Gallstone Ileus as an Uncommon Etiology of Intestinal Obstruction: A Case Report.

Although gallstones are commonly confined to the biliary system, in rare instances, they may migrate into the intestinal tract and cause obstruction, a scenario most often observed in older adults with a history of gallbladder pathology. We describe the case of a 46-year-old woman who developed small bowel obstruction secondary to a large impacted gallstone, despite having no prior biliary disease. Her clinical presentation included a 15-day history of abdominal pain, bilious vomiting, and diarrhea. Laboratory tests revealed leukocytosis and acute kidney injury. Contrast-enhanced computed tomo

A Unique Case of Gallstone Ileus in a Patient With Crohn's Disease: Stone Impaction in an Ileal Adenocarcinoma.

Gallstone ileus represents an uncommon etiology of mechanical intestinal obstruction. Individuals with Crohn's disease are predisposed to both increased gallstone formation and a heightened risk of gastrointestinal malignancy. This case highlights the necessity of excluding malignancy when evaluating newly developed strictures in this patient population.

Atypical Gallstone Ileus in a Young Adult: A Diagnostic Challenge and Surgical Resolution.

Gallstone ileus is a rare but serious complication of cholelithiasis resulting in mechanical bowel obstruction. We present the case of a 40-year-old woman with no prior medical history who developed classic features of gallstone ileus. Radiological investigations revealed Rigler's triad (small bowel obstruction, pneumobilia, ectopic gallstone), and the patient underwent exploratory laparotomy and enterotomy with successful extraction of multiple gallstones. Postoperative recovery was uneventful, and this case emphasizes the importance of early imaging and surgical intervention in such cases.

Gallstone ileus associated with obstructive jaundice and intestinal ischemia: a case report.

Typical gallstone ileus includes small intestine obstruction caused by the gallstone passing from the gallbladder through the common bile duct (CBD). The case of gallstone ileus with simultaneous obstructive jaundice and intestinal ischemia is an extremely rare scenario. Computed tomography of the abdomen and pelvis is the method of choice for the diagnosis. While some cases may be treated conservatively, surgery is often required, with the approach tailored to the patient's general condition and medical history. Hereby, we present a case of an 88-year-old lady with gallstone ileus, obstructiv

Gallstone Ileus Presenting As Small Bowel Obstruction: A Surgical Case Report.

Gallstone ileus is an uncommon complication of cholelithiasis. This case report discusses an 83-year-old patient who presented with chronic, intermittent abdominal discomfort that progressed to persistent nausea and vomiting, necessitating hospital admission. A CT scan confirmed the diagnosis of gallstone ileus, demonstrating Rigler's triad. The patient was successfully managed with a surgical enterotomy, specifically an enterolithotomy. Notably, the patient had no prior admissions for gallstone-related issues, despite a history of recurrent abdominal symptoms that had not been previously inve

Diagnostic Value of Multimodal Imaging and Histopathology in Gallstone Ileus: A Retrospective Analysis.

Background: Gallstone ileus is a rare cause of bowel obstruction, often presenting with nonspecific symptoms that delay diagnosis. This study assessed the diagnostic value of CT imaging and its correlation with histopathological findings in confirmed cases. Methods: A retrospective analysis of 14 patients with surgically confirmed gallstone ileus was performed. All underwent abdominal radiography, ultrasound, and CT. Imaging findings were evaluated for calcification type, impaction site, and fistula presence. Histopathology from surgical specimens was used for correlation. Relative risk (RR) a

Gallstone Ileus Mimicking a Colonic Tumor: A Case Report.

INTRODUCTION: Gall stone ileus is a rare complication of cholelithiasis which typically presents with obstruction in the small intestine. However, it can rarely mimic a mass when it presents in unusual sites like the sigmoid colon as in our case. CASE PRESENTATION: We present a 42-year-old woman with a history of bariatric surgery, diverticulitis status post sigmoid colectomy and decompensated cirrhosis complicated by hepatic encephalopathy who presented to the hospital with concern for altered mental status and was diagnosed with grade III hepatic encephalopathy due to lactulose non adherenc

Gallstone ileus 16 years postcholecystectomy?

Gallstone ileus, a rare complication of cholecystitis, occurs when the distal ileum becomes obstructed by gallstone causing, mechanical bowel obstruction. Gallstone ileus with history of cholecystectomy is exceptionally rare, with few cases documented in literature. This case report details the discovery of a large, impacted gallstone 16 years postcholecystectomy. An 87-year-old male presented with progressively worsening right-sided abdominal pain over 2-3 days. Exploratory laparotomy unveiled a 2.0 cm × 2.5 cm gallstone was obstructing the terminal ileum. Patient improved remarkably postoper

Gossypiboma following cesarean section presenting as acute abdomen and sepsis: a case report.

INTRODUCTION: Gossypiboma is a problematic but rare and highly preventable surgical complication. The incidences of gossypiboma are vastly underreported; therefore, the actual incidence is unknown. Women undergoing obstetrical procedures constitute a fairly high proportion of patients with gossypibomas. Most cases present after months or years, and only a few cases have been reported that presented acutely within days. CASE PRESENTATION: We present a case of a woman who presented with severe symptoms of intestinal obstruction leading to acute abdomen merely a week after her caesarean section.

The Silent Presentation of Gallstone Ileus in a Surgery-Naïve Abdomen: A Case Report.

As an uncommon etiology of bowel obstruction that predominantly affects the elderly, gallstone ileus warrants attention because it frequently arises from cholecystoenteric fistulas and carries disproportionate morbidity when recognition is delayed. This case report details an unusual presentation in an elderly male patient with no history of abdominal surgery who exhibited nonspecific abdominal symptoms. Imaging studies identified Rigler's triad, and computed tomography (CT) confirmed the presence of an ectopic gallstone in the proximal ileum. The patient promptly underwent surgical interventi

Radiological diagnosis of gallstone sigmoid ileus or coleus: Case report and literature review.

