Nonoperative Versus Neurosurgical Treatment of Brain Abscess: An Emulated Trial Nested Within a Nationwide, Population-Based Cohort.

BACKGROUND: Uncertainty about the role of neurosurgery in brain abscess treatment is reflected in surveys and observational studies. METHODS: This was a nationwide population-based cohort study of all adults (≥18 years) diagnosed with brain abscess in Denmark from 2007 until 2023. Inverse probability weighting was applied to balance covariates according to neurosurgical treatment strategy. Outcomes included risks of mortality, rupture, and unfavorable outcome (Glasgow Outcome Scale score 1-4) 6 months after discharge and were assessed using modified Poisson and Cox regression with 95% confide

The Use of Adjunctive Interferon-γ Therapy for Complex Mycobacterial Disease in Patients with Advanced HIV: A Case Series.

Management of mycobacterial opportunistic infection remains challenging in patients living with HIV, particularly when infection persists despite antiretroviral therapy (ART) and optimised antimycobacterial therapy (AMT). Interferon-γ is a key mediator of macrophage activation and intracellular pathogen killing, providing a rationale for its use as adjunctive host-directed therapy in selected patients. We describe three patients with advanced HIV and complex mycobacterial disease treated with adjunctive interferon-γ. One patient had disseminated tuberculosis with recurrent psoas abscesses afte

Subpericranial and epidural abscess with skull osteomyelitis as a complication of microneedling for alopecia: A singular case report.

BACKGROUND: Subpericranial hematoma (SPH) is unusual in adults and is usually seen in neonates as cephalohematoma. Closed scalp hematoma is usually not associated with infection and abscess formation. CASE DESCRIPTION: A 28-year-old male presented with complaints of swelling on the left forehead. The patient at that time was on self-treatment for alopecia following a routine of application of minoxidil hair spray followed by microneedling with his derma roller. Radiological evaluation revealed a mass in the left frontal scalp region and communicating through frontal bone defect with mass in t

Initial Management of Intra-Abdominal Abscess in Crohn's Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Intra-abdominal abscess (IAA) is a serious complication of Crohn's disease (CD). Management strategies include medical therapy, percutaneous drainage (PD), and initial surgery, but the optimal approach is debated. We performed a systematic review and meta-analysis to compare these strategies. METHODS: A systematic search of 4 electronic databases was conducted. The primary outcome was the need for surgical intervention (resection or reoperation). Secondary outcomes included recurrence and complications. Data were pooled using random-effects models. RESULTS: Twenty-three studies w

Case of unilateral biportal endoscopic management of multilevel thoracic spinal epidural abscess with acute paraplegia.

BACKGROUND: Spinal epidural abscess (SEA) may cause rapidly progressive neurological deterioration and requires urgent decompression when severe spinal cord compression is present. Unilateral biportal endoscopy (UBE) has been increasingly applied to spinal infection, but its use for extensive thoracic SEA has rarely been reported. CASE DESCRIPTION: A 49-year-old female with poorly controlled type 2 diabetes mellitus presented with severe thoracic back pain and the acute onset of paraplegia. Thoracic magnetic resonance demonstrated a posterior epidural abscess extending from T7 to T11 resultin

Precise Strain Tuning of PtPdRhNi Nanozyme Boosts Multi-Pathogen and Multi-Model Antibacterial Therapy.

Nanozymes are emerging antimicrobial agents that catalyze reactive oxygen species to eliminate pathogenic threats; however, their ability to combat multidrug-resistant infections remains limited by catalytic efficiency, substrate affinity and instability. Herein, we report a new strategy incorporating Ni into ultrathin PtPdRh nanosheets to engineer lattice strain for enhancing substrate affinity and boosting enzyme-mimicking catalytic activity. The PtPdRhNi nanozyme achieves a catalytic efficiency (Kcat/Km = 2.05 × 106 m-1 s-1), 56.5-fold higher than PtPdRh and maintains over 90% activity afte

Iliopsoas Abscess: A Narrative Review.

