Anorectal function in elderly patients with fecal impaction.

Manometric and other investigations were carried out in 55 elderly patients who had impacted masses of feces in the rectum upon admission to hospital and in 36 elderly age- and sex-matched control subjects. Maximum basal and maximum squeeze sphincter pressures in the patients were similar to those in the elderly controls. Most elderly patients in the impacted group and all control subjects were able to pass a 50-ml balloon from the rectum, although a lower proportion of patients, admitted with impaction, could expel a small solid sphere. In patients the rectum had to be distended with larger v

Pediatric gastrointestinal foreign body ingestions.

One hundred twenty-five consecutive emergency department cases of pediatric gastrointestinal foreign body ingestions were analyzed retrospectively to tabulate data, identify high risk ingestions, and draw conclusions regarding the standard of care. Eleven patients (9%) were admitted for endoscopy or observation of high risk situations. Twenty patients (16%) were managed invasively, 19 with esophageal foreign bodies; 17 of 20 attempts at invasive management were successful. There was a 2% incidence of minor complications; no major complications (perforation, obstruction, bleeding, or mediastina

[Constipation in the aged. I. Significance, prevalence, causes and treatment].

There is no reason to assume that in old age defecation is different from younger people. Old people who are sound and active often have normal defecation. Their intestinal transit time has been shown to be normal. Nevertheless about fifty percent of the elderly use laxatives. In the very old and in institutions the use is even more frequent. The main cause of this real or supposed constipation is the lack of dietary fibre in the food. In the aged constipation can have dangerous complications, e.g. acute mental confusion, urinary retention, urinary incontinence and fecal impaction. Treatment m

Experimental studies of drug-induced impaction colic in the horse.

Colic was induced in horses and ponies following topical or intravenous (iv) administration of amitraz, a formamidine acaricide. The condition was characterised by rapid cessation of intestinal sounds, stasis, extensive impaction and tympany throughout the large colon. Three animals that were necropsied had a faecalith obstructing the proximal small colon aboral to marked colonic impaction. A reproducible and reversible impaction colic syndrome could be induced by an iv injection of 1 mg amitraz/kg body weight in solvent. There were immediate central nervous system and intestinal signs. Large

Surgical pathology of acute appendicitis.

Pathologic findings and clinicopathologic correlations in 276 consecutive appendectomies performed in a university hospital are reviewed. In 59 cases, appendectomy was incidental to another elective procedure. In the other 217 cases exploration was performed as an emergency, and acute appendicitis was present in 160 of the cases (74 per cent); 54 of these were complicated by perforation. In the remaining 57 cases there was either some other pathologic process clinically distinct from appendicitis or no identifiable source of the preoperative symptoms. The major findings were as follows: (1) Al

Rectal bleeding secondary to fecal disimpaction: angiographic diagnosis and treatment.

Two cases of bleeding caused by manual disimpaction of fecal material are presented. Angiography played a critical role in the diagnosis and treatment. One case was managed with balloon tamponade, and the other was successfully treated by selective intra-arterial vasopressin infusion, after prior intravenous infusion had been ineffective. A history of fecal disimpaction should suggest the possibility of low rectal bleeding which can be diagnosed and treated angiographically.

[Coprolith-ileus of the small intestine].

The case of a 71 year old man is reported, in which coproliths developed in the small intestines over a period of 13 years and which finally lead to a complete ileus. This condition is discussed in connection with other rare forms of ileus; specific clinical phenomena are pointed out, which might be helpful in establishing a correct diagnosis. Among the transient forms of ileus associated with food intake, which may cause the patient only slight discomfort, one should bear in mind a corpus alienum-ileus. Only operative therapy of ileus is promising success.

Diagnostic and therapeutic considerations for fecal impaction.

During an 18-month period, 18 patients were admitted to the Beth Israel Hospital because of fecal impaction or its complications. The records of the 18 patients were reviewed to determine the presenting signs and symptoms, radiologic findings, course and etiology of fecal impaction. Prior use of drugs that slow gastrointestinal motility was found in seven cases, and seven of the 18 patients had severe neuropsychiatric illness. The presenting signs and symptoms in almost all instances were consistent with a diagnosis of intestinal obstruction. The difficulty in differentiating intestinal obstru
