BACKGROUND: Burkholderia pseudomallei, an environmental saprophyte bacterium, causes melioidosis in humans and animals. It was first discovered in Iran between 1967 and 1976 in small ruminants, equines, environments and humans. No subsequent studies have been conducted to determine the existence and prevalence of this pathogen in the country.
OBJECTIVES: The present study aims to monitor the presence of B. pseudomallei in the ruminant population of the Golestan province of Iran, which largely depends on pastures. The ruminants can serve as sentinels to indicate the presence of the bacteria in

INTRODUCTION: Melioidosis may occasionally be encountered in non-endemic areas and medical imaging is frequently used to identify and characterise sites of disease. The purpose of this study is to describe the spectrum of imaging findings encountered in melioidosis patients treated in the tertiary public hospitals of Perth, Western Australia, between 2002 and 2022.
METHODS: A database search and electronic medical record review was used to identify cases. Cases were included if they had Burkholderia pseudomallei isolated on culture and if they had at least one diagnostic imaging study perform

Melioidosis is caused by community-acquired gram-negative bacillus Burkholderia pseudomallei which resides in soil and water. It was first described in 1912 in Burma and 1927 in Sri Lanka. Melioidosis presents with non-specific clinical and biochemical findings. Diagnosis is confirmed by the isolation of bacteria in cultures or demonstrating antibody response. Once the diagnosis is made, patients are managed with a course of intravenous antibiotics followed by a long course of oral antibiotics. Even with antibiotic treatments, most patients do not achieve complete recovery which results in chr

BACKGROUND: Melioidosis is one of the emerging infections in India and is often neglected due to lack of awareness among clinicians. The diseases usually present as multifocal or focal abscesses with bacteremia, yet the isolated septic monoarthritic complicated with septic venous thrombosis is a rare presentation.
CASE: This case report describes a 48-year-old male business executive with diabetes and alcoholism who developed septic arthritis caused by Burkholderia pseudomallei and no clear history of exposure to the organism. The patient initially experienced fever, chills, fatigue, reduced

Melioidosis is a rare disease caused by Burkholderia pseudomallei, which has recently acquired prominence in India as an emerging pathogen. It is a gram-negative bacteria found in soil. As delayed diagnosis and treatment are linked to increased mortality, early diagnosis is crucial. We present here a unique instance of melioidosis that was made worse by a rare disorder known as hemophagocytic lymphohistiocytosis (HLH), which necessitates early treatment.

BACKGROUND: Burkholderia thailandensis is an environmental bacteria closely related to Burkholderia pseudomallei that rarely causes infection in humans. Some environmental isolates have shown to express a capsular polysaccharide known as B. thailandensis capsular variant (BTCV), but human infection has not previously been reported. Although B. thailandenisis has been identified in environmental samples in Laos before, there have not been any human cases reported.
CASE: A 44-year-old man presented to a district hospital in Laos with a short history of fever and pain in his left foot. Physical

Melioidosis is a fatal infectious disease caused by Burkholderia pseudomallei. Its clinical manifestations are so varied that it has been called a great mimic. The primary melioidotic prostatic disease usually presents as an abscess of the prostate. This is the only documented case misdiagnosed as benign prostatic hyperplasia (BPH) because it does not show as a prostate abscess. The patient was a 66 years old male with a history of hypertension and alcoholism. Symptoms of difficulty urinating and fever had persisted for three days. Laboratory tests of the patient showed a marked increase in in

BACKGROUND: Melioidosis is a potentially fatal tropical infection. Little is known about mediastinal involvement in melioidosis. This study aimed to (a) describe the prevalence and demographics of various morphologies of mediastinal melioidosis, (b) propose a classification for radiological morphologies of mediastinal melioidosis.
METHODS: A retrospective cohort study was performed. Case records of consecutive patients with culture-positive melioidosis who underwent computed tomography (CT) thorax from January 1, 2018-February 28, 2022, were reviewed.
RESULTS: 486 culture-positive melioidosi

BACKGROUND: Melioidosis is uncommon but endemic in Southeast Asia and parts of Northern Australia. Cerebral melioidosis is rare but can be spread through several routes, such as hematogenous spreading or the direct inoculation of organisms from wound contamination with soil. It can cause devastating sequelae if the treatment is delayed. However, with early and adequate treatment, patients can recover and have a good quality of life.
CASE DESCRIPTION: A 62-year-old diabetic male presented with epilepsy 2 months after a head injury. A computed tomography scan revealed an abscess extending from

