colonoscopy was performed. The procedure involved percutaneous puncture and fixation using T-sutures and endoscopic clips. Both patients were discharged without complications and experienced no recurrence of volvulus six months post-procedure. The article highlights the need for further research to validate this method as an alternative to surgery for high-risk patients.

Gastric volvulus, a surgical or endoscopic emergency?

We present the case of a 62 year old male who presented to the emergency room with abdominal pain and vomiting in coffee grounds. In the diagnostic study, a gastric volvulus was detected and initial endoscopic management was decided. He presented a torpid clinical evolution, which eventually required surgical intervention as the final treatment. We highlight gastric volvulus as a rare urgent pathology, whose optimal management is still to be defined.

The Mesenteric Whirl Sign: A Surgical Decision Signal for Bowel-Threatening Mechanical Disease.

The mesenteric whirl sign on computed tomography (CT) should be approached as a surgical decision signal for potentially bowel-threatening mechanical disease rather than as an isolated radiologic label. Not every whirl pattern represents fixed active volvulus; however, underestimating a true whirl sign can delay recognition of closed-loop obstruction, internal hernia, strangulation, bowel necrosis, perforation, sepsis, and death. This review reframes the sign around the management question most relevant to general surgeons: when should imaging evidence of mesenteric rotation alter care before

A Twisted Presentation: Adult Intestinal Malrotation Complicated by Caecal Volvulus and Perforation.

Intestinal malrotation is a congenital anomaly caused by incomplete rotation and fixation of the midgut during embryological development. Although it is usually diagnosed in infancy, rare adult cases may present with acute intestinal obstruction or perforation. Cecal volvulus is an uncommon but potentially life-threatening complication of malrotation. We report a 35-year-old woman who presented with severe abdominal pain, progressive distension, and absolute constipation for two days. Clinical examination revealed generalized peritonitis with absent bowel sounds, and an erect radiograph demons

Adult Onset Hirschsprung's Disease With Overlapping Presentation of Sigmoid Volvulus: Two Case Reports With Literature Review at Tikur Ambessa Specialized Hospital, AAU, Ethiopia.

Adult-onset Hirschsprung's disease (HSD) is uncommon and frequently leads to diagnostic difficulty after infancy. It is a congenital illness that causes intestinal obstruction due to a lack of ganglion cells. Approximately 1 in 5000 live newborns have this illness, and 95% of HSD cases are identified in infants. This case report will fill a knowledge gap at a low-resource setting where published papers on this topic are extremely rare, and will provide insight into diagnosis and surgical management options that are feasible in resource-limited environments with good outcomes. In this case repo

Sigmoid Volvulus in a Young Adult With Cerebral Palsy: A Case Managed by the Paul-Mikulicz Procedure.

A 25-year-old male with cerebral palsy presented with sigmoid volvulus complicated by ischemia and fecal impaction-an underrecognized but serious risk in this population. Emergency Paul-Mikulicz colostomy achieved safe resection. Clinicians should suspect volvulus early in CP patients, and proactive bowel management is essential.

Adult intestinal malrotation: imaging findings.

Intestinal malrotation is the most common congenital anomaly of the small intestine. However, it is associated with delayed diagnosis due to the lack of specificity of its symptoms, which can lead to devastating consequences such as intestinal volvulus or massive intestinal necrosis. We present a clinical case in which we highlight the importance of abdominal computed tomography and the detection of its characteristic signs for the early identification of this pathology.

Exploring Neural Mechanisms Underlying Error-related Impairments in Active Working Memory Suggests an Adaptive Shielding of Contents during Cognitive Control.

Goal-directed behavior relies on cognitive flexibility-the ability to rapidly adapt ongoing thoughts and behaviors while preserving task-relevant information. The performance monitoring system optimizes such behavior by detecting and evaluating errors, whereas the working memory (WM) system maintains relevant information and protects it from interference. We previously showed that motor errors impaired active WM maintenance (Error-Related Impairment of Active working Memory [ERIAM]; [Wessel, J. R., Jiang, J., & Stolley, J. J. Effect of Adhesion Barrier Use During Ladd Procedure for Intestinal Malrotation on Postoperative Midgut Volvulus and Postoperative Small-Bowel Obstruction: A Retrospective Cohort Study Using a National Inpatient Database.

BACKGROUND: Postoperative midgut volvulus is a life-threatening complication after the Ladd procedure. An adhesion barrier to prevent postoperative small-bowel obstruction could increase postoperative midgut volvulus incidence due to reduced adhesions. We aimed to examine the effect of adhesion barrier use during Ladd procedures on postoperative midgut volvulus and postoperative small-bowel obstruction. STUDY DESIGN: We retrospectively identified patients younger than 18 years who underwent the Ladd procedure between July 2010 and March 2021, using a Japanese nationwide inpatient database. We

Nutritional risk versus established malnutrition in older adults: metabolic considerations for the association with depression.

