Recurrent sigmoid volvulus in pregnancy.

Intestinal volvulus during pregnancy is a rare and potentially severe cause of bowel obstruction, associated with high maternal and fetal morbidity and mortality. Diagnosis is challenging due to nonspecific symptoms, requiring a high index of suspicion. We report a primigravida in her late 30s diagnosed with sigmoid volvulus at 25 weeks of gestation, initially treated successfully with endoscopic decompression. Recurrence occurred at 36 weeks, leading to an urgent caesarean section with sigmoidopexy, achieving excellent maternal and fetal outcomes. This case emphasises the diagnostic challenge

Case Report: Midgut malrotation presenting as left-sided appendicitis with secondary omental torsion: a diagnostic challenge in a child.

BACKGROUND: Omental torsion is an uncommon cause of acute abdomen in children and is usually secondary to other intra-abdominal pathology. Congenital midgut malrotation can alter anatomic relationships and lead to atypical presentations. This case report aims to highlight that congenital malrotation should be considered when left-sided appendicitis is suspected, especially when omental abnormalities are present. CASE PRESENTATION: We report a 12-year-old child who presented to a tertiary children's hospital in Southwest China with more than 48 h of abdominal pain and 24 h of fever after heavy

Ischaemic giant Meckel's diverticulum causing small bowel obstruction.

Meckel's diverticulum (MD) is the most common congenital abnormality of the small bowel, present in about 2% of the population. While typically asymptomatic, MD can cause complications such as small bowel obstruction (SBO), particularly in adults. Giant MD, defined as a diverticulum larger than 5 cm, is a rare cause of SBO and may lead to torsion, volvulus, or kinking of adjacent bowel. Preoperative diagnosis is often challenging as imaging findings are nonspecific, and MD may be misdiagnosed unless complications like inflammation or torsion occur. Surgical intervention, usually resection, is

Hyperinsulinemia After Extensive Small Bowel Resection and Total Parenteral Nutrition in a Neonate: A Case Study.

BACKGROUND: Neonatal hyperinsulinemia is a rare condition, but poses a significant increase in the risk of recurrent and persistent hypoglycemia in newborns, which can lead to long-term neurodevelopmental complications if not identified early. This condition may be due to a congenital disorder, or arise transiently due to perinatal stress, and may develop as a consequence of interventions such as total parenteral nutrition (TPN). Infants diagnosed with SBO following extensive intestinal resection are particularly at higher risk. CASE PRESENTATION: This report is of a 2-week-old male neonate,

Prophylactic appendectomy during Ladd's procedure: a multicenter survey of institutional approaches and their rationales in Japan.

PURPOSE: To describe institutional approaches to prophylactic appendectomy during Ladd's procedure in Japan, including rationales and prespecified exceptions. METHODS: We conducted a cross-sectional questionnaire survey through the Japanese Society of Pediatric Surgeons Under-45 Working Group. Member institutions reported their usual institutional approach in a standardized index scenario of emergency Ladd's procedure for midgut volvulus in a stable term neonate without intestinal necrosis or perforation. Responses were analyzed descriptively. RESULTS: Forty-six of 50 invited institutions re

[Use of teduglutide in short bowel syndrome in infants, children and adolescents: Position paper of the working group "Chronic intestinal failure" of the Society for Paediatric Gastroenterology and Nutrition (GPGE)].

Paediatric short bowel syndrome (SBS) with chronic intestinal failure (IF) is a rare, complex and potentially life-threatening condition. The main causes in children are necrotizing enterocolitis, volvulus, congenital malformations, and other enteropathies. The resulting reduced intestinal absorption capacity often requires parenteral nutrition (PN), which is associated with complications such as catheter infections, liver disease, and thrombosis, as well as significant limitations in quality of life. The goal of multidisciplinary treatment is to achieve enteral autonomy, prevent complications

Safety and Efficacy of Endoscopic Derotation in Colonic Volvulus Occlusion: Systematic Review and Meta-Analysis.

Background: Sigmoid volvulus is a time-critical cause of large-bowel obstruction. While endoscopic detorsion (ED) is the primary intervention for rapid decompression and the assessment of mucosal viability, reported success, recurrence, and mortality rates vary significantly across the literature, complicating evidence-based clinical decision-making. Methods: A systematic review and meta-analysis were conducted following PRISMA guidelines (protocol submitted to PROSPERO). MEDLINE/PubMed and Embase were searched from inception to 20 October 2025, supplemented by manual reference screening. We i

Idiopathic double-site jejunoileal intussusception with volvulus and transmural necrosis in a young adult: case report.

BACKGROUND: Adult intussusception is a rare condition accounting for less than 5% of all intussusceptions, with over 90% of cases associated with an identifiable pathological lead point. Idiopathic presentations are exceptional. This case is reportable due to its unique constellation of features: simultaneous double-site small bowel intussusception, concurrent volvulus, transmural necrosis, and complete absence of any structural pathology in a previously healthy young adult. CASE PRESENTATION: A 26-year-old male presented with a 3-day history of abdominal pain and vomiting. Examination reveale

Subacute Small Bowel Obstruction in a 50-Year-Old Woman With Complete Intestinal Malrotation: A Case Report.

