Cesarean Delivery after Abdominal Wall Reconstruction Using an Anterolateral Thigh Flap with Vascularized Fascia Lata.

Techniques for abdominal wall reconstruction include the use of prosthetic materials (synthetic or biological meshes), component separation, autologous tissue reconstruction, or a combination thereof. Reconstruction using prosthetic materials avoids additional trauma to healthy tissues but carries risks such as infection, adhesion to surrounding structures, and bowel obstruction caused by intestinal adhesions. Therefore, abdominal wall reconstruction using autologous tissue may be considered as an option suitable for women of childbearing potential. Only a few cases of successful childbirth fo

A rare case of lesser omental hernia in a male patient following total colectomy.

INTRODUCTION AND IMPORTANCE: A lesser omental hernia is a rare internal hernia in which intestinal loops become entrapped through a defect in the lesser omentum, potentially causing bowel obstruction. Its diagnosis is challenging because of nonspecific symptoms that resemble those of other gastrointestinal diseases. This report presents a case of lesser omental hernia following total colectomy. CASE PRESENTATION: A 65-year-old male with a history of laparoscopic total colectomy for ulcerative colitis 4 years prior presented with acute abdominal pain and distension. Computed tomography (CT) re

Giant Jejunal Diverticulosis Causing Acute Mesenteric Volvulus: a Case Report.

BACKGROUND: Large jejunal diverticula are usually asymptomatic but can cause rare, life-threatening complications including obstruction, perforation, and bleeding. Secondary mesenteric volvulus caused by jejunal diverticulosis is exceptionally rare. It should be strongly considered when computed tomography (CT) reveals a mesenteric "whirl sign" in an adult presenting with acute obstructive symptoms. CASE PRESENTATION: A 76-year-old female presented to the emergency department with acute, diffuse abdominal pain and clinical evidence of mechanical intestinal obstruction. A CT scan demonstrated

Duodenum inversum presenting with intestinal obstruction in a 40-day-old infant: a case report.

BACKGROUND: Duodenum inversum is a rare congenital anomaly characterized by an abnormal course of the duodenum with preservation of a normally positioned duodenojejunal junction. Clinical presentation is highly variable, ranging from incidental findings to symptoms of upper gastrointestinal obstruction. Due to its rarity, diagnosis and management remain challenging, particularly in pediatric patients. CASE PRESENTATION: We report the case of a 40-day-old female infant presenting with recurrent postprandial vomiting and poor weight gain since the first days of life. Initial clinical, metabolic

The vicious cycle of nonoperatively managed adhesive small bowel obstruction: does weekend admission play a role?

BACKGROUND: Data on the influence of nonpatient factors, such as day of admission, on adhesive small bowel obstruction (aSBO) management are limited. This study assesses the association between weekend admission, management approach, and postdischarge outcomes. METHODS: We conducted a retrospective cohort study using the 2020 and 2021 National Readmission Database. Adult patients admitted with aSBO between January and March of each study year were followed for 9 months after discharge. Patients were stratified by admission day (weekend vs weekday) and management (nonoperative [NOM] vs operati

Case Report: Erdheim-Chester disease presenting as abdominal cocoon syndrome.

Erdheim-Chester disease (ECD) is a rare histiocytic neoplasm driven by MAPK pathway mutations. Although it frequently involves the bones, heart, and kidneys, initial manifestations predominantly affecting the gastrointestinal tract and mesentery are exceedingly rare, often leading to significant delays in clinical diagnosis. This report describes a 61-year-old female patient who presented with recurrent abdominal pain over several months and was initially diagnosed with a conventional "adhesive bowel obstruction" at an external hospital. However, surgical exploration revealed a classic "abdomi

A Rare Mimicker of Malignancy: Inflammatory Cloacogenic Polyp Presenting as Obstruction in a Colon Cancer Survivor.

