[Surgical treatment of diverticulitis of the large intestine--a plea for early elective resection].

PURPOSE: Diverticulitis of the colon is observed more and more frequently in highly developed countries. The spontaneous course of the disease is not predictably; an exact definition of risk factors for perforated diverticulitis is missing. Therefore the decision for the conservative therapy or for an early elective resection is subject of a controversial discussion. The results of an offensive surgical concept, aiming at an early elective resection for definite therapy of diverticulitis have been analysed in a retrospective study. RESULTS: From 1980-1995 337 patients were operated due to div

[The computed tomography of acute sigmoid diverticulitis].

PURPOSE: To study the ranking of CT in the diagnosis of sigma diverticulitis. Comparison of results of the colon monocontrast enema with those obtained via CT and histopathology as gold standard. MATERIAL AND METHODS: 32 patients were included who were operated on for sigma diverticulitis. In 30 patients both CT and monocontrast enema were performed and in two patients CT only. RESULTS: Acute inflammatory wall alterations are identified by CT with a sensitivity of 89.7% vs 96.3% via contrast enema. Pericolic reactions of the environment were seen via CT with 89.7% sensitivity. In case of cov

[Acute diverticulitis of the appendix. A clinical case].

The authors describe a case of acute diverticulitis of the appendix presenting as an inflammatory mass in the right lower quadrant of the abdomen. The histologic and clinical findings are discussed. This uncommon condition may mimic acute appendicitis, but in most instances it departs from typical appendicitis for later age of appearance, more indolent clinical course and increased tendency to perforation. In these cases the disease may progress with subacute interstitial inflammation, with or without abscess formation, and present as a tumor-like mass of the cecum.

Diverticulitis mimicking a tuboovarian abscess. Report of a case in a young woman.

BACKGROUND: Diverticulitis is an uncommon condition in young women. When it occurs, it is often not recognized until complications such as perforation or fistulization occur. There has been no recent discussion in the gynecologic literature of diverticulitis in a young woman presenting as gynecologic disease. CASE: A 31-year-old woman with a long history of "irritable bowel syndrome" developed a 4-5-cm left adnexal mass associated with mild discomfort and dysparunia. Acute worsening of her pain led to abdominal exploration and left salpingo-oophorectomy for an unruptured tuboovarian abscess.

[Colon diverticulitis: imaging diagnosis with ultrasound--a prospective study].

In a prospective study, we investigated the value of high resolution sonography for the imaging diagnosis of colonic diverticulitis. A total of 161 patients with abdominal complaints on account of which acute diverticulitis entered into differential diagnosis were included in the study. 74 of these finally had proven colonic diverticulitis (prevalence, 46%). The results of intestinal sonography and the working diagnoses on admission were evaluated on the basis of final diagnoses. According to history, symptoms, and clinical findings on admission, the working diagnosis of diverticulitis was gra

Perforated diverticulitis: a surgical dilemma.

Thirty-eight patients, 18 males and 20 females (mean age 61 years) have been operated on for perforated diverticulitis, over the past 14 years. We assigned the patients into stage A (n = 12) when the inflammation was confined to the mesosigmoid folds, stage B (n = 16) when an intraabdominal abscess was walled off and stage C (n = 10) when a generalized peritonitis had developed. Resection of the perforated sigmoid was carried out in 24 patients either as Hartmann's procedure (n = 18) or with primary anastomosis (n = 6). The mortality rate in this setting was 0.0% and 16.6% respectively. In 14

[Sonography in the diagnosis and follow-up of colonic diverticulitis].

17 patients with colonic diverticulitis of various degree were examined by conventional abdominal ultrasound in comparison to either colonoscopy or operation. The sonography of the inflammated region showed an echo-poor enlargement of the colonic wall (normal less than 3 mm) ranging from 5 to 15 mm (mean 7.8 mm) over a short distance. In cases with bowel wall enlargement of more than 11 mm a spontaneous evacuation of pus from the diverticles was endoscopically seen. A sonographic enlargement of the colonic wall of more than 15 mm is very suspicious to be a peridiverticulitis or a beginning abs

Primary anastomosis in emergency distal colonic surgery after on-table colonic lavage.

The traditional operative management of emergency distal colon pathology has involved staged procedures, but recently Hartmann's operation has gained popularity. The indications for primary anastomosis without covering colostomy have remained controversial, but the technique of on-table lavage has extended the use of this approach in the acute setting. A series of seven patients having this procedure for diverticular abscess (three), obstructing colonic carcinoma (one), perforating colonic carcinoma (one) and sigmoid volvulus (two) is presented. The saline irrigation is introduced by a Foley c

The Hartmann procedure.

