Early ileal perforation with fecal peritonitis secondary to adhesive small bowel obstruction after repeat cesarean section: a case report.

Adhesive small bowel obstruction (ASBO) in the immediate postpartum period is uncommon and may mimic postoperative ileus. A 26-year-old woman returned 48 hours after repeat cesarean section with abdominal distension, vomiting, and obstipation. Her history included a previous cesarean delivery and prior midline laparotomy for ovarian torsion with appendectomy and right salpingo-oophorectomy. Radiography showed dilated small-bowel loops with air-fluid levels, without initial peritoneal signs. After nasogastric decompression and observation, she deteriorated ~10 hours later with worsening pain, p

[Indications and management of nasogastric tube].

. Indications and management of nasogastric tube. The review by Alverdy et al examines current evidence on the indications, placement, and management of nasogastric tubes (NGTs) used for gastric decompression. Historically employed since the 17th century, modern NGTs - particularly the Salem sump - enable effective aspiration while minimizing mucosal injury. The primary indication remains small bowel obstruction, most often caused by adhesions. In uncomplicated cases without ischemia or peritonitis, NGT based conservative management resolves approximately 75% of obstructions. Water soluble con

Closed duodenal loop syndrome in newborns: a rare case report of concomitant pyloric and distal duodenal atresias and focused literature review.

Congenital pyloric atresia (CPA) is a rare cause of neonatal gastric outlet obstruction. Although usually isolated, CPA may coexist with additional gastrointestinal atresias. Its association with distal duodenal atresia (DA) is particularly relevant because the segment between the two obstructions may become a closed duodenal loop into which biliary and pancreatic secretions continue to drain. We report the case of a male newborn delivered at 35 + 3 weeks of gestation after prenatal detection of severe polyhydramnios, recurrent bowel dilatation, abdominal effusion, and a cystic abdominal lesio

Layer-Resolved Spatial Transcriptomic Profiling Reveals Distinct Fibrotic and Muscularizing Submucosal Niches in Fibrostenotic Crohn's Disease.

BACKGROUND AND AIMS: Fibrostenotic stricture is a severe complication of Crohn's disease (CD) that affect up to 50% of patients over their lifetime, causing irreversible narrowing of the intestinal lumen and bowel obstruction, but the molecular programs that distinguish stenotic from non-stenotic inflammation and the relative contributions of fibrosis and smooth-muscle remodeling remain incompletely characterized. We aimed to dissect bowel-wall compartment-specific and histology-defined remodeling programs in fibrostenotic CD. METHODS: Matched full-thickness terminal ileal tissues from 4 pati

Strangulated Adult Bochdalek Hernia With Extensive Small Bowel Necrosis and Septic Shock.

Bochdalek diaphragmatic hernias are congenital diaphragmatic defects through which abdominal viscera herniate into the thoracic cavity and are typically diagnosed in newborns and infants. Adult presentations are uncommon and may present with heterogeneous manifestations, ranging from asymptomatic to life-threatening complications. A 72-year-old male with a history of urinary retention presented with acute lower abdominal pain and vomiting. Initial physical examination and laboratory studies suggested a urinary tract infection. Despite adequate treatment, the patient's condition deteriorated, a

Multicenter Long-Term Outcomes of Laparoscopic Versus Open Ladd's Procedure for Intestinal Malrotation.

PURPOSE: The Ladd's procedure is the surgical treatment for symptomatic intestinal malrotation in children. Although laparoscopic techniques have gained popularity, there is concern that it may lead to decreased adhesion formation and thus increased susceptibility to volvulus. This study examined long-term outcomes of laparoscopic versus open Ladd's procedure. METHODS: Retrospective review was conducted of 281 patients who underwent Ladd's procedure at a large integrated healthcare system from 2010 - 2024. Choice of technique (laparoscopic or open) was based on surgeon preference. 109 patient

Intracorporeal anastomosis versus extracorporeal anastomosis for laparoscopic right hemicolectomy: short-term and long-term outcomes.

BACKGROUND: Laparoscopic right hemicolectomy is the standard of care for non-locally advanced right-sided colonic neoplasms and endoscopically unresectable polyps amenable to laparoscopic resection, but the optimal ileocolic anastomotic technique remains debated. This study compares intracorporeal anastomosis (ICA) and extracorporeal anastomosis (ECA) outcomes in laparoscopic right hemicolectomy performed by three surgeons at a Canadian community hospital. METHODS: This retrospective study analyzed patients who underwent elective laparoscopic right hemicolectomy with ICA using Pfannenstiel ex

Case report: retentive fibroid tumor of the ileocecal region during pregnancy.

BACKGROUND: Desmoid type fibromatosis (DTF) is a rare, non-metastasizing but locally invasive intermediate soft tissue tumor. With an annual incidence of 2-4 per million, it is particularly rare during pregnancy. The particularity of this case is that the patient is pregnant, 14 weeks gestation, placenta previa, and a large mass is found in the abdominal pelvic cavity. The patient requests to terminate the pregnancy and remove the abdominal pelvic cavity mass. The large mass originates from ileocecal area, which is extremely rare and can be an excellent supplement to the literature. CASE DESC

A Bedside Prediction Score for Strangulation in Adults With Acute Mechanical Intestinal Obstruction: A Prospective Cohort Study in Taiz, Yemen.

Background Early recognition of bowel strangulation in acute mechanical intestinal obstruction (AMIO) is critical but remains difficult when computed tomography (CT) is unavailable, delayed, or not immediately actionable. This study aimed to develop and internally validate a CT-independent bedside prediction score for estimating the risk of bowel strangulation in adults with AMIO in Taiz, Yemen. Materials and methods This 12-month prospective observational cohort study, with diagnostic and prediction-model components, enrolled consecutive adults with suspected AMIO at Al-Thawra Hospital and Al

Surgical Treatment for Bacteremia due to Translocation of Enterococcus gallinarum: A Case Report.

