Gallstone ileus is a rare mechanical bowel obstruction caused by the migration of a gallstone through a biliary-enteric fistula, most commonly impacting at the ileocecal valve. Proximal obstruction is uncommon and is typically classified as Bouveret syndrome when involving the stomach or proximal duodenum. Obstruction near the ligament of Treitz represents an exceedingly rare variant. A 76-year-old female presented with nausea, vomiting, and abdominal distention. Computed tomography demonstrated gastric and proximal small bowel dilation secondary to an obstructing gallstone in the distal duode

INTRODUCTION AND IMPORTANCE: Bouveret syndrome is a rare form of gallstone ileus characterized by gastric outlet obstruction secondary to impaction of a gallstone that has migrated into the gastrointestinal tract through a cholecystoduodenal fistula. It predominantly occurs in elderly patients with multiple comorbidities. Cases involving giant gallstones complicated by acute pancreatitis are exceedingly uncommon and pose significant therapeutic challenges.
CASE PRESENTATION: A 72-year-old woman was admitted with a 6-day history of persistent right upper quadrant abdominal pain and distension,

INTRODUCTION AND IMPORTANCE: Bouveret syndrome is a rare (0.3-0.5%) form of gallstone ileus caused by gallstone impaction in the duodenum via a cholecystoenteric fistula. Its nonspecific presentation and high mortality rate (12-30%) make early diagnosis and appropriate management essential. Dual gallstone impaction - combining Bouveret syndrome with distal small bowel obstruction - is even more uncommon and poses additional diagnostic and therapeutic challenges.
CASE PRESENTATION: A 55-year-old woman with a 3-year history of gallstones and partially controlled type 2 diabetes presented with p

Bouveret syndrome is an uncommon form of gallstone ileus that results from a cholecystoenteric fistula. Impaction of gallstone in the proximal duodenum blocks the gastric outlet. We report the first case from a resource-limited setting. A 75-year-old man with calculous cholecystitis arrived with gastric obstruction, melena, coffee-ground emesis, and weight loss. Lab data showed leukocytosis and mild anemia. Computed tomography scan showed gastric distension, pneumobilia, and a gallstone in the duodenum: the characteristic findings in Rigler's triad. Due to its large size and calcification, end

Gallstone ileus is a rare cause of mechanical bowel obstruction, predominantly affecting elderly patients with multiple comorbidities. We present a series of three patients illustrating the variability in clinical presentation, imaging findings, and surgical management. An 86-year-old male underwent successful laparoscopic enterolithotomy with an uneventful postoperative course and discharge on postoperative day 4. In contrast, two female patients (aged 77 and 84 years) required open surgery due to more severe clinical presentation and developed significant postoperative complications, includi

Bouveret syndrome (BS) is a rare phenomenon of gallstone ileus causing gastric outlet obstruction that can present significant challenges in management, given its lack of formal treatment guidelines and endoscopic difficulty. We present a case of an 84-year-old woman with vomiting and abdominal pain who was found to have a large gallstone in the duodenum due to a cholecystoduodenal fistula, as well as findings consistent with acute cholecystitis. She was treated with antibiotics, and gastroenterology and general surgery were consulted. Endoscopy was performed due to high suspicion for BS, reve

INTRODUCTION: Bouveret's syndrome is a rare etiology of gastric outlet obstruction, comprising 3% of gallstone ileus (GI) cases. GI occurs with cholecystoenteric fistulating disease resulting in gallstone impaction within the gastrointestinal tract. There is a lack of unified guidance in GI management for surgeons. This, combined with its unpredictable symptomatology at presentation, results in delayed diagnoses and high mortality rates, despite advancements in surgical care.

