Clinical and haematological correlates of severe dengue using 2023 epidemic data: A multicentre tertiary hospital-based analysis from Dhaka, Bangladesh.

BACKGROUND: Dengue fever, caused by the dengue virus (DENV), remains a major public health threat in tropical and subtropical regions. Bangladesh recoded its largest dengue outbreak in 2023. Understanding factors associated with dengue severity is essential to improve clinical triage and outcome. METHODS: We conducted a hospital-based study of 466 laboratory-confirmed dengue cases admitted to four tertiary hospitals in Dhaka during the peak epidemic period (September 1 to October 31, 2023). Demographic, clinical, and laboratory data were collected using a semi-structured questionnaire. Cases

Molecular analysis and computational modeling reveal temporally separable responses triggered by DENV-induced soluble factors in endothelial cells.

Dengue virus (DENV) represents a growing global health challenge with billions of people at risk. Severe Dengue (SD), a complication of DENV infection that involves generalized hemorrhage, is driven, at least in part, by endothelial dysfunction. Endothelial dysfunction refers to increased permeability due to inflammation, mechanical injury and/or modification of the genetic program of endothelial cells. Previous work showed that exposure of endothelial cells to conditioned media from DENV-infected cells (CMDV) increased permeability and cellular stiffness, repressed endothelial markers and ind

Extensive intraventricular hemorrhage, complicated by dengue-induced coagulopathy, initially misdiagnosed as gastroenteritis in a toddler: A case report.

Incomplete physical examinations especially omissions of Central nervous system (CNS) assessments remain a major cause of missed or delayed diagnoses of serious neurological conditions in febrile children with nonspecific symptoms. I report the case of a 22-month-old male initially presenting with prolonged fever, coffee-ground vomiting, nonbloody diarrhea, and recurrent convulsions, leading to a presumptive diagnosis of gastroenteritis with sepsis at Mwananyamala regional referral hospital. Despite initial management, his condition deteriorated. Subsequent evaluation at Muhimbili National Hos

Platelet Transfusion in Dengue-Associated Thrombocytopenia: A Systematic Review and Meta-Analysis.

Thrombocytopenia is a known and potentially severe manifestation of dengue hemorrhagic fever, raising concerns regarding the risk of hemorrhage. To prevent such complications, prophylactic platelet transfusion (PT) has been suggested as a possible intervention. We aimed to evaluate the efficacy and safety of PT in dengue-associated thrombocytopenia. We conducted a systematic literature search for randomized controlled trials and cohorts across PubMed, Cochrane Library, Web of Science, Scopus, and Embase from inception to November 2025. Primary outcomes were time to platelet count ≥ 50 × 10³ pl

Dengue virus envelope epitope-specific antibodies are associated with protection from severe dengue.

Our understanding of a protective humoral immune response to dengue virus (DENV) remains limited. The envelope (E) protein is the main antibody (Ab) target. While anti-fusion loop (FL) epitope monoclonal Abs (mAbs) can induce antibody dependent enhancement (ADE) in vitro, some mAbs targeting quaternary epitopes can cross-neutralize different DENV serotypes. However, the contribution of each Ab subset to disease outcome remains poorly characterized. We defined DENV2 E epitope-specific Abs dynamics and assessed their association with disease outcome in a cohort of hospitalized and subclinical de

A Clinical and Operational Preparedness Framework for Dengue in Colombia: Integrating Surveillance, Clinical Response, Prevention, and Health System Planning.

Dengue is an arboviral disease of major public health relevance in Colombia, where recurrent endemic-epidemic transmission intersects with climatic variability, urbanization, population mobility, co-circulation of viral serotypes and lineages, seasonal healthcare pressure, and substantial direct and indirect costs. Although dengue prevention and response in Colombia include epidemiological surveillance, clinical diagnosis, management of warning signs, vector-control activities, outbreak response, and assessment of preventive interventions, these components may be implemented unevenly across te

Bulk transcriptomics of peripheral blood mononuclear cells delineates systems-level immune dysregulation in pediatric dengue infection.

Children are disproportionately at a greater risk of developing severe dengue disease, yet the molecular mechanisms driving the heightened susceptibility remains poorly understood. To elucidate the immune determinants of pediatric dengue pathogenesis, we profiled the transcriptomic landscape by performing RNA sequencing (RNA-Seq) on peripheral blood mononuclear cells (PBMCs) from laboratory-confirmed cases of primary and secondary pediatric dengue, as well as pediatric healthy controls. Differential gene expression (DEG) and pathway analyses were performed to delineate the transcriptional alte

IFNL1 gene promoter single nucleotide polymorphism rs7247086 enhances transcription through a STAT-binding site.

A single nucleotide polymorphism (SNP) within the human interferon lambda 1 (IFN-L1, IFN-λ1) gene promoter, rs7247086 (C/T) has been reported to be associated with severe dengue and possibly with psoriasis and COVID-19. However, its functional nature is unknown. The present study was undertaken to examine the effect of rs7247086 on transcription, by utilizing promoter and enhancer-reporter assays. We see that the T allele completes a consensus signal transducer and activator of transcription (STAT)-binding site. While we did not find strong evidence to show that the STAT-binding site drove tra

Association of pre-existing antithrombotic therapy with clinical outcomes in patients with acute dengue infection.

BACKGROUND: Dengue infection frequently causes thrombocytopenia and bleeding complications. Patients with cardiovascular disease who receive antiplatelet or anticoagulant therapy represent a clinically challenging population during acute dengue infection. Data regarding clinical outcomes and platelet recovery in these patients remain limited. METHODS: This observational study included a single-center retrospective cardiovascular dengue cohort and an exploratory platelet-monitoring cohort. In the retrospective cohort, patients were classified according to pre-existing antithrombotic therapy (A

The evolution of paediatric dengue management over the past three decades-a historical perspective.

Over the past three decades, the recognition and management of dengue fever (DF) and severe dengue (SD) in children have undergone significant transformation. In the 1990s, limited understanding of dengue pathophysiology led to delayed recognition, with aggressive fluid resuscitation, invasive ventilation, and frequent blood product use, often resulting in adverse outcomes. The 1997 World Health Organisation (WHO) Guidelines introduced a structured classification and emphasized cautious fluid management, improving survival. Advances in diagnostics, particularly non-structural (NS) protein anti

Invasive Aspergillosis Following Dengue: An Autopsy Case and Review of the Literature.

Invasive aspergillosis typically occurs in immunocompromised hosts but has been increasingly recognized following viral illnesses, including dengue and coronavirus disease 2019. Herein, a fatal case of disseminated aspergillosis following severe dengue is described, and the association is systematically reviewed. A previously healthy 25-year-old woman presented with fever and multiorgan dysfunction. Dengue IgM results were positive, and other tropical infections were excluded. Despite intensive care, the patient developed progressive respiratory failure, refractory shock, and acute kidney inju

Approximating Bayesian spatial relative risk of dengue in Bandung: A gradient boosting surrogate model with demographic and climatic features.

Dengue Hemorrhagic Fever (DHF), a disease transmitted by Aedes aegypti mosquitoes, remains a critical public health issue. Bandung is recorded as having one of the highest numbers of DHF cases in Indonesia, exhibiting a concerning increasing trend annually. This persistent rise indicates that the current control efforts for DHF have not been optimal. In this study, the relative risk of DHF transmission across subdistricts in Bandung is estimated using the Besag, York, and Mollié (BYM) model within a Bayesian inference framework. The BYM model is extended by incorporating additional covariates:
