Gastrointestinal bypass surgery for non-strangulated adhesive small bowel obstruction.

The effectiveness of gastrointestinal bypass surgery for treating non-strangulated adhesive small bowel obstruction (ASBO) remains unclear. We aimed to compare the outcomes of gastrointestinal bypass and conventional surgery in patients with non-strangulated ASBO who were unresponsive to conservative treatment. The patients (n = 55) were divided into a bypass group (n = 15) undergoing bypass surgery and a non-bypass group (n = 40) undergoing conventional surgery, including adhesiolysis or small bowel resection. The serum albumin levels and prognostic nutritional index were comparable between t

Congenital band as a rare cause of small bowel obstruction: a case series and review of the literature.

INTRODUCTION: Small bowel obstruction (SBO) is a common surgical emergency, most often caused by adhesions, whereas congenital peritoneal bands (CPBs) represent a rare etiology in adults. Limited contemporary data exist, with most adult series reporting fewer than ten cases. This study describes seven adult patients treated for CPB-related SBO and compares their characteristics and outcomes with those in the literature. METHODS: A retrospective review was conducted of adult patients presenting with SBO due to congenital bands at a single tertiary center between 2010 and 2022. Inclusion criter

Gastrointestinal Basidiobolomycosis Mimicking Adhesive Bowel Disease and Ileocaecal Malignancy in an Adult HIV Patient: A Case Report Highlighting a Diagnostic Pitfall.

INTRODUCTION: Fungi cause gastrointestinal basidiobolomycosis (GIB). It often presents with features that are difficult to differentiate from other gastrointestinal disorders such as malignancy and inflammatory bowel disease. This results in a misdiagnosis and delayed initiation of appropriate treatment. CASE PRESENTATION: We present a case of a 41-year-old female, who had been HIV positive for over 20 years on antiretroviral therapy. The patient presented with intestinal obstruction. She had experienced similar episodes over the preceding month and was managed conservatively but without comp

Strangulated Small Bowel Obstruction Caused by a Migrated Ring-Shaped Intrauterine Device: Two Case Reports and a Literature Review.

BACKGROUND Small bowel obstruction (SBO) is a common surgical emergency, most frequently caused by postoperative adhesions or hernias. SBO resulting from migration of an intrauterine device (IUD) is exceedingly rare but can be life‑threatening. CASE REPORT We present 2 cases of postmenopausal women with long-term IUD retention (>30 years) who developed acute abdominal pain and clinical signs of intestinal obstruction. Preoperative abdominal CT revealed dilated small bowel loops with surrounding fluid and inflammatory changes, along with an ectopic ring-shaped IUD through which a segment of ile

National survey on management practices for adhesional small bowel obstruction and nasogastric tube use among general surgeons in Australia.

BACKGROUND: The nasogastric tube (NGT) is recommended routinely by current guidelines in the non-operative management of adhesional small bowel obstruction (ASBO). However, several retrospective studies have reported poorer outcomes in ASBO patients managed with an NGT. We sought to examine current practices among Australian General Surgeons in managing ASBO and using NGTs, assess the presence of surgical equipoise for a potential randomised controlled trial (RCT), and evaluate interest in multi-centre collaboration. METHODS: In October 2021, we conducted an online survey of all active member

Predictive value of gastrointestinal decompression volume and its trajectory over time for surgical intervention in children with postoperative adhesive small bowel obstruction.

OBJECTIVE: To investigate the predictive value of gastrointestinal decompression volume and its temporal trend for surgical intervention in children with postoperative adhesive small bowel obstruction (ASBO). METHODS: This retrospective study included 131 children with postoperative ASBO who initially received conservative treatment from January 2015 to December 2024. Based on treatment outcomes, patients were categorized into a successful conservative treatment group (CT, n = 80) and a surgical treatment group (ST, n = 51). Clinical data were collected, with emphasis on daily gastrointestina

Minimally invasive versus open surgery for adhesive small bowel obstruction: a systematic review and meta-analysis.

BACKGROUND: Adhesive small bowel obstruction (ASBO) is a common surgical emergency. However, open adhesiolysis has traditionally been the standard operative approach after failed conservative management. The aim was comparing postoperative outcomes between laparoscopic and open adhesiolysis in ASBO. METHODS: A systematic review and meta-analysis was conducted according to PRISMA 2020 and Cochrane guidelines. PubMed, Embase, Scopus, Web of Science, and BVS were searched from inception to September 2025. Randomized controlled trials (RCTs) and comparative cohort studies evaluating laparoscopic

Clinical performance of the Antoine Béclère score in predicting operative requirement in adhesive small bowel obstruction.

