The introduction of cystic fibrosis transmembrane conductance regulator modulators (CFTRms) in the treatment of cystic fibrosis (CF) has significantly improved both the life expectancy and quality of life for patients affected by the disease. While this new therapy shows promising results in CF patients, data about the use of CFTRms during pregnancy in women who are carriers with fetuses affected by CF are limited. We present a new case report and literature review of fetal CF treated by the maternal assumption of CFTRms to understand the safety and the effectiveness of the treatment in resolv

BACKGROUND: Small bowel obstruction (SBO) in patients with a virgin abdomen (VA)-no prior abdominal surgery-remains understudied, with traditional assumptions favoring mandatory surgical exploration due to suspected non-adhesive etiologies like malignancy or hernias. However, emerging evidence suggests adhesions are also prevalent in SBO-VA, challenging this paradigm. This study investigates the etiologies, management outcomes, and prognostic factors of SBO-VA to guide evidence-based treatment strategies.
METHODS: A retrospective analysis was conducted on 312 SBO-VA patients treated at a tert

We present a unique case of a 43-year-old woman with a complex bowel obstruction caused by the connecting tubing of a gastric band placed three years prior. The tubing formed a loop with a twist acting as a noose, causing an internal hernia involving the distal ileum. The surrounding adhesions to the transverse colon also caused her mobile cecum to be distended and to be reflected upward. We managed her complex closed-loop bowel obstruction laparoscopically by lysing adhesions and transecting the gastric band tubing. The patient recovered well without sequelae, and the gastric band was later r

INTRODUCTION AND IMPORTANCE: Small bowel volvulus (SBV) is the twisting of a small bowel loop around its mesentery, leading to closed-loop intestinal obstruction. SBV following laparoscopic procedures is a rare complication. Patients often present with non-specific symptoms, creating a diagnostic dilemma and delaying surgical intervention. A high index of suspicion and timely imaging are essential. Understanding the mechanisms, risk factors, clinical presentation, and management is paramount.
CASE PRESENTATION: A 21-year-old woman with a confirmed diagnosis of adult Hirschsprung disease under

A 66-year-old man with type 2 diabetes mellitus and known gastroparesis presented to the emergency department with fatigue, confusion, and hypoglycemia. During hospitalization, he developed worsening abdominal pain and distention. CT revealed marked gastric distention, extensive portal venous gas, and mesenteric air, raising concern for emphysematous gastritis, gastric outlet obstruction, or volvulus. Despite these alarming radiographic findings, the patient remained hemodynamically stable, with no peritoneal signs. Surgical consultation was obtained; however, given his clinical stability and

Acute esophageal necrosis (AEN), or black esophagus, is a rare endoscopic syndrome typically encountered in critically ill patients. AEN associated with gastric volvulus is particularly uncommon and may be underrecognized when volvulus appears clinically asymptomatic. An 87-year-old woman admitted for pelvic fractures was incidentally found on CT to have a large paraesophageal hernia with organoaxial gastric volvulus and suspected gastric outlet obstruction, without gastrointestinal symptoms. She developed acute hypoxemic respiratory failure; during emergent intubation, a large volume of upper

Adult intestinal malrotation is a rare condition and may remain undiagnosed until the occurrence of acute complications. We report the case of a 33-year-old woman presenting with acute abdominal pain. Contrast-enhanced computed tomography (CT) demonstrated an abnormal relationship between the superior mesenteric artery and vein, right-sided small bowel loops, left-sided colonic segments and a mesenteric whirlpool sign, consistent with midgut volvulus associated with intestinal malrotation. Although CT allows reliable identification of intestinal malrotation and midgut volvulus, the precise obs

BACKGROUND Parahiatal hernias (PHHs) are uncommon diaphragmatic defects that can resemble paraesophageal hiatal hernias (PEHs), resulting in a diagnostic challenge and the potential for inappropriate preoperative surgical planning. Preoperative differentiation of PHH from PEH is difficult due to their anatomic proximity to the esophageal hiatus. This report highlights the importance of intraoperative recognition of PHH and the alteration of the surgical plan. Robotic assistance was used to facilitate visualization and repair of the defect. CASE REPORT A 71-year-old female patient presented wit

