Aetiologies and prognosis of small bowel obstruction in virgin abdomen: a retrospective cohort study.

BACKGROUND: Small bowel obstruction (SBO) in patients with a virgin abdomen (VA)-no prior abdominal surgery-remains understudied, with traditional assumptions favoring mandatory surgical exploration due to suspected non-adhesive etiologies like malignancy or hernias. However, emerging evidence suggests adhesions are also prevalent in SBO-VA, challenging this paradigm. This study investigates the etiologies, management outcomes, and prognostic factors of SBO-VA to guide evidence-based treatment strategies. METHODS: A retrospective analysis was conducted on 312 SBO-VA patients treated at a tert

Case report: small bowel volvulus following laparoscopic-assisted Soave pull-through for adult Hirschsprung disease: a diagnostic dilemma.

INTRODUCTION AND IMPORTANCE: Small bowel volvulus (SBV) is the twisting of a small bowel loop around its mesentery, leading to closed-loop intestinal obstruction. SBV following laparoscopic procedures is a rare complication. Patients often present with non-specific symptoms, creating a diagnostic dilemma and delaying surgical intervention. A high index of suspicion and timely imaging are essential. Understanding the mechanisms, risk factors, clinical presentation, and management is paramount. CASE PRESENTATION: A 21-year-old woman with a confirmed diagnosis of adult Hirschsprung disease under

Primary Small Intestinal Volvulus in a 91-Year-Old Patient: A Rare Case of Successful Laparotomy With Resection and Anastomosis.

Primary small intestinal volvulus (PSIV) is a rare cause of acute bowel obstruction in adults and is exceptionally uncommon in the very elderly. It occurs when the small intestine twists around its mesenteric axis without an identifiable secondary cause, such as adhesions or hernias. This case report describes a 91-year-old woman who presented with abdominal pain and minimal rectal bleeding, posing a diagnostic challenge in the context of multiple comorbidities and atypical symptoms. CT imaging demonstrated a persistent "whirlpool sign," raising suspicion for volvulus, and the patient underwen

The flower bouquet sign and the fat notch sign in adhesive small bowel obstruction.

A 55-year-old woman presented with vomiting and abdominal pain. Physical examination revealed abdominal tenderness, but no rebound tenderness. A computed tomography (CT) scan revealed a distended small intestine called the flower bouquet sign, as well as the fat notch sign. These findings supported the suspicion of adhesive small bowel obstruction (SBO). Emergency surgery confirmed the presence of an adhesive band formed and it was resected. The patient recovered uneventfully. Most cases of SBO are caused by adhesions, followed by internal hernias and volvulus. CT scanning is essential for dia

A Case of Congenital Peritoneal Encapsulation Complicated with Left Paraduodenal Hernia Presenting as Recurrent Small Bowel Obstruction.

INTRODUCTION: Congenital peritoneal encapsulation (CPE) is a rare anomaly in which the small intestine is enclosed in a thin peritoneal sac. Although mostly asymptomatic, CPE can cause severe intestinal obstruction due to strangulation and torsion. CASE PRESENTATION: A 16-year-old male patient had been experiencing recurrent intestinal obstruction of unknown cause since the age of 13. He was admitted with persistent abdominal pain as his chief complaint, and emergency laparoscopic surgery was performed due to a suspected strangulating intestinal obstruction. The findings included an abnormal

Meckel's diverticulum band adhesion causing internal hernia, small bowel obstruction complicated by peritonitis and massive ascites: a rare pediatric case.

Meckel's diverticulum (MD) is one of the most common congenital anomalies of the gastrointestinal tract in children, although it may occasionally be detected in adults. It is typically located within 100 cm of the ileocecal valve in the distal ileum. The diverticulum varies in size and morphology; many patients remain asymptomatic throughout life. In children, the most common clinical presentation is painless gastrointestinal bleeding, followed by intestinal obstruction. Intussusception may also occur. Once complications develop, patients often present with acute abdomen. Failure to establish

Early postoperative ileoileal intussusception following adhesiolysis and enterouterine fistula resection in an adult: a rare case report.

Adult intussusception is a rare cause of intestinal obstruction and is typically associated with an underlying pathological lead point. Postoperative intussusception without an identifiable lesion is particularly uncommon and may present diagnostic and therapeutic challenges. We report the case of a 44-year-old woman who developed epigastric pain, nausea, and vomiting on postoperative Day 3 following small bowel resection and enterouterine fistula excision for adhesive bowel obstruction. Contrast-enhanced abdominal computed tomography demonstrated a 20-cm ileoileal intussusception with a chara

Proximal Roux-en-Y Limb Intussusception After Pediatric Living Donor Liver Transplantation for Biliary Atresia: A Rare Surgical Emergency.