Inflammatory cloacogenic polyps are rare and often discovered either during routine colorectal cancer screenings or in symptomatic patients presenting with hematochezia. In unusual cases, they can present as an adhesive obstruction with radiological and clinical findings suggestive of a suspicious recurrent colorectal malignancy lesion. Because these polyps can mimic cancer during endoscopy, a detailed tissue examination is crucial for making the correct diagnosis and planning treatment. In this report, we share a challenging case where a patient with a history of cancer presented with signs t

Jejunal transection caused by postoperative adhesive small bowel obstruction: managed simultaneously with abdominal aortic aneurysm.

A man in his 90s presented with abdominal pain and was referred to our hospital. Contrast-enhanced CT revealed an abdominal aortic aneurysm (AAA) without evidence of rupture and marked dilatation of the small bowel, leading to a diagnosis of postoperative adhesive small bowel obstruction. Conservative management with a long intestinal tube failed to relieve bowel obstruction, but the patient initially declined surgery and was transferred back to the referring hospital. After his symptoms persisted, he agreed to operative treatment and was readmitted. Following a preoperative multidisciplinary

Adhesive small bowel obstruction in the virgin abdomen: comparable phenotype, divergent management.

BACKGROUND: In small bowel obstruction (SBO), the absence of prior abdominal surgery is commonly interpreted as a marker of higher-risk pathology and is often associated with a lower threshold for early operative intervention. Whether this assumption reflects differences in disease phenotype or decision-making remains uncertain. METHODS: This post hoc analysis of a prospective, multicenter cohort study included consecutive adults with CT-confirmed mechanical SBO. Analyses were restricted to patients with adhesion-related SBO to enable comparison between virgin abdomen adhesive SBO (vaASBO) an

Surgical Management of Intestinal Obstruction: A Prospective Observational Study.

BACKGROUND: Intestinal obstruction remains one of the most common surgical emergencies encountered in tertiary care settings globally, carrying significant morbidity and mortality if not promptly diagnosed and managed. The etiological spectrum, clinical presentation, and optimal surgical strategies vary considerably between small bowel and large bowel obstruction (LBO) and are further influenced by the patient's comorbidity burden, nutritional status, and oncological history. This study was undertaken to systematically evaluate the surgical management of intestinal obstruction at a tertiary ca

Hydrogels strategies for organ adhesions: from mechanism to application.

Organ adhesions pose a significant clinical challenge, arising from factors such as congenital adhesions caused by embryonic developmental abnormalities, as well as acquired adhesions resulting from inflammation or surgery. This phenomenon commonly occurs in conditions such as peritonitis, pelvic inflammatory disease, and malignant tumor infiltration. Postoperative adhesions might cause serious complications such as chronic pain, infertility and intestinal obstruction. Traditional treatment methods (anti-adhesion membranes and drug therapy), have some limitations, including insufficient mechan

Dogma, data, and decision-making: a history of treatment for small-bowel obstruction.

BACKGROUND: Mechanical small-bowel obstruction (SBO) has been recognized since antiquity. We systematically review the evolution of its diagnosis and treatment, with emphasis on surgical milestones, influential surgeons, and procedural advances alongside the development of imaging and non-operative therapy. METHODS: We searched primary historical texts, monographs, and PubMed-indexed articles (inception to July 2025) for descriptions of mechanical SBO management. Data on key innovations, figures, outcomes, and global knowledge sharing were extracted and chronologically synthesized. Narrative

Initial Adhesive Small Bowel Obstruction Management Pathway Drives Infectious Complication Occurrence.

INTRODUCTION: The Bologna guideline outlines three small bowel obstruction (SBO) management pathways. It remains unclear how pathway selection influences post-operative infections. METHODS: A multi-national, prospective, observational, audit of SBO management (November 1, 2023-May 31, 2024) captured demographics, care, and outcomes. Patients were grouped by pathway (successful non-operative management [NOM], NOM followed by surgery [NOM-T], direct to surgery [DTS]). Intergroup comparisons by chi-square or Fisher exact test, significance for p < 0.05. RESULTS: A total of 1,737 patients were a

Abdominal Pain Syndromes: Small Bowel Obstruction and Ileus.

