Gallstone ileus is a rare but significant cause of abdominal pain and mechanical bowel obstruction. Its management usually involves minimally invasive surgical intervention. This case report describes a 65-year-old woman with a history of Caroli disease and prior abdominal surgery, who presented with a one-month history of progressive, cramp-like abdominal pain, accompanied by nausea and intermittent vomiting. Due to these persistent symptoms, she underwent a CT scan, which confirmed gallstone ileus with obstruction of a jejunal loop, necessitating surgical removal of the gallstone. This case

Cholecystocolonic fistula (CCF) is a rare entity resulting from chronic gallstone disease. Here we describe a 60-year-old female who presented with abdominal distension, colicky pain, and constipation for a period of five days. Imaging studies revealed pneumobilia and small bowel obstruction. Exploration via laparotomy revealed a 3.5 cm gallstone lodged in the terminal end of the ileum and a fistulous tract from the gallbladder to the hepatic flexure of the colonic bowel. Surgical intervention consisted of enterolithotomy, cholecystectomy, and colonic resection with primary anastomosis. Fortun

Biliodigestive fistulas are a complication of gastrointestinal pathologies, often arising from chronic recurrent cholecystitis. These fistulas disrupt normal biliary physiology, leading to bile reflux or the entry of intestinal contents into the biliary system, with gallstone ileus as a significant consequence. This condition typically results in mechanical bowel obstruction due to the migration of gallstones into the intestinal lumen. Diagnosis can be challenging due to nonspecific symptoms, and surgical intervention is often required. This retrospective study reviews cases of gallstone ileus

Bouveret syndrome is a rare form of gallstone ileus in which a gallstone causes gastric outlet obstruction via a cholecystoenteric fistula. We present the case of an elderly patient who underwent emergency surgery due to clinical signs of gastric outlet obstruction. Intraoperatively, a large gallstone was identified and removed from the duodenum. A subtotal cholecystectomy was performed, during which the identification of the course of the cystic duct was limited due to considerable inflammatory changes. In the early postoperative period, bile-stained output from the surgical drain raised susp

Bouveret's syndrome is a form of gastric outlet obstruction characterised by gallstones traversing through a cholecystoduodenal fistula and lodging in the proximal duodenum and pylorus. This condition often requires complex surgical management due to disruption of the normal biliary and duodenal anatomy. The management approach is usually tailored to the patient's disease, the surgeon's and endoscopist's skills and instrument availability. The principles for treatment are ectopic gallstone extraction, management of duodenotomy, cholecystectomy and repair of cholecystoduodenal fistula. This cas

INTRODUCTION AND IMPORTANCE: Biliary ileus is a rare yet potentially life-threatening cause of mechanical intestinal obstruction, predominantly affecting elderly, comorbid patients. Often underdiagnosed due to non-specific symptoms, it can now be detected earlier with point-of-care ultrasound (PoCUS).
CASE PRESENTATION: We present the case of an 85-year-old male with non-specific abdominal symptoms where PoCUS at the bedside facilitated early identification of a distal ileal stone, significantly expediting radiological confirmation and surgical management.
CLINICAL DISCUSSION: This case unde

BACKGROUND: Cholecystolithiasis is the most common disease of the gallbladder. Both acute obstructive suppurative cholangitis (AOSC) and gallstone ileus are critical clinical conditions requiring urgent intervention. However, their synchronous occurrence, particularly in elderly patients, presents a significant therapeutic challenge. In such scenarios, an optimal treatment strategy is essential to ensure patient safety while minimizing procedural risks.
CASE PRESENTATION: Herein, we described a 91-year-old women with cholecystolithiasis who was admitted for a day of abdominal pain accompanied

Gallstone ileus is a rare complication of chronic cholelithiasis and an exceedingly uncommon cause of bowel obstruction following Roux-en-Y gastric bypass (RYGB), particularly after cholecystectomy. We report the first known case of a large gallstone causing obstruction of the biliopancreatic limb in an 80-year-old woman with prior RYGB, cholecystectomy, and choledochoduodenostomy. She underwent successful open enterolithotomy with stone extraction. This case highlights the diagnostic and technical challenges posed by altered post-RYGB anatomy and emphasizes the importance of maintaining a hig

Gallstone ileus occurring decades after cholecystectomy is exceptionally rare and represents an uncommon cause of small bowel obstruction (SBO) in elderly patients with prior gallstone disease. A 72-year-old male with a remote history of cholecystectomy presented with nausea, vomiting, and diffuse abdominal pain. CT imaging demonstrated an SBO with a calcified intraluminal mass in the distal ileum. After initial conservative management, the patient underwent exploratory laparotomy with extensive adhesiolysis, enterolithotomy, and primary ventral hernia repair. A large, calcified gallstone was

Background and Objectives: Gallstone ileus (GI) is a rare mechanical intestinal obstruction resulting from gallstone impaction through a bilioenteric fistula, accounting for 1-4% of mechanical intestinal obstructions. This study aimed to evaluate the clinical approach, surgical management, and outcomes in a cohort of surgically treated GI patients. Materials and Methods: A retrospective cohort analysis of 12 patients diagnosed with GI and treated surgically between January 2018 and December 2024 was conducted. Data collected included demographics, clinical presentation, imaging findings, surgi

Gallstone ileus is a rare complication of gallstone disease, wherein a dislodged gallstone causes bowel obstruction by impaction of the intestines. Elderly patients and those with a history of gallstones are most commonly affected. Symptoms are usually vague and obscure, with patients reporting bloating, nausea and vomiting, early satiety, and constipation. To confirm the diagnosis, a contrast-enhanced computed tomography (CECT) scan is most commonly used, wherein Rigler's triad of small bowel distention, aerobilia, and an ectopic gallstone may be seen, which prompts further management. Gallstone ileus is an uncommon cause of mechanical intestinal obstruction. It results from the impaction of one or more gallstones within the gastrointestinal tract, usually after migrating through a cholecysto-duodenal fistula. The clinical presentation is nonspecific, including abdominal pain, vomiting, distension, and constipation. Computed tomography (CT) scan is the diagnostic modality of choice due to its high sensitivity. The treatment is surgical, with enterolithotomy being the initial procedure to extract the impacted stone. This case report describes a 74-year-old woman diagnosed wit

Gallstone ileus is a rare mechanical bowel obstruction caused by the impaction of a gallstone in the GI tract, most frequently in the terminal ileum and the ileocecal valve, secondary to a cholecystoenteric fistula. It predominantly affects elderly women with multiple comorbidities, which, together with non-specific and intermittent symptoms, delay the diagnosis. Imaging studies play a crucial role in diagnosis. Despite that, gallstone ileus continues to be associated with relatively high rates of morbidity and mortality. The optimal management of acute gallstone ileus remains controversial. T

Gallstone ileus is a rare cause of mechanical bowel obstruction, typically affecting elderly patients with a history of gallstones. It results from the migration of a gallstone through a cholecystoenteric fistula into the bowel, most commonly lodging in the ileum. We report a case of a 59-year-old female who presented with small bowel obstruction caused by a 22 mm gallstone. During diagnostic laparoscopy, no obstructing stone was found, and subsequent imaging revealed its migration to the descending colon. The patient was successfully managed conservatively without surgical intervention. This