UNLABELLED: In human cells TORC1 mTOR (target of rapamycin) protein kinase complex renders heat shock transcription factor 1 (Hsf1) competent for stress activation. In such cells, as well as in yeast, the selective TORC1 inhibitor rapamycin blocks this activation in contrast to Hsp90 inhibitors which potently activate Hsf1. Potentially therefore rapamycin could prevent the Hsf1 activation that frequently compromises the efficiency of Hsp90 inhibitor cancer drugs. Little synergy was found between the effects of rapamycin and the Hsp90 inhibitor radicicol on yeast growth. However certain rapamyc

Twisted Mystery: Identifying Gallbladder Volvulus: Challenges in Preoperative Diagnosis.

BACKGROUND Gallbladder volvulus (GV) is a rare surgical emergency caused by the twisting of the gallbladder on its mesentery, leading to vascular compromise and potential necrosis. It primarily affects elderly women but can occur at any age or in either sex. Excessive gallbladder mobility is a key predisposing factor, and the condition often mimics acute cholecystitis, making early diagnosis challenging. Timely recognition is crucial to prevent severe complications. Given its rarity and frequent diagnostic uncertainty, each reported case provides valuable insight, and the present report contri

[Bloody stool and bilious vomiting - Volvulus in non-rotation].

A volvulus in cases of non-rotation in newborns is a neonatal surgical emergency that is associated with high mortality if there is a delay in diagnosis and therapy. In addition to clinical signs such as bilious vomiting, bloody stools, a tender abdomen, and a lethargic patient, the diagnosis can be quickly made by sonographic evidence showing the "whirlpool sign," allowing for the initiation of surgical therapy. We report on a full-term male newborn who was unremarkable at the routine U2 examination except for a weight loss of just under 10%. On the third day of life, the newborn began biliou

Surgical Management of Sigmoid Volvulus: A Retrospective Review of Six Cases with a Focus on the Sharon Operation.

INTRODUCTION: Sigmoid volvulus is a clinically significant cause of large bowel obstruction that occurs particularly in older patients with a reduced physical function. Although endoscopic detorsion is the first-line treatment, volvulus recurrence is common and often requires elective surgery. Despite various reports on the surgical procedures, the optimal approach remains controversial. The Sharon operation, first introduced in 1985, is a minimally invasive technique that avoids mobilization of the sigmoid colon via a small incision in the left lower quadrant. Although it offers several advan

Mesenteric Venous Malformation Case Report in an Adolescent With Midgut Volvulus.

INTRODUCTION: Although the reported incidence of congenital vascular malformations is ~1.5% of the general population, the true incidence of these lesions is difficult to assess due to the heterogeneity of vascular anomalies and the variability in terminology used in reporting. These vascular anomalies can involve capillaries, lymphatics, venous, and/or arterial structures and can occur anywhere in the body. Rarely does a vascular malformation originate from the gastrointestinal (GI) mesentery and present as a bowel obstruction. CASE REPORT: This report describes an adolescent patient with an

Multidisciplinary artificial intelligence systems versus single-model approaches for the diagnosis and management of ileus and volvulus.

BACKGROUND AND AIMS: The accurate and timely diagnosis of ileus versus volvulus is essential in emergency care, as treatment choices directly influence patient outcomes. In this study, the diagnostic accuracy and adherence to guidelines of multidisciplinary AI systems were compared with those of single-model approaches and actual clinical decisions in managing these acute gastrointestinal conditions. METHODS: We conducted a retrospective analysis of 234 adult patients diagnosed with ileus (n = 120, 51. 3%) or volvulus (n = 114, 48. 7%) from January 2018 to December 2024. We assessed three app

Acute intestinal obstruction caused by multifocal nodular inflammatory disease of the small intestine: a case report and literature review.

BACKGROUND: Multifocal nodular inflammatory disease of the small intestine is a rare cause of acute intestinal obstruction. We report such a case to discuss its diagnostic approach, emphasizing the role of surgical intervention and pathological examination, thereby aiming to improve clinical recognition of this condition. CLINICAL PRESENTATION: We analyzed the clinical data of a 33-year-old male patient who was admitted with intermittent abdominal pain and distension for one week, which aggravated over one day. Emergency computed tomography (CT) revealed obstruction of the proximal jejunum wit

Paediatric caecal volvulus, a rare presentation of african degenerative leiomyopathy - a case report.

BACKGROUND: Caecal volvulus is a rare condition with an unknown prevalence, particularly in paediatric patients. Predisposing factors include fixation anomalies of the colon (with or without malrotation) and significant intestinal distension due to conditions such as chronic constipation, post-operative ileus, Hirschsprung disease (HD), or paediatric intestinal pseudo-obstruction (PIPO). African degenerative leiomyopathy (ADL) is a regional variant of visceral myopathy characterised by a fibrotic "tiger-striped" degeneration of the muscular layers of the colon, which causes PIPO. It is endemic
