The Effect of Albumin/Glutaraldehyde Glue (Bioglue) on Colonic Anastomosis Under Intestinal Obstruction: An Experimental Study in Rats.

Background/Objectives: Healing of colonic anastomoses is critical to surgical recovery, particularly under obstructive ileus conditions. Adhesive biological materials such as albumin/glutaraldehyde glue (Bioglue) show potential in enhancing anastomotic healing and minimizing complications. This study investigates the effect of Bioglue on colonic anastomoses healing under obstructive ileus conditions in rats. Methods: Eighty albino Wistar rats were divided into control, ileus, Bioglue, and ileus + Bioglue groups (n = 20 each). Subgroups (n = 10) were sacrificed on the 4th or 8th postoperative d

Small bowel obstruction outcomes according to compliance with the World Society of Emergency Surgery Bologna guidelines.

BACKGROUND: Small bowel obstruction (SBO) is a common surgical emergency associated with substantial morbidity, hospital length of stay (LOS), and healthcare cost. The World Society of Emergency Surgery (WSES) Bologna guidelines provide evidence-informed recommendations for managing adhesive SBO, promoting timely surgical intervention (or non-operative management (NOM) when ischaemia, strangulation, or peritonitis are absent). However, guideline adoption and its impact on outcomes remain under studied. Compliance with the Bologna guidelines was evaluated to determine the impact of compliance o

Water-soluble contrast agents in adhesional small bowel obstruction: meta-analysis and PRECIS-2 assessment of trials.

BACKGROUND: Adhesional small bowel obstruction is a common presentation to acute general surgical services. Initial management is typically conservative and includes the use of water-soluble contrast agents. Current trials assessing water-soluble contrast agents are limited by sample size and demonstrate contrasting results. The aim of this review was to systematically appraise the use of water-soluble contrast agents in adhesional small bowel obstruction. METHODS: This systematic review and meta-analysis was registered with PROSPERO (CRD42024573136) and conducted in line with PRISMA guidelin

Small Bowel Obstruction due to Hemobezoar Following Roux-En-Y Gastric Bypass: A Case Report.

Roux-en-Y Gastric Bypass (RYGB) is a widely performed bariatric surgery with well-documented benefits, but it can lead to rare complications such as small bowel obstruction (SBO). While internal hernias and adhesions are common causes, hemobezoar formation is an extremely rare etiology. We report two cases of acute SBO due to hemobezoar following laparoscopic RYGB. Both patients presented with severe nausea within the early postoperative period. CT imaging revealed marked small bowel distension in both cases, prompting surgical intervention. Diagnostic laparoscopy confirmed the presence of obs

Traditional Chinese medicine for adhesive intestinal obstruction: theory, methods and mechanisms of action.

Adhesive intestinal obstruction (AIO) represents a common postoperative complication, particularly following abdominal surgery, with reported incidence rates varying between 50 and 80%. Traditional Chinese Medicine (TCM) has proven clinically effective in managing AIO, offering diverse therapeutic approaches that facilitate multi-pathway and multi-target treatment. Clinical evidence consistently supports the favorable safety profile of Traditional Chinese Medicine (TCM). Nevertheless, several critical challenges remain to be addressed, including its complex multi-component nature, insufficient

[Prospects for the treatment of malignant intestinal obstruction associated with peritoneal metastasis in colorectal cancer].

Malignant intestinal obstruction is one of the common clinical symptoms of peritoneal metastasis of colorectal cancer, and its pathophysiological mechanism involves various factors such as local invasion and compression of tumors, intestinal adhesions caused by the formation of scattered nodules in the abdominal cavity, inflammatory response in the abdominal cavity, and neuromodulation disorders. Patients with traditional intestinal obstruction are generally treated with surgery, but patients with malignant intestinal obstruction associated with peritoneal metastasis of colorectal cancer can p

Small bowel obstruction after caesarean section: an unusual complication of surgical haemostatic agent.