INTRODUCTION: Bacterial translocation (BT) followed by bacteremia is usually managed with antibiotics, but some cases remain refractory. Herein, we report an immunocompromised patient with bacteremia due to the translocation of Enterococcus gallinarum, a rare pathogen among enterococcal infections, who was successfully treated with surgical intervention. CASE PRESENTATION: A 69-year-old woman with prior low anterior resection for rectal cancer developed adhesive intestinal obstruction during steroid treatment for suspected pyoderma gangrenosum. She progressed to septic shock despite antibioti

Midgut malrotation and ovarian dermoid cyst as a dual cause of neonatal intestinal obstruction: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Midgut malrotation is a congenital anomaly characterized by impaired rotation of the gut around the superior mesenteric artery during embryonic development. It is one of the most common causes of neonatal intestinal obstruction. A dermoid cyst arises from the trapping of ectodermal elements during embryonic development. CASE PRESENTATION: A 6-day-old female neonate was brought to the hospital with a history of bilious emesis and failure to pass stool. The antenatal history was uneventful. She was diagnosed with intestinal obstruction. The patient was treated initi

Recurrent duodenal obstruction caused by Ladd's bands in an adult: a case report and review of the literature.

INTRODUCTION: Intestinal malrotation is a congenital anomaly resulting from incomplete midgut rotation. While it most commonly becomes symptomatic in infancy, adult presentations are rare and pose diagnostic difficulties. In particular, obstruction due to persistent Ladd's bands in adults is uncommon and requires surgical attention. PRESENTATION OF THE CASE: We describe a 43-year-old male patient with recurrent episodes of small-bowel obstruction over 3 years. The initial presentation included bilious vomiting and CT evidence of duodenal obstruction, which was managed conservatively. Upon rec

Ileosigmoid knotting: a rare case of intestinal obstruction with intra-operative finding of type II B ileosigmoid knotting.

Ileosigmoid knotting is one of the rarest emergencies, occurring when the sigmoid colon or ileum wraps around the base of the other, obstructing the flow. It is considered a rare cause of intestinal obstruction. Clinical signs and symptoms are nonspecific, making preoperative diagnosis challenging. Delays in diagnosis and management may lead to bowel necrosis or peritonitis. Here, we present the case of a 37-year-old male who presented with severe abdominal pain, initially misdiagnosed as adhesive bowel obstruction. ISK was diagnosed intra-operatively.

A Rare Case of Gangrenous Meckel's Diverticulum and Acute Appendicitis Presenting As Closed-Loop Obstruction in a Young Adult.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract, arising from incomplete obliteration of the vitelline duct. Although often asymptomatic, it can present with life-threatening complications such as inflammation, bleeding, intestinal obstruction, and perforation. Closed-loop obstruction secondary to Meckel's diverticulum is rare, and its coexistence with acute appendicitis is exceptionally uncommon, posing a diagnostic challenge. We report the case of a 22-year-old male who presented to the emergency department with a two-day history of diffuse abdominal

Incidental Diagnosis of a Gossypiboma With Complete Transmural Migration Into the Jejunum After a Hysterectomy: A Case Report.

Gossypiboma, or retained surgical sponge, is a rare and preventable surgical complication. Complete transmural migration of a retained sponge into the gastrointestinal tract without overt intestinal perforation is an exceptionally unusual phenomenon that poses significant diagnostic and therapeutic challenges. A 41-year-old woman with a history of open hysterectomy seven months prior presented with progressive neurological deterioration, severe metabolic encephalopathy, and suspected urosepsis, without classical signs of mechanical bowel obstruction. Abdominal computed tomography (CT) demonstr

Strangulated Small Bowel Obstruction Caused by a Hernia-Related Adhesive Band after Successful Reduction of an Incarcerated Inguinal Hernia.

INTRODUCTION: Incarcerated inguinal hernia is a common surgical emergency. Persistent bowel obstruction after successful reduction is typically attributed to reduction en masse, whereas other mechanisms of ongoing strangulation are rarely reported. We describe a rare case in which bowel strangulation persisted despite successful reduction of an incarcerated inguinal hernia due to a hernia-related adhesive band. CASE PRESENTATION: A 92-year-old man presented with an incarcerated left inguinal hernia. Manual reduction was successfully performed, and contrast-enhanced CT confirmed reduction of t

Cecal Volvulus Caused by a Strangulating Fallopian Tube: A Case Report and Brief Literature Review.

Cecal volvulus occurs when the cecum, ascending colon, and terminal ileum twist around the mesenteric pedicle. It is a rare cause of intestinal obstruction, and several associated factors include cecal hypermobility, previous abdominal surgery, adhesions, colonic tumors, pregnancy, abdominal masses, and congenital conditions such as Ehlers-Danlos syndrome. Without prompt recognition, cecal volvulus may progress to necrosis, gangrene, or perforation, resulting in sepsis and necessitating emergent operative intervention. Cecal volvulus secondary to strangulation by a fallopian tube is an even ra

Dual-mechanism small bowel obstruction from Meckel diverticulum in an 8-year-old: a case report.

We report a dual-etiology obstruction in an 8-year-old boy: congenital closed-loop small bowel obstruction (SBO) from a volvulized Meckel diverticulum tethered by an omphalomesenteric duct remnant, followed by early postoperative focal SBO from an omental band. He presented after one week of abdominal pain with worsening and non-bloody, non-bilious emesis. Contrast-enhanced computed tomography showed distal SBO with a mesenteric swirl. Laparoscopy confirmed a necrotic Meckel diverticulum with volvulized distal ileum; the tethering band was divided and segmental ileal resection with anastomosis