In 1923, Hartmann described an operation for cancer of the rectosigmoid area in high-risk patients not amenable to abdominoperineal resection. In the period 1972 to 1984 at the Queen Elizabeth Hospital in Montreal interest in the Hartmann procedure, with resection, end-colostomy and suture closure of the distal colon intraperitoneally, was renewed. The main indication for the procedure was diverticulitis, with cancer second. The author reviews the experience with 64 patients (average age 80 years) who underwent the Hartmann procedure. Obstruction, perforation and abscesses were the commoner in

The "coffee bean" sign in periappendiceal and peridiverticular abscess.

Four cases of right lower quadrant abscess, each a clinical diagnostic dilemma, were recognized as abscesses surrounding a perforated viscus by application of the "coffee bean" sign on sonographic examination. The appearance in the right lower quadrant of a brightly echogenic finger-like projection extending into a cystic mass, with or without scattered internal echoes, should suggest the possibility of periappendiceal or diverticular abscess.

The treatment of perforated diverticulitis by one-stage intracolonic bypass procedure.

The one-stage intracolonic bypass procedure prevents gastrointestinal secretions and fecal content from coming in contact with an anastomotic site without interrupting the intraluminal continuity of the fecal flow from proximal to distal colon. This can be achieved by the intraluminal implantation of a soft, pliable tube above the anastomotic site. Previous clinical and experimental data have indicated that the intracolonic bypass procedure can protect an anastomosis in the presence of maximal colonic loading, gross dehiscences, or fecal peritonitis. This report presents 28 patients with perfo

Pneumopylephlebitis and intramesocolic diverticular perforation.

The insidious presentation of intramesocolic perforation in diverticulitis has been reviewed, and two cases of intramesocolic perforation associated with pneumopylephlebitis have been reported. Review of the two previously reported cases of pneumopylephlebitis associated with diverticular perforation suggests that these may have been intramesocolic perforations as well. Exploratory laparotomy is clearly indicated in cases of pneumopylephlebitis. In patients with recurrent sepsis without a probable source, a water-soluble contrast enema is recommended. If conservative measures fail, an explorat

Diverticular disease in the young patient.

The records of 115 patients less than 40 years of age with roentgenographic or operative documentation of diverticular disease were reviewed. Ninety-two patients presented with acute diverticulitis; five, with bleeding diverticulosis, and 18 were asymptomatic. An urgent operation for fistula, abscess or free perforation was necessary upon initial presentation in 16 patients. Nine patients underwent resection during the initial hospitalization after a cooling-off period. The remaining 67 patients were managed medically and discharged. Fifty-five per cent of the medically managed patients requir

[Present role of Hartmann's operation. Apropos of 50 cases].

Of 50 Hartmann's operation conducted, 22 (including 19 for sigmoid diverticulitis) were for non-neoplastic lesions. Emergency surgery was necessary for lesions complicated by perforation, abscess, or occlusion in 34 cases, the indications for operation being local and general conditions, where immediate re-establishment of colorectal continuity was not possible, in the other 16 patients. Overall mortality was 22 p. cent, being much higher in cancer cases (32 p. cent) than in those with sigmoiditis (52 p. cent). Postoperative morbidity was marked, and was dominated by parietal complications and

[Diverticulosis of the colon and its complications. Surgical and clinical aspects].

Colonic diverticula in reality are herniations of the mucosa through a thickened musculature of the bowel wall where it is penetrated by the vasa recta. Their prevalence increases with western food and with age. Low-residue diet and increased intraluminal pressure are considered to be etiologic factors. Diverticular disease is characterised by inflammatory and bleeding complications. Diverticulitis may lead to peridiverticulitis and pericolitis with frank peritonitis. Uncomplicated diverticulosis warrants observation only under dietary guidance. Repeated complications indicate elective operati

[Diverticulosis-diverticulitis].

Diverticulosis of the colon and its clinical sequelae--diverticulitis, peridiverticulitis, and pericolitis--are typical diseases of elderly people. The main causes are weakness of the connective tissue in the colonic wall increasing with age and pathologic motility pattern of the colon due to low dietary fibre consumption. In about 20% of all people with diverticula, acute or chronic-recurrent diverticulitis develops, often with serious complications as perforation, abscess or fistula formation, obstruction, inflammatory pseudotumor and intestinal bleeding. Diagnosis is mainly based on clinica
