Gallstone ileus due to a visible cholecystoduodenal fistula with pneumobilia CT diagnosis and surgical management: A case report.

Gallstone ileus is an uncommon cause of mechanical small-bowel obstruction, usually occurring in elderly patients with comorbidities. It results from migration of a gallstone through a cholecystoenteric fistula, most commonly a cholecystoduodenal fistula, with subsequent impaction within the bowel lumen. Although computed tomography (CT) is the established imaging modality for diagnosis, direct demonstration of the fistulous tract is not always straightforward in routine emergency imaging. We report the case of a 69-year-old woman with type 2 diabetes mellitus and ischemic heart disease who pr

Gallstone Ileus: A Case Report of an Uncommon Cause of Mechanical Intestinal Obstruction.

Biliary ileus is an uncommon cause of mechanical intestinal obstruction secondary to the passage of gallstones into the gastrointestinal tract through a cholecystoenteric fistula. Its nonspecific clinical presentation contributes to diagnostic and therapeutic delays. Although computed tomography is considered the diagnostic modality of choice, identification of Rigler's triad on plain abdominal radiography may provide an early diagnostic orientation in patients with a compatible clinical presentation. We present the case of a 58-year-old male patient without known biliary disease who presented

Gallstone Ileus With Atypical Gastrointestinal Bleeding: A Case Report and Literature Review.

Gallstone ileus is a rare complication of cholelithiasis. Its diagnosis is challenging due to a nonspecific presentation and high morbidity and mortality related to age and comorbidities. An 87-year-old female with a history of hypertension presented with obstructive abdominal pain, hyporexia, and "coffee-ground" emesis. Upon admission, she showed hemodynamic instability [blood pressure (BP) 83/51 mmHg] and acute renal failure [Creatinine (Cr) 1.34 mg/dL]. Computed tomography revealed Rigler's triad, consisting of small bowel obstruction, neumobilia, and an ectopic gallstone. Exploratory lapar

Gallstone ileus developing two months after endoscopic papillary large balloon dilatation: a case report.

BACKGROUND: Gallstone ileus is a rare clinical entity causing bowel obstruction, and transpapillary migration of gallstones after endoscopic retrograde cholangiopancreatography is exceptionally uncommon. Although endoscopic papillary large balloon dilatation is an effective procedure that dilates the Vater's papilla to remove larger size calculi, gallstone ileus following the procedure as a complication has rarely been reported in the literature. CASE PRESENTATION: We present the case of an 89-year-old Japanese woman diagnosed with acute cholangitis due to choledocholithiasis, exhibiting epig

Ectopic Gallstone Presenting As Small Bowel Obstruction: A Diagnostic Challenge.

Gallstone ileus represents an infrequent etiology of mechanical bowel obstruction and may be difficult to identify because of its nonspecific clinical manifestations. We describe the case of an 80-year-old woman who presented with abdominal pain, nausea, vomiting, and constipation. Computed tomography demonstrated pneumobilia, a cholecystoduodenal fistula, and an ectopic gallstone lodged at the ileocecal valve. Surgical exploration confirmed the diagnosis, and the patient underwent enterotomy with stone extraction, partial cholecystectomy, and fistula repair. Recovery after surgery was favorab

Laparoscopic enterolithotomy for large gallstone ileus in an elderly high-risk patient: a case report.

BACKGROUND: Gallstone ileus is an uncommon complication of cholelithiasis leading to mechanical small bowel obstruction that typically affects geriatric populations with significant baseline medical conditions. The optimal management approach is highly debated, specifically when choosing between simple emergency enterolithotomy alone or a complex one-stage definitive biliary procedure in high-risk patients. CASE PRESENTATION: A 79-year-old male with an ECOG performance status of 2, ASA Class IV risk classification, and known severe ischemic heart disease presented with acute mechanical small

Gallstone Ileus: A Case Report of an Uncommon Cause of Bowel Obstruction in the Emergency Department.

Gallstone ileus is an uncommon cause of mechanical small bowel obstruction that most often affects older adults with comorbid disease and may be difficult to diagnose because symptoms and laboratory findings are frequently nonspecific. We report the case of a 70-year-old man with hypertension, hypercholesterolemia, coronary artery disease, and gastroesophageal reflux disease who presented to the emergency department with progressive abdominal pain, distention, and recurrent non-bilious emesis after being discharged from another facility following symptomatic treatment. On examination, he had a

Gallstone ileus: a single-center experience.