A retrospective review of culture confirmed musculoskeletal melioidosis reported between December 2014 and February 2024 was conducted. 22 of 100 patients with culture proven melioidosis infection had musculoskeletal involvement. Median age was 47 years with 95.4 % being males. Osteomyelitis, septic arthritis and septic arthritis along with adjacent osteomyelitis were observed in 12, 9 and 4 cases respectively. All isolates were susceptible to meropenem and susceptible increased exposure to ceftazidime, doxycycline and cotrimoxazole. Twelve patients underwent surgical procedures. Four patients

Neurologic melioidosis warrants attention in view of its rarity and lethal consequences. We performed a retrospective review of culture confirmed neurologic melioidosis reported between 2017 and 2023. Thirteen patients were identified with median age 43 years and all but three were males. Brain parenchyma was involved in 57 % cases (7/13) as cerebritis, encephalitis and macro abscess or micro abscess. Three patients had myelitis with spinal cord micro abscess and cerebral venous thrombosis in three other patients. Five patients died during hospitalization. Two were discharged against medical a

Melioidosis infecting a periurethral diverticulum and behaving as an abscess is a rare complication. A 76-year-old woman presented with pelvic pain, dysuria and urinary frequency. CT identified a large periurethral cystic collection and melioidosis was cultured in her urine. Cystoscopy revealed communication between urethra and diverticulum, requiring multiple transvaginal aspirations for re-accumulation and relapsing symptoms. No risk factors for melioidosis were identified, and was likely that isolated urinary infection is due to her urinary tract pathology. This challenging case with a rare

We report a case of cutaneous melioidosis in a 54-year-old male with a meropenem-resistant sub-population. He was empirically treated with episodic doxycycline and trimethoprim-sulfamethoxazole; however, the abscess re-accumulated. The patient had no prior exposure to meropenem. A sub-population of the isolate was meropenem resistant with an MIC >32 µg ml-1 and the identification was re-confirmed as Burkholderia pseudomallei. Whole-genome sequencing with ARDaP analysis only revealed a resistance determinant to doxycycline and did not reveal a resistance determinant to meropenem. Furthermore, n

BACKGROUND: Melioidosis, caused by Burkholderia pseudomallei, is an important disease that is endemic in areas of northern Australia and can cause a wide range of clinical signs in people and animals. There is limited published data on the disease in marsupials.
CASE REPORT: Two captive red-legged pademelons (Thylogale stigmatica) with collective signs of lethargy, dysphagia and bloody oral discharge were submitted for necropsy and showed variations in pathology findings ranging from multifocal abscessation in various tissues to evidence of fulminant septicaemia. In both cases Burkholderia ps

BACKGROUND: Streptobacillus moniliformis is the primary causative agent of rat bite fever, an infectious disease transmitted through contact with rats through bites, scratches, or exposure to excrement. Before this report, only two instances of spinal epidural abscess (SEA) due to S. moniliformis infection have been documented. We present the case of a 76-year-old male who developed a cervical SEA secondary to S. moniliformis infection, requiring neurosurgical decompression of the spinal cord.
CASE DESCRIPTION: A 76-year-old male presented to the emergency department with bilateral shoulder a

Melioidosis is caused by percutaneous inoculation or inhalation of Burkholderia pseudomallei, predominantly among individuals with risk factors (diabetes mellitus, immunosuppression, etc.) from endemic areas of South Asia, Southeast Asia and Northern Australia. While some patients present acutely with sepsis and multi-organ failure, others present with a subacute to chronic course characterised by abscess formation. We present nine representative cases, each with an abscess at a separate site (lung, skin, bone/ joint, prostate, parotid gland, liver, spleen, brain and orbits). Using these cases

Glanders is a rare zoonotic disease caused by Burkholderia mallei (B. mallei). B. mallei can cause pneumonia, abscesses, osteomyelitis in severe cases, sepsis, and even death in humans. In this report, we present a 15-year-old male patient living in a rural area who was diagnosed with glanders. The patient, who did not have any previous disease, was followed up with a diagnosis of pneumonia in the hospital, where he was admitted with complaints of cough and abdominal pain and presented to us with pain, redness, and swelling in his leg. Magnetic resonance imaging of the lower extremity revealed