UNLABELLED: Gallstone ileus is a mechanical bowel obstruction caused by a gallstone within the intestinal tract. It occurs secondary to a fistulous communication between the gallbladder and intestine and commonly causes a small bowel obstruction termed gallstone ileus. It rarely causes a large bowel obstruction termed gallstone sigmoid ileus (GSI) or gallstone coleus. CONTRIBUTION: A case of gallstone sigmoid ileus with typical plain radiography and CT scan findings and a review of different imaging modalities for this condition.

"An unusual cause of small bowel obstruction: Gallstone ileus, a rare presentation of chronic gallstone disease".

INTRODUCTION AND IMPORTANCE: Gallstone ileus is a rare cause of mechanical small bowel obstruction, accounting for 1-4 % of cases and is most common in elderly females. It arises from the passage of a large gallstone through a cholecysto-enteric fistula into the intestinal lumen, where it lodges-typically in the terminal ileum-causing obstruction. PRESENTATION OF CASE: A 52-year-old male presented with four days of abdominal pain, vomiting, distension, and constipation. He had a 22-year history of gallstones but no prior biliary symptoms. Imaging revealed small bowel obstruction with a cholec

Large Jejunal Gallstone Ileus After Sleeve Gastrectomy.

Gallstone ileus is a rare but serious complication in patients with cholelithiasis, resulting in an increased risk of mortality, highlighting the importance of timely diagnosis and management. Excess weight loss after bariatric surgery is associated with more pronounced symptomatic gallstone disease. We present a 44-year-old female with a previous vertical sleeve gastrectomy who presented with progressively worsening mechanical small bowel obstruction. Computed tomography showed cholelithiasis, and magnetic resonance cholangiopancreatography identified extravasation of contrast from the gallbl

Gallstone ileus presenting as intestinal obstruction in the elderly: Two case reports and literature review.

INTRODUCTION AND IMPORTANCE: Gallstone ileus is a rare and potentially life-threatening complication of chronic cholecystitis in the elderly. It results from the passage of a large gallstone through a bilio-digestive fistula into the gastrointestinal tract, causing mechanical obstruction. Early diagnosis and surgical intervention are crucial to reducing associated morbidity and mortality. CASE PRESENTATION: We report two cases of gallstone ileus. Case 1: A 76-year-old man presented with vomiting, abdominal pain, and bowel obstruction. CT scan revealed pneumobilia and an obstructing gallstone

Gallstone ileus in a young patient complicated by double biliary-enteric fistula: A rare case report.

BACKGROUND: Gallstone ileus is an uncommon complication of chronic cholecystitis, characterized by mechanical small bowel obstruction due to gallstone migration through a biliodigestive fistula. The occurrence of double biliary-enteric fistulas (e.g., cholecysto-duodenal and cholecysto-colic) is infrequent and presents additional diagnostic and surgical challenges. CASE PRESENTATION: A 41-year-old woman with no significant past medical history presented with progressive abdominal pain, distension, and cessation of bowel movements. Laboratory findings indicated a severe inflammatory response (

Concurrent Bouveret's Syndrome and Gallstone Ileus: A Rare Case of Dual-Site Obstruction in Gallstone Disease.

Bouveret's syndrome is a rare cause of gastric outlet obstruction, resulting from gallstone migration into the duodenum via a biliary-enteric fistula. Even rarer is its coexistence with distal gallstone ileus. We report the case of an elderly woman presenting with gastric outlet obstruction who was found to have dual-site obstruction caused by two impacted gallstones. Despite multidisciplinary management involving imaging, endoscopy, and surgery, her postoperative course was complicated by sepsis and multiorgan failure, leading to death. This case underscores the severe morbidity and mortality

When Stones Strike Twice: A Rare Case of Post-endoscopic Retrograde Cholangiopancreatography (ERCP) Gallstone Ileus.

Gallstone ileus is a rare complication, typically resulting from a cholecystoenteric fistula. Its occurrence following endoscopic retrograde cholangiopancreatography (ERCP), particularly without evidence of fistula formation, is exceedingly uncommon. We present the case of a 69‑year‑old man with choledocholithiasis who developed gallstone ileus 48 hours after successful ERCP and stone extraction. The risk of gallstone ileus was anticipated during the procedure due to the large size of the bile duct stones. The patient subsequently developed symptoms of small bowel obstruction and underwent sur

Gallstone ileus secondary to cholecystoduodenostomy causing mechanical duodenal obstruction in a cat.

CASE SUMMARY: A 9-year-old neutered male domestic shorthair cat was presented for anorexia and lethargy with a 2-day history of ptyalism and dysorexia. The cat had undergone a cholecystoduodenostomy 8 years before presentation and had suffered from episodes of clinical cholangitis ever since. Abdominal ultrasound revealed duodenal obstruction. Two stones were surgically removed, subjected to spectrophotometric examination and identified as choleliths. Clinical improvement was noted postoperatively, and 12 months after surgery the cat was doing well and had no further digestive problems. RELEV

On-table cholangioscopy for hepatolithiasis in a patient with gallstone ileus post hepaticojejunostomy.

Roux-en-Y hepaticojejunostomies (RYHJs) can be associated with biliary complications. We describe a very rare case of gallstone ileus causing a Roux limb obstruction in a patient who had recurrent hepatolithiasis and cholangitis secondary to residual stones despite repeated cholangioscopy. The obstruction was managed with enterotomy and removal of gallstone via an open surgical approach, with complete clearance of hepatolithiasis through on-table trans-enteric cholangioscopy.
