Sigmoid wall thickness correlates with fecal calprotectin during irritable bowel syndrome exacerbation.

AIM: Patients with diarrhea-predominant irritable bowel syndrome (IBS-D) who present with symptom exacerbation [defined by a high IBS-Symptom Severity Scale (IBS-SSS)] may have increased fecal calprotectin (FC). Intestinal ultrasound (IUS) is used to measure bowel wall thickness (BWT). The aim of our study was to measure BWT in IBS-D patients, ulcerative colitis (UC) and healthy controls (HC), and compare BWT, IBS-SSS score, and FC level. MATERIAL AND METHOD: Patients with IBS-D and symptom exacerbation (n=100), active UC (n=25), and HC (n=30) were included. FC was measured in all groups. IBS

A single test strategy using spot fecal bile acid test may be a feasible strategy for the diagnosis of bile acid malabsorption.

BACKGROUND: Bile acid malabsorption (BAM) is often missed in patients with chronic diarrhea as diagnostic tests are technically challenging and not available widely. Quantitative estimation of fecal bile acids (FBA) in a single stool sample has been reported recently for the diagnosis of BAM and may be easily applied. METHODS: We performed a pilot observational cross-sectional study to estimate the optimal FBA cut-point for the diagnosis of BAM, using the IDK® Bile Acid test, an enzymatic spectrophotometry-based assay for measuring total stool bile acids. We estimated FBA concentrations in he

PM2.5 exacerbates brain injury in diarrhea-predominant irritable bowel syndrome mice via the disruption of the microbiota-gut-brain axis and the restoring effect of resveratrol.

Fine particulate matter (PM2.5) poses escalating neurological risks, but its impact on susceptible populations, such as those with diarrhea-predominant irritable bowel syndrome (IBS-D), via the gut-brain axis remains unclear. This study investigated whether PM2.5 exacerbates gut-brain axis dysfunction in IBS-D mice and whether resveratrol (RES) offers protection. Six mouse models were established: CON, IBS-D, IBS-D + RES, PM2.5, PM2.5+IBS-D, and PM2.5+IBS-D + RES. Using 16S rRNA sequencing and non-targeted metabolomics, we assessed gut microbiota, metabolic profiles, intestinal sensitivity, br

Comparative effectiveness and underlying mechanisms of acupuncture approaches in IBS-D animal models: a protocol for systematic review and network meta-analysis.

Irritable bowel syndrome with predominant diarrhea (IBS-D) is a type of functional gastrointestinal disorder mainly marked by abdominal discomfort and frequent loose stools. Its complex pathogenesis involves multiple factors, such as low-grade intestinal inflammation, brain-gut axis dysregulation, and visceral hypersensitivity. Although acupuncture therapies show the potential to alleviate IBS-D symptoms, systematic evidence regarding their comparative efficacy and underlying mechanisms remains lacking. Methods A systematic search will be conducted across eight major databases: CNKI, WanFang D

BACKGROUND & AIMS: The low Fermentable Oligosaccharides, Disaccharides, Monosaccharides, And Polyols (FODMAP) diet (LFD) and rifaximin are effective in <50% individuals with irritable bowel syndrome (IBS), highlighting the need to identify predictors of treatment response. We therefore conducted a randomized controlled trial comparing LFD and rifaximin to identify microbial predictors of response. METHODS: Sixty-five adults with diarrhea-predominant IBS (IBS-D) were randomized to LFD or rifaximin for 5 weeks. Primary endpoints were changes in mean daily abdominal pain and bloating at week 5 v

Glucagon-Like Peptide-1 Receptor Agonists for Bile Acid Diarrhea: Emerging Evidence and Clinical Implications.

Bile acid diarrhea (BAD) is an underrecognized cause of chronic diarrhea that makes a significant contribution to the symptom burden and impaired quality of life. Despite increasing awareness, BAD is frequently misdiagnosed as diarrhoea-predominant irritable bowel syndrome (IBS-D), leading to delayed diagnosis and incomplete symptom control. Current management relies primarily on bile acid sequestrants, which are effective for many patients but limited by poor tolerability, variable adherence, and incomplete response in a subset, prompting interest in alternative therapeutic approaches targeti

Serum FGL2 is correlated with the severity and psychological status of diarrhea-predominant irritable bowel syndrome.

BACKGROUND: Fibrinogen-like protein 2 (FGL2) is involved in immune-mediated inflammatory responses. However, its role in diarrhea-predominant irritable bowel syndrome (IBS-D) remains to be elucidated. This study investigated serum FGL2 levels in patients with IBS-D and examined their association with inflammatory cytokines, disease severity, and psychological status. Patients with inflammatory bowel disease (IBD), including Crohn's disease and ulcerative colitis, were excluded to avoid diagnostic overlaps. IBS-D diagnosis was based on the Rome III criteria. METHODS: A total of 117 patients wi

A Previous History of Endometriosis Elevates Irritable Bowel Syndrome Risk in Middle-Aged and Older Women: A UK Biobank Cohort Study of 225,703 Participants.

OBJECTIVE: Endometriosis (EMT) and irritable bowel syndrome (IBS) disproportionately affect middle-aged and older women, where overlapping symptoms (e.g., chronic pelvic pain, bloating) and shared inflammatory mechanisms may drive comorbidity. However, large-scale evidence linking EMT to IBS incidence in this population remains scarce. Leveraging the UK Biobank cohort (N = 225,703), we investigate their longitudinal association to address this critical gap in women's health research. METHOD: We sourced data on pre-existing endometriosis (EMT) status and incident IBS diagnoses from the UK Biob