BACKGROUND: Adhesive small bowel obstruction (ASBO) remains a common surgical emergency, yet distinguishing patients who will fail conservative management continues to be difficult. Although many clinical and radiologic predictors have been proposed, their accuracy is inconsistent, and simple objective tools are still lacking. METHODS: A retrospective cohort study was conducted including adults admitted with CT-confirmed ASBO between 2020 and 2024. Demographics, comorbidities, laboratory values, PNI, PLR, radiologic parameters, and Antoine Béclère (AB) scores were collected. Predictors of sur

Revisiting the enigma of a large symptomatic peritoneal loose body: a case report and critical literature review.

INTRODUCTION AND IMPORTANCE: Peritoneal loose bodies (PLBs), also known as "peritoneal mice," are rare calcified structures found in the peritoneal cavity. Smaller PLBs are usually asymptomatic, while larger PLBs can sometimes lead to abdominal discomfort, urinary retention, or intestinal obstruction. Diagnostic imaging, particularly computed tomography scanning, identifies these centrally calcified soft tissue masses. CASE PRESENTATION: A 55-year-old Indo-Aryan man with chronic constipation presented with signs of intestinal obstruction. Initial imaging suggested a mesenteric mass, leading t

Intestinal stent with polydopamine-enhanced sodium alginate/polyacrylamide hydrogel coating: Design, fabrication and anti-migration performance evaluation.

Endoscopic stent placement is an established effective intervention for relieving acute malignant obstruction secondary to colorectal tumors. However, clinically used stents are frequently associated with complications such as tumor ingrowth and stent migration, which limit their long-term efficacy. In this study, we developed a novel 3D-printed flexible tubular intestinal stent coated with polydopamine-enhanced sodium alginate/polyacrylamide (SA/PAM) hydrogel (hereafter referred to as PSP hydrogel). The stent itself acts as a physical barrier to inhibit tumor ingrowth, whereas the PSP coated

Biomimetic natural polysaccharide-peptide hydrogels via hierarchical co-assembly: A dynamic nanofibrous shield for active intervention of postoperative adhesions.

Postoperative adhesions are common and serious complications following abdominal and gynecological surgery, leading to chronic pain, infertility, and intestinal obstruction, yet safe and effective clinical options remain limited. Inspired by the ordered architecture of the extracellular matrix (ECM), we engineered an injectable biomimetic hydrogel composed of naturally derived building blocks through the hierarchical co-assembly of hyaluronic acid (HA) and the antimicrobial peptide Temporin-SHF (SHF). Driven primarily by hydrophobic interactions and further stabilized by hydrogen bonding and π

Supramolecular nanogel aggregate as injectable regulatory barrier for preventing postoperative adhesion in diabetic colonic anastomosis.

Postoperative tissue adhesions, especially after intestinal surgeries like colonic anastomosis, frequently cause serious complications such as intestinal obstruction and chronic pain, which are worsened in diabetic conditions. Although barrier materials have advanced, it remains challenging to simultaneously achieve targeted localization, dynamic mechanical adaptability, anti-adhesion efficacy, and bio-regulatory functions. Here, we develop an injectable supramolecular aggregate barrier formed by aggregation-induced gelation of thermosensitive amphoteric nanogels. The resulting aggregate posse

Robotic adhesiolysis: current evidence, technical considerations, and future directions.

Adhesiolysis is a technically demanding component of abdominal and pelvic reoperation. Robotic platforms may offer theoretical advantages in complex dissections; however, the clinical evidence specific to robotic adhesiolysis is limited. This review summarizes the current evidence, robotic-specific technical considerations, clinical rationale, patient selection, and future research priorities for robotic surgery in operations requiring adhesiolysis. A targeted narrative review of peer-reviewed literature on adhesion-related morbidity, adhesive small bowel obstruction (ASBO), laparoscopic adhes

Clinical features of failure of conservative management in adult patients with adhesive small bowel obstruction: a retrospective cohort study.

BACKGROUND: Adhesive small bowel obstruction (ASBO) is a common cause of acute abdominal pain in adults. Most patients with ASBO without strangulation are successfully managed conservatively. However, some patients fail nonoperative treatment, develop intestinal ischemia and undergo surgical management. The clinical features of these patients require further investigation. METHODS: Patients who received conservative management as the initial treatment for ASBO between January 2017 and December 2025 were selected from the electronic medical records of a single institution. The patients were ca
