Ileosigmoid knotting (ISK) is a rare form of compound volvulus in which ileal loops wrap around the sigmoid colon or vice versa, producing a rapidly progressive closed-loop obstruction with a high risk of ischemia. Prompt computed tomography (CT) recognition is essential, as clinical findings are often non-specific, and deterioration can occur quickly. We report the case of a previously healthy adult who presented with abrupt, severe abdominal pain and progressive distension. CT revealed a double closed-loop configuration involving the distal ileum and sigmoid colon, a prominent whirlpool sign

Hiatal hernias are a common condition, particularly in the elderly. While often asymptomatic, they can lead to life-threatening complications such as incarceration, strangulation, and gastric volvulus. Obstructive shock due to cardiac compression is a rare but critical presentation. We present the case of a 77-year-old Japanese man with a history of renal transplantation who was admitted for conservative management of an incarcerated hiatal hernia. On the second day of admission, he developed profound shock, requiring ICU admission. A malpositioned nasogastric tube had failed to decompress the

BACKGROUND: Sigmoid volvulus stands out as a frequent surgical emergency, particularly in older adults. Initial management centers on prompt detorsion of the affected colon segment, coupled with strategies to avoid future episodes. Preventing recurrence remains a persistent clinical challenge. Advances in endoscopic technology have positioned this modality as a pivotal tool in volvulus care, offering the dual advantage of therapeutic detorsion and diagnostic evaluation of predisposing factors.
METHODS: We prospectively included all patients who presented to the emergency department at Zagazig

The present study describes a rare case of intestinal malrotation in which the common hepatic artery originated from the superior mesenteric artery, focusing on the morphology of the gastrointestinal tract and vascular distribution. The cadaver was that of an 84-year-old woman donated for research and education; the cause of death was acute renal failure, without a history of gastrointestinal disease or abdominal surgery except for gastrostomy. During anatomical dissection, an abnormal colonic position was identified; the entire gastrointestinal tract was resected. The isolated tract was exami

INTRODUCTION: Sigmoid volvulus is a life-threatening condition requiring urgent intervention. Endoscopic detorsion is the preferred initial treatment; however, delayed surgical management in emergent cases may increase complications and mortality. Additionally, early recurrence following successful detorsion remains a significant challenge. This study aims to identify predictors of emergency surgery and early recurrence in patients with sigmoid volvulus.
METHODS: A retrospective analysis was conducted on patients with sigmoid volvulus treated between January 2015 and January 2025. Patients we

Bradycardia and hypotension are well-recognized clinical presentations in the emergency department, and common causes include cardiac, metabolic, or drug-related factors. However, gastrointestinal pathologies are seldom considered within the initial differential diagnosis. We report the case of a 75-year-old man who presented with light-headedness due to life-threatening bradycardia and hypotension. A chest radiograph incidentally revealed marked colonic distension beneath the diaphragm. After stabilizing his vital signs with vasopressors, a computed tomography revealed a sigmoid volvulus. Eme

An 87-year-old woman with no prior history of laparotomy visited her local doctor complaining of anorexia. She was diagnosed with intestinal obstruction and initially managed conservatively with fasting; however, her condition did not improve, and she was referred to our hospital on the seventh day after symptom onset. Abdominal CT revealed a 2-cm cystic lesion on the anterior surface of the sacral pelvic cavity and a whirlpool sign near the end of the ileum. Based on these findings, a diagnosis of small intestinal volvulus and an ovarian cyst-previously noted-was made, and emergency surgery w

We read with great interest the article written by Tapiolas et al. on percutaneous sigmoidopexy (PS) in sigmoid volvulus (SV). Although SV is not an over-rife disease worldwide, Eastern Anatolia is an endemic area of SV. As a result, our 1,101 patients with SV treated over 59.5 years (between June 1966 and January 2026) constitute the most comprehensive single-centre SV series over the world. Based on this clinical practice, our comments relate to PS in SV.

