INTRODUCTION AND IMPORTANCE: Intestinal malrotation encompasses a spectrum of congenital anomalies resulting from incomplete rotation and fixation of the midgut during embryogenesis. Incomplete common mesentery is the most common form encountered in adulthood and is associated with a narrow mesenteric base, predisposing patients to midgut volvulus due to abnormal peritoneal attachments (Ladd's bands).

Pouch volvulus is a rare but potentially morbid complication after total proctocolectomy with ileal pouch anal anastomosis, typically managed surgically. Evidence for definitive endoscopic therapy is limited. We report 2 patients with ulcerative colitis who developed pouch volvulus after ileal pouch anal anastomosis and were poor surgical candidates due to medical complexity.
