First-in-Human Axillary Vein Implantation of a Leadless Pacemaker: Breaking the Access Barrier.

BACKGROUND: Leadless pacemakers (LPs) are conventionally implanted transfemorally, with transjugular and subclavian routes reported as alternatives. Axillary access has not previously been reported. FIH SUMMARY: A 78-year-old woman with chronic sacral wounds became temporary-pacing dependent following an inferior ST-segment elevation myocardial infarction. An LP was selected given infection risk. Because of a right femoral deep venous thrombosis, an abscess at the left femoral site, and restricted neck mobility limiting internal jugular access, she underwent left axillary vein implantation of

Case Report: Multiple brain abscess-like lesions with concurrent detection of Nocardia amamiensis and Leishmania infantum in cerebrospinal fluid.

Central nervous system nocardiosis is rare and potentially life-threatening, and human infection due to Nocardia amamiensis has seldom been reported; central nervous system involvement in visceral leishmaniasis is likewise uncommon. We report a 69-year-old man who developed progressive impairment of consciousness and coma after recurrent fever. Brain magnetic resonance imaging revealed multiple abscess-like lesions in both cerebral hemispheres, the cerebellum, and the brainstem, accompanied by acute or subacute cerebral infarctions. The cerebrospinal fluid opening pressure was 300 mmH2O. The w

Cardiac Magnetic Resonance-Supported Diagnosis of Acute Myocarditis Mimicking Acute Coronary Syndrome in a Patient With Sepsis Due to a Massive Perianal Abscess.

Acute myocarditis is a rare but potentially life-threatening inflammatory disease that may mimic acute myocardial infarction, particularly in septic patients where myocardial injury may have other causes. We report the case of a 46-year-old man with sepsis secondary to a massive perianal abscess who underwent emergency surgical drainage under general anesthesia. Postoperatively, persistent respiratory deterioration and elevated cardiac biomarkers prompted further cardiac evaluation. Echocardiography showed newly reduced left ventricular (LV) systolic function, with an ejection fraction (EF) of

Urogenital abscess with a complex etiology: a case report.

BACKGROUND: Scrotal abscesses requiring hospitalization are uncommon, with progression to sepsis being even rarer. A feared complication of a scrotal abscess is Fournier gangrene, an uncommon type of necrotizing fasciitis that starts in the scrotum or perineum. Fournier gangrene is associated with a mortality rate of about 40% and accounts for less than 0.02% of all hospital admissions. The patient in this report was hospitalized for a complex scrotal abscess and, during repeat incision and drainage (I&D), was found to have Fournier gangrene. CASE DESCRIPTION: The patient was a 49-year-old ma

Structural and Kinetic Basis for Selective Inhibition of Canine Myeloperoxidase by Staphylococcal Peroxidase Inhibitor (SPIN) Orthologs.

Staphylococcus aureus and related species are highly adapted to their hosts and have evolved numerous strategies to evade the innate immune system. The Staphylococcal Peroxidase Inhibitor (SPIN) is a small, secreted protein that inhibits myeloperoxidase (MPO). SPIN blocks the MPO active site and prevents production of cytotoxic hypohalous acids within the neutrophil phagosome, thereby enabling evasion of oxidative killing. S. aureus is primarily a human pathogen, and its SPIN protein cannot inhibit MPO from most non-human species. This raises questions as to whether staphylococci adapted to al

Investigating the Use of Routine Swabs in Incision and Drainage of Abscesses: An Audit of Western Sydney Hospitals.

BACKGROUND: Incision and drainage of subcutaneous abscesses is a common surgical procedure. It is the standard of treatment, with or without adjunctive antibiotics. Interestingly, there exists a lack of evidence regarding the role of intraoperative sampling of the purulent discharge from abscesses. This study aimed to demonstrate whether intraoperative swab results were used to tailor antibiotic management and the circumstances surrounding these decisions. This will guide local practice for judicious collection of swabs and empirical antibiotic management. This is needed in the Western Sydney

Metastatic HER2-positive breast cancer in a 19-year-old woman following a 4-year diagnostic delay: a case report.

