Disseminated Peritoneal Nematode Infestation Causing Adhesive Small Bowel Obstruction and Bowel Gangrene Following Perforated Appendicitis: A Case Report.

Disseminated peritoneal nematode infestation is a rare extra-intestinal manifestation of ascariasis, typically occurring following bowel perforation. We report a case of a male in his 60s presenting with features of acute small bowel obstruction, including abdominal pain, distension, obstipation, and bilious vomiting, with a history of perforated appendicitis managed surgically six months prior. Contrast-enhanced computed tomography suggested small bowel obstruction. Intraoperative findings revealed dense adhesions, gangrenous ileum, and multiple nodular lesions over the small bowel, mesentery

Development and implementation of "Mazboot Bunyad": a digital health intervention for the screening and management of undernutrition among children aged 6-59 months, at private provider clinics in urban slums of Karachi, Pakistan.

BACKGROUND: Although factor Xa inhibitors have become a popular choice for chronic oral anticoagulation, effective drug reversal remains difficult due to a lack of specific antidote. Currently, 4-factor prothrombin complex concentrate (4F-PCC) is considered the treatment of choice for factor Xa inhibitor-related major bleeding. However, safety of 4F-PCC and its risk of thrombosis when used for this off-label purpose remain unclear. The purpose of this retrospective study is to determine the rate of thromboembolism when 4F-PCC is used for the emergent reversal of factor Xa inhibitors. METHODS:

Intestinal Ischemia Secondary to Ladd's Bands Obstruction Associated With Intestinal Malrotation in a One-Year-Old Pediatric Patient: A Case Report.

Intestinal malrotation is a congenital anomaly resulting from an alteration in the rotation and fixation of the midgut during embryonic development. Ladd's bands are abnormal peritoneal adhesions that can cause extrinsic compression of the bowel, obstruction, and vascular compromise, leading to intestinal ischemia. Although the diagnosis is typically made in the neonatal period, late presentations exist, posing a diagnostic and therapeutic challenge. We describe the case of a previously healthy one-year-old male patient who was admitted for feeding intolerance, accompanied by vomiting, progres

System-level opportunities to reduce mortality in intestinal obstruction: US trends, 1999-2023.

BACKGROUND: Intestinal obstruction is among the most common surgical emergencies worldwide, yet contemporary, population-level mortality patterns in the United States, especially across demographic and geographic strata, remain incompletely defined. METHODS: We used National Vital Statistics System death certificates, accessed via Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research (1999-2023), to identify United States deaths among persons aged 35 years or older with intestinal obstruction as the underlying cause. Age-adjusted mortality rates per 10

Butyrate mitigates multi-organ damage in urosepsis: associated changes in the renal NOX4/ROS/NF-κB axis and gut microbiota.

INTRODUCTION: Urosepsis is a kind of sepsis resulting from urogenital infections, marked by a significant prevalence of organ failure and mortality. This study sought to examine the preventive effect and possible mechanism of butyrate against multi-organ damage caused by urosepsis. METHODS: This work utilized a ureteral obstruction model complicated by infection to more precisely replicate acute kidney injury and the systemic inflammatory response associated with urosepsis. Rats received butyrate supplementation or antibiotic therapy following modeling, multi-organ function, histopathology, i

Human bacteraemia caused by Pseudomonas plecoglossicida, a fish pathogen of ayu (Plecoglossus altivelis): a case report.

Pseudomonas plecoglossicida is a gram-negative bacterium that primarily infects fish and is the causative agent of bacterial haemorrhagic ascites in ayu (Plecoglossus altivelis). Although sporadic human infections such as pneumonia and meningitis have been reported in recent years, human bacteraemia caused by P. plecoglossicida has not yet been documented. Here, we report the first known case of P. plecoglossicida bacteraemia in an 89-year-old male with a history of gastrectomy and recurrent adhesive intestinal obstruction. Following treatment for intestinal obstruction and aspiration pneumoni

Peritoneal and nodal abdominal tuberculosis presenting as intestinal obstruction mimicking malignancy: a case report.

