Possible association of pyogenic liver abscess with an inflamed sigmoid colonic lipoma: a case report and literature review.

BACKGROUND: Pyogenic liver abscess (PLA) is a severe infectious condition that is often associated with an identifiable biliary or gastrointestinal source. Benign colonic lesions are rarely considered during the etiological evaluation of PLA. CASE PRESENTATION: We report the case of a 47-year-old man admitted with febrile diarrhea. Contrast-enhanced computed tomography (CT) demonstrated confluent hepatic abscesses involving segments II, III, and IV, measuring 10 cm × 9 cm × 7 cm, without a CT-detectable biliary source. The etiological investigation identified an approximately 2 cm inflammator

Dynamic left main occlusion after aortic valve surgery: recognizing extrinsic compression by aortic root pseudoaneurysm-a case report.

BACKGROUND: Extrinsic compression of the left main (LM) coronary artery by an aortic root pseudoaneurysm is an exceedingly rare but life-threatening cause of acute coronary syndrome following aortic valve surgery. Multimodality imaging is essential for diagnosis and surgical planning. CASE SUMMARY: A 52-year-old male underwent mechanical aortic valve replacement and aortic root reconstruction for infective endocarditis complicated by a 23 mm aortic root abscess. Following initial recovery, he re-presented 2 months later with persistent chest pain on a background of 10 days of intermittent sym

Bacteroides fragilis Empyema Secondary to Ruptured Pyogenic Liver Abscess: A Rare Cause of Septic Shock.

Pyogenic liver abscess is an uncommon but potentially life-threatening intra-abdominal infection associated with significant morbidity and mortality. Although hepatic abscesses most commonly present with systemic infection or intra-abdominal complications, transdiaphragmatic spread into the pleural cavity resulting in empyema is rare. Bacteroides fragilis, an anaerobic gram-negative bacillus that forms part of the normal gastrointestinal flora, is a well-recognized pathogen in polymicrobial intra-abdominal infections but is infrequently reported as a primary pathogen in empyema originating fro

Purpureocillium lilacinum Fungemia Masquerading as an Unidentified Yeast in a Heavily Immunosuppressed Patient With Relapsed Multiple Myeloma and Therapy-Related Mixed-Phenotype Acute Leukemia: A Case Report.

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is an environmental saprophytic mold and an increasingly recognized opportunistic pathogen in immunocompromised hosts, most often causing cutaneous, ocular, or sinopulmonary disease; bloodstream infection (fungemia) is rare. The initial microscopic morphology on Gram stain of a positive blood culture can simulate yeast, delaying correct identification and appropriate antifungal therapy. We describe a 67-year-old man with relapsed multiple myeloma and therapy-related mixed-phenotype acute leukemia, further complicated by a prior Mycoba

Anti-PD-1 Therapy Alleviates Klebsiella pneumoniae-Induced Liver Abscess by Effectively Activating Kupffer Cells.

Klebsiella pneumoniae-induced liver abscess (KPLA) is the leading cause of pyogenic liver abscess (LA) worldwide. Hepatic Kupffer cells exert protective effects during KPLA progression; thus, elucidating their protective mechanisms and developing novel therapeutic strategies are imperative. A murine KPLA model was established. Hematoxylineosin (HE) staining and tissue bacterial load were analyzed to verify successful K. pneumoniae infection. RT-qPCR, Western blot (WB), and flow cytometry were employed to quantify PD-1 expression in Kupffer cells. PD-1-knockdown Kupffer-cell transfection system

Skull infarction and chronic subdural exudates in a pediatric patient with sickle cell anemia: illustrative case.

BACKGROUND: Skull infarction is a rare sickle cell disease (SCD) complication. While epidural hematomas from calvarial infarction have been sporadically reported, subdural collections and sterile exudates mimicking intracranial empyema remain poorly characterized. OBSERVATIONS: A 6-year-old boy with nonverbal autism and SCD presented with progressive periorbital swelling, fever, and extremity pain. CT raised concern for orbital cellulitis, subperiosteal abscess, and subdural empyema. MRI revealed a diffuse heterogeneous calvarial marrow signal with multifocal nonenhancement and restricted dif

Contrast-Enhanced Ultrasonography of Orbital and Intraocular Pathologies in 24 Dogs and 6 Cats.

OBJECTIVE: Contrast-enhanced ultrasonography (CEUS) is a noninvasive and relatively safe imaging technique to assess ocular perfusion, showing high sensitivity and specificity for tumor characterization in human oncology, while reports on its application in canine and feline diseased eyes are limited. This study aimed to describe the CEUS features of orbital and intraocular pathologies in dogs and cats in order to evaluate its diagnostic value in detecting and characterizing various lesions. ANIMAL STUDIED: CEUS examinations of 30 animals (24 dogs, 6 cats) were evaluated retrospectively. PRO

Implant-Free Breast Volume Restoration Using Abdominal Free Flaps in Nononcologic Patients: A Retrospective Cohort Study.

Abdominal free flaps are established in oncologic breast reconstruction but are less frequently used for implant-free breast volume restoration in nononcologic patients. The aim of this study was to describe the indications, operative characteristics, complications, follow-up, and secondary procedures associated with abdominal free-flap breast reconstruction or augmentation in a nononcologic cohort. The authors performed a retrospective single-center cohort study of consecutive patients who underwent deep inferior epigastric perforator (DIEP) or muscle-sparing transverse rectus abdominis myocu

Gut microbiota and innate immune barrier dysfunction in hepatic portal venous gas: a hypothesis-generating review.

