Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 2.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that often starts in the second or third decade of life, induces both inflammatory and cicatricial cutaneous lesions, and frequently involves intertriginous and anogenital anatomic sites. Abscesses, nodules, and draining tunnels are reversible inflammatory lesions that cycle over time through unpredictable periods of exacerbation; cicatricial lesions are permanent and persist as fibrotic scars and nondraining tunnels, the latter reflecting the shells of prior draining tunnels. HS has a profound adverse impact on overall

Peritonsillar Abscess Status Post Prior Tonsillotomy: A Potential Long-Term Risk.

OBJECTIVE: Peritonsillar abscess (PTA) after surgical tonsil removal is an infrequent complication. Residual lymphoid tissue after tonsillotomy may increase this risk compared to tonsillectomy. This study aimed to identify cases of PTA following tonsillotomy or tonsillectomy in a pediatric population and compare the incidence and case details. STUDY DESIGN: Retrospective comparative case series. SETTING: Tertiary pediatric hospital. METHODS: A review was conducted of patients ≤18 years of age who underwent tonsillotomy or tonsillectomy from January 2010 to January 2025 and later presented w

Prostatic Involvement in Granulomatosis with Polyangiitis: A Case Report and Scoping Review.

Background and Clinical Significance: Granulomatosis with polyangiitis (GPA) is a systemic antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that typically affects the respiratory tract and kidneys. Prostatic involvement is rare and may mimic infection or malignancy, delaying diagnosis. We report a case of exceptionally extensive urogenital destruction and provide a scoping review of prostatic involvement in GPA published since the last comprehensive review in 2019. Case Presentation: A 60-year-old man underwent repeated antibiotic courses and urological procedures for sterile,

Concurrent Middle Ear and Vertebral Tuberculosis Secondary to Mycobacterium Africanum in an Immunocompetent Young Adult in a Low-Incidence Setting: Diagnostic Value of Middle Ear Biopsy.

Otic tuberculosis is an exceedingly rare manifestation of extrapulmonary tuberculosis (EPTB) and typically mimics more common otologic conditions, leading to delayed diagnosis and treatment. This report discusses a case of middle ear tuberculosis in a New York City-born-and-raised young woman. She presented with ear drainage, hearing loss, generalized weakness, and a painful lump on her posterior neck. She was subsequently diagnosed with multiple abscesses along the cervical and thoracic spine and a large mass in her middle ear that protruded through her tympanic membrane into the external aud

Nocardiosis: death from the ground.

Nocardiosis is a rare but potentially life-threatening infection caused by aerobic, filamentous, gram-positive bacteria of the genus Nocardia found in soil and water. Although traditionally found in immunocompromised patients, up to one-third of cases occur in immunocompetent individuals, contributing to frequent diagnostic delays. Historically fatal before the antibiotic era, outcomes have substantially improved with modern antimicrobial therapy and diagnostic advances. However, nocardiosis remains underrecognized due to its nonspecific presentation and ability to mimic more common diseases.

Ultrasonographic Features and Diagnostic Challenges of Hepatic Tuberculosis: Insights from Taiwan and Southeast Asia.

Hepatic tuberculosis (HTB) is a rare manifestation of extrapulmonary tuberculosis (TB) and represents fewer than 1% of all tuberculosis cases. From an imaging perspective, HTB poses a significant diagnostic challenge because its ultrasonographic appearances are highly variable and frequently mimic hepatic malignancy or pyogenic abscess, often resulting in delayed or incorrect diagnosis. This review focuses on the ultrasonographic features of hepatic and hepatobiliary TB, with reference to the epidemiologic data from Southeast Asia and Taiwan. Ultrasonography remains the most commonly used firs

Brain Abscess Caused by Enterococcus Faecalis Following Intracerebral Hemorrhage and Mimicking Chronic Encapsulated Intracerebral Hematoma: A Case Report.

Brain abscess developing at the site of intracerebral hemorrhage has rarely been reported. We report herein a case of brain abscess following intracerebral hemorrhage that mimicked a chronic encapsulated intracerebral hematoma. An 80-year-old man presented with right hemiparesis caused by a left frontoparietal subcortical intracerebral hemorrhage, which was managed conservatively. During hospitalization, progressive motor aphasia developed. Two weeks after admission, magnetic resonance imaging demonstrated worsening perihematomal edema and ring enhancement along the hematoma margin. Diffusion-

Postoperative Infection Risk in Sacrocolpopexy: Single vs Dual Antibiotic Prophylaxis.