BACKGROUND: Gallstone ileus (GSI) is a rare surgical emergency. Its diagnosis is challenging and frequently delayed. Moreover, there is no consensus regarding the optimal surgical approach. The primary point of debate is whether cholecystectomy and fistula repair should be performed and, if so, when. Several studies have suggested that omitting cholecystectomy and fistula repair may increase the risk of cholangitis, gallbladder cancer, and recurrent gallstone ileus. However, most of these data originate from studies conducted in the previous century and have not been supported by contemporary

Gallstone ileus with spontaneous passage of the obstructing gallstone and resolution of small bowel obstruction: a case report.

Gallstone ileus is a rare cause of mechanical small bowel obstruction that typically occurs in elderly patients with longstanding gallstone disease. It results from migration of a gallstone into the gastrointestinal tract through a biliary-enteric fistula, where it may become impacted within the bowel lumen. We present the case of an elderly female with insulin-dependent diabetes mellitus and heart failure who presented with abdominal pain, nausea and vomiting. Imaging established the diagnosis of gallstone ileus, and prior imaging demonstrated underlying cholelithiasis. Follow-up imaging seve

Gallstone Ileus Diagnosed During Endoscopic Evaluation: A Case Report.

Gallstone ileus is a rare complication of calculous cholecystitis, resulting from the migration of a gallstone through a bilio-digestive fistula into the gastrointestinal tract, where it may cause mechanical obstruction. Diagnosis is often delayed due to a nonspecific clinical presentation. We report the case of a 73-year-old man with a history of unoperated vesicular lithiasis who presented with intense epigastric pain, intractable vomiting, and clinical and biological signs of dehydration. After intravenous rehydration, esophagogastroduodenoscopy revealed a fistulous orifice in the duodenal

Gallstone Ileus After Conservative Management of Acute Cholecystitis and Refusal of Interval Cholecystectomy: A Case Report.

Background and Clinical Significance: Gallstone ileus (GI) is a rare but serious complication of gallstone disease. This condition is characterized by migration of gallstones through a cholecystoenteric fistula, resulting in mechanical bowel obstruction. GI primarily affects older patients with multiple comorbidities. Case Presentation: Herein, we describe a case of 68-year-old Saudi woman with a history of end-stage renal disease, pulmonary hypertension, diabetes mellitus, bronchial asthma, and atrial fibrillation. She presented with acute calculous cholecystitis. Due to her condition, she wa

Case Report: A typical, nearly missed jejunal gallstone ileus.

BACKGROUND: Gallstone ileus is a rare cause of mechanical intestinal obstruction, predominantly affecting older adults. The ileum and ileocecal valve are the most common sites of impaction, whereas jejunal involvement is considerably less frequent. Contrast-enhanced computed tomography (CT) is the primary imaging modality for diagnosis; however, minimally calcified ectopic stones are prone to being overlooked, leading to delayed surgical intervention. CASE PRESENTATION: A 68-year-old woman presented with a 12-day history of abdominal pain, nausea, vomiting, and obstipation. She underwent two

Imaging manifestations of Bouveret's syndrome: A rare case report of gallstone-induced ileus.

Bouveret's syndrome, a rare cholelithiasis complication (gallstone impaction via cholecystoduodenal fistula causing gastric outlet obstruction), is reported in an 81-year-old male with chronic cholecystitis, coronary heart disease, and left nephrectomy. He presented with spasmodic epigastric pain, nausea, and vomiting of coffee-ground-like gastric contents. Multimodal imaging confirmed the diagnosis, and laparoscopic gallstone extraction was successful.

Cholecystogastric fistula presenting with transient gallstone ileus and incidental Fitz-Hugh-Curtis syndrome: A rare radiological and intraoperative association.

Cholecystogastric fistula (CGF) is the rarest subtype of cholecystoenteric fistula and is an uncommon complication of chronic gallstone disease. Diagnosis is often challenging because of nonspecific clinical features and subtle imaging findings. We report a rare case of CGF presenting with transient gallstone ileus and an incidental intraoperative finding of Fitz-Hugh-Curtis syndrome, a previously unreported association. A 42-year-old woman presented with acute epigastric pain and vomiting on a background of intermittent chronic epigastric discomfort. Contrast-enhanced computed tomography demo

Emphysematous gastritis with portal venous gas: conservative management in a high-risk patient.

Emphysematous gastritis is a rare, often fatal condition characterized by gas within the gastric wall, usually in patients with significant comorbidities. We describe a 75-year-old man with incomplete tetraparesis, chronic respiratory failure and type 2 diabetes who presented with abdominal pain, vomiting and hematemesis. Initial computed tomography (CT) was reported as non-obstructive small-bowel dilatation with possible gallstone ileus, but subspecialty radiology review identified intramural gastric gas with portal venous gas consistent with emphysematous gastritis. He was managed non-operat