BACKGROUND: Iliopsoas abscess (IPA), a purulent collection in the iliopsoas compartment, is an infrequent disease that is insidious, easily overlooked, life-threatening, and rarely diagnosed early due to nonspecific symptoms and signs. The classic triad of fever, flank pain, and hip movement limitation is present in only 30% of patients. Delays in treatment are followed by increased morbidity and mortality. The microbial etiology of IPA is variable and depends on the geographical area. IPA can be primary or secondary due to the origin of the infectious focus. This review aims to outline the cu

Development of a bacterin-toxoid vaccine using a Korean isolate for protection against caseous lymphadenitis in goats.

Caseous lymphadenitis (CLA), a chronic infectious disease caused by Corynebacterium pseudotuberculosis, remains a major concern in small ruminant populations owing to its persistent nature and impact on animal productivity. Despite the availability of commercial vaccines, their efficacy in goats has been inconsistent and field-based evidence supporting their use remains limited. In this study, we report the development and evaluation of a novel bacterin-toxoid vaccine formulated with C. pseudotuberculosis strain 51-12A obtained from a Korean native goat. The toxoid, prepared from culture super

Impact of abscess type on outcomes following posterior fixation for thoracolumbar pyogenic spondylitis: a multicenter retrospective cohort study.

STUDY DESIGN: Multicenter retrospective cohort study. PURPOSE: To evaluate the impact of abscess presence and type on treatment duration and clinical outcomes in patients undergoing minimally invasive posterior fixation for thoracolumbar pyogenic spondylitis. OVERVIEW OF LITERATURE: Surgical management is increasingly favored for pyogenic spondylitis. Abscesses, particularly epidural and iliopsoas, have been linked to poorer prognoses, and empyema, though uncommon, tends to be particularly severe. However, the effect of specific abscess types on surgical outcomes remains unclear. METHODS: T

Percutaneous Drainage of a Post-ERCP Liver Abscess: Successful Management of a Rare Complication-A Case Report.

Endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy is an established treatment for choledocholithiasis but may rarely lead to pyogenic liver abscess (PLA). We report the case of a 72-year-old woman with a history of biliary stenting who presented with jaundice and cholestasis. ERCP with lithotripsy and sphincterotomy successfully cleared multiple stones, but she developed high fever and leukocytosis within 48 h. Imaging revealed a 6-cm abscess in segment 7 of the liver. Ultrasound-guided percutaneous catheter drainage was performed, yielding purulent material that cultur

Basic principles of ultrasound applied to infectious diseases.

Point-of-care ultrasound (POCUS) is an increasingly valuable diagnostic tool in infectious diseases, offering real-time, bedside imaging without radiation. It enables the detection of key findings such as fluid collections, lung consolidations, abscesses, and cardiac involvement, supporting rapid diagnosis, targeted interventions, and improved outcomes. This article reviews the fundamental physical principles of ultrasound - wave propagation, acoustic impedance, resolution, and artifacts - along with technical aspects like image acquisition, probe selection, and optimization strategies. From Sound to Silence: Cerebellar Abscesses and Herniation Due to Cochlear Implant Infection With Escherichia coli and Bacteroides fragilis.

Cochlear implantation (CI) is a safe and well-established intervention for sensorineural hearing loss, with a low incidence of severe postoperative infections. We present the first reported case of cerebellar abscess and herniation due to CI infection. This unique case involves a 57-year-old man with recurrent cochlear implant infections, necessitating multiple debridements and eventual removal of the implant body while retaining the electrode array. This ultimately led to the formation of two right-sided cerebellar abscesses. His course was further complicated by cerebellar herniation due to

Vaginal Mesh Exposure Following Pessary Use After Transvaginal Mesh Surgery: A Case Report.

INTRODUCTION: Vaginal pessaries are a widely used treatment for pelvic organ prolapse. They are considered minimally invasive, effective, and easy. However, pessary-related adverse events are sometimes underestimated. Here, we report on an important case of vaginal mesh exposure as a pessary complication. CASE PRESENTATION: A patient underwent transvaginal mesh surgery for pelvic organ prolapse, and after 10 years, had a mild recurrence. Gynecologists recommended that she use a pessary. After 3 years, she had vaginal bleeding and malodor. Gynecologists detected mesh exposure, but they downpla