Hepatic portal venous gas (HPVG) is an uncommon radiographic sign whose prognosis is driven by bowel necrosis and shock rather than by intraportal gas. Immunological events along the gut-liver axis have not been synthesized for adult HPVG, and dedicated microbiome, metabolomic, gut-vascular barrier (GVB), and immune-profiling data are lacking. This hypothesis-generating review proposes gut dysbiosis as a putative contributor to mucosal and hepatic innate immune failure, inferred from cirrhosis, porto-sinusoidal vascular disease (PSVD), and intestinal ischemia/reperfusion. In those proxy states

Parvimonas micra and central nervous system infections: mechanisms of transformation from oral commensal to intracranial pathogen and clinical evidence.

Parvimonas micra (P. micra), an anaerobic oral coccus long dismissed as a culture contaminant, is increasingly recognized as a causative agent of pyogenic central nervous system (CNS) infections, yet its neuropathogenesis remains poorly defined. This narrative review synthesizes 41 peer-reviewed studies identified via a structured comprehensive literature search of PubMed, Embase and Web of Science through June 2026. Epidemiologically, pooling available case reports and small series (n=64 confirmed CNS cases), P. micra is among the most frequently isolated Gram-positive anaerobic cocci in repo

Pulmonary dermoid cyst mimicking hydatid cyst: a case report with literature review.

Intrapulmonary dermoid cyst (mature cystic teratoma) is rare and may mimic common cystic lung diseases, particularly pulmonary hydatid disease. A 26-year-old woman presented with dyspnea and left-sided chest, shoulder, and neck pain. Computed tomography demonstrated a large complex cystic lesion in the left upper lobe with gas locules and associated pleural and pericardial effusions, initially suggesting a hydatid cyst. Video-assisted thoracoscopic excision revealed hair and heterogeneous cyst contents, and histopathology showed mature tissues from multiple germ layers with xanthogranulomatous

Tuberculous spinal epidural abscess: a cautionary case report of permanent paraplegia due to delayed diagnosis and management.

BACKGROUND: Spinal epidural abscess caused by tuberculosis is a rare but serious complication of spinal tuberculosis. Its atypical presentation predisposes to diagnostic delay and a poor prognosis. This case not only documents the clinical and radiologic evolution of tuberculous spinal epidural abscess but also suggests that delays in diagnosis and treatment may jointly contribute to serious neurological outcomes. CASE PRESENTATION: A 39-year-old man initially underwent acupotomy for recurrent low back pain with recent exacerbation. Subsequent imaging revealed a paraspinal abscess within the

Tuberculous Cervical Lymphadenitis (Scrofula) Associated with Massive Pleural Effusion in an Immunocompetent Adult: A Rare Presentation of Extrapulmonary Tuberculosis Confirmed by GeneXpert MTB/RIF.

Background: Tuberculous cervical lymphadenitis (scrofula) is the most common form of extrapulmonary tuberculosis, although its coexistence with massive pleural effusion is uncommon and may delay diagnosis because of its nonspecific clinical presentation. We report the case of a 47-year-old man presenting with a large right-sided pleural effusion associated with a suppurative cervical lymphadenitis, in whom rapid molecular testing established the diagnosis of disseminated extrapulmonary tuberculosis. A Fish Bone Penetrating the Esophagus Into the Left Atrium.

BACKGROUND: Fish bone migration into the left atrium is extremely rare and carries high mortality. CASE SUMMARY: A 68-year-old woman presented with chest pain, fever, and dyspnea 5 days after swallowing a fish bone; she had attempted to dislodge it with a rice ball. Contrast-enhanced computed tomography showed a 2.9-cm linear foreign body penetrating the mid-esophagus into the left atrium. Transthoracic echocardiography revealed a 2.3-cm echogenic mass in the left atrium without pericardial effusion. Emergency surgery under cardiopulmonary bypass extracted the fish bone, debrided a large vege

Prostatic abscess in a 16-year-old adolescent secondary to intraurethral water instillation: A case report and review of the literature.

While acute bacterial prostatitis is seen occasionally in adult males, the development of a prostatic abscess (PA) is exceedingly rare in pediatric and adolescent populations. When PA does occur in younger patients, congenital anomalies, immune compromise, trauma, or genitourinary instrumentation are commonly associated. We report a unique case of a 16-year-old boy who developed a prostatic abscess after deliberately instilling shower water into his urethra. This report details the clinical presentation, diagnostic workup, and treatment course of this patient, followed by a literature review o

Pyogenic Sacroiliitis in Older Adults Presenting With Massive Retroperitoneal Abscess: A Case Report.

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

C-Reactive protein and the CALLY index for predicting complicated appendicitis: a retrospective cohort study of 5,428 patients.

BACKGROUND: Distinguishing complicated from uncomplicated appendicitis before surgery remains a critical yet unresolved clinical challenge. Delayed recognition of complicated appendicitis, particularly in cases of perforation or abscess formation, is associated with increased morbidity and mortality. Although various laboratory markers and inflammatory indices have been proposed as markers associated with disease severity, their relative diagnostic performance remains unclear. This study aimed to evaluate and compare the discriminatory performance of conventional laboratory parameters, leukocy

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 1.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that usually starts in the second or third decade of life and has a profound negative impact on quality of life of those who are affected. HS presents with both inflammatory and cicatricial lesions that are often markedly symptomatic, including pain and purulent discharge, and commonly affect areas that are intertriginous, resulting in additional frictional and mechanical discomfort. The pathophysiology is complex and incompletely understood and results in changes that are transcutaneous, involving the epidermis, superf