BACKGROUND: Breast cancer in adolescents and young adults (AYA) under age 30 is exceedingly rare, representing a very small proportion of all breast cancer cases. When it occurs, it demonstrates more aggressive biological behavior with higher rates of advanced-stage disease and poorer outcomes compared to older populations. Although breast cancer remains uncommon in this age group, population-based data demonstrate that the overall incidence of carcinomas among adolescents and AYA has increased steadily in the United States since 1975, with the greatest relative increase observed among adolesc

Severe Transaminitis Without Rhabdomyolysis: An Unusual Presentation of Daptomycin-Induced Liver Injury.

Daptomycin is a lipopeptide antibiotic commonly used for methicillin-resistant Staphylococcus aureus infections. Abnormal liver function tests occur in approximately 3% of patients receiving daptomycin; severe isolated transaminitis is exceptionally rare. We report a 43-year-old man with methicillin-resistant Staphylococcus aureus bacteremia and multiple abscesses who developed marked transaminitis with aspartate aminotransferase and alanine aminotransferase levels peaking at 1,965 U/L and 2,253 U/L, respectively, within 6 days of initiating daptomycin therapy (8 mg/kg daily). Notably, the pat

A hepatic malignant spindle-cell neoplasm of uncertain lineage clinically favoring sarcomatoid hepatocellular carcinoma: a case report.

BACKGROUND: Sarcomatoid hepatocellular carcinoma (SHC) is an uncommon and aggressive variant of primary liver cancer. Its clinical and radiological features are often nonspecific, and it may mimic inflammatory liver lesions. Evidence for unresectable SHC is limited, so management is frequently individualized. CASE DESCRIPTION: A 43-year-old man with chronic hepatitis B presented with recurrent right upper abdominal discomfort and 3 kg weight loss over 1 month. He was provisionally diagnosed with a hepatic abscess at an outside hospital, and percutaneous aspiration and drainage were performed;

A large iliopsoas abscess due to migration of ureteral stent: a case report.

BACKGROUND: Iliopsoas abscess (IPA) is a rare but potentially life-threatening condition. Due to its nonspecific clinical presentation and low incidence, IPA often leads to diagnostic delays and suboptimal initial management. Secondary IPA originating from urinary tract complications is extremely rare, and IPA induced by ureteral stent migration appears to be exceptionally uncommon. Fungal IPA caused by Candida tropicalis is also a scarcely reported clinical entity, especially in immunocompromised patients with long-term rheumatoid arthritis (RA). CASE PRESENTATION: We report a 63-year-old wo

Endobronchial Pulmonary Typical Carcinoid Tumors in Young Adults: A Three-Case Series Highlighting Obstructive Complications and Surgical Outcomes.

BACKGROUND: Typical pulmonary carcinoid tumors are uncommon, well-differentiated neuroendocrine neoplasms that often arise in the central airways and may present with nonspecific obstruction-related respiratory symptoms, resulting in delayed diagnosis. We report a three-case series of endobronchial typical pulmonary carcinoid tumors in young adults, highlighting their variable obstructive complications and surgical outcomes. METHODS: This retrospective case series included three consecutive patients managed at a tertiary university hospital in Syria. Clinical presentation, imaging, bronchosco

Cat scratch disease with fever and lymphadenopathy without definite cat exposure diagnosed via tissue metagenomic next-generation sequencing technology: a case report.

BACKGROUND: Cat scratch disease (CSD), a disease caused by Bartonella henselae (B. henselae), is often associated with exposure to cats. However, in rare cases, a history of cat exposure is absent, and thus achieving a definitive diagnosis may be challenging. Metagenomic next-generation sequencing (mNGS) is a relatively new technology that can analyze pathogens without introducing inherent biases. In this report, we describe a case of CSD diagnosed via mNGS, which supports the use of mNGS in identifying rare types of pathogenic infection. CASE DESCRIPTION: A 54-year-old male exhibited fever a

Targeting an IL-17/IL-36 pathogenic hub in hidradenitis suppurativa with a bispecific antibody.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disorder with a high unmet need, particularly for treatment-resistant tunnels. Integrated multiomics analysis revealed that HS tunnels are spatially organized, self-amplifying inflammatory circuits driven by synergistic IL-17/IL-36γ activation of a unique keratinocyte subset, which activates neutrophils and inflammatory fibroblasts. This mechanistic insight coupled with spatial coenrichment of IL-17A and IL-36R provided the rationale for HB0043, a bispecific antibody that leverages IL-36R binding to locally dual-block both cytokines.
