Dengue is the most prevalent arthropod-borne viral disease, caused by infection with the dengue virus (DENV). Severe dengue is characterized by significant vasculopathy involving a proinflammatory and procoagulant state associated with increased vascular permeability. However, the host-virus interactions driving this process remain incompletely elucidated. Monocytes (Mø) are primary target cells during DENV infection and actively release extracellular vesicles, like microparticles (MPs), mediating intercellular communication, contributing to dengue pathogenesis. Here, we evaluated whether MPs

BACKGROUND: Dengue virus (DENV) infections cause a range of clinical symptoms, from a mild febrile illness to severe disease. Higher levels of DENV RNAemia are associated with severe dengue, although this relationship is incompletely understood. Dengue Human Infection Models (DHIMs), in which volunteers are experimentally infected with underattenuated DENV strains, provide an invaluable tool for studying early virologic, transcriptional, and immunologic features of infection. DHIM studies using DENV-1, DENV-3, and DENV-4 have demonstrated qualitatively distinct clinical features, however, the

This case report discusses a rare and challenging presentation of dengue haemorrhagic fever (DHF) in a 62- year-old male, complicated by acute coronary syndrome (ACS) and multi-organ failure (MOF). The patient was initially afebrile, presented with respiratory symptoms, and was diagnosed with non-ST elevation myocardial infarction (NSTEMI) and cardiogenic shock. Dengue serology confirmed a secondary dengue infection with profound thrombocytopenia, respiratory failure, and cardiogenic shock. Coronary angiography revealed triple-vessel disease, prompting consideration of coronary artery bypass g

Vitamin D affects immune function and possibly, dengue severity, but evidence is inconsistent. Observational studies comparing vitamin D levels between severe and nonsevere dengue were systematically searched. A random-effects meta-analysis was conducted. Heterogeneity was quantified using I2. Subgroup and sensitivity analyses were performed. Six studies with 1,130 participants were included. The pooled odds ratio (OR) for severe dengue with vitamin D deficiency was 1.77 (95% CI: 0.75-4.16, P = 0.194) with substantial heterogeneity (I2 = 61.6%). A subgroup analysis showed a significant associa

BACKGROUND: Thrombocytopenia is a characteristic of dengue infection; however, the underlying mechanisms of these phenotypes remain underexplored clinically. Platelet indices may serve as a swift and accessible method for mechanistic distinction and risk assessment.
OBJECTIVE: The objective of this study was to evaluate platelet indices for mechanistic categorization of thrombocytopenia phenotypes in dengue patients, assess their association with bleeding risk, determine their diagnostic performance for bleeding prediction, and develop a preliminary Dengue Bleeding Risk Score (DBRS) for risk

Dengue in pregnancy is difficult to manage because warning signs, thrombocytopenia, hematocrit changes, and bleeding risk may overlap with pregnancy-related physiology and obstetric conditions. Data on in-hospital progression and transfer from maternity hospitals in dengue-endemic settings remain limited. This retrospective observational study included pregnant women admitted to Hung Vuong Hospital, Ho Chi Minh City, during 2018-2022 with acute dengue confirmed by positive nonstructural protein 1 antigen and/or dengue immunoglobulin M. Dengue severity was reclassified using World Health Organi

Human endogenous retroviruses (HERVs), remnants of ancient viral infections, are increasingly recognized as potential regulatory elements influencing gene expression and immune responses. However, their role in infectious disease severity remains poorly understood. Here, we investigate the transcriptional impact of HERVs in dengue disease progression by leveraging RNA sequencing data from a large cohort of hospitalized patients across different severity subgroups. We identify differentially expressed HERVs and their co-expressed genes, revealing a severity-dependent transcriptional signature.

BACKGROUND & AIMS: Severe dengue (SD) is the leading cause of morbidity and mortality resulting from dengue virus (DENV) infection, with liver injury being one of the most common complications. However, besides symptomatic treatment, there is currently no targeted therapy, and the specific mechanism underlying liver injury in SD remains elusive. This study aimed to investigate the molecular mechanisms underlying liver injury in SD to facilitate the development of prophylactic and therapeutic interventions.
METHODS: To systematically characterize dynamic liver lesions in an SD mouse model, we

Hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening hyperinflammatory syndrome caused by uncontrolled immune activation and increased release of pro-inflammatory cytokines, resulting in multiorgan dysfunction. Secondary HLH is often activated by infections, especially viral infections. Dengue fever, the most common arboviral infection in tropical regions, is an uncommon but known precipitating factor of HLH and continues to challenge diagnosis due to overlapping clinical and laboratory manifestations. We describe the case of a 23-year-old previously healthy woman who presen

PURPOSE OF REVIEW: Despite recent advances in understanding dengue infection, substantial gaps remain in its epidemiology, pathogenesis, diagnosis, and prevention, including vaccine development and its long-term impact. This review summarizes recent findings and highlights the most pressing research needs to advance the field and reduce individual and public health burden.
RECENT FINDINGS: Dengue burden has increased, particularly among children and adolescents, and it has expanded into previously unaffected regions. Still, current data likely underestimates the true burden due to suboptimal

PROBLEM: In early 2024, Lao People's Democratic Republic experienced a surge in dengue cases approaching levels of its 2013 epidemic, underscoring the persistent threat of dengue during the monsoon season. Although awareness of preventive behaviours is high, inconsistent adoption has limited their effectiveness.
CONTEXT: In response, the Ministry of Health, in collaboration with the World Health Organization, implemented an innovative, humour-driven risk communication and community engagement (RCCE) campaign targeting individuals who perceived dengue as a routine health concern.

Dengue virus (DENV) poses a serious global health challenge, particularly in cases of dengue hemorrhagic fever (DHF). Patients with preexisting mitochondrial disorders may be at increased risk for complications due to the specific impact of DENV on mitochondrial-dependent cellular processes and immune function. We describe a 16-year-old male with known mitochondrial complex I deficiency caused by a homozygous likely pathogenic variant in NDUFV1 who subsequently developed DHF. His illness was marked by rapidly worsening weakness, respiratory distress, intracranial hemorrhage, and seizures. His

Severe dengue is characterized by marked vascular leakage, hemorrhagic manifestations, and organ impairment, largely driven by immunopathological mechanisms. Traditional vaccine approaches focusing on inducing neutralizing antibodies against DENV structural antigens have shown limitations and, under certain conditions, may also worsen infection through antibody-dependent enhancement (ADE) of infection. More recently, increasing evidence has highlighted the importance of T cell-mediated immunity and anti-non-structural protein 1 (NS1)-specific responses in suppressing DENV replication and reduc