BACKGROUND: Abdominal tuberculosis (TB) is an uncommon form of extra pulmonary TB that often presents with nonspecific symptoms and poses significant diagnostic challenges. Its clinical and radiologic features can closely mimic malignancy and other abdominal pathologies, frequently leading to delayed diagnosis and management, especially in resource-limited settings. CASE PRESENTATION: We report a 20-year-old Eritrean male presenting with a 1-month history of abdominal pain, fever, weight loss, and later respiratory symptoms. Initial evaluation suggested pneumonia; however, persistent symptoms

Adapting the Daily Inventory of Stressful Events for SMS Delivery: Recommendations for Research.

Vernal keratoconjunctivitis (VKC) is a chronic and potentially sight-threatening disease. Topical corticosteroids (Cs) seem to be the only effective treatment for this condition, although severe side effects may occur owing to their prolonged use. More recently, cyclosporine (Cyc) eye drops have been reported as a valid alternative, but so far such treatment has only been successfully experimented for a short time and in small numbers of patients. The aim of our study is to evaluate the long term safety and efficacy of topical cyclosporine eye drops in children suffering from VKC. Over a perio

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Ileocecal CD5-Positive Diffuse Large B-Cell Lymphoma Presenting with Acute Obstructive Symptoms: A Case Report.

INTRODUCTION: Primary gastrointestinal lymphoma is a rare form of extranodal non-Hodgkin lymphoma, accounting for approximately 1%-4% of all gastrointestinal malignancies. Among primary gastrointestinal lymphomas, involvement of the small intestine accounts for approximately 20%-30% of cases. CD5-positive diffuse large B-cell lymphoma (DLBCL) is a rare variant, representing approximately 5%-10% of all DLBCL cases and is associated with advanced age, extranodal involvement, and poorer prognosis. We report a rare case of ileocecal CD5-positive DLBCL presenting with acute obstructive symptoms. C

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Expression of V‑domain Ig suppressor of T cell activation in ovarian cancer and its regulation of follicular regulatory T‑cell function via the AKT/MAPK pathway.

BACKGROUND: Patients with eating disorders (ED) often suffer from compulsive exercise behavior, which is associated with lower short-term response to treatment and poorer long-term outcome. Evidence-based interventions specifically targeting compulsive exercise behavior have been scarce so far. We developed a manualized group therapeutic approach integrating cognitive-behavioral therapy, exercise therapy and exposure with response management to promote healthy exercise behavior. Our objective was to examine the feasibility and acceptance of this new approach as add-on to regular inpatient trea

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Ifebemtinib plus garsorasib in previously treated metastatic colorectal cancer with KRASG12C mutation: a multicentre, randomised, phase 1b/2 trial.

BACKGROUND: Ifebemtinib is a potent oral focal adhesion kinase inhibitor. Preclinical evidence supports combining ifebemtinib with the KRASG12C inhibitor garsorasib. This study aimed to evaluate this combination in KRASG12C-mutated solid tumours. METHODS: This multicentre, phase 1b/2 study had a phase 1b component to establish the recommended phase 2 dose and a phase 2 multitumour expansion component. In phase 1b, the safety and tolerability of ifebemtinib combined with garsorasib was assessed using a 3 + 3 design in KRASG12C-mutated solid tumours. No dose-limiting toxic effects were observed

Coexisting adhesive small-bowel obstruction and giant peritoneal loose body in a surgically naïve patient: a case report.

INTRODUCTION AND IMPORTANCE: Small-bowel obstruction (SBO) is a common surgical emergency, most often caused by postoperative adhesions. However, it may also occur in patients without prior abdominal surgery, commonly referred to as the "virgin abdomen." In such cases, the causes include congenital bands, inflammation, neoplasms, or rare intra-abdominal entities such as peritoneal loose bodies, which are typically discovered incidentally during imaging or surgery. CASE PRESENTATION: A 42-year-old man presented with a 3-day history of progressive crampy abdominal pain, bilious vomiting, abdomi

Gut microbiota in patients receiving dialysis: Mechanisms, complications and the road to personalized microbiome‑based care (Review).

UNLABELLED: In vivo evaluation of the immunotoxicity of xenobiotics is mainly carried out in rodents. Recent trends following the 3R (reduction, refinement, replacement) principle have driven development of in vitro methods for screening immunotoxic chemicals. The recently reported IMMUNOTOX-T immunotoxicity assay uses the human monocytic leukemia cell line THP-1, and evaluates potential immunotoxicity by profiling production of various cytokines by cells treated with test chemicals. Development of the assay involved small-scale evaluation of transferability, within-laboratory repeatability (W