PURPOSE: This study aimed to identify genetic variants contributing to the development of early-diagnosed isolated intestinal malrotation.
METHODS: We conducted Genome sequencing on ten young children diagnosed with midgut volvulus due to intestinal malrotation who presented no other malformations or comorbidities. Our analysis focused on a panel of 442 genes previously associated with intestinal development, malrotation, ciliopathies and/or situs abnormalities.
RESULTS: In one male patent we discovered two heterozygous variants in TTC7A, c.433G > A p.(Ala145Thr) and c.1057G > A p.(Glu353Lys

Meckel's diverticulum (MD) represents a remnant of the embryonic vitelline duct and is recognized as a congenital anomaly of the small intestine. While it is more frequently identified in children, the majority of cases remain clinically silent, with symptomatic presentation in adults being relatively rare. When present, symptoms typically reflect complications, including gastrointestinal hemorrhage, diverticulitis, or intestinal obstruction. The probability of developing complications decreases with advancing age, making clinically significant presentations in elderly individuals uncommon and

Type IV paraesophageal hiatal hernias are rare but carry a significant risk of life-threatening complications, including gastric volvulus, incarceration, and the involvement of other intra-abdominal organs. Pancreatic herniation and associated pancreatitis are exceedingly uncommon. We report the case of a 74-year-old woman with Barrett's esophagus, chronic obstructive pulmonary disease (COPD), and obesity who presented with nausea, vomiting, and abdominal pain after a recent increase in the dose of tirzepatide. Imaging demonstrated a massive type IV paraesophageal hernia with organoaxial gastr

Symptomatic congenital intestinal malrotation during adulthood is a rare condition usually presented as a chronic clinical condition superimposable to an upper intestinal obstruction. Symptoms tend to be nonspecific, varying from intermittent episodes of vomiting, food intolerance, and weight loss, leading to a delayed diagnosis worsening the patient's functional reserve. High clinical suspicion leads to the request of the request of requesting adequate diagnostic imaging, with computerized tomography being the imaging of choice. While Ladd surgery is the standard of care, the approach is cont

Right paraduodenal hernia is a rare congenital internal hernia resulting from abnormal midgut rotation. Although paraduodenal hernias represent the most common type of congenital internal hernia, the right-sided variant is an uncommon cause of small bowel obstruction and is often challenging to diagnose preoperatively. We report the case of a 46-year-old woman who presented with a 48-hour history of acute abdominal pain, nausea, and vomiting, without any prior history of abdominal surgery. Physical examination revealed tenderness in the epigastric and mesogastric regions without signs of perit

BACKGROUND: Omphalocele with a concomitant patent vitelline duct is a rare presentation, with very few cases reported in the literature. Most reported cases present with visible bowel prolapse, meconium drainage or mucosal exposure at birth. Presentations without obvious intestinal herniation or mucosal exposure are extremely uncommon and pose diagnostic challenges. CASE PRESENTATION: A male neonate was born to a G3P2 mother at 38 + 6/7 weeks via spontaneous vaginal delivery after a pregnancy notable for a 20-week prenatal ultrasound concerning for omphalocele. Upon delivery, the omphalocele m

Chronic intestinal pseudo-obstruction (CIPO) is a rare, debilitating gastrointestinal motility disorder characterized by symptoms of intestinal obstruction in the absence of a mechanical cause. Although no definitive treatment currently exists, multidisciplinary therapies such as percutaneous endoscopic transgastric jejunostomy (PEG-J) for intestinal decompression have improved symptom control, enabled the partial resumption of oral intake, and facilitated a transition to outpatient management. We report the case of a 65-year-old woman who was diagnosed with CIPO following emergency surgery fo

Large bowel obstruction (LBO) accounts for approximately one-quarter of all intestinal obstructions. LBO may arise from mechanical obstruction due to benign or malignant etiologies, or from functional obstruction such as acute colonic pseudo-obstruction. Because clinical presentation varies by acuity, etiology, and ileocecal valve competency, early recognition and prompt evaluation are essential to prevent complications, including ischemia and perforation with potential for sepsis and mortality. Patients with acute obstruction typically present with abdominal pain, distension, and obstipation,

BACKGROUND: Dolichocolon (DC), or colonic redundancy, is an elongated and tortuous colon described as early as 1820, yet it remains underrecognized in clinical gastroenterology. Advances in imaging and motility assessment offer new insights into its prevalence, mechanisms, and clinical implications.
AIMS: To summarize current evidence on the anatomy, epidemiology, and potential clinical significance of DC and to explore possible pathophysiological mechanisms linking this variant to gastrointestinal disorders.
METHODS: A targeted literature review of studies published between 1900 and 2024 wa