Small bowel obstruction and ileus are common surgical concerns that family physicians may encounter in a consultative role with surgical specialties. The most common cause of small bowel obstruction in adults and children is adhesions, followed by internal and external hernias. Postoperative ileus is a common complication that can occur after any abdominal surgery; however, with the implementation of enhanced recovery after surgery protocols, the incidence is decreasing. For small bowel obstruction, treatment focuses on gastric decompression, fluid resuscitation, electrolyte replacement, and p

Acute intestinal obstruction in children: a comparison of laparoscopic treatment and open surgery.

OBJECTIVE: This study aimed to evaluate the clinical efficacy of laparoscopic surgery for acute small bowel obstruction (SBO) in children. METHODS: The retrospective study included children with acute SBO who were treated at Shanghai Children's Hospital from June 2014 to December 2023. Patients were categorized into one of the following two groups based on the operative approach they underwent: the laparoscopic surgery group and the open surgery group. Perioperative variables, including operative time, fasting duration, hospital stay, and complication rates, were evaluated. The primary outcom

High bilirubin and blood urea nitrogen predict intestinal necrosis in incarcerated groin hernia.

PURPOSE: Groin hernias are one of the most common surgical conditions worldwide, leading to substantial numbers of incarcerated cases. Incarcerated hernias pose a potential bowel necrosis risk, complicating surgical decision-making. Therefore, preoperative prediction of intestinal necrosis is crucial for determining treatment strategies. While previous studies on intestinal necrosis primarily focused on adhesive obstruction, few addressed incarcerated hernias. This study investigated risk factors for intestinal necrosis using electronic medical records. METHODS: Patients with incarcerated groi

Comparison of Hepaticoduodenostomy with Hepaticojejunostomy in the Laparoscopic Management of Choledochal Cyst in Children: A Systematic Review and Meta-Analysis.

BACKGROUND: There has been systematic review and meta analysis (SRMA) on comparison of hepaticoduodenostomy (HD) and hepaticojejunostomy (HJ) in choledochal cysts (CDC), which included both open and laparoscopic procedures. There is no SRMA, comparing them in pediatric CDC which were managed exclusively by laparoscopy. METHODS: Studies comparing laparoscopic HD and HJ after CDC excision were identified from Medline, Ovid, Google Scholar, and Cochrane library. Suitable studies were reviewed and meta-analyzed. Outcomes evaluated included operative time, hospital stay, and incidence of postopera

Immunohistochemical detection of putative pathogenetic factors in adhesions of infants: a pilot study.

Newborns' intestinal adhesions have been reported in 4.7% infants who underwent a laparotomy, but adhesions can also appear idiopathically. Etiology and pathogenesis of adhesions is still to be determined, but evidence shows relation to inflammation, formation of fibrin bands, hypoxia and tissue remodelation. Multiple candidate genes have been associated with adhesion development. The aim of this study was to evaluate the appearance of Sonic Hedgehog (SHH), Indian Hedgehog (IHH), Forkhead-box F1 (FOXF1), caudal type homeobox 1 (CDX1), HCLS1-associated protein X-1 (HAX-1), GATA Binding Protein

Outcomes of Small Bowel Obstruction in Patients With No Prior Surgery: A Systematic Review.

INTRODUCTION: Small bowel obstruction in patients without prior abdominal surgery (NPA-SBO) presents a diagnostic and therapeutic challenge. Although traditionally thought to require operative exploration, recent studies suggest that most patients may be managed nonoperatively. METHODS: We performed a systematic review and meta-analysis of studies reporting outcomes for adult patients with NPA-SBO published between 1996 and 2023. Databases were queried on September 30, 2024. The primary outcome was the proportion of patients successfully treated by nonoperative management. Secondary outcomes

Outcomes of small bowel obstruction management in previously unoperated patients with a mid-term follow-up: a retrospective cohort study.

BACKGROUND: Small bowel obstruction (SBO) is a widespread cause of emergency admissions for acute abdominal pain. While adhesions from prior surgeries are the most common cause, SBO occurs in patients without previous abdominal operations (virgin abdomen, VA-SBO). In 2021, the World Society of Emergency Surgery (WSES) re-evaluated the recommended management for these cases. This study aimed to investigate the causes and management of SBO in patients without prior abdominal surgeries. METHODS: A retrospective review was conducted to examine the data of patients hospitalised with SBO between 201
