Severe dengue is typically linked to secondary infection. We report dengue with warning signs due to primary DENV-3 infection in a healthy Italian adolescent returning from the Philippines. Severe thrombocytopenia and mucosal bleeding, with clinical and laboratory features suggestive of possible plasma leakage, were managed without shock, highlighting the need for multidisciplinary monitoring and care even in primary dengue.

Background/Objectives: Arboviral diseases transmitted by Aedes mosquitoes, including Dengue (DENV), Zika (ZIKV), and Chikungunya (CHIKV), represent a major public health challenge in tropical regions. Their clinical similarity complicates differential diagnosis, particularly in settings of viral co-circulation, and may lead to underdiagnosis. The objective was to detect acute dengue infection and assess serological evidence of Chikungunya and Zika virus exposure among patients with acute febrile syndrome and clinical suspicion of dengue in the department of Córdoba, Colombia. Methods: A prospe

Dengue is a major cause of pediatric hospitalization in tropical countries and low- and middle-income countries (LMICs), where early recognition of severe disease remains challenging. Severe dengue is characterized by dysregulated immune activation and systemic inflammation, leading to organ dysfunction. Ferritin, an acute-phase reactant, has been increasingly studied as a potential biomarker of dengue severity, although pediatric data from LMIC settings remain limited. The aim of this study was to assess the prevalence of hyperferritinemia among children admitted with dengue with warning sign

BACKGROUND: Bangladesh has experienced recurrent dengue outbreaks since 2000, driven by climatic changes and socio-demographic vulnerabilities. Limited evidence on circulating serotypes and immune diversity has hindered preparedness for severe dengue (SD). Identifying clinical and laboratory predictors of progression to severe dengue is key to strengthening outbreak response and clinical management.
METHODS: A longitudinal cohort study was conducted in four hospitals in Dhaka between January and November 2024. Patients with confirmed dengue with warning signs were enrolled. Clinical data and

Secondary dengue infections are often linked to more severe clinical outcomes, yet pre-existing antibodies may exert either protective or pathogenic effects. To evaluate the role of acute-phase dengue antibodies in pediatric dengue, we analyzed clinical and laboratory features, viremia, and antibody profiles in children infected with DENV-1. We conducted a retrospective study of patients under 18 years diagnosed with DENV-1 in Salta, Argentina. Viremia was quantified by real-time RT-PCR; acute-phase IgG antibodies (within 7 days from symptom onset) against DENV, DENV-1, and DENV-NS1 were measu

BACKGROUND: Dengue virus (DENV) infection poses a major global public health burden, particularly in endemic regions where repeated exposure increases the risk of severe disease. Despite revisions to the World Health Organization (WHO) dengue classification, early prediction of progression to severe dengue remains challenging due to overlapping clinical and laboratory features. Current management strategies rely primarily on supportive care and reactive monitoring, underscoring the need for predictive biomarkers that enable early risk stratification and timely intervention.

BACKGROUND: Dengue can manifest as a mild febrile illness to life-threatening haemorrhagic fever. The early identification of patients at risk of disease progression remains challenging, particularly in non-severe cases. Therefore, this study aims to investigate the capability of extracellular vesicle plasma microRNA (miRNA) expression as a biomarker in dengue infection.
METHODOLOGY: A total of 52 dengue patients (26 dengue with warning signs and 26 dengue without warning signs) were enrolled. Microarray analysis was utilised to profile miRNA derived from plasma extracellular vesicles to iden

BACKGROUND: The World Health Organization (WHO) classification system, comprising dengue fever (DF), dengue with warning signs (DWS), and severe dengue (SD), has been widely adopted globally; however, clinical management remains challenging. Current warning sign criteria lack specificity, frequently failing to distinguish patients progressing to severe disease from those with transient illness, thereby contributing to delayed interventions and missed therapeutic windows. Although Toll-like receptors (TLRs) mediate innate antiviral immunity and are implicated in severe dengue pathogenesis, thei

BACKGROUND: Dengue fever is a widely prevalent viral disease, with one of the most common laboratory findings being thrombocytopenia. Although platelet indices like mean platelet volume (MPV), platelet distribution width (PDW), platelet large cell ratio (P-LCR), and plateletcrit (PCT) are routinely measured, their utility in clinical practice has been limited. This study was planned to evaluate platelet indices across the range of disease severity, understand their importance in terms of their trends over the disease course, and correlate them with other hematological parameters in pediatric d

INTRODUCTION: Dengue fever (DF) remains a major global public health concern, especially in tropical and low-resource settings. The neutrophil-to-lymphocyte ratio (NLR) has been proposed as an accessible biomarker for diagnosis and severity assessment, but the existing evidence is heterogeneous. To summarize the current evidence on the behavior of the NLR in DF.
METHODS: This study conducted a systematic search in five databases for observational studies published until April 2024. Inclusion and exclusion criteria were based on predefined eligibility criteria, and quality and methodological a

BACKGROUND: Dengue is a mosquito-borne disease that caused a large outbreak in Taiwan in 2023. Some published studies about this outbreak suggest higher disease severity while we observed the opposite epidemiological trend in the southern part of Taiwan. Therefore, we conducted comparative analysis for clinical manifestation and disease severity between two outbreak periods.
METHODS: We retrospectively analyzed 4009 cases diagnosed at one medical center and two regional hospitals in Taiwan, including 3588 cases from 2014 to 2015 and 421 cases from 2023. Moderate-to-severe dengue was defined a

Dengue virus hijacks host cell mechanisms and immune responses in order to replicate efficiently. The interaction between the host and the virus affects the host's gene expression, which remains largely unexplored. This pilot study aimed to profile the host transcriptome as a potential strategy for identifying specific biomarkers for dengue prediction and detection. High-throughput RNA sequencing (RNA-seq) was employed to generate host transcriptome profiles in 16 dengue patients and 10 healthy controls. Differentially expressed genes (DEGs) were identified in patients with severe dengue and t

INTRODUCTION: Dengue viruses cause either symptomatic infections or asymptomatic seroconversion. Symptomatic dengue has a wide clinical spectrum ranging from self-limiting infection to severe manifestations, mostly characterized by plasma leakage with or without hemorrhage. World Health Organization classification in 2009 classified dengue into dengue without warning signs, dengue with warning signs, and severe dengue. This study aimed to determine patient characteristics and hematological parameters (leukopenia, and thrombocytopenia) in patients with dengue infection in the emergency departme

Dengue is the most common arboviral disease globally. It is caused by four distinct but closely related Dengue viruses (DENV-1, -2, -3, and - 4) transmitted through bites of infected Aedes species mosquito vectors. In the last 50 years, incidence has increased 30-fold with increasing geographic expansion to new countries. Here we report the most important autochthonous Dengue epidemic in Italy ever recorded with a total of 86 confirmed cases occurring in September 2024 in Fano. They were caused by DENV-2. They were 61 Dengue fever, 21 Dengue with warning signs and no cases of severe Dengue. Ou