BACKGROUND: Intussusception of the Roux-en-Y limb after pediatric liver transplantation (LT) is an extremely rare but potentially life-threatening complication. Altered anatomy, postoperative adhesions, and disrupted intestinal motility create a unique pathophysiological environment, which may delay diagnosis and increase morbidity. METHODS: We retrospectively analyzed a patient with acute intestinal obstruction after living donor liver transplantation. Clinical presentation, laboratory findings, imaging results, intraoperative observations, surgical management, and outcomes were reviewed. Th

Meckel's Diverticulum Causing Small Bowel Obstruction in Male Patients: A Case Series.

Meckel's diverticulum is a congenital abnormality of the gastrointestinal tract, resulting from an incomplete obliteration of the vitello-intestinal duct. Generally, Meckel's diverticulum is asymptomatic, but it can present as intestinal perforation, obstruction, hemorrhage, and diverticulitis. These complications can develop through various mechanisms such as volvulus, adhesions, intussusception, and internal hernia. Small bowel obstruction due to a meso-diverticular band of Meckel's diverticulum is a rare complication. Herein, we present a case series of three male patients presenting with i

SPARC induction and correlation with inflammation and fibrosis in a mouse model of alkali-induced abdominal adhesions.

Postoperative abdominal adhesions can cause several complications, such as intestinal obstruction, female infertility, and chronic pelvic pain. By alkaline cauterization of the cecum, we established a mouse abdominal adhesion model to investigate the roles of secreted protein acidic and rich in cysteine (SPARC) in adhesion formation. Alkaline cauterization led to elevation of adhesion scores and mRNA levels of SPARC, monocyte chemoattractant protein-1 (MCP-1), tumor necrosis factor-α (TNF-α), and collagen I on Days 3, 7, and 14, with a positive correlation between SPARC levels and MCP-1, TNF-α

Predictors of surgical intervention and timing of ileus presentation in patients with adhesive small bowel obstruction.

BACKGROUND: Adhesive small bowel obstruction remains a common long-term complication following abdominal surgery and continues to represent a significant cause of morbidity and healthcare utilization. While several factors contributing to adhesion formation have been described, determinants influencing both the need for surgical intervention and the timing of clinical presentation are not yet fully understood. This study aimed to identify factors associated with surgical management in patients presenting with adhesive ileus after a single prior abdominal operation and to evaluate the relations

Mechanical Small Bowel Obstruction Secondary to Low-Grade Appendiceal Mucinous Neoplasm: A Case Report.

Low-grade appendiceal mucinous neoplasm (LAMN) is an uncommon epithelial tumor that is frequently diagnosed incidentally and only rarely produces mechanical small bowel obstruction, with previous reports limited to isolated cases. We describe a 53-year-old man with a four-day history of periumbilical pain, obstipation, nausea, and bilious vomiting. Examination showed abdominal distension, decreased bowel sounds, and rebound tenderness. Contrast-enhanced computed tomography (CT) demonstrated a 7 cm heterogeneous right hemiabdominal mass with pericecal fat stranding that favored, but did not def

Small bowel obstruction in older adults: similar nonoperative success but worse outcomes across management strategies - insights from the ESTES SnapSBO study.

BACKGROUND: Small bowel obstruction (SBO) is a common surgical emergency. It remains unclear whether adverse outcomes in older patients reflect impaired obstruction resolution, differences in disease phenotype, or reduced physiologic tolerance. METHODS: We performed a prespecified age-stratified analysis of the multicenter SnapSBO cohort, which consecutively enrolled adults with radiologically-confirmed mechanical SBO, stratified as younger (< 65 years) or older (≥ 65 years). Presentation, etiology, management, and outcomes were compared using risk differences and odds ratios. Exploratory ana

Cocoon Syndrome: A Clinical Case of Intestinal Obstruction Highlighting the Diagnostic and Therapeutic Importance of Exploratory Laparoscopy in the Face of Ambiguous Imaging.

Cocoon syndrome, also known as sclerosing encapsulating peritonitis (SEP), is a rare condition characterized by the encapsulation of small bowel loops by a thick fibrocollagenous membrane. Its non-specific clinical presentation and ambiguous findings on conventional imaging often lead to significant diagnostic delays or misdiagnosis, frequently being mistaken for internal hernias or neoplastic processes. We present the case of a 61-year-old male patient with no prior abdominal surgical history who presented with a six-month history of recurrent abdominal pain, vomiting, and weight loss. While

Development and internal validation of the Operate-Now Score for early prediction of clinically significant intestinal ischemia in adhesive small bowel obstruction.

BACKGROUND: Adhesive small bowel obstruction (ASBO) is the leading cause of mechanical small bowel obstruction in developed countries and represents a major burden in emergency surgery. Early identification of clinically significant intestinal ischemia remains challenging. Delayed surgical intervention increases the risk of bowel resection and postoperative morbidity. This study aimed to develop and internally validate a simplified clinical-radiological score for early prediction of intestinal ischemia requiring resection. METHODS: A retrospective single-center observational study was conduct