An unusual case of small bowel obstruction (SBO) due to haemostatic gelatin sponge placed during caesarean delivery is presented. A primigravida in their 30s underwent caesarean delivery at 39 weeks, and developed symptoms of SBO from the second postoperative day. Given the worsening condition of the patient and increasing abdominal girth, CT of the abdomen and pelvis was done which revealed features of SBO. Due to no improvement despite conservative management for 3 days, the decision for surgical management was taken. Exploratory laparotomy revealed adhesive obstruction with haemostatic agen

Are the systemic immune-inflammation index and panimmune-inflammation value predictive indicators for the decision of operative treatment in adhesive small bowel obstruction?

BACKGROUND: Emergency surgical pathologies constitute a significant portion of general surgery practice. Small bowel obstructions are a common cause of surgical emergencies in general surgical practice. This study aimed to investigate the predictive role of the Systemic Immune-Inflammation Index and Pan-Immune-Inflammation Value in determining the need for operative treatment in adhesive small bowel obstructions. These obstructions are significant in general surgery, yet clinicians lack consensus on treatment selection and clinical follow-up. This study also seeks to address controversial ques

Nanotechnology-Based Therapies for Preventing Post-Surgical Adhesions.

Adhesions are the body's natural response to various inflammatory causes, with surgery being the most common cause. However, the formation of postoperative adhesions can lead to significant complications, including intestinal obstruction and chronic pain. To prevent such postoperative complications associated with adhesions, developing effective strategies for adhesion prevention has been a major focus of research. Currently, several therapeutic models have been developed to achieve this objective. These include pharmaceuticals, inert polymers, functional biomaterials, and nanotherapeutics. Am

Band Adhesion Causing Intestinal Obstruction Without Previous Surgery in Pregnancy: A Rare Case.

Intestinal obstruction (IO) during pregnancy is a rare occurrence. Although uncommon, IO in pregnancy carries a significant risk of maternal and fetal mortality. Diagnosing IO can be challenging, as abdominal pain and vomiting in pregnant patients may be mistaken for labor pain or hyperemesis gravidarum. Early reports strongly recommend exploratory surgery as the standard treatment once IO is diagnosed. The delivery of the baby during the same procedure may be necessary if the surgical emergency jeopardizes the pregnancy. However, this approach is not ideal if the pregnancy is not at term. In

Platelet-rich fibrin decreases adhesion to polypropylene prosthetic mesh material in ventral hernia repair.

Mesh adhesion is an important complication in prosthetic hernia repairs which may lead to intestinal obstruction, enterocutaneous fistula and viscus perforation. Therefore, direct contact between visceral organs and mesh should be avoided. The aim of this study was to investigate the efficacy of platelet rich fibrin (PRF) as an adhesion barrier. Thirty-six Wistar-Albino rats were used in the study. Four rats were used to obtain platelet-rich fibrin. Remaining rats were divided to four separate groups. In sham group, abdominal wall was repaired by suture without mesh following laparotomy. Abdom

[Abdominal pain out of proportion by a young, healthy patient].

We report on a 29-year-old patient with highly intense, acute abdominal pain of idiopathic origin (out of proportion), admitted to the emergency department with suspected appendicitis. An ultrasound revealed signs of bowel obstruction, and due to persistent pain despite intravenous morphine analgesia, a computed tomography was performed, revealing a closed-loop obstruction. The patient underwent laparoscopy, where an omental adhesion was identified and surgically resolved. The postoperative course was uneventful. This potentially dangerous diagnosis should be considered even in healthy patient

Case Report: Intestinal Diverticulum in a Golden Retriever.

A 1-year-old female spayed Golden Retriever presented with a 2-day history of vomiting and a chronic 4-month history of intermittent diarrhoea. Blood work revealed hepatocellular hepatopathy with mildly elevated baseline cortisol. Abdominal radiographs revealed mineral opacities in the right caudal abdomen, suspicious for foreign material, but with no evidence of small intestinal obstruction. Owners elected to pursue exploratory laparotomy. A firm, tan, irregular structure was observed in the right caudal abdomen with a small adhesion to an adjacent loop of jejunum. The structure placed caudal

MTOR signaling regulates the development of airway mucous cell metaplasia associated with severe asthma.