Intestinal malrotation is a congenital anomaly resulting from the failure of the midgut to undergo its normal 270° counterclockwise rotation during embryological development. While the vast majority of cases are seen in neonates, adult presentation is exceptionally rare and often presents with vague abdominal complaints, leading to significant diagnostic delays. A 50-year-old woman presented with diffuse, colicky abdominal pain and recurrent bilious vomiting for three days. Vital signs revealed hypertension (170/100 mmHg) and tachycardia (107 bpm). The physical examination revealed a soft, but

Congenital ileal stenosis.

Congenital jejunal-ileal atresia (JIA) is a critical neonatal diagnosis, as morbidity and mortality can be high if not treated. JIA usually presents with obstipation, and diagnosis can be established with history and physical exam, radiography, and contrast fluoroscopy. This case demonstrates ileal stenosis, a rare type of JIA, presents with intermediate obstructive symptoms. A 39-week newborn female presented with bilious emesis and dilated loops of bowel on X-ray. Contrast fluoroscopy ruled out volvulus and colonic pathology. Patient had persistent bowel function and had evolving but persist

Long-term surgical outcomes for the surgical management of complete intrathoracic stomach (CIS).

BACKGROUND & AIMS: Surgery is indicated for symptomatic CIS patients to avoid complications (volvulus, obstruction and strangulation) associated with high morbidity and even mortality. There remains limited evidence of the long-term outcomes and improvement of symptoms from this procedure. Previous studies have been unable to compare preoperative symptoms with post-operative data. Although it appears the procedure has favourable outcomes, further data are required to demonstrate a significant improvement in quality of life (QoL). We therefore aim to assess the long-term outcomes, including cha

Mesh Safety Under Contamination Across Incarcerated Hernias: A Single-Center Cohort Analysis With a Systematic Review of Adult Bochdalek Hernia Complicated by Gastric Pathologies.

BackgroundWhether prosthetic mesh increases infection in contaminated fields remains controversial, particularly in incarcerated hernias and adult Bochdalek hernia (BH) with gastric pathology.MethodsWe combined a PRISMA-guided systematic review of adult BH with gastric pathology (14 cases, 1981-2025) with a single-center retrospective cohort of incarcerated hernias treated laparoscopically or laparoscopy-assisted (n = 313; inguinal 177, incisional 111, diaphragmatic 10 [4 BH, 1 Morgagni], and hiatal 15). Gallbladder Volvulus: A Great Mimicker of Acute Cholecystitis.

Gallbladder volvulus is a rare disease process, with approximately 500 reported cases in the current literature. Preoperative diagnosis is challenging, with only about 80 cases reported as being diagnosed prior to surgery, as it is frequently misdiagnosed as acute cholecystitis. Prompt recognition is vital, as the mainstay of treatment is emergent surgical intervention. This case describes an 84-year-old female who presented with right upper quadrant abdominal pain. Initial computed tomography (CT) demonstrated biliary anatomic abnormalities with relatively unremarkable laboratory values, and

Sigmoid Volvulus in a Healthy Adolescent Male.

We report the case of a 15-year-old previously healthy male who presented with several days of progressive abdominal pain, vomiting, and anorexia. On arrival to the emergency department, he was hemodynamically stable. Patient's abdominal exam revealed a soft, benign abdomen with right-sided greater than left-sided tenderness. Cross-sectional imaging demonstrated sigmoid volvulus without evidence of pneumatosis, perforation, or free air. He underwent multidisciplinary evaluation by pediatric surgery and gastroenterology, and urgent endoscopic detorsion was performed. Pediatric sigmoid volvulus

Bochdalek Hernia as a Discrepant Intraoperative Diagnosis in a Paraesophageal Hernia Surgery: A Surgical Case Report.

Bochdalek hernia (BH) is a rare paediatric condition, but late diagnoses occur in adults. It occurs following incomplete posterolateral development of the diaphragm. Adult BH is asymptomatic in most cases, but it has the potential of being life-threatening. This case report is of a 35-year-old female patient who presented to us with a history of childhood abdominal discomfort leading to frequent hospitalisation. Presently, she came with complaints of increasing abdominal pain, postprandial vomiting, and a 31% weight loss over 3 months before the current admission to our facility. A preoperativ

Acute intrathoracic sigmoid volvulus in major congenital diaphragmatic eventration: a rare thoracoabdominal emergency in an 18-year-old.

Acute intrathoracic sigmoid volvulus secondary to major congenital atic eventration in an adult is exceptionally rare. We report the case of an 18-year-old male with lifelong constipation who presented an acute bowel obstruction caused by intra thoracic sigmoid volvulus in major left diaphragmatic eventration. Despite complete functional loss of the left lung, the patient had no significant respiratory complaints. Emergency endoscopic decompression was performed, followed by sigmoidectomy and diaphragm plication, preventing recurrence of volvulus and improving respiratory function. This case h