Primary small intestinal volvulus (PSIV) is a rare cause of acute bowel obstruction in adults and is exceptionally uncommon in the very elderly. It occurs when the small intestine twists around its mesenteric axis without an identifiable secondary cause, such as adhesions or hernias. This case report describes a 91-year-old woman who presented with abdominal pain and minimal rectal bleeding, posing a diagnostic challenge in the context of multiple comorbidities and atypical symptoms. CT imaging demonstrated a persistent "whirlpool sign," raising suspicion for volvulus, and the patient underwen

A 55-year-old woman presented with vomiting and abdominal pain. Physical examination revealed abdominal tenderness, but no rebound tenderness. A computed tomography (CT) scan revealed a distended small intestine called the flower bouquet sign, as well as the fat notch sign. These findings supported the suspicion of adhesive small bowel obstruction (SBO). Emergency surgery confirmed the presence of an adhesive band formed and it was resected. The patient recovered uneventfully. Most cases of SBO are caused by adhesions, followed by internal hernias and volvulus. CT scanning is essential for dia

Gallbladder agenesia is a rare congenital anomaly that is frequently overlooked in clinical practice, often misdiagnosed as chronic sclero-fibrous cholecystitis or cholelithiasis. Failure to recognize it intraoperatively can prompt unnecessary and hazardous exploration of the biliary tree and liver, increasing the risk of iatrogenic injuries.

Acute intestinal obstruction is a common surgical emergency, defined as a partial or complete interruption of the forward flow of intestinal contents. It is associated with significant morbidity and mortality, and timely diagnosis and management are essential. This study aimed to evaluate the causes, clinical presentation, management, and outcomes of patients admitted with intestinal obstruction to Al-Thawra Modern General Hospital, Sana'a, Yemen. A prospective descriptive study was conducted on 105 patients diagnosed with intestinal obstruction between January 1 and December 31, 2024. Patient

Gastric volvulus is a rare but potentially life-threatening complication of a hiatal hernia that can result in gastric ischemia or necrosis if not promptly recognized and treated. A 51-year-old man presented with several hours of severe nausea, vomiting, and chest pain. Physical examination was notable for signs of acute distress, and the patient reported dark-colored emesis. Initial laboratory evaluation demonstrated leukocytosis and elevated serum lactate levels. Computed tomography of the chest and abdomen revealed a large hiatal hernia with herniation of the majority of the stomach into th

OBJECTIVE: To describe associations between abnormalities in splenic tissue removed secondary to gastric dilatation-volvulus (GDV) correction and histopathologic diagnosis of splenic malignancy.
METHODS: Dogs treated with splenectomy secondary to GDV correction at any of 83 US veterinary referral hospitals between January 1, 2013 and May 1, 2025 were retrospectively reviewed. Data related to signalment, visual, and tactile description of the spleen during surgery, and results from the splenic histopathology report were recorded. Descriptive statistics were used to summarize histopathologic di

UNLABELLED: Malrotation is classically a neonatal condition, but it can present in older children. This population has a non-specific presentation that can delay diagnosis with fatal consequences. We aimed to identify presentation patterns and efficacy of imaging modalities to inform the management of children with suspected malrotation. Children aged 1-18 who had surgery for malrotation between 2014 and 2024 at four centres in the south of England were retrospectively reviewed. Hospital presentations, radiology, operative findings and outcomes were reviewed. Seventy-five. patients were identi

BACKGROUND A gallbladder volvulus is a rare and life-threatening emergency requiring immediate surgery to prevent gallbladder perforation and sepsis. A correct preoperative diagnosis is often difficult. CASE REPORT We describe the case of a 93-year-old woman with an acute gallbladder volvulus that was initially misdiagnosed as acalculous cholecystitis. She was started on i.v. antibiotics for conservative management due to a symptomatic, high-grade aortic valve stenosis. After her clinical condition worsened, an emergency cholecystectomy was scheduled. After an interdisciplinary discussion with

A cecal bascule is a specific type of volvulus that confers a high degree of mortality if it is not addressed. Timely and proper diagnosis is critical, as it generally requires surgical correction. Given the rarity of this phenomenon, it is important for clinicians to be aware of this complication to facilitate treatment. In this case report, we discuss the diagnosis and management of a patient who experienced a cecal bascule postoperatively after cardiac surgery. Prompt diagnosis allowed the patient to obtain appropriate treatment, and he was ultimately discharged after a positive outcome.