In asthma, airway epithelial remodeling is characterized by aberrant goblet cell metaplastic differentiation accompanied by epithelial cell hyperplasia and hypertrophy. These pathologic features in severe asthma indicate a loss of control of proliferation, cell size, differentiation, and migration. MTOR is a highly conserved pathway that regulates protein synthesis, cell size, and proliferation. We hypothesized that the balance between MTOR and autophagy regulates mucous cell metaplasia. Airways from individuals with severe asthma showed increased MTOR signaling by RPS6 phosphorylation, which

Small Intestinal Strangulation Secondary to Omental Adhesion Entrapment in a Dog.

A 6-year-old, male neutered, German Shorthaired Pointer was evaluated for acute onset of emesis, tenesmus, and restlessness. Abdominal radiographs suggested gastrointestinal obstruction or torsion. An exploratory laparotomy revealed an omental-to-mesenteric adhesion causing entrapment and strangulation of a segment of the jejunum, along with several other adhesions. The patient had undergone two previous laparotomies, which likely contributed to the extensive adhesion formation found intraoperatively. While postsurgical adhesion formation is a known complication in both human and veterinary me

Water-soluble contrast agent use in adhesional small bowel obstruction: a survey of surgical practices and clinical trial considerations.

BACKGROUND: The role of water-soluble contrast agent (WSCA) as a therapeutic tool in the nonoperative management of adhesional small bowel obstruction (ASBO) remains indeterminate. This survey aimed to understand current surgical practices in WSCA use in the conservative approach to ASBO, and to evaluate key design and feasibility factors to inform a future controlled trial of WSCA in ASBO. METHODS: This study is reported in line with CROSS guidance. The survey consisted of 16 questions organised into three sections: respondent characteristics, current decision-making and WSCA use, and opinio

A Sandwiched Patch for Prevention of Pelvic Adhesion After Uterine Operation.

The prevention of postoperative adhesions remains a critical clinical challenge in gynecological surgery, as these pathological formations may lead to intestinal obstruction, chronic pelvic pain, and reproductive complications. Regarding the clinical application of anti-adhesion barriers, current products including but not limited to Interceed and chitosan-based dressings have demonstrated suboptimal efficacy in practice. We developed a triple-layer patch comprising: (1) a tissue-adhesive inner layer of polyacrylic acid-gelatin composite, (2) a mechanically dissipative intermediate layer of po

Homocysteine(HCY), a novel biomarker for predicting irreversible transmural intestinal necrosis in patients with adhesive small bowel obstruction: results from a prospective observational study.

BACKGROUND: Whether elevated homocysteine level is causally associated with small bowel necrosis remains unestablished. We conducted a prospective observational study to analyze the value of serum homocysteine (HCY) in predicting irreversible transmural intestinal necrosis (ITIN) of adhesive small bowel obstruction (ASBO). METHODS: This prospective observational study was performed between Feb 2023 and Feb 2025 in patients with adhesive small bowel obstruction. The primary outcome was the occurrence of ITIN. The serum levels of different biomarkers in different groups were calculated and comp

Meat bezoar-induced small bowel obstruction: a rare but critical cause of acute intestinal distress.

Acute intestinal obstruction is a common surgical emergency, usually caused by adhesions, tumors, or hernias. Bezoars are a rare but significant cause of small bowel obstruction (SBO), even in patients without prior gastrointestinal surgery. We report a case of a 60-year-old man with no surgical history who presented with nausea, vomiting, abdominal pain, and absence of bowel movements. Imaging suggested SBO, and diagnostic laparoscopy confirmed a meat bezoar obstructing the terminal ileum. Histopathological examination revealed thermally altered liver parenchyma, consistent with undigested co

Application of modified Roux-en-Y digestive tract reconstruction in total gastrectomy for patients with gastric cancer.

BACKGROUND: At present, the concept of surgical treatment of gastric cancer (GC) has changed from "radical treatment" to "care for patients" to a certain extent. The reconstruction method is the most likely to affect the postoperative life of the patient. Currently, the traditional Roux-en-Y esophagojejunostomy anastomosis is a commonly used method for gastrointestinal reconstruction after total gastrectomy for GC. However, more recent studies have shown that the traditional Roux-en-Y anastomosis is complicated in operation procedure, with more reconstruction steps and longer reconstruction ti
