[Ileal Crohn's Disease as a Form of Presentation of Inflammatory Bowel Disease in Pediatrics].

Pediatric Crohn's disease localized exclusively to the ileum is a rare condition in our setting. Due to its overlapping clinical presentation, a high index of suspicion is required, and differential diagnoses must be considered with respect to infectious, infiltrative, and other less frequent conditions. We present the case of an 11-year-old patient with a history of anemia, pain in the right iliac fossa, weight loss, and fever. Imaging studies revealed thickening of the terminal ileum and intestinal stenosis near the ileocecal valve, with prestenotic dilation. Video colonoscopy showed that th

Giant Sigmoid Fecaloma Causing Complete Large Bowel Obstruction in a Patient With Intellectual Disability: A Case Report.

Fecaloma is a rare but clinically significant cause of large bowel obstruction, often associated with chronic constipation and underlying conditions affecting bowel motility. We report the case of a 50-year-old man with a history of intellectual disability who presented with a 10-day history of progressive abdominal pain, obstipation, abdominal distension, vomiting, and intolerance to oral intake. Physical examination revealed a distended abdomen with decreased bowel sounds and no signs of peritoneal irritation. Laboratory findings were unremarkable except for a mild elevation of lactate dehyd

Chronic Pica Resulting in Perforative Peritonitis: A Case Report.

Foreign body (FB) ingestion is a frequent clinical presentation, particularly among children and individuals with intellectual disabilities. However, recurrent ingestion associated with pica represents a high-risk subset, often leading to cumulative gastrointestinal injury, operative complexity, and increased mortality. We report the case of a 23-year-old male patient with severe intellectual disability who presented with abdominal pain and distension of five-day duration. The patient had a significant history of recurrent FB ingestion, predominantly metallic objects, and his status was post p

Rapunzel Syndrome in a Pediatric Female Patient: A Case Report of Trichobezoar Causing Intestinal Obstruction.

Rapunzel syndrome is a very rare type of trichobezoar in which a hair mass extends to the stomach and small intestine forming a body and tail. It usually affects young females with a psychiatric background. Symptoms are usually nonspecific and often delay the diagnosis. We present in this report a case of a six-year-old previously healthy girl who presented with two months of intermittent abdominal pain, vomiting, and constipation. Initial laboratory tests and two times abdominal ultrasounds were unremarkable. Abdominal X-ray showed moderate dilatation of the bowel. Further physical examinatio

Intestinal tuberculosis in a child: a case report.

Tuberculosis (TB) is an infectious disease and one of the major causes of mortality worldwide, especially in children under the age of 15 years. Abdominal TB includes TB of bowel, peritoneum, lymph nodes, or other solid organs, with a predominance of intestinal and peritoneal forms of the disease. Most pediatric cases may be misdiagnosed due to subclinical symptoms. This case report describes an 11-year-old boy diagnosed with intestinal tuberculosis (ITB) complicated by bowel obstruction. No prior recommendation for surgical therapy resulted in a significant complication of enterobiasis. ITB i

Myeloid Sarcoma Arising in a Rare Anatomical Location: A Case Report.

Myeloid sarcoma (MS) is a rare extramedullary tumor of immature myeloid cells that can occur concurrently with acute myeloid leukemia (AML), precede its onset, or present in isolation. Gastrointestinal involvement is uncommon, and diagnosis is often challenging due to non-specific imaging features and clinical presentation. We report the case of a 75-year-old patient with newly diagnosed AML who was found to have synchronous cecal wall thickening and an adjacent hypodense lesion on abdominal computed tomography (CT). Due to an impending obstruction, surgery was performed, and the patient under

Adult Annular Pancreas Presenting With Gastric Outlet Obstruction Following Semaglutide Initiation: A Case Report.

Annular pancreas is a rare congenital anomaly that may remain clinically silent until adulthood. Its symptoms can be masked or exacerbated by glucagon-like peptide-1 receptor agonists (GLP-1 RAs), which delay gastric emptying. We report a 55-year-old male with type 2 diabetes who developed progressive postprandial vomiting and nine-kg weight loss following semaglutide initiation. Imaging revealed a partial annular pancreas causing second-part duodenal stenosis without malignancy. Symptoms persisted despite semaglutide discontinuation, supporting a fixed mechanical obstruction as the primary ae

The Gamboa-Hoil Point for Primary Perineal Closure During Total Pelvic Exenteration.

Introduction Total pelvic exenteration (TPE) is a radical procedure for selected patients with advanced pelvic malignancies and is associated with significant perineal morbidity. Despite multiple reconstructive strategies, no consensus exists regarding the optimal strategy for perineal closure. The Gamboa-Hoil point is a standardized primary closure technique proposed by the authors, designed to reinforce perineal wound closure using a simple suture-based approach. Methods We conducted a single-center, retrospective descriptive study including patients with colorectal cancer (rectal, sigmoid c

Acute Postoperative Intestinal Obstruction Due to Abdominal Drains: A Case Report.

Early small bowel obstruction caused by a drain in the postoperative period is a rare complication, with few cases reported in the literature. Its diagnosis is difficult, and its complications can be severe. The objective of this case report is to describe the clinical and radiological features suggestive of intestinal occlusion associated with abdominal drains and to summarize the management strategies reported in the literature for this rare complication. We report a case of acute intestinal obstruction caused by an abdominal drain after rectal surgery, documented in the oncology surgery dep

CT Angiography for the Diagnosis of Hemorrhagic Jejunal Diverticulosis: A Rare Cause of Obscure GI Bleeding.

