Giant colonic diverticulum is a very rare entity in colonic diverticular disease and is characterized by a high rate of complications such as perforation, abscess formation and even carcinoma. We report a case of a complicated giant diverticulum of the transverse colon accompanied by a right inguinal hernia of the greater omentum in a 52-year-old man, as demonstrated on CT.

PURPOSE: The spontaneous passage per rectum of a full-thickness colon "cast" is a rare consequence of acute colonic ischemia. Left-sided diverticulosis coli is a common condition in western communities, with 30% to 50% of adults over the age of 60 being affected. It predominantly involves the sigmoid colon. The purpose of this study is to evaluate the application of computed tomography (CT) in the diagnosis of colonic diverticulitis in the emergency department (ED). Clinical information and CT images of 38 proved cases of colonic diverticulitis were reviewed retrospectively. The most common CT findings were pericolic fat infiltration (100%), thickened fascia (78.9%), muscular hypertrophy (26.3%), and arrowhead sign (23.7%). Between June 1, 1990 and December 31, 1996, 58 consecutive patients with unprepared colons were urgently explored for nontraumatic disease with intent to proceed with primary left-sided colonic anastomosis.