INTRODUCTION AND HYPOTHESIS: The optimal antibiotic prophylaxis for mesh-based prolapse surgery remains uncertain. Although cefazolin is widely used, its limited anaerobic coverage may increase the risk of postoperative infections, particularly in women undergoing concomitant hysterectomy. Evidence from hysterectomy cohorts suggests that adding metronidazole reduces surgical-site infections, prompting evaluation of this approach in laparoscopic mesh-augmented apical prolapse repair. The objective was to compare postoperative infection rates in women receiving single-agent versus dual-agent pro

Diagnostic uncertainty in a patient with severe hidradenitis suppurativa revealing Buschke-Löwenstein tumor.

Buschke-Löwenstein tumor (BLT), also known as giant condyloma acuminatum, is a rare human papillomavirus-associated lesion characterized by locally aggressive growth with the potential for malignant transformation. Diagnosis may be particularly challenging when BLT arises in the setting of severe chronic inflammatory disease. We present a 44-year-old man with severe hidradenitis suppurativa, well-controlled HIV infection, recurrent gluteal abscesses, and chronic sacral osteomyelitis who presented with worsening perineal pain and chronic drainage. Although imaging demonstrated extensive infiltr

Biomarkers for Early Detection of Postoperative Intra-Abdominal Abscess in Children with Perforated Appendicitis.

Background/Objectives: Appendicitis is a common pediatric surgical emergency, with perforation leading to significant complications such as intra-abdominal abscesses. Current diagnostic methods are not sufficiently investigated in terms of timeliness, which emphasizes the need for reliable, non-invasive biomarkers for early signaling of postoperative abscess formation. This study aims to evaluate the associations between laboratory markers and postoperative intra-abdominal abscess formation in children treated for perforated appendicitis. Methods: This study retrospectively analyzed records of

Epidural Abscess in Routine Preoperative MRI Reports Versus Documented Operative Findings, and Exploratory Mortality Risk Factors in Surgically Treated Spondylodiscitis: A Retrospective Single-Centre Study.

Background: Surgical decisions in spondylodiscitis rely on the preoperative magnetic resonance imaging (MRI) report, and postoperative mortality remains high. We examined how often routine MRI reports described an epidural abscess later documented at surgery and explored factors associated with mortality. Methods: Twenty-seven adults operated on for pyogenic or fungal spondylodiscitis at one centre (2017-2025) were studied retrospectively. Findings were abstracted from routine MRI reports and operative notes, with epidural pus documented at surgery as the reference. Mortality was explored with

Histopathologic spectrum of focal liver lesions diagnosed by ultrasound-guided percutaneous liver biopsy and predictors of hepatocellular carcinoma: A single-center experience from Sub-Saharan Africa.

BACKGROUND: Focal liver lesions (FLLs) encompass a wide spectrum of benign and malignant pathologies, and accurate diagnosis is essential for appropriate management. Although advances in imaging have improved lesion characterization, histopathologic assessment remains the diagnostic gold standard for indeterminate lesions. Data on the histopathologic spectrum and diagnostic utility of ultrasound-guided percutaneous liver biopsy (US-PLB) in sub-Saharan Africa (SSA) are limited. This study aimed to characterize the histopathologic findings of US-PLB performed for FLLs at a tertiary referral cent

[Liver abscess caused by hypermucoviscous Klebsiella pneumoniae].

Hypervirulent Klebsiella pneumoniae (hvKP) represents an emerging highly virulent subtype, associated with cryptogenic liver abscesses and severe metastatic infections. First described in Asia in the 1980s, its global spread has been progressive, with a recent increase in reported cases in Argentina. It is characterized by a hypermucoviscous phenotype demonstrated by the "string test," capsular serotypes K1 and K2, and specific genetic determinants that confer resistance to phagocytosis and complement-mediated killing. We report the case of a 34-year-old man without major comorbidities who pre

Temporal Neuroimmune Changes During Primary and Persistent Experimental Toxoplasmic Encephalitis.

Toxoplasma gondii persists in the central nervous system and may cause severe toxoplasmic encephalitis, particularly in immunocompromised hosts. Our previous studies demonstrated that T. gondii-induced neuropathology involves oxidative and nitrosative stress, apoptosis, glial activation, blood-brain barrier injury, and GMF-β- and SAP/NF-κB-associated responses. This study investigated the temporal immunoreactivity of CD68, S100B, Foxp3, and IL-5 during early and chronic experimental infection. Twenty-four female Swiss albino mice were assigned to one uninfected control group and three ME49-inf