Jejunal diverticulosis (JD) is a rare condition of the small intestine characterized by acquired pseudodiverticula arising along the mesenteric border. Although most cases remain asymptomatic, complications may occur in a minority of patients and include diverticulitis, perforation, intestinal obstruction, malabsorption, and, more rarely, GI hemorrhage. Bleeding originating from jejunal diverticula is an uncommon but potentially life-threatening cause of obscure GI bleeding (OGIB). We report the case of a 59-year-old woman who presented to the ED with melena associated with symptomatic anemia.

A 10-Year-Old Boy With Chronic Atrial and Intestinal Dysrhythmia (CAID) and Chronic Intestinal Pseudo-Obstruction (CIPO).

Chronic intestinal pseudo-obstruction (CIPO) is a severe gastrointestinal syndrome characterized by disruption of normal gut movement, resembling a mechanical obstruction, despite the absence of any physical obstruction. It can occur idiopathically or as a secondary manifestation of various underlying conditions, such as certain genetic disorders. Chronic atrial and intestinal dysrhythmia (CAID) is one of the rare causes of CIPO. This syndrome is an autosomal recessive cohesinopathy in which the SGOL1 (K23E) mutation disrupts cardiac and intestinal pacemaker cells, leading to sick sinus syndro

Marrow to Bowel or Bowel to Marrow: Intestinal Myeloid Sarcoma Presenting With Acute Obstruction.

Myeloid sarcoma (MS) is an extramedullary tumor composed of immature myeloid precursor cells and represents a tissue manifestation of acute myeloid leukemia (AML). It may occur concurrently with AML, precede marrow disease, or present at relapse. Gastrointestinal (GI) involvement is uncommon and frequently poses a diagnostic challenge due to morphological overlap with lymphoid and other GI malignancies. Rapunzel Syndrome Complicated by Cholecystoduodenal Fistula Secondary to Biliary Compression: A Case Report of a Patient With Cerebral Palsy.

Rapunzel syndrome is a rare form of gastric trichobezoar that extends into the small intestine, leading to intestinal obstruction. Biliary-enteric fistulas are abnormal communications between the biliary system and the gastrointestinal tract, generally occurring spontaneously. We report the case of a 26-year-old female patient with a history of cerebral palsy who presented to the emergency department with an acute abdomen. Computed tomography revealed a mass occupying the stomach and intestine. A laparotomy was performed, identifying a gastric and duodenal trichobezoar complicated by an acute

A Case Report of Bouveret Syndrome.

The present study aimed to describe the diagnosis and surgical management of Bouveret syndrome, an uncommon cause of intestinal obstruction. We report a case of a 74-year-old woman presenting with clinical features consistent with intestinal obstruction, in whom clinical evaluation, laboratory tests, and contrast-enhanced abdominopelvic computed tomography were performed to establish the diagnosis. Surgical treatment was subsequently carried out by exploratory laparotomy and enterotomy for gallstone extraction. Computed tomography revealed pneumobilia and the presence of an ectopic gallstone a

Spatulated umbilical cord technique embracing Wharton's jelly for gastroschisis repair.

The purpose was to prospectively analyze outcomes of gastroschisis closure using the Spatulated Umbilical Cord technique (SUCT). In SUCT, the umbilical cord is split open and used to cover the abdominal defect. The procedure involved opening the amnion layer from the base, exposing Wharton’s jelly and ‘spatulating’ using longitudinal incisions and securing it as a patch over the defect. A pilot interventional study was conducted prospectively at 3 centres from 2016 to 2023 with follow-up till 2024. The inclusion criteria included uncomplicated gastroschisis, where primary fascial closure was a

Primary tumor resection in the setting of unresectable metastatic disease in small bowel NETs: consensus and controversy.

Small bowel neuroendocrine tumors represent a rare malignancy, and most patients present with metastatic disease at diagnosis. While current guidelines from the North American Neuroendocrine Tumor Society and the European Neuroendocrine Tumor Society agree that resection is indicated for symptomatic tumors causing obstruction, ischemia, or bleeding, the role of primary tumor resection in otherwise asymptomatic patients with unresectable metastatic disease remains controversial. This article reviews the existing evidence and highlights the role of primary tumor resection in the setting of metas

Malignant peripheral nerve sheath tumor of the ileocecal region: a case report.

BACKGROUND: Malignant peripheral nerve sheath tumor (MPNST) is a rare and aggressive soft tissue sarcoma that rarely occurs in the gastrointestinal tract. To date, fewer than ten cases of primary colonic MPNST have been reported, and this case may represent the first case report of an MPNST arising in the ileocecal region. CASE PRESENTATION: A 62-year-old male patient presented with right lower abdominal pain for two days. A firm, fixed mass approximately 10 cm in diameter was palpable in his right lower abdomen. Auxiliary examinations including abdominal CT and colonoscopy revealed various f

Use of barbed suture in surgery and the risk of intestinal obstruction: analysis of a large institutional cohort.

BACKGROUND: Barbed sutures have become increasingly popular across multiple surgical specialties due to their self-anchoring design, which eliminates the need for knot tying and reduces operative time. However, concerns persist that exposed barbs may cause postoperative intestinal obstruction by entangling adjacent bowel loops or mesentery. The true incidence of such events in large surgical populations remains unclear. METHODS: A retrospective analysis was conducted using an institutional database encompassing all intra-abdominal surgeries performed with barbed sutures from August 2017 to Ap