An 88-year-old woman presented with acute post-prandial abdominal pain, nausea, and rigors, with biochemical evidence of obstructive jaundice. Computed tomography demonstrated choledocholithiasis with acute cholecystitis and an incidental segment of thickened descending colon. Following endoscopic sphincterotomy with stone extraction and subsequent laparoscopic cholecystectomy, she was discharged home. She re-presented 6 days later with constipation and lower abdominal pain. Repeat imaging revealed large bowel obstruction with marked cecal dilatation due to migration of a gallstone impacted at

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage.
METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: A correlation between elevation of pro-inflammatory cytokines and white matter injury or abnormal neurologic outcome has been established in the preterm infant. In the full-term neonate, few studies exist linking elevation of cytokines with encephalopathy and poor neurodevelopmental outcome. Our aims were to investigate if serum interleukin-6 concentrations in delivering mothers and their offspring at birth are associated with perinatal asphyxia, and to examine the relation of interleukin-6 levels to the severity of hypoxic-ischemic encephalopathy and to the neurological outcome.

We present a case of a woman in her 70s who arrived in the emergency department with signs of small-bowel obstruction. CT scanning revealed acute cholecystitis with a cholecystoduodenal fistula, pneumobilia and small-bowel obstruction possibly secondary to gallstone ileus although no radio-opaque gallstones were seen. The patient underwent an emergency operation and intra-operative findings revealed mechanical small-bowel obstruction of the proximal jejunum where a 4×2 x 3 cm gallstone was impacted. A laparoscopic-assisted enterotomy was performed with stone extraction and primary closure. The

An 84-year-old lady presented with 1 day history of sudden onset generalized abdominal pain, fevers, and peritonism. Computed tomography was suggestive of a mid-small bowel perforation associated with a distal ovoid soft tissue density structure without pneumobilia. An urgent laparotomy demonstrated two areas of jejunal diverticula necrosis and perforation associated with a 3 cm luminal mass in the proximal ileum, and proximal small bowel dilatation. A 100 cm small bowel resection incorporating the mass and perforated jejunal diverticula and primary stapled anastomosis were performed. Histopat

INTRODUCTION AND SIGNIFICANCE: Gallstone ileus is a mechanical intestinal obstruction caused by gallstones or stones from the common bile duct entering the intestinal canal through a biliary-intestinal fistula and becoming impacted. Multidetector computed tomography (MDCT) scanning is the most important auxiliary diagnostic method for this disease.
CASE PRESENTATION: We report a rare case of gallstone ileus involving an elderly female patient and analyze the characteristics of the MDCT images: atrophic cholecystitis, gallstones, the formation of a local duodenal-gallbladder fistula, pneumatos

Bouveret's syndrome, a rare and severe complication of gallstone disease, is characterized by gastric outlet obstruction resulting from the passage of a gallstone through a bilioenteric fistula, typically a cholecystoduodenal fistula. We present the case of a 68-year-old female patient with bilateral low back pain, nausea, vomiting, and constipation. Imaging revealed jejunal obstruction due to gallstone migration through a cholecystoduodenal fistula. Initial management involved nasogastric decompression and fluid resuscitation, followed by enterolithotomy. Diagnosis of Bouveret's syndrome is c

Cholelithiasis is a common condition, and complications of gallstone disease include cholecystitis, cholangitis, pancreatitis, and, rarely, gallstone ileus or Bouveret syndrome. We present a case of a 79-year-old male with multiple comorbidities who was treated using endoscopic techniques to avoid the risks associated with surgery. The approach utilized holmium laser lithotripsy and electrohydraulic lithotripsy over multiple sessions to fragment and remove the large gallstone causing a duodenal obstruction. Conservative management of a subsequent duodenal perforation led to successful recovery

We present a case of a large bowel obstruction secondary to a large impacted gallstone measuring 5cm in the sigmoid colon. Significant features include a cholecysto-colonic fistula and the presence of a perforation initially missed on imaging. Endoscopic retrieval was attempted, followed by a Hartmann's procedure. This case highlights the significance of operative management in ensuring the safe treatment of colonic gallstone ileus, especially in a rural setting.
